Analysis of Induced Heterophoria Variations by Repeated Binocular Vision Examinations (반복된 양안시 검사로 유발된 사위도 변화 분석)

  • The purpose of this study was to evaluate the degree of heterophoria variations according to visual fatigue induced by increasing number of examinations while repeating binocular vision examinations. Twenty - one adults in their twenties who had no heterotropia other eye disease or systemic disease underwent binocular vision examinations. As the examinations progressed, the degree of heterophoria increased in both orthophoria and heterophoria. When the distance examinations was repeated 5 times, orthophoria increased degree of $1.56{\Delta}$ heterophoria. and heterophoria increased degree of $1.63{\Delta}$ heterophoria. When the near examinations was repeated 5 times, orthophoria increased degree of $1.78{\Delta}$ heterophoria. and heterophoria increased degree of $2{\Delta}$ heterophoria.. As the number of examinations increased, the result gradually deviated from the normal range as visual fatigue increased. The degree of the variation is judged to be enough to make the wrong prescription in the clinic. In conclusion, it was found that the binocular vision examinations should be conducted more accurately and promptly.

Comparison of Purchasing Decision Factors for Eyeglasses Buyer according to Demographic Characteristics (인구 사회학적 특성에 따른 안경구매자의 구매결정요소 비교)

  • Purpose: We study the eyeglasses consumer's purchasing decision, brand equity, purchase satisfaction and word of mouth. Methods: For two month, from October to December, 437 questionnaires distributed through eyeglasses shops. Sampling was performed using a convenience sampling, participants in the survey-based selflegislation (self-adminstration method) were to answer. Results: It was found that demographic characteristics - gender, age, education, income - acts as parameters representing difference of purchase decision, brand equity and word of mouth. Conclusions: Consumer's demographic characteristics such as gender, age, education, income serve as parameters that represents the differences. Purchase satisfaction by age is statistically significant. Word of mouth is also classified according to their income level is statistically significant.

A Longitudinal Study for 3 Years on Myopic Refractive Error Changes of Myopic Children Among Patients of a Korean Optometry Clinic (국내 한 안경원을 방문한 근시 어린이의 3년간 근시도 변화의 추적 연구)

  • Purpose: To investigate amount of myopic progression with increase of age for children myopes among patients of a Korean optometry clinic. Methods: It has followed up 99 children subjects (male 55, female 44) who had no ocular disease and have visited a Korean optometric clinic for mean $33{\pm}8$ months (13 to 54 months) since June of 2001. Mean age of subjects at first visit was $118{\pm}23$months. Non-cycloplegic refractive error were measured 6 times using Canon RK-3(Japan) every mean 6 months. Results: For all subjects mean of refractive errors increased -0.78 D per year from $-2.02{\pm}1.05D$ at first visit to $-4.18{\pm}1.30D$ at final visit with longitudinal study, but -0.19 D per year with cross-section study, which showed a big difference between two methods. Mean of astigmatic refractive error increased -0.15 D per year. As progression of refractive error according to ages at first visit, refractive errors increased -1.04 D per year for 6 years old, -0.9 D for 7 years old, -0.89 D for 8 years old, -0.89 D for 9 years old, -0.74 D for 10 years old, -0.74 D for 11 years old and -0.72 D for 12 years old. And it showed a tendency that the younger age was the higher progression of myopia. However it was not significantly different between each groups. Conclusions: Follow-up results for myopic children among patients of a Korean optometry clinic showed increase of -0.78 D for myopic refractive error and -0.14 D for astigmatic refractive error per year.

Research of Difference between the Refractive Powers by Autorefractometer and the Prescription using Phoropter (자동 굴절력계의 굴절력값과 포롭터를 이용한 처방값의 차이에 관한 연구)

  • Purpose: This study is research of the conditions which causes difference between the refractive power of the measurement of autorefractometer and the prescription using phoropter. Methods: Autorefractometer (SR-7000) and phoroptor (AV-9000) were used to measure 60 eyes of 30 participants who had no eye diseases and wore the corrective lens due to Ametropia. To prevent the dependence of the prescription value of the refractive power on the testers, two testers measured the refractive power of the eyes of the participants at the same measuring conditions. Results: Statistically, the prescribed values of the refractive power by two testers were not significantly different. Most of the prescribed values of the refractive power were smaller than the refractive power by autorefractometer In case of myopic eyes, the difference between refractive powers by the measurement of autorefractometer and the prescription using phoropter showed the trend of increase as the spherical refractive power became larger. The result was analyzed by the range of the different cylindrical refractive power for the myopic astigmatic eyes. In this case, the difference between refractive powers showed the trend of decrease as the cylindrical refractive power became larger. Conclusions: No difference between the prescribed value by two testers was observed. In case of myopic or myopic astigmatic eyes, the difference between refractive powers by autorefractometer and the prescription were measured to be approximately proportional to the refractive powers of ametropic eyes. As the this difference become larger for the participant who needs the lens of larger refractive power, additional caution is needed in the prescription of the refractive power of the corrective lens.

