Home visiting oral health program of Longterm home care service and the change of some pathogenic microorganism counts in denture (재가장기요양기관의 방문구강보건교육 전·후 의치의 일부 병원성 미생물의 집락수 변화)

  • Purpose: This study is a repetitive comparative analysis of a qualitative case study that carried out a home visiting oral health education program. Method: This study conducted an interview survey through medical examination by interview before and after the home visiting oral health education and measured some pathogenic microorganism counts in the denture. Results: There was a positive change in the self oral care ability of the elderly at home after home visiting oral health education, including the behavior of self management of dentures, and some pathogenic microorganism counts in the dentures. Conclusion: The home visiting oral health education of home care service centers can improve oral health care for the elderly at home.

SHOCK-ABSORBING BEHAVIOR OF TEMPORARY SOFT DENTURE LINERS (임시 연성 의치상 이장재의 충격 흡수에 관한 연구)

  • Temporary soft liners can be used to prevent chronic soreness from dentures or to aid in its treatment are as adjuncts in tissue conditioning, for temporary obturators, and to stabilize baseplate or surgical stent. The purpose of this study was to evaluate the shock absorption properties of several temporary soft denture liners using a free drop test with an accelerometer. The materials tested inclued Coe-comfort, Softone, Tissue conditioner and Viscogel. The specimens were fabricated with the thickness of 1, 2, 3mm and were stored in distilled water at $37^{\circ}C$ for a day, 1, 2, and 3 weeks. Six samples were made with each material for each test condition and the shock-absorbing behavior was evaluated according to material, thickness and duration. The results were as following : 1. Softone of 3mm thickness stored for a day showed the most excellent shock absorbability. 2. The shock absorbing behavior of duration according to materials and thickness showed a day to be the highest and decreased in 1 week, 2 weeks and 3 weeks in that order(p<0.05). And there was no significant difference between durations in Tissue conditioner. 3. The shock absorbability of thickness according to materials and duration showed 3mm to be highest and decreased in the order of 2mm, 1mm(p<0.05). 4. In comparison of the shock absorbability of temporary soft denture liners according to thickness, there was statistically significant difference between Softone and Visocgel, Tissue conditioner, Coe-comfort / Viscogel and Tissue conditioner, Coe-comfort in 1,2mm thickness, and between Softone, Viscogel and Tissue conditioner, Coe-comfort in 3mm thickness (p<0.05).

TIME-DEPENDENT DEFORMATION OF POLYMER-BASED PROVISIONAL CROWN AND FIXED PARTIAL DENTURE MATERIALS

  • Statement of problem. One of the common problems of provisional crown and fixed partial denture materials is that when they are subjected to constant loads for a long period of time, they exhibit a dimensional change (creep). Purpose. The aim of this study was to investigate the viscoelastic behaviour of polymer-based provisional crown and fixed partial denture materials with time at constant compressive load. Material and methods. Three dimethacrylate-based materials (Protemp 3 Garant, Temphase, Luxatemp) and one monomethacrylate-based material (Trim) were selected. Dimensional changes of the specimens were recorded by a LVDT to evaluate their viscoelastic behavior and creep strain. For all specimens, two loading procedures were used. At first, static compressive stress of 4 MPa was applied for 30 minutes and followed by 1 hour of strain recovery. Then, after 24 hours of water storage, the specimens were loaded again. The creep values between materials were statistically analyzed using one-way ANOVA and multiple comparison $Scheff\acute{e}$ test. Independent samples t-test was also used to identify the difference of creep strain between first and secondary loading conditions at the significance level of 0.05. Results. Following application of the first loading, Trim showed the highest maximum creep strain (32.7%) followed by Luxatemp, Protemp 3 Garant and Temphase, with values of 3.78%, 2.86% and 1.77%, respectively. Trim was significantly different from other materials (P<0.05), while there were no significant differences among Luxatemp, Protemp 3 Garant and Temphase (P>0.05). The highest recovery and permanent set of Trim, were significantly different from those of others (P<0.05). At the secondary loading of the dimethacrylate-based materials, creep deformation, recovery and permanent set decreased and the percentage of recovery increased, while in Trim, all values of the measurements increased. This result showed that the secondary loading at 24 hours produced a significant creep magnitude. Conclusion. The dimethacrylate-based provisional crown and fixed partial denture materials showed significantly higher creep resistance and lower deformation than the monomethacrylate-based material. Thus, monomethacrylate-based materials should not be used in long-term stress-bearing situations.

