국소의치 장착시 잔존치아의 치태침착에 관한 연구 (A STUDY ON PLAQUE ACCUMULATION FOLLOWING THE WEARING OF REMOVABLE PARTIAL DENTURE)

  • The purpose of this study was to investigate the effect of wearing the removable partial denture on plaque accumulation. Twelve removable partial denture cases were examined. Patients were recalled three consecutive 3 day periods. Plaque index was measured after each 3 day period. (1) not wearing the denture, (2) wearing the denture, (3) wearing the denture after intensive tooth brushing instruction. The results were as follows: 1. Plaque indices of all the remaining teeth were higher following the wearing of removable partial denture than those of teeth not wearing the denture and those of teeth after intensive tooth brushing instruction. 2. Plaque indices showed statistical difference between all the tooth surfaces which were in contact with the denture and those which were not. 3. Plaque indices showed no statistical difference between buccal and lingual surfaces which were in contact with the denture and those which were not. 4. Plaque indices showed statistical difference between proximal surfaces which were in contact with the denture and those which were not.

노인들의 의치 장착 기간과 의치 사후 관리에 영향을 주는 요인 - 경북 농촌지역을 중심으로 - (Factors Affecting Period of weared and Post Management of Denture of the Elderly (Rural areas in Gyoungbook Province))

  • Purpose: In order to validate empirically the theoretical model of this study subjects, who were limited in Andong gun, Youngyang gun, Uiseong gun, Bonghwa gun, Chilgok gun, Cheongsong gun according to residence-specified distribution and over aged 65, were focused to the elderly in rural areas with geographical characteristics where they have difficulties in accessibility of dental facilities and dental care. Methods: The collected data were analyzed by the SPSS 19.0 program. First, the frequency analysis was performed for each independent variable and the dependent variable. Second, reliability analysis and correlation analysis were conducted for daily living skills and Biomedical Vigilance. Third, multivariate analysis was performed in terms of and the post-administration management. Fourth, multiple regression analysis was performed to identify the factors affecting the post-management of dentures and the period of using dentures. Results: By results of the multiple regression analysis, the elder the subjects are(${\beta}$ = 0.03), the lower their income activities are(${\beta}$ = -0.18), the smaller their monthly allowances are(${\beta}$ = -0.16), the worse their daily activity skills are(${\beta}$ = -0.12), the more they were mounted their dentures by an unlicensed contractor(${\beta}$ = 0.07), and the more they cost to their dentures(${\beta}$ = 0.14), the longer the period of use dentures are. And the beneficiaries of Basic Livelihood Security(${\beta}$ = -0.14) who got free denture business are shorten the period of using dentures. According to multiple regression analysis for post management of dentures, the independent variables like the health education(${\beta}$ = 0.24), smoking(${\beta}$ = -0.18), periodic oral examination(${\beta}$ = 0.13), dentures mounted by an unlicensed contractor(${\beta}$ = -0.13) are shown statistically significant results. In that study subjects are taking part in health education and non-smokers, they are likely to have more chances to post management for their dentures like periodic oral examination. However the subjects who were weared their denture by an unlicensed contractor get less chance to post management for their dentures. Conclusion: As the elderly who want to wear denture are increasing due to the denture insurance for the elderly, the need for oral health education for the elderly is an urgent request, especially continuos education for the way of use their dentures and follow-up management for the elderly who were already mounted their dentures. And also, the need for more systematic researches is requested for securing objective data.

Bisphosphonate-related osteonecrosis of the jaw in metastatic breast cancer patients: a review of 25 cases

  • Background: Intravenous bisphosphonates have been used in metastatic breast cancer patients to reduce pathologic bone fracture and bone pain. However, necrosis of the jaw has been reported in those who received intravenous bisphosphonates. Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is caused by dental extraction, dental implant surgery, and denture wearing; however, it occurs spontaneously. The purpose of this study was to report BRONJ in metastatic breast cancer patients. Methods: Consecutive 25 female patients were referred from the Department of Oncology from 2008 to 2014 for jaw bone discomfort. Staging of breast cancer, history of bisphosphonate infusion, etiology of BRONJ, and treatment results were reviewed. Average age of the patients was 55.4 years old (38-74). Twelve maxillae and 16 mandibles were involved. Conservative treatments such as irrigation, antibiotic medication, analgesics, and oral gargle were applied for all patients for the initial treatment. Patients who had sequestrum underwent debridement and primary closure. Results: The etiologies of BRONJ were dental extraction (19 cases), dental implant (2 cases), and endodontic treatment (1 case). However, three patients did not have any risk factors to cause BRONJ. Three patients died of progression of metastasis during follow-up periods. Surgical debridement was performed in 21 patients with success in 18 patients. Three patients showed recurred bone exposure and infection after operation. Conclusions: Prevention of the BRONJ is critical in metastatic breast cancer patients. Conservative treatment to reduce pain, discomfort, and infection is recommended for the initial therapy. However, if there is a sequestrum, surgical debridement and primary closure is the key to treat the BRONJ.

가철성 국소의치와 후방연장 계속가공의치를 장착한 환자의 치주 및 보철 상태 (PERIODONTAL AND PROSTHETIC FINDINGS IN PATIENTS TREATED WITH REMOVABLE PARTIAL DENTURES OR DISTALLY EXTENDING CANTILEVER BRIDGES)

  • This study was perfomed to investigate the prosthodontic and periodontal status of the abutment teeth in distally extending bridges(DEBs) (78 cases) and removable partial dentures(RPDs) (43 cases) for 122 patients (55 males and 57 females) visiting department of Periodontology, Seoul National University Hospital. The average wearing periods were 67.8 months for DEBs and 66.4 months for RPDs. 38.4% of the patients in DEBs and 35.9% in RPDs complainted of chewing discomfort and 22.6% and 24.4% were not chewing on the prosthodontically - treated sides, respectively. In DEBs, when the restoration for 2nd molar supported by 1st molar & 2nd premolar was grouped to type 1, 1st molar supported by 1st & 2nd premolars was type 2, and 1st & 2nd molars supported by 1st & 2nd premolars was type 3, there was a significant differences only in the tooth mobility score among clinical parameters (type 2>type 1>type 3). In RPDs, when bilateral free-end case was grouped to type 1, and unilateral case was type 2, there was a significant difference in the Gingival index (type 1>type 2). In DEBs, 62.8% of restoration were overcontoured, 72% had interproximal space closures, 30.5% overextended pontics and 86.6% overhanging margins. In RPDs, 24.4% of the restorations were overcontoured, 45.5% had interproxinal space closures and 58.3% overhanging margins. From these results of this study, periodontal problems caused by prosthodontic defects were considered to be contributing factors in chewing discomfort.