Relationship between the Deposition of Tear Constituents and the Adherence of Candida albicans according to Soft Contact Lens Materials and Pigmentation (소프트콘택트렌즈 재질과 착색에 따른 눈물성분 침착과 칸디다균 흡착의 상관관계)

  • Purpose: The aim of this study was to figure out how the characteristics of soft contact lens materials and pigmentation affect the adherence of C. albicans on soft contact lenses pre-deposited with tear constituents. Methods: The adherent number of C. albicans on clear soft contact lenses (hereinafter clear lenses) and circle soft contact lenses (hereinafter circle lenses) made of etafilcon A, hilaiflcon B and nelfilcon A, respectively, was measured before and after the deposition of artificial tear. Also, bacteria adherence on lenses were observed by a scanning electron microscope. Results: Adherence of C. albicans was significantly different according to lens materials. The amount of adsorption was not different between clear lenses and circle lenses made of etafilcon A however, the number of bacteria absorption was bigger in hilafilcon B and nelfilcon A lenses. More absorption of C. albicans was found in the non-pigmented central area compared the pigmented area, and non-pigmented peripheral area has more bacterial absorption than non-pigmented central area. The number of C. albicans decreased in the case that tear protein was pre-deposited. The maintenance of antibacterial activity against C. albicans was different according to lens materials thus, etafilcon A has the longest period of its maintenance. Conclusions: It was revealed that the number of C. albicans was different according to the characteristics of lens materials, pigmentation or non-pigmentation, the pigmented area of soft contact lenses. Thus, it is suggested that the management method should be different according to the adsorption characteristics of C. albicans.

A Correlation between Axis-Rotation and Corneal Astigmatism in Toric Soft Contact Lens Fitting (토릭소프트렌즈 피팅 시 축 회전과 각막난시와의 상관관계)

  • Purpose: The present has analyzed the correlation between the direction of lens and the amount of rotation upon soft toric contact lens fitting after classifying the corneal astigmatism. Methods: Soft toric contact lens was fitted on 114 with-the-rule astigmatic eyes with total astigmatism of at least -0.75 D in their 20s and 30s according to the fitting guideline of the manufacturer and the correlation between the astigmatic degree and the rotational direction/amount of rotation was analyzed by when keeping the eyes on the front and by changing the direction of gaze. As for re-orientation movement. The speed of lens re-orientation and total amount of lens rotation was compared and analyzed by corneal astigmatism after mis-location of lens of $45^{\circ}$ to temporal and nasal direction, respectively. Results: The positive correlations were shown between corneal astigmatism and the direction of lens rotation and between corneal astigmatism and the amount of lens rotation. Meanwhile, the amount of lens rotation was different by the direction of gaze however, there was no correlation with corneal astigmatism. The speed of lens re-orientation was fastest in the group of high astigmatic degree when the lens was mis-located to both temporal and nasal directions. Conclusions: For optimal axis stabilization of toric soft lens, it is proposed that the adjustment of fitting guideline considering corneal astigmatism is necessary since the current fitting guideline is only based on total astigmatism.

Comparisons of Adherence Level of Micro-organisms According to Contact Lens Materials and Protein Deposition and Disinfection Efficacy of Multipurpose Solution (콘택트렌즈 재질 및 침착 단백질에 따른 균 흡착 정도와 다목적용액의 살균력 비교)

  • Purpose: The present study was aimed to compare the difference in adherence level of microorganisms according to contact lens materials and protein deposition and to evaluate disinfection efficacy of multipurpose solution. Methods: The evaluations of micro-organisms' adherence and disinfection efficacy of multi-purpose solution were conducted by employing the Part 2. Regimen Procedure for Disinfecting Regiments in the Disinfection Efficacy Testing under the "FDA Evaluation Criteria & Method". Results: Pseudomonas aeruginosa, Serratia marcescens, Candida albicans except Staphylococcus aureus adhered more on etafilcon A lens and disinfection efficacy of total 4 products investigated was almost perfect except Candida albicans. The 3 micro-organisms except Serratia marcescens adhered more to albumin-predeposited lens. Disinfection efficacy of multi-purpose solution was higher against the micro-organisms adhered to albumin-deposited lens than against the micro-organisms adhered to the lysozyme-deposited lens. Furthermore, disinfection efficacy of multi-purpose solution was different according to types of micro-organisms. Conclusions: It was revealed that the type of micro-organisms, the lens materials and type of absorbed tear protein affected the amount of adhered micro-organisms to contact lens and that adhesion of tear protein could induce the change of disinfection efficacy of multi-purpose solution. It suggest that the hygienic condition of contact lens can vary by these factors influencing on disinfection efficacy and the occurrence of adverse effect can be affected.