Oral health and behavior by diabetic status: the fifth Korea national health and nutrition examination survey (당뇨상태에 따른 구강상태 및 행태: 제5기 국민건강영양조사)

  • Objectives: The aim of this study was to evaluate the oral health status and behavior in Korean diabetic adults. Methods: The study subjects were 11,840 adults who participated in the fifth Korea National Health and Nutrition Examination Survey(2010-2012). Diabetic status was defined by doctors and fasting blood sugar(FBS) level. Oral health status was assessed by decayed-missing-filled teeth(DMFT), community periodontal index(CPI), periodontal disease, denture needs, limitation of oral function, and chewing difficulty. The oral health behavior was evaluated by oral examination within a year, brushing times a day, and use of auxiliary oral product. The data were analyzed by descriptive analysis, chi-square tests and multiple logistic regression analyses. Results: The prevalence rate of diabetes mellitus diagnosed by doctor and FBS was 8.3% and 10.2%, respectively. The prevalence rate of periodontitis was 25.4%. The proportions of $DMFT{\geq}20$, $CPI{\geq}3$, periodontal disease, denture needs, oral function limitation and chewing difficulty in the confirmed diabetic group by doctor were significantly higher than those of the non-diabetic group(p<0.05). In multiple logistic regression analysis, the adjusted odds ratio(aOR) for periodontal disease(aOR=1.73, 95% CI=1.41-2.12), presence of denture needs(aOR=1.40, 95% CI=1.06-1.84), limited oral function(aOR=1.43, 95% CI=1.15-1.78) and chewing difficulty(aOR=1.41 95% CI=1.13-1.77) in diabetic subjects were significantly higher than those of the non-diabetic subjects. There were similar associations between diabetes defined with FBS and oral health. In oral health behavior, diabetic subjects had significantly lower odds ratios for oral examination(aOR=0.76, 95% CI=0.60-0.98), brushing time ${\geq}2$ times(aOR=0.73, 95% CI=0.57-0.93), and auxiliary oral products(aOR=0.74, 95% CI=0.59-0.94). Conclusions: There was a significant relationship between oral health status and behavior in Korean diabetic adults. Further study is needed to evaluate the underlying mechanisms between diabetes mellitus and oral health status.

The relationship of snack habits, oral health behavior and oral health status in psychiatric patients (일부 정신병원 입원환자의 구강건강상태와 구강건강행위 및 간식습관과의 관련성)

  • Objectives : The purpose of the study is to investigate the relationship between variables associated with oral health status in psychiatric patients and to provide basic information on oral health education program development for the psychiatric patients. Methods : Subjects were 151 psychiatric patients who filled out the structured questionnaire. The data were analyzed for independent t-test, one-way ANOVA and hierarchical multiple regression by using SPSS Win Program 18.0 version. Results : Higher DMFT index was shown in the older age(p<0.001) and low perceived oral health group(p<0.001). Higher DMFT index included no tooth brushing after breakfast(p=0.045), wrong tooth brushing method(p<0.001). Chewing discomfort(p=0.027) and denture(p=0.027) were closely related to higher DMFT index. Bread(p=0.033) and snack(p<0.001) increased DMFT index. Factors affecting oral health status were age, denture, snack and bread. The explanation power of the final model was 29.4%. Conclusions : Dietary control for the psychiatric patients is very important to prevent dental caries. Tooth brushing and use of oral hygiene devices can improve good oral health care. Furthermore, this result can improve the oral health care for the disabled people.

A study of subjective periodontal disease of elderly people (노인의 주관적 치주질환에 관한 연구)

  • Objectives : This study was conducted to evaluated the subjective oral health state related periodontal disease of elderly people. Methods : Data were collected through the National Health and Nutrition Survey in 2005. Participants consisted of 1,091 elderly people above 65 years old. Independent variables in the survey were social characteristic, health behavior, oral health condition, oral health management. The data were analyzed by using the SPSS 12.0. Results : The more elderly people of 69.0% recognized own oral health as is not healthy, elderly people of 74.0% recognized own oral health as is not healthy about subjective oral health and a lot of stressed(82.8%) has felt highly about periodontal disease(p<0.001). Respondents of have a sound teeth(58.9%), have a lower denture(75.0%) and have a no problem in mastication(74.5%) has felt highly about periodontal disease(p<0.001). Elderly people recognized own oral health as is not healthy about subjective oral health were 0.316 times(p<0.01), elderly people recognized own oral health as is common healthy about subjective oral health were 0.241 times(p<0.001), a lot of stressed were 1.410 times has felt highly about periodontal disease. Elderly people of have a lower denture were 1.159 times, have a upper denture were 1.159 times, have a lower and upper denture were 0.464 times has felt highly about periodontal disease(p<0.05). Also respondents of have a no problem in mastication were 7.464 times compared with problem in mastication(p<0.001). Conclusions : Quality of life from disease of Korean elderly people can be fallen, and improve quality of life that medical treatment is healthy numerical value state numerical value state. Study's findings of above may be used to inform the importance of health numerical value state while establish dental health policy that is string.

