Towards Safety Based Design Procedure for Ships

  • Present-day rules and regulations for the design and construction of ships are almost without exemption of a prescriptive and deterministic nature. Often it is argued that this situation is far from ideal; it does no right to the advances, which have been made during the past decades in engineering tools in marine technology, both in methodology and in computational power. Within IMO this has been realized for some time and has resulted in proposals to use Formal Safety Assessment(FSA) as a tool to improve and to modernize the rule making process. The present paper makes use of elements of the FSA methodology, but instead of working towards generic regulations or requirements, a Risk Assessment Approach, not unlike a 'safety case'; valid for a certain ship or type of ship is worked out. Delft University of Technology investigated the application of safely assessment procedures in ship design, in co-operation with Anthony Veder Shipowners and safety experts from Safely Service Center BV. The ship considered is a semi-pressurized-fully refrigerated LPG carrier. On the basis of the assumption that a major accident occurs, various accident, scenarios were considered and assessed, which would impair the safety of the carrier. In a so-called Risk Matrix, in which accident frequencies versus the consequence of the scenarios are depicted, the calculated risks all appeared lo be in the ALARP('as low as reasonable practicable') region. A number of design alternatives were compared, both on safety merits and cost-effectiveness. The experience gained with this scenario-based approach will be used to establish a set of general requirements for safety assessment techniques in ship design. In the view that assessment results will be most probably presented in a quasi-quantified manner, the requirements are concerned with uniformity of both the safety assessment. These requirements make it possible that valid comparison between various assessment studies can be made. Safety assessment, founded on these requirements, provides a validated and helpful source of data during the coming years, and provides naval architects and engineers with tools experience and data for safety assessment procedures in ship design. However a lot of effort has to be spent in order to make the methods applicable in day-to-day practice.

  • The increased demand on the transport of both passengers and goods through rail systems implies higher traffic intensity and congestion on the railways, resulting in greater likelihood of accidents and also degraded passenger services. To cope with the issues, development of an integrated communication network for rails has attracted great deal of attention lately. GSM-R is such an example developed in Europe, which seems to have restrictions in providing various communication services due to network speed limit. For the reason, an LTE-based approach is under study in Korea. After the network development, operation management of the network is necessary. Design of operation management systems has been studied little and thus is the objective of this paper. To do so, a conceptual design has been carried out based on model-based approach. Specifically, a context model has first been created using the use case diagram. Then, SysML models of operational scenarios were developed for the management system. The SysML models have alternatively been expressed as EFFBD models to simulate and verify them. Consequently, the result of the conceptual system design for the operation management of the integrated wireless network is expected to be used as a basis for the detailed design later.

Study on the Damage Pattern Analysis of a 3 Phase 22.9/3.3kV Oil Immersed Transformer and Judgment of the Cause of Its Ignition (3상 22.9/3.3kV 유입변압기의 소손패턴 해석 및 발화원인 판정에 관한 연구)

  • The purpose of this paper is to present the manufacturing defect and damage pattern of a 3 phase 22.9/3.3kV oil immersed transformer, as well as to present an objective basis for the prevention of a similar accident and to secure data for the settlement of PL related disputes. It was found that in order to prevent the occurrence of accidents to transformers, insulating oil analysis, thermal image measurement, and corona discharge diagnosis, etc., were performed by establishing relevant regulation. The result of analysis performed on the external appearance of a transformer to which an accident occurred, the internal insulation resistance and protection system, etc., showed that most of the analysis items were judged to be acceptable. However, it was found that the insulation characteristics between the primary winding and the enclosure, those between the ground and the secondary winding, and those between the primary and secondary windings were inappropriate due to an insulating oil leak caused by damage to the pressure relief valve. From the analysis of the acidity values measured over the past 5 years, it is thought that an increase in carbon dioxide (CO2) caused an increase in the temperature inside the transformer and the increase in the ethylene gas increased the possibility of ignition. Even though 17 years have passed since the transformer was installed, it was found that the system's design, manufacture, maintenance and management have been performed well and the insulating paper was in good condition, and that there was no trace of public access or vandalism. However, in the case of transformers to which accidents have occurred, a melted area between the upper and the intermediate bobbins of the W-phase secondary winding as well as between its intermediate and lower bobbins. It can be seen that a V-pattern was formed at the carbonized area of the transformer and that the depth of the carbonization is deeper at the upper side than the lower side. In addition, it was found that physical bending and deformation occurred inside the secondary winding due to non-uniform pressure while performing transformer winding work. Therefore, since it is obvious that the accident occurred due to a manufacturing defect (winding work defect), it is thought that the manufacturer of the transformer is responsible for the accident and that it is lawful for the manufacture to investigate and prove the concrete cause of the accident according to the Product Liability Law (PLL).

