Health-related quality of life in elderly patients with diabetes mellitus according to age: based on Korea National Health and Nutrition Examination Survey (노인 당뇨병 인지자의 연령에 따른 삶의 질 수준 분석: 국민건강영양조사를 이용하여)

  • Purpose: Although the importance of life satisfaction in patients with chronic diseases has been emphasized, limited studies have been performed on the health-related quality of life (HRQoL) among elderly patients with chronic diseases. This study examined the HRQoL among Korean patients with diabetes aged 65 years or older. Methods: This study used data from the Korea National Health and Nutrition Examination Survey 2008-2016. The analysis included data of 2,353 elderly patients with diabetes aged 65 years or older who were aware that they had type 2 diabetes. Each participant was categorized into one of three age groups (65-69 years, 70-74 years, and ≥ 75 years). The EuroQol-5 Dimensions (EQ-5D) scale was computed to estimate the HRQoL. Multivariate linear regression analysis was used to compare the demographic characteristics, lifestyle characteristics, nutrient intake, and EQ-5D scores between the age groups. Results: The intake levels of most nutrients were similar in the three age groups, but the total energy intake levels were significantly lower, and the sodium intake levels were significantly higher in the older age groups (p < 0.001). In addition, all items of the EQ-5D index, except for anxiety/depression, tended to decrease with age. In particular, the oldest age group (≥ 75 years) showed significantly lower levels on most items and the EQ-5D index than those in the other age groups (p < 0.001). Conclusion: Elderly patients with type 2 diabetes may have low levels of life satisfaction, which worsens with increasing age. Therefore, it is important to develop and manage personalized guidelines and medical systems so that elderly patients have a higher quality of life for the remainder of their lives. The results of this study are expected to be used as basic data for the systematic management of diabetes in elderly Korean patients.

Correction of Secondary Cleft Lip Deformities by Scar Excision and Abbe Flap Coverage: Photogrammetric Analysis (이차구순열변형에서 상순반흔제거술 후 Abbe 피판을 이용한 재건: 사진계측학적 연구)

  • Purpose: The Abbe flap procedure has been used to correct disharmony of the upper and lower lips as well as for making a philtrum for patients with secondary cleft lip deformities. But the Abbe flap procedure adds two scars in addition to the prior operative scar on the upper lip. This study was conducted to determine the treatment outcomes of esthetic subunit excision of the scar on the philtrum and Abbe flap coverage for correction of cleft lip deformities with photogrammetric analysis. Methods: This study investigated a total of 11 patients with cleft lip deformities who underwent scar excision with Abbe flap coverage, and the patients were followed up for at least 6 months. Under general anesthesia, a mushroom-shaped Abbe flap was drawn on the lower lip with a width of 8 mm and a height 1~2 mm longer than that of the philtral midline. The epidermis and dermis of the scar on the upper lip were excised. In the cases with alar base depression, the orbicularis oris muscle was split vertically and transposed to the alar base. The Abbe flap was harvested as a pedicled flap containing a small amount of muscle and this was rotated 180-degree to be inserted into the upper lip. Mucosa, muscle, subcutaneous tissue and skin were closed in layers. The flap was divided at the 7~14 postoperative day. The postoperative outcomes were evaluated by using photogrammetric analysis. Three indices were measured from the standard clinical photographs taken before and after the surgery. For anthroposcopic assessment, observers described the postoperative outcomes using an ordinary scale method. Results: The postoperative values obtained in the photogrammetric analysis showed improvement as compared with the preoperative ones. Improved anthroposcopic outcomes were also noted. Conclusion: Scar excision and Abbe flap coverage were proven to be effective in improving protrusion and the height of the upper lip, the scar of the upper lip and the symmetry of Cupid's bow and the philtral column, as well as formation of the philtral dimple.

