상아질 접착 시스템의 국내 사용 동향에 관한 연구 (Survey study on the using state of dentin bonding systems in Korea)

  • Dentin bonding systems are unique dental materials which are rapidly evolute and essential materials for bonding of restorative materials to dentin. Now, 4th generation, 5th generation, 6th generation and 7th generation of dentin bonding systems are used in clinic. We investigated the frequency, motivation of choosing dentin bonding systems and satisfaction of them by survey in 2014. 5th generation of dentin bonding systems are mostly used in Korea and young dentist have tendency to choose newly developed dentin bonding systems.

산 표면처리된 상아질 표층의 교원섬유 용해가 레진-상아질간 결합에 미치는 영향 (EFFECT OF COLLAGEN DISSOLUTION IN ACID CONDITIONED DENTIN LAYER ON RESIN-DENTIN ADHESION)

  • The effect of collagen dissolution in acid conditioned dentin layer on resin - dentin adhesion was investigated. 160 freshly extracted human molars were divided into 4 groups randomly and dentin surfaces were exposed. 40 exposed dentin surfaces were not acid conditioned and each 10 of them were applied with bonding agents within dentin bonding systems of All Bond 2, Scotchbond Multipurpose, Clearfil Photobond and Superbond D - Liner respectively. Each 10 of another 40 exposed dentin surfaces were acid conditioned by the acid within the above four bonding systems respectively and applied with corresponding bonding systems. After acid conditioning of the other 40 exposed dentin surfaces as above, they were treated with 5% NaOCl for 2 minutes, and each 10 of them were applied with the above four dentin bonding systems respectively. The remaining 40 dentin surfaces were acid conditioned and treated with 10% NaOCl for 2 minutes, and each 10 of them were applied with corresponding bonding agents as the above. After the procedures were finished, composite resin (Z -100, 3M Dent. Prod., USA) were applied on the dentin surfaces and light cured. Shear bond strength values were measured. Surface changes of fractured dentin specimens were observed using SEM (Hitachi S-2350, Japan). The following results were obtained. 1. In all of dentin bonding systems, shear bond strengths of non - conditioned specimens were significantly lower than those of acid conditioned specimens (P<0.05). 2. A statistically significant difference of bond strengths did not exist between acid conditioned specimens and 5% NaGCI retreated specimens applied with All Bond 2, Scotchbond Multipurpose and Clearfil Photobond (P>0.05). However, strength values of 5% NaOCl retreated specimens applied with Superbond D - Liner were lower than those of acid conditioned specimens (P<0.05). 3. In all the applied dentin bonding systems except Clearfil Photobond, bond strengths of 10% NaOCl retreated specimens were lower than those of acid conditioned and 5% NaOCl retreated specimens (P<0.05). 4. The resin - dentin hybrid layer of 4 - $5{\mu}m$ thickness was formed in the acid conditioned specimens applied with All Bond 2, Scotchbond Multipurpose and Superbond D-Liner. 5. The resin - dentin hybrid layer of 3 - $4{\mu}m$ thickness was still formed in the 5% NaOCl retreated specimens applied with All Bond 2 and Scotchbond Multipurpose. In addition, this layer was not completely removed after the retreatment with 10% NaOCl. Above results indicate that the dissolution of collagen in acid conditioned dentin layer by NaOCl solution can not be achieved completely and the collagens contribute to the resin - dentin adhesion considerably.

  • The aim of this study was to determine the shear bond properties of four dentin bonding systems to internal cervical dentin, and to investigate the effect of the pretreatment for removing smear layer and position of dentin on shear bond strength of dentin bonding agents. The materials tested in this study were consisted of four commercially available dentin bonding systems[Allbond 2(AB), Clearfil Linerbond 2(CL), Optibond FL(OP), Scotchbond Multi-purpose(SB)], a restorative light-cured composite resin[Z100]J and a chelating agent[RC-prep(RC)]. Fifty-six freshly extracted human molars were used in this study. Dentin specimens were prepared by first cutting the root of the tooth 1mm below the cementoenamel junction with a diamond bur in a high speed handpiece under air-water coolant, and then removing occlusal part at pulp horn level by means of a second parallel section, The root canal areas were exposed by means of cutting the dent in specimens perpendicular to the root axis. Dentin specimens were randomly assigned to two groups(pretreated group, not-pretreated group) based on the pretreatment method of dentin surface. In pretreated group, RC was applied to dentin surface for 1minute and then rinsed with NaOCl. In not-pretreated group, dentin surface was rinsed with saline Each groups were subdevided into four groups according to dentin bonding systems. Four dentin bonding systems and a restorative resin were applied according to the directions of manufacturer. The dentin-resin specimens were embedded in a cold cure acrylic resin, and were cut with a low speed diamond saw to the dimension of $1{\times}1mm$. The cut specimens were divided into three groups according to the position of internal cervical dentin. The shear bond properties of dentin-resin specimens were measured with Universal testing machine (Zwick, 020, Germany) with the cross head speed of 0.5mm/min. From this experiment. the following results were obtained : 1. In case of shear bond strength, there was no significant difference among dentin bonding systems in not-pretreated groups, whereas in pretreated groups, the shear bond strengths of AB and of SB were statistically significantly higher than those of CL and of OP. 2. The shear bond strengths of AB and of SB in pretreated groups were significantly higher than those in not-pretreated groups. 3. The shear bond strengths of radicular layer of OP were higher than those of occlusal layer of OP in not-pretreated groups, and of AB in pretreated groups. The shear bond strengths of radicular layer of AB and of CL in not-pretreated groups were higher than those in pretreated group.

