영유아 및 소아의 단순복부 X-선 사진 (Plain Abdominal Radiography in Infants and Children)

  • Plain X-ray radiographs are the first line of investigation taken in the diagnosis of abdominal pathology and are considered an important diagnostic tool to provide guidelines for further imaging studies and comprehensive therapeutic management. Although most abdominal pathology demonstrates non-specific radiologic findings, the plain abdominal radiography is very useful in specific diseases, including certain gastrointestinal anomalies. This review provides image findings of normal plain abdominal radiography and some common abdominal pathology in infants and children.

The ability of panoramic radiography in assessing maxillary sinus inflammatory diseases

  • Purpose : To evaluate the relative diagnostic accuracy of panoramic radiography and Water's projection in maxillary sinus inflammatory diseases by comparing the radiodensities of the images with those of CT. Materials and Methods : Panoramic radiographs, Waters' projection, and CT images from 55 subjects (11O sinuses) were included in this retrospective study. The radiodensity of each maxillary sinus in panoramic radiography was recorded separately as upper and lower divided horizontally by hard palate. In Waters' projection, the overall sinus radiodensity was recorded. The CT images were considered as gold standard. Results : In panoramic radiography, 83 sinuses had same upper and lower radiodensity and 72 of these were consistent with those of CT, 26 sinuses had different upper and lower radiodensity and 15 of these, upper radiodensity was consistent with CT, the remaining 11, lower radiodensity was consistent with CT. One sinus had upper radiolucency with lower radiopacity and both were consistent with those of CT. Altogether 73 (66.4%) among 110 sinuses in panoramic radiography showed full agreement with CT, 26 (23.6%) showed partial agreement with CT. 9 sinuses had no lower image under the hard palate in panoramic radiography due to the smaller size of sinus. In Waters' projection, the radiodensity of 105 sinuses (95.5%) were consistent with that of CT. Conclusion : The panoramic radiography showed 90.0% of the sinus conditions fully or partially which may appear less accurate than that of Water's view (95.5%) but with more detailed information of the inferior part of sinuses. 용접공진폐증 집단검진을 위한 단순 흉부방사선 촬영과 고해상 흉부전산화 단층촬영의 진단적 의의 (Diagnostic Meaning of High Resolution Computed Tomography Compared with Chest Radiography for Screening of Welder's lung)

  • Pneumoconiosis is one of the major problem in the field of occupational health at Korea. Therefore, the efficient diagnosis of pneumoconiosis is a hot issue on the occupational health program. The author executed this study to estimate the diagnostic value of high resolution computed tomography(HRCT) compared with chest radiography for screening of welder's lung. HRCT was introduced very recently for the diagnosis of pneumoconiosis, however, the diagnostic value for screening of welder's lung - principally nonfibrogenic and reversible - has not been evaluated. The subjects were fifty cases of welder's lung or suspected cases who had been collected between 1989 and 1994 from one shipyard and continuously followed-up on the basis of in-plant periodic health check program. We applied both chest radiography and HRCT on the same subjects from May 1 to 30, 1996. The images were evaluated by two careered radiologists independently. The findings of chest radiography were classified into four category by ILO classification, and the findings of HRCT according to the criteria of Bergin et al. The concordance between two radiologists expressed with Kendall's tau-b was 0.72 by chest radiography and 0.44 by HRCT- that is, interobserver variation of HRCT was bigger than that of chest radiography. The concordance between the two different methods was highly variable as 0.44 by radiologist A and 0.06 by radiologist B - that is, interobserver variation was very big. However, HRCT looked more detectable for the minor parenchymal change. These findings suggested that it is not appropriate to use HRCT routinely for screening of welder's lung due to lack of diagnostic criteria, and feasibility, acceptability and economic aspects. Nevertheless, HRCT might be recommendable in the case of equivocal parenchymal features on the chest radiography, unexplained respiratory symptoms, and/or lung function abnormalities suggestive of interstitial fibrosis.

Reliability of two different presurgical preparation methods for implant dentistry based on panoramic radiography and cone-beam computed tomography in cadavers

  • Purpose: Special care is necessary to avoid invading important anatomic structures during surgery when presurgical planning is made based on radiographs. However, none of these types of radiography represents a perfect modality. The purpose of this study was to determine the reliability of presurgical planning based on the use of two types of radiographic image (digital panoramic radiography [DPR] and cone-beam computed tomography [CBCT]) by beginner dentists to place implants, and to quantify differences in measurements between radiographic images and real specimens. Methods: Ten fresh cadavers without posterior teeth were used, and twelve practitioners who had no experience of implant surgery performed implant surgery after 10 hours of basic instruction using conventional surgical guide based on CBCT or DPR. Two types of measurement error were evaluated: 1) the presurgical measurement error, defined as that between the presurgical and postsurgical measurements in each modality of radiographic analysis, and 2) the measurement error between postsurgical radiography and the real specimen. Results: The mean presurgical measurement error was significantly smaller for CBCT than for DPR in the maxillary region, whereas it did not differ significantly between the two imaging modalities in the mandibular region. The mean measurement error between radiography and real specimens was significantly smaller for CBCT than for DPR in the maxillary region, but did not differ significantly in the mandibular region. Conclusions: Presurgical planning can be performed safely using DPR in the mandible; however, presurgical planning using CBCT is recommended in the maxilla when a structure in a buccolingual location needs to be evaluated because this imaging modality supplies buccolingual information that cannot be obtained from DPR.