A Trial for Development of Health Profile (KHP 1.0) to Measure the Self-Perceived Health Status of Korean (한국인의 자가평가 건강수준 측정도구(KHP 1.0) 개발)

  • Objectives : The 1990s has seen advances in the conceptualization of self-perceived health status which has important roles for individual health and the quality of life. Many types of standardized questionnaires have been developed with the current wide use of SF-36, NHP, andEuroQol. However, the outcomes of these tools may be different with regard to regional, cultural and emotional backgrounds. The purpose of this study was to trial the development of a Korean Health Profile (KHP 1.0) to measure the self-perceived health stati of Koreans. Methods : The KHP 1.0 was designed on the basis of the Medical Outcome Study Form 36 (SF-36), the Nottingham Health Profile (NHP), and the EuroQOL. It was composed of 9 scales; physical functioning, role limitation-physical, pain, general health, energy, social isolation, sleep, role limitation-emotional, and e-motional health. Self-reported chronic disease conditions, and the Zung's Self-Rating Depression Scale (SDS), were also checked for the evaluation of clinical validity. This study was conducted, from December 2000 to January 2001, on 800 middle-aged parents, with four high school students, with 100 retest sets being conducted two weeks later. From the 800 subjects there were 588 complete responses (effective response 73.5%). The reliability of the test-retest results, and the factor analysis on the validity of the KHP 1.0 components, were evaluated using the SPSS (ver 10.0) software. Results : The reliability of the KHP 1.0 was good with Cronbach's alpha (>0.6), test-retest correlation coefficients (>0.5), but with no significant differences from the paired t-test. From the psychometric validity tests, the 9 scales of the KHP 1.0 were divided into two components; physical and mental, and trimmed to the established model with 55% of the total variance, with the exception of role limitation-emotional. The clinical validity on the basis of the comparison for the four characteristic groups; healthy, physical conditions only, mental conditions only, and physical and mental conditions were also good. Conclusions : The KHP 1.0 appears to be a valid measurement tool of self-perceived health stati of Koreans, although there are limitations, i.e. sample size was too small, a limited number of middle-aged subjects, and it was based on unconfirmed diagnoses, etc. Therefore, further study is required to standardize the assessment.

Symptom severity, Functional Impairment, and Personality Profiles between Partial and Full Post-traumatic Stress Disorder Patients among the Adolescent Survivors from the Incheon Fire Disaster (인천 화재사건 청소년 생존자를 대상으로 한 PTSD 하위유형간 증상, 기능 및 기질성격 특성 비교연구)

  • Objectives : The aim of this study was to investigate Post Traumatic Stress Disorder (PTSD)-related symptom severity, level of functional impairment and personality profiles between full-blown PTSD, partial PTSD and non-PTSD groups among 59 adolescent survivals from the Incheon fire disaster. Method : Using Short Screening Scale for DSM-IV PTSD, victims of the disaster were assigned to a full-blown PTSD group (n=18), a partial PTSD (n=22), or a non-PTSD group (n=19). Assessments included the Beck Depression Inventory (BDI), the State-Trait Anxiety Inventory (STAI), the Impact of Event Scales (IES), the McBride's Degree of General Labor Loss, and the Korean Version of Temperament and Character Inventory (K-TCI). Results : Significantly severe PTSD symptoms (F=4.832, df=2, p<.05) and functional impairment (F=12.144, df=2, p<.01) were demonstrated by PTSD groups as compared to the non-PTSD group. Interestingly, full and partial PTSD did not differ in these comparisons. Similarly, the subtypes of PTSD did not differ with respect to personality profiles using the K-TCI ; however, personality profiles were sharply differentiated between the PTSD and non-PTSD group. Conclusion : Although high subject homogeneity and small sample size may limit the results of this study, the present results highlight the possibility of the underestimation as well as the insufficient, treatment and compensation of partial vs full PTSD.