자가부식형 상아질 접착제와 상아질과의 전단결합강도 비교 (Comparison of the shear bond strength of self-etching dentin bonding agents to dentin)

  • The purpose of this study was to ascertain the bonding durability of self-etching dentin bonding agents to dentin by means of shear bonding strength. Several acid-etching dentin bonding system (ESPE Z100) and self-etching dentin bonding systems (DEN-FIL, GRADIA DIRET) were used. The occlusion surface of human molars were ground flat to expose dentin and treated with the etch bonding system according to manufactures instruction and followed by composite resin application. After 24hours of storage at 37$^{\circ}C$, the shear bonding strength of the specimens was measured in a universal testing machine with a 1mm/min crosshead speed. An one-way analysis of variance and the scheffe test were performed to identify significant differences (p<0.05). The bonded interfacial surfaces and treated dentin surfaces were examined using a SEM. Through the analysis of shear bond strength data and micro-structures of dentin-resin interfaces, following results are obtained. In dentin group, the shear bond strength of DEN-FIL showed statistical superiority in comparison to the other groups and followed by ESPE Z100 and GRADIA DIRECT (p<0.05).

상아질에 대한 저점도 복합레진의 자가접착에 관한 연구 (SELF-ADHESION OF LOW-VISCOSITY COMPOSITES TO DENTIN SURFACE)

  • The objectiveness of this study was to evaluate whether low-viscosity composite can bond effectively to dentin surface without bonding resin. The low-viscosity composites being 50wt% filler content were made by the inclusion of bonding resin of two self-etching systems(Cleafil SE Bond, Unifil Bond) varied with contents as 0, 10, 20, 30, 40, 50wt%. Exposed dentin surfaces of extracted 3rd molars are used. Dentin bond strengths were measured. The tests were carried out with a micro-shear device placed testing machine at a CHS of 1mm/min after a low-viscosity composite was filled into an iris cut from micro tygon tubing with internal diameter approximately 0.8mm and height of 1.0mm. 1 Flexural strength and modulus was increased with the addition of bonding resin. 2. Micro-shear bond strength to dentin was improved according to content of bonding resin irrespective of applying or not bonding resin in bonding procedure, and that of Clearfil SE Bond groups was higher than Unifil Bond. 3. There were no significant difference whether use of each bonding resin in bonding procedure for S-40, S-50, U-50(p>0.05). 4. In SEM examination, resin was well infiltrated into dentin after primed with self-etching primer only for S-50 and U-50 in spite of the formation of thinner hybrid layer. Low viscosity composite including some functional monomer may be used as dentin bonding resin without an intermediary bonding agent. It makes a simplified bonding procedure and foresees the possibility of self-adhesive restorative material.

복합레진 수복시 복합용기 및 단일용기 상아질 접착제의 미세변연누출에 관한 연구 (MICROLEAKAGE OF CURRENT DENTIN BONDING SYSTEMS)

