  • The purpose of this study was to compare and evaluate the stress distribution and displacement developed in the abutment teeth and residual ridge area by madibular unilateral distal extension removable partial denture with 2 different retainer designs. The retainers on right and left canine and right 2nd molar were Alters clasp in one model and telescopic crown in the other model. The stress distribution of abutment teeth and residual ridge area on two model were compared and analyzed with 3-dimensional finite element method. 150N and 400N forces were applied vertically, 30 degree and horizontally on the central fossa area of left 1st molar of the removable partial denture, and then stress distribution patterns were analyzed and compared. The results were as follows 1. As the magnitude and angulation of applied force were increased, the magnitude of stress on the right and central residual ridge area and the right canine of the telescopic type increased and comparing to those of the Alters clasp type. 2. As the magnitude and angulation of applied force were increased, the mesial direction of displacement on the right residual ridge area and the right tooth of the telescopic type increased and the distal direction of displacement on left residual ridge area and the left canine increased comparing to those of Akers clasp type. 3. As the vertical force was applied, the distal direction of the displacement of the right tooth were greater and that of the left canine was smaller and the upward displacement of the right canine was greater in telescopic partial denture than those of Akers clasp type. 4. As the 30 degree force was applied, the mesial direction of the displacement of the right tooth were greater and the distal direction of the displacement of the left canine was smaller and the upward displacement of the right canine was greater in telescopic partial denture than those of Akers clasp type. In the horizontal force the results were same in right area tooth but the distal direction of displacement was greater in left canine. 5. In both removable partial dentures, as the magnitude and degree of force were increased, the stress and displacement were increased. The compressive force was dominative than the ten sile force. 6. In both removable partial dentures, the magnitude of stress was greater on mucosal tissue area than that of the alveolar bone area on distal extension residual ridge area but the result was reversed on anterior residual ridge area. The displacement was always greater on mucosal tissue area than that of alveolar bone area.

  • The purpose of this study was to evaluate the stress distribution developed in the supporting structures by mandibular distal extension removable partial dentures with 2 different direct retainer designs and with or without indirect retainer and abutment splinting. The examined direct retainers on the second bicuspid abutment tooth were Akers clasp and RPA clasp, the indirect retainer was located on the mesial fossa of the first bicuspid, and the first and second bicuspid were splinted in case of tooth splinting. Total 8 cases were compared and analyzed with 3-dimensional finite element method. 150N were applied vertically on the artificial teeth of the removable partial denture, and then stress distribution patterns were analyzed and compared. The results were as follows : 1. The forces transmitted to the abutment tooth were primarily from the occlusal rests. 2. The abutment tooth was displaced distally when the force was applied. The compressive stress was observed at the distal root surface of the abutment tooth and the tensile stress, at the mesial root surface. 3. The denture base was displaced posteriorly and inferiorly when the force was applied. At the more distal portion of the denture base, the greater displacement was observed.And the anterior portion of the major connector was displaced superiorly. 4. The occlusal rest placed on the distal part of the abutment tooth tended to tip the tooth more posteriorly than did one on the mesial part of that tooth. 5. Severe superior displacement was observed at the anterior portion of the major connector in case of removable partial dentures without indirect retainer. 6. In case of tooth-splinting, the stress was distributed through all the root surface of both abuments. In case of no tooth-splinting, the stress was concentrated on the distal root surface of the primary abutment.

고령 환자의 기존 의치를 치과기공소에서 수리한 증례 (Denture repair for elderly patients in dental laboratories: a case report)

  • The need for repairing removable dentures has grown as the population had aged. The direct methods allow existing dentures to be repaired without interrupting their use. However, if patient compliance is low, direct methods may be challenging. Moreover, attaching an artificial tooth to a metal base is a complicated procedure because it necessitates casting a retentive element and soldering it to a metal base. This clinical report describes how to add an artificial tooth to a metal base, reline denture bases, and reestablish occlusion on relined removable dentures using indirect methods. Existing removable dentures were successfully repaired and their service life was efficiently extended using the methods described.

  • 다수의 구치를 상실한 환자에서는 전방부 치아들의 마모나 변위, 교합평면의 붕괴와 교합수직고경의 감소가 일어날 수 있다. 특히 편측으로 소수의 치아만 가진 경우 국소의치는 생역학적으로 불리하므로, 소수의 임플란트 서베이드 크라운을 추가한 임플란트 보조 국소의치가 대안이 될 수 있다. 본 증례의 환자는 하악 우측 구치부 상실로 인해 전방부 치아들이 심하게 마모되고, 교합평면이 붕괴되어 있었다. 최소한의 교합 수직고경 거상과 함께 상악 고정성 보철 및 하악 임플란트 보조 국소의치로 수복을 진행하였고, 기능적 및 심미적으로 양호한 임상 결과를 얻을 수 있었다.