The Reliability and Validity of the Korean version of Working Alliance Inventory-Short Revised (WAI-SR-K) (한글판 치료 동맹 척도 단축개정판(WAI-SR-K)의 표준화연구 : 신뢰도와 타당도)

  • Objective : The Working Alliance Inventory-Short Revised (WAI-SR) has been reported as a reliable, valid and widely used tool for measuring therapeutic alliance. The aim of this study was to assess the reliability and validity of the Korean version of WAI-SR (WAI-SR-K). Methods : A sample of 196 outpatients completed the WAI-SR-K and the data were analyzed for internal consistency and factor structure. Additionally 51 outpatients re-filled WAI-SR-K after two weeks for the test-retest reliability. To test for the validity, questionnaires of treatment satisfaction, and scales including the Korean version of Drug attitude inventory-10 (K-DAI-10) and Hospital anxiety and depression scale (KHADS) were administered. Results : Internal consistency of WAI-SR-K was good (Cronbach's alpha=0.928) and an exploratory factor analysis revealed a two-factor structure. The WAI-SR-K showed a reasonable test-retest reliability (r= 0.810, p<0.01). The total score of WAI-SR-K was positively correlated with K-DAI-10 (r=0.276, p<0.01), and negatively correlated with scores of K-HADS-D (r=-0.217, p<0.01) and K-HADS-A (r=-0.159, p<0.05). Conclusion : The Korean version of Working Alliance Inventory-Short Revised proved to be a reliable and valid instrument for measuring therapeutic alliance in Korean patients with depressive symptoms.

The Physical and Mental Symptoms of Normal Subjects with Mild Life Stress (생활 스트레스가 경미한 일반 성인에서의 신체적 및 정신적 증상 - 종합병원 건강증진센터 내원자들을 대상으로 -)

  • Objectives : Aims of the study were to investigate the physical and mental symptom profiles related with perceived life stress in normal populations. Methods: The study subjects were 186 subjects who visited the Health Promotion Center at National Health Insurance Corporation Ilsan Hospital between July and December, 2000. We measured Life Stress Assessment Scale (LSAS), SCL-90-R (Symptom Check List-90 Revised), and Physical Health Check Questionnaires to evaluate perceived life stress for one year, and physical and mental symptoms. The physical markers related with stress were also measured. According to LSAS scores, we selected two groups from all subjects : Group I (LSAS score with lower 25 percentile) and Group II (LSAS score with higher 75 percentile). Each number of subjects was 46 in Group I and 47 in Group II. We analyzed stasistical significances of study variables between Group I and II. Results : Group II subjects showed higher scores in all subscales of SCL-90-R compared to Group I subjects. Comparing to Group I, Group II subjects complained more physiological symptoms related with anxiety and depression. But there were no statistical differences in the physical markers related stress between two groups. Conclusion: We found that past year-life stress caused various mental and physical symptoms, which didn't develop any physical illness related life stress in normal populations yet.

Post-operative Analgesic Effect of Epidural Midazolam Administered with Morphine (경막외강에 Morphine과 동반 투여한 Midazolam의 진통효과)

  • Background: Opioids and local anesthetics have been administered epidurally for the purpose of the postoperative analgesia. However opioids have a serious risk of respiratory depression and local anesthetics have the risks of hypotension, sensory block, or motor one. In recent years, reports of spinal administration of midazolam for acute postoperative pain control have appeared in the literature. This study was performed to observe the effect of epidural midazolam in patient-controlled analgesia (PCA) device. Methods: Forty-five patients scheduled for the elective total hysterectomy were randomly selected; epidurally take morphine only (group I, n=15), morphine plus 0.1% bupivacaine (group II, n=15), or morphine plus midazolam (group III, n=15). The visual analogue scale (VAS) at rest and with movement, the sedation score, the degree of the satisfaction, the total amounts of a morphine usage, and the incidence of the side effects were observed. Rusults: The VAS at rest of group II and III were decreased significantly than that of group I. The VAS with movement of group III was significantly decreased than that of group I and II. The sadation score and the cumulative dose of a morphine were statistically insignificant within groups. Conclusion: Epidural morphine plus midazolam was proven to be clinically effective in the post-operative pain control especially for the pain with movement, compared with epidural morphine only and morphine plus 0.1% bupivacaine.