  • The purpose of this study was to evaluate the microleakage of 5 current dentin bonding systems which are composed of 2 multi-bottle systems(Scotchbond Multi-Purpose, All Bond2) and 3 one-bottle systems(Single bond, One-Step, Prime & Bond). In this in vitro study, class V cavities were prepared on buccal and lingual surfaces of sixty extracted human premolars and molars on cementum margin. The experimental teeth were randomly divided into six groups of 10 samples (20 surfaces) each, Group 1 : Scotchbond Multi-Purpose ; Group 2 : All Bond 2 ; Group 3 : Single Bond ; Group 4 : One-Step ; Group 5 : Prime & Bond ; Group 6 : no bonding agent(control). The bonding agent and composite resin were applied for each group following the manufacturer's instructions. After 500 thermocycling between $5^{\circ}C$ and $55^{\circ}C$, the 60 teeth were placed in 2% Methylene blue dye for 24 hours, then rinsed with tab water. The specimen were embedded in clear resin, then sectioned buccolingually through the center of restoration with a low speed diamond saw. The dye penetration on each of the specimen were then observed with a stereomicroscope at ${\times}20$. The results of study were statistically analyzed using the Student-Newmann-Keul's Methods and the Mann-Whitney Rank Sum Test. The resin/dentin interfaces were examined under Scanning Electron Microscopy. The results of this study were as follows. 1. None of the dentin bonding systems used in this study showed significant difference in leakage values at both the enamel and the dentin margins (P>0.05). 2. In all groups except the control, leakage value seen at the enamel margin was significantly lower than that seen at the dentin margin (P<0.05). 3. Compared to the control group, all the groups treated with dentin bonding systems showed significantly lower leakage value at both enamel and dentin margins (P<0.05). 4. In the SEM view, gaps were observed in the composite resin / dentin interface in group 6 where no dentin bonding agent was used, and in all the other groups (group 1, 2, 3, 4, 5) composite resin, hybrid layer, and dentin were seen to be closely adhering to each other where there were no leakages. Well-developed resin tags 3~100${\mu}m$ in length infiltrated dentinal tubules past the hybrid layer and a hybrid layer 1~5${\mu}m$ thick had developed between the dentinal surface and the composite resin surface.

불소도포가 상아질 접착에 미치는 영향 (EFFECT OF FLUORIDE APPLICATION ON DENTIN BONDING)

  • The purpose of this study was to investigate the effect of stannous fluoride on the dentin bonding with three kinds of commercially available dentin bonding systems containing different adhesive monomers. Dentin specimens with exposed labial dentin prepared from freshly extracted bovine mandibular anterior teeth were divided into experimental and control groups. The specimens of experimental groups were bonded with dentin bonding systems and composite resins including All bond 2 ㅡ& Bisfil, Scotchbond Multi-Purpose & Z100, and Denthesive II Charisma after 2 % stannous& fluorided application for S minutes and washing for 1 minute. The specimens of control groups were bonded with the same dentin bonding systems and composite resins as used in the experimental groups. After bonded specimens were stored in $37^{\circ}C$ distilled water for 24 hours, the tensile bond strength and cohesive failure rate were measured, and then the pretreated dentin surfaces and the fractured dentin surfaces were examined under scanning electron microscope. The results were as follows : Mean bond strength of stannous fluoride applied groups of All bond 2, Scotchbond MP, and Denthesive II were 2.5MPa, 1.1MPa, and 1.1MPa respectively, and those of control groups were 7.5MPa, 8.1MPa, and 4.6MPa. Bond strength values of stannous fluoride applied groups were significantly lower than those of the control groups(p<0.05). SEM findings of dentin surfaces after stannous fluoride application demonstrated an appearance of partially remained smear layer and smear plugs inspite of pretreatment with 10 % phosphoric aicd or maleic acid solution, and an appearance of smear layer covered surface under Denthesive II priming. But those of control groups commonly showed clean dentin surfaces without smear layer and smear plugs. On SEM observation of the fractured dentin-resin interface, while most of the specimens of stannous fluoride applied groups showed adhesive failure mode, those of All bond 2 and Scotchbond MP control groups showed mainly adhesive-cohesive mixed failure mode, and mainly adhesive failure mode in Denthesive II control group.

단일용기 상아질 접착제 처리 후 레진-상아질 경계면에 대한 투과전자현미경적 연구 (A TEM STUDY OF THE RESIN-DENTIN INTERDIFFUSION ZONE FORMED BY ONE-BOTTLE DENTIN ADHESIVE SYSTEMS)