  • The purpose of this study was to investigate the displacement of and the stress distribution on the prosthesis, abutment, and its supporting tissues under functional load, and the effect of alteration in root length of 2nd abutment. The 3-dimensional finite element method was used and the finite element models were prepared in which the abutments of left mandibular 5 unit axed partial denture were canine, the 1st pre-molar and the 2nd molar, and the root lengths of canines were as follows. Model I : Root length of canine was 2mm longer than the 1st premolar Model II : Root length of canine was 2mm shorter than the 1st premolar Static compressive force of 300N was applied to connector between 2nd premolar & 1st molar, and then von Mises stress, displacement and reaction force were obtained. The results were as follows : 1. In fixed partial denture, prosthesis under load on pontic was rotated around mesio-distal long axis of it from longual side to buccal, and simultaneously bended in buccal and gingival direction with mesial end deformed in gingival direction and distolingual end in occlusal. 2. Clinical crowns of abutments were bended in the same directions with those in which prosthesis deforms. Due to that, roots of anterior abutments were twisted in counterclockwise with concentration of shear stress on distal or distobuccal sides of their cervices, and that of posterior was in clockwise with concentration of shear stress on mesiobuccal side of it in the same level with anterior abutments. 3. In case that root length of the 2nd abutment was longer than that of the 1st abutment, its displacement and reaction force which means the force tooth exerts on the surrounding periodontal tissues were smaller but shear stress on itself was larger than in the case root length of 2nd abutment was shorter.

후방연장 가철성 국소의치에서 임플란트의 길이와 위치가 응력분산에 미치는 영향 (Influence of the length and location of implants on distal extension removable partial dentures: finite element analysis)

  • 목적: 임플란트 위치와 길이가 하악 후방연장 가철성 국소의치(DERPD)와 연관된 임플란트의 응력 분포와 변위에 미치는 영향을 알아보는 것이다. 연구 재료 및 방법: #35, 36, 37이 소실된 시상절단면의 후방연장모형과 가철성 국소의치를 기본모형으로 사용했다. NX 9.0으로 7개의 모델을 디자인했다. 모델 A, B, C에서 각각 11, 6, 4 mm 길이의 임플란트가 #37 인공치 하방에 위치되었다. 모델 D, E, F에서 각각 11, 6, 4 mm의 임플란트가 #36 인공치 하방에 위치되었다. 모델 G는 임플란트가 없었다. 수직하중(250 N)을 #36의 중심와에 가했고, 유한요소 분석프로그램을 이용해 von Mises stress와 변위를 관찰했다. 결과: #37에 위치한 임플란트는 #36에 위치한 것과 비교시 주변골에 더 낮은 응력집중도를, #36에 위치한 임플란트는 #37에 위치한 것보다 더 적은 변위를 보였다. 결론: 임플란트 지지형 가철성 국소의치에서 후방부에 위치한 임플란트는 전방부에서보다 더 이점을 가지며, 길이가 더 긴 임플란트의 사용은 응력분산을 위해서 중요하다.

  • The purpose of this study was to analyze the stress distribution and the displacement happened to the abutment, the prosthesis, and the surrounding structure according to the location of the nonrigid connector, that is, the keyway in the distal of canine and the mesial of the implant in the three unit fixed partial denture. Two-dimensional finite element model ws constructed and analyzed for the stress distribution and the displacement using software ABAQUS(Ver 5.2 Hibbitt, Karisson & Sorenson, Inc., 1992). After finishing the finite element model, the distribution load of 15kg was applied simultaneously to the all cusp tips of the prosthesis and the concentration load of 10㎏ was applied respectively at the each cusp tip of the prosthesis. The following results were obtained : 1. The amount of displacement of the implant was greater in case of the non-rigid connection than the rigid connection, and the more favorable displacement was shown in case of the IKb than the IKa. 2. Without regard to the connection method, the stress represented at the surrounding bone was similar, and the more favorabel stress distribution was shown in case of IKb. 3. The maximum stress was concentrated at the fastening screw and the neck of implant in all experimental groups, and their stress magnitudes were in the order of IKb, IR, and IKa.

