암환자와 배우자의 부부적응에 대한 부부의사소통의 영향 - 자기효과와 상대방효과를 중심으로 - (The Effect of Communication on Marital Adjustment of Cancer Patients and their Spouses - Focusing on the Actor Effect and Partner Effect -)

  • 암환자에게 부부적응은 심리적인 안녕 뿐만 아니라 치료경과 및 생존율에 영향을 미치는 것으로 보고되고 있어 이들의 부부적응을 도모하기 위한 사회복지 노력과 관심이 절실히 필요하다 하겠다. 이에 본 연구는 암환자와 배우자의 커플자료를 수집하여 이들의 부부적응에 대해 조사하였으며, 기초자료가 미비한 실정에서 부부관계의 지표로 부부적응에의 필수고려 요인으로 지목되고 있는 부부의사소통의 영향을 조사하였다. 또한 암환자와 배우자의 상호의존성을 고려하여 부부의사소통이 부부적응에 미치는 영향을 자기 자신에게 미치는 영향(자기효과), 상대방에게 미치는 영향(상대방효과)을 고찰하였다. 이를 통해 부부적응에 긍정적 부부의사소통의 중요성을 제시하였으며, 부정적 의사소통 유형인 요구-철회의사소통에서 환자의 배우자에 대한 상대방효과가 유의한 것으로 나타나 환자에 대한 의사소통 교육에의 필요성을 제시하였다. 이러한 연구결과를 바탕으로 실천적 함의 및 후속연구에 대한 제언을 제시하였다.

Reactivation of Silenced WT1 Transgene by Hypomethylating Agents - Implications for in vitro Modeling of Chemoimmunotherapy

  • Background: A cell line with transfected Wilms' tumor protein 1 (WT1) is has been used for the preclinical evaluation of novel treatment strategies of WT1 immunotherapy for leukemia due to the lack of appropriate murine leukemia cell line with endogenous WT1. However, silencing of the transgene occurs. Regarding the effects of hypomethylating agents (HMAs) on reactivation of silenced genes, HMAs are considered to be immune enhancers. Methods: We treated murine WT1- transfected C1498 (mWT1-C1498) with increasing doses of decitabine (DAC) and azacitidine (AZA) to analyze their effects on transgene reactivation. Results: DAC and AZA decreased the number of viable cells in a dose- or time-dependent manner. Quantification of WT1 mRNA level was analyzed by real-time polymerase chain reaction after mWT1-C1498 treated with increasing dose of HMA. DAC treatment for 48 h induced 1.4-, 14.6-, and 15.5-fold increment of WT1 mRNA level, compared to untreated sample, at 0.1, 1, and $10{\mu}M$, respectively. Further increment of WT1 expression in the presence of 1 and $10{\mu}M$ DAC was evident at 72 h. AZA treatment also induced up-regulation of mRNA, but not to the same degree as with DAC treatment. The correlation between the incremental increases in WT1 mRNA by DAC was confirmed by Western blot and concomitant down-regulation of WT1 promoter methylation was revealed. Conclusion: The in vitro data show that HMA can induce reactivation of WT1 transgene and that DAC is more effective, at least in mWT1-C1498 cells, which suggests that the combination of DAC and mWT1-C1498 can be used for the development of the experimental model of HMA-combined WT1 immunotherapy targeting leukemia.

치아 외과적 발거 전 혈액검사의 필요성과 이의 정책적 제시 (Needs of Preoperative Blood Sample Test in Surgical Extraction: Suggestion of New Policy)