AN EXPERIMENTAL STUDY ON REINFORCEMENT OF ACRYLIC RESIN DENTURE BASE (아크릴릭 레진 의치상 강화에 관한 실험적 연구)

  • The denture may be fractured accidentally by an impact while outside the mouth, or may be cracked or broken while in service in the mouth. The latter is generally a fatigue failure caused by repeated flexure over a period of time. This investigation compared the flexural fatigue resistance, the impact force and the transverse strength of two denture base materials with and without the grid strengthener, the T300, the T800 and the Kevlar fiber to evaluate the fracture resistance. The distribution and behavior of fibers across fracture lines were examined by Hi-Scope Compact Microvision System. Through analyses of the data from this study, the following conclusions were obtained. 1. The flexural fatigue resistance, impact strength and transverse strength of high impact strength resin were higher than those of conventional heat polymerizing resin, but statistically there was no significant difference(p>0.05). 2. All specimens with and without the grid strengthener did not show significant differences in the flexural fatigue, the impact and the transverse strength test(p>0.05). 3. All specimens reinforced with the T300, the T800 and the Kevlar fiber showed significant increase of the fatigue resistance and the impact force(p<0.05). 4. All specimens reinforced with the T800 and the Kevlar fiber showed significant increase of the transverse strength(p<0.05). 5. All specimens reinforced with the T300, the T800 and the Kevlar fiber exhibited greenstick fractures. The fibers tended to remain enveloped in the resin, resisting pull-out.

Corrosion Characteristics of Ti alloy for Removable Partial Denture (국소의치용 티타늄 합금의 부식 특성)

  • In this study, surface characteristics and corrosion behaviors have been investigated in addition to Zr elements on the low elastic modulus Ti-30Ta alloy. Low elastic modulus Ti-30Ta-xZr(x : 3, 7 and 15 wt %) alloys were prepared by arc melting and then heat treated at $1000^{\circ}C$ for 24 hrs in an argon atmosphere. Microstructures of the alloys were examined by field emission scanning electron microscope(FE-SEM) and X-ray diffractometer(XRD). Electrochemical experiments were performed using a conventional three-electrode configuration with a sample working electrode, a high density carbon counter electrode and a saturated calomel reference electrode. According to the result of polarization behavior in the Ti-30Ta-xZr alloys, the current density of homogenized Ti-30Ta-15Zr in the passive region was lower than the other alloys.

Effects of subjective oral health condition and oral health behavior on happiness in the elderly people (일부 노인의 주관적 구강건강상태 및 구강건강행위가 행복감에 미치는 영향)

  • Objectives: The purpose of the study is to identify the relationship between happiness, subjective oral health condition and oral health behavior in Korean elderly people. Methods: A self-reported questionnaire was completed by 487 elderly people using facilities for the senior citizens in Gyeonggi-do from January 3 to November 28, 2014 by convenience sampling method. The questionnaire consisted of demographic characteristics of the subjects, subjective oral health status, oral health behavior, and happiness. Cronbach'a alpha in the happiness was 0.734 in the study. Results: Happiness was closely correlated with abscence of subjective periodontal diseases, dental fillings, and denture. The number of toothbrushing and dental health education had also influenced on happiness. Conclusions: In order to improve the quality of dental health in the elderly, it is necessary to develop a multilateral systematic dental health promotion program by Ministry of Health and Welfare.

The Effect of Education and Income Levels of Some Elderly on Oral Health Status and Management Behavior and Dental Use Behavior (일부 노인의 교육 및 소득정도가 구강건강상태 및 관리행태와 치과이용행태에 미치는 영향)

  • This study was conducted to identify the oral health status and management behavior and dental use behavior according to the level of education and income of the elderly, and to present the supporting data to prepare policies and systems for the promotion of elderly oral health. The data survey was conducted on 224 elderly people living in the Daejeon area from August 19, 2019 to October 25, 2019, and the collected data were analyzed by chi-square test and logistic regression analysis. As a result, oral health status and management behavior showed significant differences according to education and income level in denture use, implant use, oral hygiene product use, oral health education experience, and oral health management method. There was a significant difference in dental use behavior according to education and income level, and age, education level, implant use, and the main reasons for dental use were found to be factors affecting dental use. Therefore, it is expected that the quality of life of the elderly's oral health can be improved by preparing an institutional plan at the national that considers education and income level based on this study.