3차원 적층 출력과 밀링에 의해 제작된 일체형 의치의 인공치 변위 평가 (Artificial teeth displacement of monolithic complete denture manufactured by 3D printer and milling machine)

  • 목적: 의치상과 인공치가 밀링과 3차원 프린팅 방식에 의해 일체형으로 제작될 때 각기 인공치의 변위를 평가하고자 한다. 재료 및 방법: 밀링과 3차원 프린팅 방식으로 상하악 각기 12개의 의치를 제작하였다. 프린팅에 의해 제작된 상악의치 군은 Group Up이고, 하악의치군은 Group Lp이다. 밀링에 의해 제작된 상악의치 군은 Group Um이고, 하악의치 군은 Group Lm이다. 대조군 상악의치는 소프트웨어상에서 최종적으로 디자인된 것으로 상악은 Group Uc이고 하악은 Group Lc이다. 상하악 의치의 양중절치 사이(AB, ab), 양견치 사이(CD, cd), 양제1대구치 사이(EF, ef), 중절치와 제1대구치사이(AE, ae) 및 중절치와 설측점사이(AG, ag) 의 거리를 측정하였다. 결과: AG와 ag는 상하악 의치 모두에서 프린트된 의치군과 컨트롤 의치군 사이에서 뿐 만 아니라 밀링 의치군과 컨트롤 의치군 사이에서도 통계학적 유의성을 나타내었다(One-way ANOVA, P < .05). 하악 일체형의치에서는 Group Lp와 Group Lm 사이에 ab, cd, ef 모두 통계학적 유의성을 나타내었다. 결론: 밀링과 3차원 프린팅에 의해 제작된 의치의 인공치 변위는 통계학적 유의성이 나타났으나, 임상적으로 매우 양호한 변위량을 보여주었다.

Overdenture 하에서 하악응력 및 의치의 변위에 관한 유한요소법적 분석 (FINITE ELEMENT ANALYSIS OF MANDIBULAR STRESSES AND DENTURE MOVEMENTS INDUCED BY OVERDENTURES)

  • The purpose of this study was to analyze the displacement and the magnitude and the mode of distribution of the stresses in the lower overdenture, the mucous membrane, the abutment tooth and the mandibular supporting bone when various denture base materials, such as acrylic resin and 0.5mm metal base, and various denture base designs were subjected to different loading schemes. For this study, the two-dimensional finite element method was used. Mandibular arch models, with only canine remaining, were fabricated. In the first denture base design, a space, approximately 1mm thick, was prepared between the denture and the dome abutment. In the second denture base design, contact between the denture and the dome abutment was eliminated except the contact of the occlusal third of the abutment. In order to represent the same physiological condition as the fixed areas of the mandible under loading schemes, the eight nodes which lie at the mandibular angle region, the coronoid process and the mandibular condyle were assumed to be fixed. Each model was loaded with a magnitude of 10 kgs on the first molar region(P1) and 7 kgs on the central incisal region (P2) in a vertical direction. Then the force of 10 kgs was applied distributively from the first premolar to the second molar of each model in a vertical direction(P3). The results were as follows. : 1. When the testing vertical loads were given to the selected points of the overdenture, the overdenture showed the rotatory phenomenon, as well as sinking and the displacements of alveolar ridge, abutment and lower border of mandible under the metal base overdenture were less than those under the acrylic resin overdenture. 2. The maximum principal stresses(the maximum tensile stresses) being considered, high tensile stresses occured at the buccal shelf area, the posterior region of the ridge crest and the anterior border region of the mandibular ramus. 3. The minimum principal stresses(the maximum compressive stresses) being considered, high compressive stresses occured at the inferior and posterior border region of the mandible, the mandibular angle and the posterior border region of the mandibular ramus. 4. The vertical load on the central incisal region(P2) produced higher equivalent stress in the mandible than that on any other region(P1, P3) because of the long lever arm distance from the fixed points to the loading point. 5. Higher equivalent stresses were distributed throughout the metal base overdenture than the resin base overdenture under the same loading condition. 6. The case of occlusal third contact of the abutment to the denture produced higher equivalent stresses in the abutment, the mandibular area around the abutment and the overdenture than the case of a 1mm space between the denture and the abutment. 7. Without regard to overdenture base materials and designs, the amounts and distribution patterns of equivalent stresses under the same loading condition were similar in the mucous membrane.

Full Mouth Rehabilitation of a Patient with Bite Collapse in the Molar Area Using Removable Partial Denture and Dental Implant Prosthetics

  • Dental clinicians often encounter cases wherein the patient's lost molar area was neglected and left untreated for an extended period of time, thereby causing the extrusion of opposite molars and occlusal disharmony as well as occlusion in the anterior teeth and consequently resulting in anterior displacement in the area. Clinicians normally carry out prosthetic treatment via occlusal plane lifting when such becomes absolutely necessary due to the lack of sufficient space needed for prosthetic therapy aimed at proper anterior and lateral induction. In this case report, we examined occlusal disharmony and VDO loss in a patient who had lost his molars and had not received prosthetic treatment for an extended period of time. We treated the maxillary area with dental implant prosthetics and Kennedy Class I RPD and the mandibular area with residual natural tooth-based implant placement and dental implant prosthetics. The patient reported treatment outcomes that were deemed satisfactory both functionally and aesthetically.