  • A condensation heat transfer model is very important for the safety analysis of nuclear power plants. Especially, condensation under the presence of noncondensable gases (NCGs) is an important issue in nuclear safety because the presence of even a small quantity of NCGs in the vapor largely reduces the condensation rate. In this study, the condensation heat transfer model of the severe accident analysis code MELCOR 1.8.6 has been assessed using a set of condensation experiments performed under the thermal-hydraulic conditions similar to those inside a containment during design-basis accidents or severe accidents. Experiment conditions are categorized into 4 types according to the shape of the condensation surface: vertical flat plates, outer surface of vertical pipes, inner surface of vertical pipes, the inner surface of horizontal pipes. The results of the calculations show that the MELCOR code generally under-predicts the condensation heat transfer except the condensation on inner surface of vertical pipes.

  • When a car accident occurs, people who had an accident are not free from civil and criminal issues so that the accident investigator should reenact and analyze the accident situation accurately. In addition, the obtained documents through the analysis of such car accident occurrence and related factors have to be used to carry out the improvement of the areas that has numerous car accidents and complementary actions. The vehicle speed, accelerating force, braking power are currently known as the most affecting factors in accordance with many car accidents, traffic facilities, road design, etc. The vehicle's performance and rode friction coefficient road surface friction coefficient are affecting the most closely in this field. Especially, once the estimate of the speed of the accident moment relating to main eleven articles of Traffic Accident Exemption Law is very important and accuracy is required. However, currently the researches of these matters have not made exclusively yet in Korea. In this study by reflecting this current situation, until the sudden braking history is found from the car's sudden braking, it estimates accurately the transient brake time and rode friction coefficient by measuring a time of transient brake time through the precision speed detector (Vericom VC2000PC). The analysis of the experimental results calculated the transient brake time and friction coefficient to fit into the purpose of this study in the basis of different kind of various special purpose asphalt pavement and slip-prevention pavement and provided the fundamental data.

A Study on the Minimum Standards of Housing Repair for Older People Living in the Community (지역사회 노인을 위한 주택수리 및 개조 최저기준에 관한 연구)

  • The purpose of this study is 1) to clarify that the house is no long the safe place through the cases of the senior safety accidents and to argue the need for housing repair and 2) to present the minimum standards for housing repair by comparing the cases here as well as the abroad and to back up the standards with the current senior housing environment 300 people at least 60 years old living in Seoul$\cdot$Metropolitan area were interviewed using the structured questionnaire. As the result, the following conclusions were made: 1. There was high accident rates of the senior residents due to physical deficits within the house, causing excessive medical cost and decreased housing satisfaction. This problem can be sufficiently prevented by housing repair which can not only solve the safety problem but also support self sufficient living for the senior residents. 2. Proper housing repair required the architectural know how as well as the expertise knowledge of the physical characteristics of the senior people. Therefore, it is essential to secure the professional (i.e., occupational therapist) who can analyze the needs of the senior residents and evaluate and/or predict the obstacles during repair. Furthermore, development and distribution of the standardized manual are also needed. 3. The minimum standard for housing repair could be approached in view of 'barrier-free' concept. First, the bumps should be removed, slippery prevented, and safety grab-bar installed for safety. Second, the entrance should be widened and the bathroom and kitchen restructured to support for the senior residents' self sufficiency. To make housing repair policy more efficient, the legal basis is required. It can be incorporated into the existing senior citizens 'Welfare Act' or the 'Senior Residents Medical Insurance' which will be effective starting in 2007.