MASS MORTALITYS OF OYSTER DUE TO RED TIDE IN JINHAE BAY IN 1978 (1978년 진해만 적조와 양식굴의 대량폐사)

  • In early August 1978, a huge red tide occurred off Chilcheon Is, , at the mouth of Jinhae Bay. It expanded rapidly into the central part and then soon covered all the bay except the innermost part of the bay. After staying for a week it disappeared, and thereafter several small scale red tides partially occurred and disappeared untill the end of August. It differed from the red tides hitherto recorded from this bay in respect of the dominant plankter, the geographical extent and the severe damaging effect to local oyster resources. During the red tide period, nevertheless, no oyster damage was found. In September, however, oyster mortailty was found from all rafts and longlines in the bay. Except the inner most part of the bay the oyster farms were completely destroyed. Of the oyster Production in the winter 1978, $96\%$ of an estimated yield of 5,879 tons (shucked meat) was lost and it was estimated to be 2,275 million won, equivalent to $US\$$ 4.55 million. The dominant species was a dinoflagellate, Ceratium fusus and it constituted about $45\%$ of the total phytoplankton. Cell count showed $7.0\times10^4\;cells/\iota$ and chlorophyll-a, $50mg/m^3$ during red tide peak. No oxygen was found ill the bottom waters in September. Sulphides in bottom waters and in the superficial mud increased to 15 and 8-fold respectively in September compared with July ana August. Precipitation from January to May of 1978 was about a third in comparison with the past ten years average but rainfall ill June was two and half-fold more than normal year, and thereafter drought persisited till September Air and water temperatures were also higher, and sunny days continued for a long time without strong winds. Therefore, water was calm for a long time after the red tide extinguished. The result indicated that the occurrence of the Ceratium red tide occurred in that year which was characterized by the combination of the formation of almost anoxic bottom water before the red tide occurrence, high air temperature and the calmness after a great quantity of rainfall in June. The mass mortality of oysters was presumed not to be directly related with the red tide but with the depression of dissolved oxygen in the environmental waters at the bottom due to settling of the red tide organisms.

Comparison of QEEG between EEG asymmetry and Coherehnce with elderly people according to smart_phone game Addiction Tendency (노인의 스마트 폰 게임 중독 경향에 따른 뇌파 비대칭(asymmetry)와 연결성(Coherehnce)의 정량화뇌파(QEEG) 비교 분석)

  • The purpose of this study was to analyze the EEG according to the elderly's tendency to be addicted to smartphone games. We compared the effects of smartphone addiction on mental health such as brain waves, sleep problems and depression through comparative analysis of asymmetry and connectivity in quantitative EEG results. The study participants were two elderly people who were addicted to smartphone game and one elderly person who did not use smartphone (Ed- to confirm: only 3 participants?!). The participant's addiction tendency of smartphone was measured by using the smartphone addiction scale and EEG (QEEG) was used for EEG analysis. The results are as follows. First, the brain waves of elderly people and smartphone non-user elderly who showed symptoms of immersion and smartphone game showed a difference in asymmetry in both opening and closing anisles. Second, there were significant differences in the openness and the anxiety of the elderly who were immersed in the mobile phone and the elderly who did not use the smartphone. Through this, it is also meaningful to explore the relationship between senile cognitive impairment and smartphone use by exploring the effect of smartphone game use on brain cognitive function through comparison of EEG analysis.

Prevalence and Associated Risk Factors of Psychological Distress in Patients with Gastric Cancer (위암 환자의 디스트레스 유병률과 관련 위험 인자)

  • Objectives : Though gastric cancer is one of the most common cancer in Korea, there have been few studies to explore psychological distress in gastric cancer. The purpose of this study was to investigate the prevalence and associated risk factors of psychological distress among patients with gastric cancer. Methods : With consecutive sampling, a total of 274 patients with gastric cancer who admitted to a cancer center in a general hospital were recruited and assessed on psychological distress using the Hospital Anxiety and Depression Scale(HADS). Sociodemographic and cancer-related clinical variables were also evaluated. Results : One hundred fifty-three(55.8%) patients with gastric cancer showed psychological distress. Logistic regression models revealed that having alcohol drinking experience[odds ratio(OR)=2.10, p=0,034] and low performance status(OR=2.40 p=0.002) were significantly associated with psychological distress in patients with gastric cancer. Conclusions : These findings indicate that approximately half of patients with gastric cancer suffered from psychological distress and having alcohol drinking experience and low performance status would be associated risk factors, suggesting the need for distress screening and psychosocial supportive care in patients with gastric cancer.