  • Purpose: The third molar extraction is one of the mostly performed procedures in the department of oral and maxillofacial surgery. In most of dental clinic or hospital, the third molar extraction used to be frequently performed in an office-based surgery, and most patients did not have specific medical history with young ages. Medical history taking are dependent on the only way by asking to the patients about their individual conditions. Therefore, as the specialists of the oral and maxillofacial surgery in the field of dentistry, we suggest a new policy that the preoperative lab must be performed routinely before extraction of the third molar. Methods: This study is based on 1,096 patients who have been managed with third molar extractions, from March 2008 to September 2011 by a single surgeon. The preoperative lab, including complete blood count, coagulation panel, chemistry and serology, was performed before any surgical procedures. The results were informed to the patients regardless of their abnormalities, and any abnormalities related to the surgical procedures, such as platelet count and coagulation factors, were checked and corrected safely. Results: Through the preoperative blood test, systemic diseases that the patients had not recognized before, such as anemia, leukopenia, fatty liver and chronic renal disease, were identified. Patients with acute or chronic leukemia, Hepatitis B, and HIV positive, were also detected as a small number. Also, the possibilities of the cross-infection between dentists and patients or between patients and patients, and any other emergency situations can be prevented; as well as the public health condition can be improved, too. The patients were satisfied with low cost preventive blood test and high quality of medical services. Conclusion: Therefore, routine medical lab testing, including history taking are needed before an office-based minor surgery, such as third molar extractions, and these results were suggested as a new policy in the field of dentistry.

불안 및 스트레스에 대한 간호중재 효과의 메타 분석 (Effects of Nursing Interventions on Anxiety and / or Stress : A Meta-Analysis)

  • The purpose of this study was to identify the commonalities of various nursing interventions and effective intervention methods in each intervention through analysis and synthesis of many accumulated research papers. The study analyzed the effects of various nursing interventions on anxiety and /or stress. For this purpose, 64 experimental studies with randomized or nonequivalent control group pre-test-posttest design were selected from journals of medical and nursing schools, the Korean Nurse, the Korean Nurses' Academic Society Journal, the Cetnral Journal of Medicine, the New Medical Journal, the Nurse Monthly, and from theses and dissertations conducted from 1970 to 1991. The selected studies nor classified according to three characteristics : 1) the research samuel, 2) types and methods of nursing interventions, and 3) statistical tests. The following analysis was done : 1) Confirmation of the accuracy of data drawn from each study by paired review, 2) Estimation of the homogenity of pre-test scores of the dependent variable between control and experimental groups warranted the effect size of post-scores, 3) If the homogenity of pre-test scores did not warrant, the change scores from pre-test to post-test were used to estimate the effect size, 4) Use of the effect size of each study among homogenious studies was tested for each intervention method, such as relaxation, information, and touch and /or support. Finally, for the studies not showing homogenity, an ANOVA test was used to identify patterns for each intervention. Some findings are summarized as follow : The effect sizes for relaxation and information were greater than those for touch and /or supportive technique. Studies using random assignment had greater effect sizes than nonrandomized sample studies using the same intervention. For healthy people, group education was more effective than individual teaching. However, for patients, relaxation and touch and /or supportive techniques given on individual basis were more effective than when given in a group situation. Measuring anxiety and stress by biological indicators was less effective than by self-report. Budzynski's relaxation method was the most effective. The more frequently the techniques applied, the larger the effect size. On the bases of these findings, the following recommendations were made : 1. A combination of information, relaxtion, and touch-supportive techniques should be used for greater effect in reducing the level of anxiety and /or stress. 2. Information is the first choice of intervention to reduce the level of anxiety and /or stress of healthy People : other interventions may be added depending on the conditions of the subjects.

성장기 아동과 성인에서의 II급 1류 부정교합치료양상에 관한 두부방사선 계측학적 비교연구 (A COMPARATIVE ANALYSIS OF CLASS II DIVISION 1 TREATMENTS : ADOLESCENTS CONTRASTED WITH ADULTS)