A pilot study of half-value layer measurements using a semiconductor dosimeter for intraoral radiography

  • Purpose: This pilot study was conducted to evaluate half-value layer (HVL) measurements obtained using a semiconductor dosimeter for intraoral radiography. Materials and Methods: This study included 8 aluminum plates, 4 of which were low-purity (less than 99.9%) and 4 high-purity (greater than 99.9%). Intraoral radiography was performed using an intraoral X-ray unit in accordance with the dental protocol at the authors' affiliated hospital: tube voltage, 60 kVp and 70 kVp; tube current, 7 mA; and exposure time, 0.10 s. The accuracy of HVL measurements for intraoral radiography was assessed using a semiconductor dosimeter. A simple regression analysis was performed to compare the aluminum plate thickness and HVL in relation to the tube voltage (60 kVp and 70 kVp) and aluminum purity (low and high). Results: For the low-purity aluminum plates, the HVL at 60 kVp (Y) and 70 kVp (Y) was significantly correlated with the thickness of the aluminum plate (X), with Y=1.708+0.415X (r=0.999, P<0.05) and Y=1.980+0.484X (r=0.999, P<0.05), respectively. Similarly, for the high-purity aluminum plates, the HVL at 60 kVp (Y) and 70 kVp (Y) was significantly correlated with the plate thickness(X), with Y=1.696+0.454X (r=0.999, P<0.05) and Y=1.968+0.515X (r=0.998, P<0.05), respectively. Conclusion: This pilot study examined the relationship between aluminum plate thickness and HVL measurements using a semiconductor dosimeter for intraoral radiography. Semiconductor dosimeters may prove useful in HVL measurement for purposes such as quality assurance in dental X-ray imaging.

손목 관절과 발목 관절 손상 환자의 진단에 있어 응급 초음파의 유용성 (The Utility of Emergency Ultrasound for Diagnosing Wrist and Ankle Injuries)

  • Purpose: Ultrasound is of proven accuracy in abdominal and thoracic trauma and may be useful for diagnosing extremity injury in situations where radiography is not available, such as disasters and military and space applications. However, the diagnosis of fractures is suggested by history and physical examination and is typically confirmed with radiography. As a alternative to radiography, we prospectively evaluated the utility of extremity ultrasound performed by trained residents of emergency medicine (EM) one patient with wrist and ankle extremity injuries. Methods: Initially, residents of EM performed physical examinations for fractures. The emergency ultrasound (EM US) was performed by trained residents, who used a portable ultrasound device with a 10- to 5-MHz linear transducer, on suspected patients before radiography examination. The results of emergency ultrasound and radiography and the final diagnosis were recorded, and correlation among them were determined by using Kappa s test Results: Thirty-nine patients were enrolled in our study. The average age was $36.6\;{\pm}\;19.3$ years. There were radius Fx. (n=21), radius-ulna Fx. (n=1), ulna Fx. (n=1), and contusion (n=2) injuries among the wrist injury and lat.-med. malleolar Fx. (n=13), lat. malleolar Fx. (n=6), and med. malleolar Fx. (n=3) injuries among the ankle injury. Comparing EM US with radiography, we found the sensitivity, specificity, positive predictive value, and negative predictive value of EM US for Fx. diagnosis to be 100%, 66.7%, 97.3%, 100% and those of radiography to be 97.2%, 100%, 100%, and 75%, respectively. Kappa s test for a correlation between the Fx. diagnosis of EM US and the final diagnosis of Fx was performed, and Kappa's value was 0.787 (P = 0.004).Conclusion: EM US for Fx. can be performed quickly and accurately by EM residents with excellent accuracy in remote locations such as disaster areas and in military and aerospace applications. EM US was as useful as radiography in our study and had a high correlation to the final diagnosis of Fx. Therefore, ultrasound should performed on patients with extremity injury to determine whether extremity evaluation should be added to the FAST (focused abdominal sonography trauma) examination.

흉추 측면검사 영상의 CNR과 SNR 측정의 비교 연구 (Comparison Study on CNR and SNR of Thoracic Spine Lateral Radiography)

  • 흉추 측면검사에서의 T-spine breathing technique을 실제 임상에서 사용되고 있는 4가지 검출기인 computer radiography (CR), charge coupled device (CCD), indirect digital radiography (IDR)와 direct digital radiography (DDR)을 사용하여 임상 유용성을 넓히고자 하였다. 화질 평가는 흉추 측면검사의 평가요소 중 5곳 (극돌기, 추궁근, 추체, 추간공, 추간)을 Image J 프로그램을 이용하여 관심영역을 정하고 신호 평균값과 표준편차를 구하여 대조도 잡음비와 신호 대 잡음비를 측정하여 비교하였다. 실험결과 4가지 검출기에서 T-spine breathing technique에서 극돌기, 추궁근, 추체, 추간공, 추간의 5곳 구조에서 우수하게 나타났다. 기존의 T-spine exhalation technique에 비해 T-spine breathing technique으로 촬영한 영상은 우수한 화질을 제공하므로 추후 심호기가 어려운 고령환자들에게 유용한 방법이라 사료된다. 그리고 4가지 검출기에서 contrast to noise ratio (CNR)와 signal to noise ratio (SNR) 같은 정량적인 수치를 제시함으로써 T-spine breathing technique의 적용 가능성을 나타내었다.