Change in Axial Rotation of Toric Soft Contact Lens according to Tear Volume (눈물양에 따른 토릭 소프트콘택트렌즈의 축 회전양 변화)

  • Purpose: The present study was aimed to investigate the effect of tear volume on a change of axial rotation according to wearing time of toric soft contact lens and gaze directions. Method: Toric soft contact lenses with double thin zone design applied on 62 eyes. Then, changes in non invasive tear film break-up time and the rotational direction/amount of lens when changing gaze direction were respectively measured after 15 minutes and 6 hours of lens wear. Results: Lens rotation to temporal direction was more found when changing gaze direction after lens wear. However, its rotation was varied according to wearing time and the subjects' tear volume. Furthermore, the frequency of lens rotation to temporal direction was higher in dry eyes compared with normal eyes at nearly all gaze directions after 15 minutes and 6 hour of lens wear. The rotational amount of lens was generally greater in dry eyes after 15 minutes of lens wear. However, its difference between normal eyes and dry eyes was not great after 6 hours of lens wear. Conclusion: The present study revealed that axial rotation of toric soft contact lens was varied according to the wearer's tear volume and lens rotational patterns at the initial, and extending periods of lens wear were different. The change in rotational pattern of toric soft contact lens from these results means the possibility of visual change after extending lens wear, and the identification of its correlation with tear volume suggests the necessity of considering factors for choosing appropriate toric soft contact lens.

Correlations between Axial Rotation of Toric Soft Contact Lenses and Corneal Eccentricity according to the Wearing Time and Gaze Directions (착용시간 및 응시방향에 따른 토릭소프트콘택트렌즈의 축 회전과 각막이심률과의 상관관계)

  • Purpose: The present study was aimed to investigate the effect of corneal eccentricity on the axial rotation when wearing toric soft contact lenses were worn for certain time and changing the gaze directions. Methods: Toric soft contact lenses with double thin zone design applied on 85 of with-the-rule astigmatic eyes. Then, rotational direction and amount of contact lenses were measured after 15 minutes and 6 hours of lens wear. The difference was further compared and analyzed according to corneal eccentricity. Results: The rotation of toric lens showed a tendency to rotate to temporal direction in all gaze directions except temporal-upper direction in all groups of corneal eccentricity. The amount of lens rotation in the frontal gaze direction exhibited a negative correlation since the amount was decreased with increasing corneal eccentricity after both 15 minutes and 6 hours of lens wearing. In many cases, the cornea with small eccentricity also showed the lens rotation larger than $10^{\circ}$. The difference in rotational amount after 15 minutes of toric lens wear was small according to the corneal eccentricity however, the change of rotational amount of contact lens according to corneal eccentricity was shown after 6 hours of lens wear. Conclusions: The present study revealed that the amount of axial rotation was largely varied according to the wearer's corneal eccentricity when wearing toric lens and the rotational amount after certain time of lens was also affected by corneal eccentricity. Thus, it is suggested that the selection of toric soft contact lenses based on corneal eccentricity is necessary.

A Relationship between Corneal Eccentricity and Stable Centration of RGP Lens on Cornea (각막 이심률과 RGP 렌즈의 중심안정위치와의 상관관계)

  • Purpose: The present study was performed to investigate a relationship between the stable centrations of spherical RGP lens and aspherical RGP lens on cornea and corneal eccentricity. Methods: Two RGP lenses with different designs were fitted in alignment, steep or flat on total 84 eyes having corneal eccentricity of 0.28~0.78. The stable centration of lenses on cornea was analyzed by taking photographs with a high-speed digital camera. Results: The stable centrations of spherical and aspherical RGP lenses in horizontal direction were decentrated to temporal side. More centration to median side was shown when corneal eccentricity was larger. The difference between the stable centrations of spherical and aspheric RGP lenses according to corneal eccentricity was bigger when the fitting state was flatter. The difference in the stable centrations of aspherical RGP lens was smaller than that of spherical RGP lens regardless of fitting status. The stable centrations of spherical and aspherical RGP lenses in vertical direction were located below corneal apex regardless of fitting status however, there was no significant difference analyzed by the variation of corneal eccentricity. However, there were many cases that RGP lenses were in upper eyelid with increasing corneal eccentricity. Conclusions: The consideration of corneal eccentricity is required for RGP lens fitting and manufacturing aspherical RGP lens since the stable centration of spherical RGP lens as well as aspherical RPG lens' centration was changed depending on corneal eccentricity.