  • 본 연구는 II급 1류 부정교합의 치료에 있어 치료전의 환자연령에 따른 부정교합치료의 양적 차이를 알아보고, Johnston analysis를 통해 협측 치열군의 교점이 어떤 방법으로 이루어지는지, 즉, 두개저에 대한 또한 상호간에 대한 상, 하악 각각의 성장양과 기저골에 대한 구치의 이동양을 측정하여 구치부의 교정이 어떻게 이루어지는지를 알아보고자 했으며 서울대학교병윈 치과진료부 교정과에 내원하여 II급 1류 부정교합으로 진단되어 상, 하악 제1소구치를 발치하여 치료를 받은 여자환자 52명중 치료전의 연령을 기준으로 성장기 아동군과 성인군으로 대별하여 치료전,후 두부방사선사진을 계측, 비교분석하여 다음과 같은 결론을 얻었다. 1. 치료전,후의 상악골의 전후방적인 위치변화에 대한 비교를 통해서 성장기 아동군과 성인군간의 유의한 차는 없었다. 그러나 하악골의 전후방적 위치변화에 있어서는 두 군간에 유의한 차이를 보였다. (P<0.05) 2. 치료 전,후 수직고경의 변화에 있어, 성장기 아동군에서는 전안면고경, 후안면 고경, 하안면고경 등이 성장과 치료의 효과로 유의성있게 증가하였으며(P<0.001), 성인군에서는 유의성있는 변화가 없었다. 3. 치료전,후 교합평면의 변화에 있어, 성장기 아동군에서는 교합평면의 경사도의 유의성있는 변화가 발견되지 않았으나, 성인군에서는 교합평면의 경사도가 유의성있게 증가하였다. (P<0.05) 4. Johnston analysis를 이용한 분석으로, total molar correction의 양에 있어서 성장기 아동군과 성인군에서 유의한 차이를 보였다. (P<0.05) 6. Johnston analysis를 이용한 분석에 의해 성장기 아동군에서는 total molar correction의 $63\%$가 하악골성장에 의해, $37\%$가 치아이동에 의해 이루어짐을 관찰했다. 성인군에서는 total molar correction의 $99\%$가 치아이동에 의해 이루어졌다.

Effect of Partial Replacement of Wheat Flour with High Quality Cassava Flour on the Chemical Composition, Antioxidant Activity, Sensory Quality, and Microbial Quality of Bread

  • In the current study, wheat flour was mixed with high quality cassava flour (HQCF) in several ratios: 90:10, 80:20, 70:30, and 60:40, and used to prepare 10%, 20%, 30%, and 40% National Root Crops Research Institute (NRCRI) cassava bread, respectively. 100% wheat bread was prepared as a control (100% wheat bread). Five bread samples were prepared per group. Antioxidant assays [i.e., 2,2-diphenyl-1-picrylhydrazyl radical (DPPH) scavenging assay, reducing power assay] revealed that the bread samples had considerable antioxidant capacities. Substitution of wheat flour with HQCF at various concentrations resulted in dose dependent decreases in the mineral and protein contents of the resulting bread samples. The crude fiber content of the bread samples was minimal, while the carbohydrate content of the bread samples ranged from 43.86% to 48.64%. A 20% substitution of wheat flour with HQCF yielded bread samples with a general acceptability that was comparable to that of 100% wheat bread. The mean bacteria counts of the bread samples ranged from $2.0{\times}10^3CFU/mL$ to $1.4{\times}10^4CFU/mL$, while the fungal counts ranged from 0 CFU/mL to $3{\times}10^3CFU/mL$. There was a positive correlation between the DPPH antioxidant activities and the reducing powers of the bread samples ($R^2=0.871$) and a positive correlation between the DPPH antioxidant activities and the flavonoid contents of the bread samples ($R^2=0.487$). The higher microbial load of the NRCRI cassava bread samples indicates that these bread samples may have a shorter shelf life than the 100% wheat bread. The significant positive correlation between total flavonoid content and reducing power ($R^2=0.750$) suggests that the flavonoids present in the lipophilic fractions of the bread samples could be responsible for the reductive capacities of the bread samples.

