Dental care for patients taking antiresorptive drugs: a literature review

  • Antiresorptive drugs (ARDs), such as bisphosphonates or denosumab, that prevent bone resorption are widely used in patients with osteoporosis or with cancer that has metastasized to the bones. Although osteonecrosis of the jaw (ONJ) is a well-documented complication of ARD use, the benefits ARDs outweigh the complication. Thus, research has focused on finding ways to prevent or reduce the risk of developing ONJ. Dentists, as part of a multi-professional team, have a critical role in preventing ONJ. However, many dentists tend to hesitate to provide dental care to patients with ONJ, or tend to think that it is a problem to be dealt with by oral surgeons. This review gives an overview of ARD-related ONJ and provides the guidelines for dental care in patients taking ARDs to lower the risk of developing ONJ.

  • Aesthetic and cosmetic dentistry is becoming a major source of the dentists practice and source of income. Patients are demanding dentists to improve and enhance their appearance and function by means of aesthetic contouring and orthodontics and are willing to pay for such services. This creates a challenge for the dental profession to gain more knowledge and skill in this important area of their practice. Ceramic and composite veneers provide a comprehensive solution to the dentition that are severely hypoplastic or deeply stained by the ingestion of tetracycline during tooth development. A large number of dental materials and new techniques are now available that will render extremely satisfactory and beautiful solutions to these previously unsightly dentitions. This presentation will present the most commonly used dental materials and techniques of fabrication of composite and porcelain veneers. Using clinical cases, the step by step cavity preparations.Impression taking, laboratory fabrication, clinical try-in and cementation materials and techniques and repairs of broken veneers as well as long term follow up will be presentedesented

노인장기요양시설 치과촉탁의제 도입을 위한 제도 개선 방향 (A study on the amendments of long-term care-related legislations for the introduction of part-time facility dentists)

  • The coverage of the National Health Insurance for the elderly is expanding to denture and implants. Although the National Long-Term Care Insurance was just being settled, Oral health service was not provided to the Elderly in Long-Term Care Facilities. The long-term care facilities had part-time facility doctors. However, there is no dentist in the long-term care facility because of lack of long-term care insurance-related legislations. The amendments of long-term care insurance-related legislations for the introduction of part-time facility dentists are needed because the elderly in long-term care facility are vulnerable to oral health. For the substantial management of the National Long-Term Care Insurance, the development of oral health service model for the elderly and education materials for the dental team will be needed. Also, adequate dental service fee of the National longterm care insurance will be needed.

치과의사의 설문을 통해 본 치과의료사고 및 분쟁 (Dental accidents and malpractice disputes survey)

  • The dental accidents and disputes among Korean dentists have been on the increase with time. The main purpose of this study is to figure out the actual status of the dental accidents and malpractice disputes based on a questionnaire survey. The findings showed that 64.8% of respondents had been experienced the dental accidents, and 52.9% of respondents had been locked into the disputes. Within 5 years from the start of dental practice, the nearly half of the dentists responded the experience of the dental accidents and disputes. The amount of compensation for dental accidents and disputes wereindentalimplant, prosthodontics, and oral surgery treatments in descending order. The highest incident type was wrong process/treatment/procedure problem in clinical process/procedure.

소아청소년의 비약물적 행동유도 (Non-Pharmacological Behavior Guidance in Pediatric Dentistry)

  • Attitudes of parents toward behavior guidance techniques used in pediatric dentistry have changed for decades. Some techniques were regarded acceptable with authority of dentists' assistance, but now face the objection of caregivers. For the way of breeding is changing, dentist should understand the situation and try to communicate with caregiver for eliminating misunderstanding of the procedures that could induce lawsuit for abusive techniques. Most of all, dentist should try to find the best method for the patient and help to overcome the fear and anxiety. The meaningful knowledge driven from clinical experience should be shared with other dentists to build up sound theoretical foundation.

일부 고등학생의 치과불안도 평가 연구 (Dental clinic fear in high school students)

  • Objectives : The aim of this study is to investigate dental clinic fear and anxiety in high school students. Methods : The subjects were 311 high school students aged 19 years. They completed self-reported questionnaire including general characteristics, dental clinic experiences, dentist or dental hygienist confidence, and dental fear. Data were analyzed using SPSS 18.0. Results : Poor oral health status showed negative attitude towards dental clinic visit. Girl students tended to have more fear than boy students. Muscle tension was the most common experience and was closely related to anesthetic needles and drills. Dental anxiety was caused by lack of confidence to dentists and dental hygienists. Students were more afraid of dentists than dental hygienists. Higher score of dental anxiety seemed to be associated with infrequent visit to dental clinic. Conclusions : Frequent regular dental checkup may reduce dental fear and anxiety. Dental clinic staff should try to minimize dental anxiety in performing treatment.

변화하는 사회와 치과의사의 공공기관 참여 제한 (Proposal for various Social Field Participation of Korean Young Dentist)

  • The purpose of this study is to discuss the economic outlook of dentistry in relation to pessimistic economic perspectives of Korea and to present subsequent solutions. The expectation of the future economic growth rate of Korea is gloom with not only a love rate of increase in population due to an aging society and a low birthrate, but also with a declined number of productive populations. Moreover, the future of dentistry in Korea is obscure. Even with a dwindling population growth, an average of 750 new dentists graduate from dental school every year. The 30-35% of the new graduates practice in public hospitals; and 65~70% of them open up their own private practices, However, unlike in the past, the occurrence of dental practice bankruptcy has been increasing with a competitive environment, excessive initial investment, knowledgeable patients, and etc. Therefore, it is essential to re-evaluate the number of new graduates. Also, it is necessary to supply a greater number of public hospitals with newly licensed dentists, who have various clinic experience, participate in research and development, and experience appropriate hospital management skills.

섭식연하장애의 이해와 실제 (Understanding and Practice of Eating-Swallowing Disorder)

  • Eating-swallowing disorder (dysphagia) is a very important functional problem for the elderly, and it has a significant impact on the quality of their life. Because the eating and swallowing processes are affected by oral tissues including teeth, tongue, and oral muscles, it is natural that the dentist as a specialist in oro-facial region, intervenes in the diagnosis and treatment of the disorder. Nevertheless, Korean dentists still lack interest and understanding of dysphagia. In aged society, it is necessary that the dentists understand the functional disorders as well as oral diseases. The purpose of this study is to introduce the evaluation and treatment methods of eating and swallowing functional disorders in order to cope with eating-swallowing disordered patients who are frequently encountered in aged society.