구강검진에서 임상검사와 비교한 파노라마방사선사진의 유용성 (Efficacy of panoramic radiography as a screening procedure in dental examination compared with clinical evaluation)

  • Purpose : To evaluate the efficacy of panoramic radiography by comparing the results of clinical examination with radiographic findings. Materials and Methods: We studied 190 patients (20 men and 170 women; mean age, 40 years; range, 22 to 68 years) who visited the health promotion center of Korea Medical Science Institute and were examined both clinically and by panoramic radiography. We compared results from both examinations. Treatment options by clinical examination were described as "no treatment indicated", "treatment of dental caries", "removal of calculus", "treatment of periodontal disease", "prothodontic treatment" and "extraction of the third molar". Findings taken from the panoramic radiography were: dental caries, periapical lesion, alveolar bone loss, calculus deposition, retained root, impaction of the third molar, disease of maxillary sinus, bony change of mandibular condyle, etc. Results: The prevalence of panoramic findings were: 37.9% of dental caries, 17.4% of periapical lesions, 44.7% of alveolar bone losses, 62.6% of calculi deposition, 7.9% of retained roots, 26.8% of third molar impactions, 6.3% of diseases of maxillary sinus, 2.1 % of bony changes of mandibular condlye and 35.8% of miscellaneous lesions. Abnormal conditions revealed by panoramic radiography which had not been discovered on clinical examination were: 24.2% of the patients had dental caries, 17.4% had periapical lesions, 7.4% had calculi deposition, 5.3% had retained roots, 15.3% had third molar impactions. The opposite cases were: 5.2% had dental caries, 12.6% had calculi deposition, and 9.5% had third molar impactions. Conclusion: The use of panoramic radiography as a supplement to the clinical examination might be a valuable screening technique.

Comparison of digital and conventional radiography for the detection of proximal surface caries

  • Conventional intraoral radiography continues to be the most widely used for the diagnosis of dental caries. But conventional intraoral film has several shortcomings, including processing error, increased radiation dose, etc. Recently, various digital radiographs substitude for conventional radiography to overcome these disadvantages. The digital radiographies are numerous advantages, including elimination of processing errors, lower radiation dose, image quality enhancements such as contrast and density modulation.(omitted)

흉부 촬영시 심음영 확대에 따른 유용성에 관한 연구 (The Usefulness of Magnification of the Heart Shadow in Chest Radiography)

  • In order to demonstrate the value of long-distance radiography, we have studied how distance affects images in chest frontal radiography and compared short-distance and long-distance images in chest lateral radiography. Cardiothoracic ratio(CTR %) of 50 patients with no disease in the chest(10 each at the age of 20~60) were evaluated in Supine AP(100 cm), Sitting AP(100 cm), Sitting AP(180 cm), and Erect PA(180 cm). In lateral radiography, we evaluated and compared left lateral radiography(100 cm and 180 cm) of the patients based on the horizontal maximum of the heart. The average value of CTR(%) were 0.48 in Erect PA(180 cm), 0.52 in Supine AP(100 cm), 0.50 in Sitting AP(100 cm), 0.49 in Sitting AP(180 cm), which were Supine AP(100 cm) > Sitting AP(100 cm) > Sitting AP(180 cm) > Erect PA(180 cm). The average value of Maximum transverse diameter of left of the cardiac(MLD), which showed how much axis of spine was slanted to the left, was 90.67 mm in Erect PA(180 cm), 103.92 mm in Supine AP(100 cm), 93.54 mm in Sitting(100 cm), 89.84 mm in Sitting AP(180 cm), 58.11 mm in the minimum value and 118.79 mm in the maximum value. The average value of Maximum transverse diameter of right side of the cardiac(MRD), which suggested how much axis of spine was slanted to the right, was 47.18 mm in Erect PA(180 cm), 48.12 mm in Supine AP(100 cm), 44.98 mm in Sitting AP(180 cm), and the minimum value 26.84 mm and the maximum value 65.30 mm. There was no standard method to calculate; therefore, the horizontal maximum of the heart was used for lateral radiography. The average value was 121.07 mm in 100 cm and 109.76 mm in 180 cm. Sitting AP(180 cm) among the types was closest to C-PA(180 cm). As a result, during C-AP radiography, long-distance radiography lessened shadow of the heart more than that of short distance, Sitting position more than Supine position.