인간 $\alpha$-fetoprotein에 대한 모노클로날 항체의 제조 및 모노클로날 항체를 이용한 효소면역분석법의 개발 (Production of a Monoclonal Antibody to Human $\alpha$-Fetopotein and Development of Monoclonal Antibody-Based Enzyme-Linked Immunosorbent Assays for Human $\alpha$-Fetoprotein)

  • 본 연구에서는 혈장이나 양수에 있는 $\alpha$-fetoprotein (AFP)을 인식 할 수 있는 모노클로날 항체를 제조하고, 모노클로날 항체를 이용한 효소면역분석법을 개발하고자 하였다. 양수로부터 얻은 AFP를 쥐에 주사한 후 비장을 분리하여 종양세포 (Sp2/O-Ag-14)와 융합하였고, 하이브리도마 기술을 이용하여 모노클로날 항체를 제조하였다. 모노클로날 항체를 클로닝하였으며, 생성된 항체를 MabF22로 명명하였고, IgG1 중사슬과 k 경사슬의 isotype을 나타냈다. 또한 immunoblotting 방법과 ELISA로 특이도를 조사한 결과 모노클로날 항체는 AFP와만 반응하였고, 결합 친화상수는 0.8$\times$$10^{-10}$M이었다. 두 종류의 효소면역분석법 -경쟁적 또는 비경쟁적 분석 -을 이용하여 항체의 효용성을 조사하였으며, 두 방법 모두 AFP와 농도에 비례하여 반응하였다. 따라서 본 연구에서 생산된 모노클로날 항체는 연구목적으로 뿐만 아니라 AFP 농도를 측정하기 위한 면역진단시약의 개발에도 유용할 것으로 생각된다.

성인의 구강건강 상태와 건강관련 삶의 질 관련성: EuroQoL-5 Dimension 구성요소를 중심으로 (Association between Oral Health Status and Health Related Quality of Life (EuroQoL-5 Dimension))

  • 본 연구는 2013년 KNHANE 자료를 이용하여 우리나라 성인의 구강건강 상태와 HRQoL 관련성을 EQ5D 구성요소를 중심으로 평가하고자 하였다. 16~64세 성인 3,252명을 최종 분석대상자로 하여 다음과 같은 결론을 얻을 수 있었다. 치아통증과 운동능력, 통증 및 불편은 통계적으로 유의한 관련성을 나타냈고, 저작 불편과 말하기 불편은 EQ5D 구성요소 5가지 모두 통계적으로 유의한 관련성을 나타냈다. 주관적 구강건강 인식은 자기관리, 일상활동, 통증 및 불편, 불안 및 우울과 통계적으로 유의한 관련성을 나타내어 구강건강 상태와 EQ5D 구성요소의 관련성을 확인하였다. 또한 EQ5D 구성요소에 미치는 영향을 통제변수를 보정 후 로지스틱 회귀분석으로 살펴본 결과 치아통증, 저작 불편, 말하기 불편, 주관적 구강건강 인식이 불안 및 우울에 영향을 미치는 것으로 나타났다. 구강건강 증진은 HRQoL을 향상시킬 수 있는 방안이 되므로, 구강건강의 중요성을 인식하고 예방차원의 교육 및 프로그램이 필요하다고 생각된다.

유치원 아동의 흡연태도 및 흡연의도에 미치는 영향요인 (Factors Affecting Smoking Attitude and Intention among Preschool Children)