  • One bottle system was recently developed in order to simplify the clinical skills and save chair time after continuous improvements on dentin bonding agents. There has been many studies to measure the bond strength of one bottle systems but no actual work has been done on micromorphologic study of resin-dentin interdiffusion zone after one bottle system application. To evaluate the bonding patterns of various commercially available one bottle systems to dentin, observation of resin-dentin interdiffusion zone under TEM was performed. Caries-free human third molars within one month of extractions were chosen for the experiments. The molars were sectioned 1mm above the cementoenamel junction and got rid of the root portions. Crown portions of the teeth were sectioned parallel to occlusal surface so that dentin discs of 1mm in thickness were remained. 7 one bottle systems and 1 two bottle system were applied according to manufacturer's instructions and followings were the results. 1. In every experimental groups, cross bandings of collagen fiber were distinguishable and tight bon dings between the bonding agents and dentin were observed. 2. Hybrid layer was clearly observed in ONE-STEP$^{(R)}$, Prime & Bond$^{(R)}$ 2.1, Syntac$^{(R)}$ SC, MAC-BOND II groups but it was not clear in Single Bond, D-Liner Dual PLUS, ONE COAT BOND groups. 3. Electron-density of hybrid layer was uniform in pattern in MAC-BOND II, Prime & Bond$^{(R)}$ 2.1 groups but not so uniform in ONE-STEP$^{(R)}$ group. 4. Electron-dense amorphous phase in most superior layer of the resin-dentin interdiffusion zone was characteristically observed in Single Bond, Syntac$^{(R)}$ SC, ONE COAT BOND groups. It can be concluded that bondings between the dentin bonding agents and dentin can be various in pattern according to their chemical compositions and the condition during applications.

유치 상아질에 대한 수종의 상아질 결합제의 전단결합강도에 대한 연구 (THE STUDY ON SHEAR BOND STRENGTH OF VARIOUS DENTIN BONDING SYSTEMS IN PRIMARY DENTIN)

  • 소아환자의 수복치료에서 시술시간과 시술과정을 줄이는 것은 중요한 점이다. 복합레진은 물성의 증가로 영구치, 유치 전치부 심미수복재로써 뿐만 아니라 구치부 수복에도 많이 쓰이고 있다. 최근에 개발된 7세대 상아질 결합제는 기존의 복합레진을 치면에 부착하기 위해 필요했던 산부식, 프라이밍, 접착제 도포의 복잡한 과정을 한번의 처리로 단순화하였다. 본 연구에서는 유치 상아질에서 7세대 상아질 결합제의 결합력을 기존에 사용되고 있는 여러 결합제의 결합력과 비교하였다. 1, 2, 3, 4군은 각각 4, 5, 6, 7세대 상아질 결합제를 사용하였다. 유치 상아질을 노출시키고 군에 따라 상아질 면을 처리한 후에 직경 2.5mm, 높이 2.0mm의 주형을 이용하여 복합레진을 접착시키고 열순환을 시행한 다음 전단결합강도를 측정하여 다음과 같은 결론을 얻었다. 1. 5세대 상아질 결합제인 Clearfil SE Bond가 가장 높은 전단결합강도인 $27.87{\pm}5.33\;MPa$를 보였다. 이것은 4, 6, 7세대와 비교했을 때 통계학적으로 유의하였다. 2. 4세대 상아질 결합제의 전단결합강토는 $20.11{\pm}5.73\;MPa$로 6, 7세대보다는 약간 높게 나타났지만 통계적으로 유의한 차이는 보이지 않았다. 3. 6, 7세대 상아질 결합제의 전단결합강도는 유사하게 나타났다.

유지놀 처리된 상아질 표면의 상아질 접착제의 전단결합강도에 관한 연구 (SHEAR BOND STRENGTH OF DENTIN BONDING AGENTS ON DENTIN SURFACE TREATED WITH EUGENOL)

  • In this study, shear bond strength of two bonding systems were measured and appearance of dentin surfaces were observed with SEM according to the storage time of eugenol on dentin surface, thus evaluated the effect of eugenol on bond strength of two dentin bonding systems. Control groups were directly bonded to dentin surface with One Step, Prime & Bond 2.1. Experimental groups were divided into experimental I and II according to dentin bonding agents. After eugenol application, dentin surfaces were bonded with One Step and Prime & Bond 2.1 according to the each storage time of immediately, 3min, 24hour, 48hour and 1week, and then control and experimental groups were filled with light cured composite resin(Z-100). After 24 hours water storage at $37^{\circ}C$, all samples were subjected to a shear load to fracture at a cross head 1.0mm/min with Instron universal testing machine(No. 4467). Etched dentin surface storaged for each time of immediately, 3min, 24hour, 48hour and 1 week after eugenol application were observed under Scanning Electron Microscope(Hitachi S-2300) at 20kvp. The data were evaluated statistically with two-way ANOVA and Tukey's HSD. The results were as follows; 1. Shear bond strengths were higher in control groups than in the experimental groups. 2. As storage time was increased after eugenol application, the shear bond strengths were decreased in experimental groups. 3. In experimental I, II the shear bond strengths were the lowest in which storage time was 1 week after eugenol application. 4. As storage time was increased after eugenol application, etched dentin surfaces showed obstructed dentinal tubule.