  • All over the nation, a lot of industrial complex utilize gas as their energy source. Possibilities are fire, explosion, and leakage could happen any time in these large complexes. To prevent these tragic accidents and to minimize the damage when the accident occurs, the development of diagnostic technology for these facilities is imperative. The safety check is conducted on an individual and partial basis, currently. Accordingly, the accumulation and improvement of the safety management technology is necessary in order to make all the different checking techniques and management systems compatible, since checking processes, result interpretation techniques, and subsequent prognoses are not the same. The program provides damage scenarios from gas leakage. The output enables policy makers to predict the degree of infliction. Through this program, engineers are able to design an effective gas safety program to operate and maintain ubiquitous gas facilities.

Finding and Analysis of Defective Elements of an LED Streetlight Lamp Based on the Product Liability (PL에 근거한 LED 가로등의 결함 요소 발굴 및 분석)

  • The purpose of this study is to present the range of defects mentioned in the Product Liability (PL) and to establish an objective basis and grounds for the analysis of accidents expected to happen by analyzing and presenting the external flame pattern and electrical characteristics of an LED streetlight lamp, a new lighting lamp. From the analysis of the cross-section of a cable carbonized by an external flame, it was observed that the wire's strand and insulation material had solidly adhered, and that greater voids were formed at the surface than at the center. Irregular carbide lumps were formed in the globe directly exposed to the flame, and the globe carbonized by the indirect flame showed characteristics that they had melted and flowed downward. It was found that the forward and backward resistances of the normal LED were approximately 1.74 [$M{\Omega}$] and 140 [$M{\Omega}$], respectively. The lamp burnt by the strong flame exhibited infinite forward and backward resistances and the LED did not emit light. The carbonized LED lamp was gray and exhibited fine delaminations. According to the Product Liability, a product defect signifies a simple product defect. Most of the defects were caused by the lack of stability, and the defect of the product itself occurred during the design and manufacture. The defects in warnings and markings include an insufficiency of handling manuals and warnings, expressive warranty violations, defective markings, etc. In order to prevent an accident resulting from a product, it is necessary to prepare safety warnings and documentation, establish clear-cut lines of liabilities, and subscribe insurances. However, it could be seen that important factors against the Product Liability were product improvement, response to compensation requests and law suits, credit restoration, etc.

INTEGRAL BEHAVIOR OF THE ATLAS FACILITY FOR A 3-INCH SMALL BREAK LOSS OF COOLANT ACCIDENT

  • A small-break loss of coolant accident (SB-LOCA) test with a break size equivalent to a 3-inch cold leg break of the APR1400 was carried out as the first transient integral effect test using the ATLAS (Advanced Thermal-hydraulic Test Loop for Accident Simulation). This was the first integral effect test to investigate the integral performance of the test facility and to verify its simulation capability for one of the design-basis accidents. Reasonably good thermal hydraulic data was obtained so that an integral performance of the fluid sub-systems was identified and control performance of the ATLAS was confirmed under real thermal hydraulic conditions. Based on the measured data, a post-test calculation was carried out using the best-estimate thermal hydraulic safety analysis code, MARS 3.1, and the similarity between the expected and actual data was investigated. On the whole, the post-test calculation reasonably predicts the major thermal hydraulic parameters measured during the SB-LOCA test. The obtained data will be used to enhance the simulation capability of the ATLAS and to improve an input model of the ATLAS for simulation of other target scenarios.