  • Parental smoking status and smoking environment do not only affect children's health through exposure to nicotine, but are also related to children's smoking attitude, intention and knowledge of smoking. Since this study was conducted to examine a relationship between family members smoking and smoking attitude, intention and knowledge of their children, little is known about psychosocial factors influencing smoking among Korean preschool children. This study aimed to determine the psychosocial factors (sociodemographic factors, education for smoking cigarette, knowledge about smoke cigarette) influencing smoking attitude and smoking intention, with a purpose of providing directions for future education programs for Korean preschool children. A cross sectional sample of 840 children, ages 4-7 years, living in a rural district were interviewed during August 6 - November 14, 2003. Among them, 834 children whose missing data were less than $10\%$ were included the analysis. The independent variables included sociodemographic characteristics and education for smoking cigarette, while the dependent variables were knowledge about smoking, the children's smoking attitude and their intention of smoking. The data on sociodemographic characteristics were analyzed using descriptive statistics including frequency, percentage, mean, and standard deviation. The correlations between the independent variables and attitude and intention of smoking were analyzed using chi-square test, t-test, and logistic regression. The results of this study were s follows. The factors that were found to be significantly correlated to smoking attitude include mother's smoking $(x^2=4.86,\;p=.03)$, grandmother's smoking $(x^2=5.56,\;p=.02)$, and knowledge of smoking cigarette $(x^2=-5.13 p<.01)$. The factors that were significantly correlated to the intention of smoking include gender $(x^2=.08,\; p=.02)$, education for smoking$(x^2=17.65,\;p<.01)$, mother's smoking $(x^2=19.05,\; p<.01)$, grandmother's smoking $(x^2=17.67,\; p<.01)$, and knowledge$(x^2=-2.17,\;p=.03)$. The children who had non smoking mother were more likely to have negative attitude (OR=2.29). The children who had a higher knowledge about smoking were more likely to have negative attitude (OR=1.58). The findings suggest that a smoking prevention program specific for preschool children is necessary to prevent early onset of smoking. The findings also suggest that the program should involve children's family members who influence their smoking attitude and intention.

개심술을 받은 환자의 체위에 따른 심박출량 및 불편감에 관한 연구 (Effects of changing position on cardiac output & on patient's discomforts after cardiac surgery)

  • Invasive hemodynamic monitoring has become a valuable assessment parameters in critical care nursing in patients undergoing open heart surgery patients. During cardiac surgery, the Swan Ganz catheter is placed in the pulmonary artery. Critical care nurses routinely obtain cardiac output, cardiac index, and pulmonary arterial pressure in these patients. Traditionally, patients are positioned flat and supine for cardiac output measurement. Numerous studies have dealt with the effects of changing position on the hemodynamic variables. However, there are a few studies dealing with patients who undergo cardiac surgery in Korea. Thus, the purpose of this study was to determine the effects of changing position on cardiac output, PAP, CVP, BP, HR and discomfort in patients after cardiac surgery. A sample of 21 adults who had CABG and/or valve replacement with Swan Ganz catheters in place was studied. The data were collected in the cardiac ICU of a university hospital in Seoul during the period from July 28, 1999 to August 30. 1999. In this study, the independent variable is patient position in the supine, 30 degree, and 45 degree angles. Dependent variables are C.O., C.I., CVP, PAP, MAP, HR and patients' perceived discomforts. Subject discomfort was measured subjectively by visual analogue scale. Other hemodynamic data where collected by the thermodilution method and by direct measurement. The data were analyzed by percentile, t-test, ANOVA, Linear regression analysis using SPSS-/WIN program. The results are as follows : 1) Changes in cardiac output were absent in different angle positions, 0, 30. 45 degrees(F=.070, P=.932). Changes in cardiac index were absent in different angle positions, 0. 30, 45 degrees(P>.05). 2) Changes in central venous pressure were absent in different angle positions, 0, 30, 45 degree(P>.05). 3) PAP had no change in different angle 0, 30, 45 degree positions; systolic PAP(P>.05), diastolic PAP(P>.05). 4) Changes in systolic blood pressure were absent in different angle positions, 0, 30, 45 degree(P>.05). 5) Changes in heart rates were absent in different angle positions, 0, 30, 45 degree(P>.05). 6) Patients' perceived discomfort was absent in different angle positions, 0, 30, 45 degree(p<.05). In conclusion, critical care nurses can measure C.O., C.I., PAP, BP, & CVP in cardiac surgery patients at 30 degree or 45 degree positions. This can improve the patients' comfort.
