Peripheral Nerve Injuries Related to Local Dental Clinic Anesthesia in the Dental Clinic (치과에서 발생하는 국소마취에 의한 신경손상)

  • Local anesthesia known as the safe and essential procedure to control pain in dentistry may cause sensory changes such as paresthesia or altered taste at the affected sites after even successful local anesthesia. Although the prognosis of the nerve injuries after local anesthesia is favorable, it might cause prolonged problems such as dysesthesia. The lingual nerve is a single fascicle at the level of the lingual among 1/3 of patients and more movable during regeneration compared to the inferior alveolar nerve after the injury. As a result, the lingual nerve is more vulnerable and has poorer outcomes. More vigilant clinical considerations are required to the lingual nerve injury after local anesthesia. Generally, more than 80% of cases are spontaneously resolved within 2 weeks after the local anesthesia even without any specific treatment. However, the patient having long lasting abnormal sensations more than 2 weeks needs specialists' care for further assessment. In case of dysesthesia which is a symptom of neuropathic pain, immediate referral to specialists is mandatory. The exact mechanism, how to prevent its occurrence, or specific treatments of the nerve injury related to the local anesthesia have not been elucidated. To prepare clinical or medicolegal problems, many cautious considerations are given to the patients who complain sensory changes after local anesthesia.

MIDAZOLAM ORAL PREMEDICATION IN AUTISTIC PATIENT WHO IS AFRAID TO ENTER DENTAL OFFICE -A CASE REPORT- (치과 진료실에 들어오지 않으려는 자폐환자에서 미다졸람 경구투여 -증례보고-)

  • Dental treatment of mentally challenged patients under general anesthesia is a series of challenging procedures not only for dental operators but also for dental anesthesiologists. Patients presenting with uncooperative behavior often resist the perioperative management for adesthestic administration. This case report suggests oral premedication as a conjuctive method for anestheitic induction. A 26-year-old male dental patient with autism was referred to dental treatment under general anesthesia. The patient refused to enter dental clinic office and was not able to receive preoperative assessment. In the day of operation, 15 mg of midazolam was given to the patient for oral premedication prior to anesthetic induction. Ater 20 minutes, the patient presented with drowziness and was transferred to the office. Anesthestic staff were able to achieve appropriate intravenous access and mask inhalation. The patient recieved 8 hrs long dental treatment and recovered in a noncomplicated way. Oral midazolam is commonly used to reduce anxiety for combative and irritated pateints. In this case, oral midazolam sedation was used as a preanesthetic management of a highly uncoopearive patient.

Study on the perception of health science college students about the personal protection of dental personnels (치과 의료종사자의 개인보호장구 착용에 대한 보건계열 대학생의 인식)

  • Objectives : This study was performed to enhance infection control and to establish the fundamental and practical improvement by the survey on personal protection of dental personnels with health science college students. Methods : We performed the survey with 585 individual students affiliated with health science in Jeonbuk province and statistically analyzed by SPSS 12.0 program. Results : The frequencies of the clinic gown as a personal protection were 96.0% of dentists (p<0.01) and 88.0% of dental hygienists (p<0.05) respectively in college-associated dental hospital when we compared with institution scale. The frequencies of the medical gloves as a compulsory personal protection were 47.2% of dentists and 34.6% of dental hygienists respectively (p<0.001). The frequencies of the medical mask as a compulsory personal protection were 84.0% of dentists (p<0.001) and 52.2% of dental hygienists (p<0.05) respectively shown by freshman students. The frequencies of the safety glasses as a compulsory personal protection were 12.3% of dentists and 2.5% of dental hygienists shown by freshman students (p<0.001). Conclusions : The results reflect that current dental personnel's concern of personal protection need to be increased continuously in accordance with current demands and eventually suggest their effort to put their action on personal protection in their dental office.

Oral health knowledge and behavior of visiting health care personnel (방문건강관리인력의 구강건강지식과 실천)

  • The purpose of this study was to survey visiting health care personnel who worked for their public medical health institutions and thereby investigate their oral health knowledge and behavior. Findings of this study are summarized as follow; First, our visiting health care personnel had a significant tendency toward higher score at correct answers to questions about oral health knowledge (mean knowledge about periodontal disease: 67%, mean knowledge about dental caries: 68%, mean knowledge about common dentistry: 68.4% or higher). Secondly, in terms of oral health care behavior, 50% or higher of all our visiting health care personnel visited dental clinic for preventive purposes, and 60.1% visited dental clinic for oral treatment. 46.6% of health care personnel relied on dental clinic for regular scaling services, and 89.6% relied on general hospital or dental clinic for oral treatment. 89.5% of health care personnel brushed their teeth 3 times or more per day; more than half (58%) of them preferred dental floss to other oral hygiene supplies; and 46% practiced roll technique to brush their teeth. Thirdly, health care personnel's oral health knowledge was significantly correlated with their age (F=5.25, p<0.01) and career of health care (F=3.94, p<0.01), while their oral health behavior was significantly associated with their career of visiting health care (F=3.20, p< 0.05).

The Study for Treatment of Temporo-mandibular Joint Pain (악관절의 통증에 관한 연구)

  • Temporomandibular disorders typically present findings of limited or asymmetric patterns of jaw opening and joint sounds usually described as clicking, popping, grating, or crepitus. Recently, patients with temporomandibular disorders have received an increasingly aggressive treatment with a greater emphasis on surgical and dental reconstruction. Scientific studies have not clearly identified the specific causes of the temporomandibular disorders and therefore some of the treatments are empiric, without a firm scientific foundation. We carried out a study on the patients of pain clinic OPD and concluded that the causes of the temporomandibular joint(TMJ) pain are the prolonged contraction of the muscles of mastication, especially the masseter muscle. Therefore, the spasmolytic treatment of masseter muscle would be a better treatment for TMJ syndrome rather than the surgical and dental reconstruction.

Analysis of the positive word-of-mouth, Quality of medical service and customer satisfaction of patients in dental clinics (치과의원 내원환자들의 긍정적 구전과 치과 의료서비스 품질 및 고객만족도 분석)

  • Dental health care in modern society has gradually been developing with patients'expectation, desire, and medical knowledge according to the changes in the medical industry. Dental marketing strategy is necessary to construct a better dental health environment in order to cope with customers' changes and desire. The purpose of this study was to evaluate the positive comments by word-of-mouth experience, quality of dental clinic service and customer satisfaction among dental patients. The study surveyed 5oo subjects being treated at 6 dental clinics in Seoul and Gyeonggi province. It was conducted from March 22 to April 9, 2010. The subject groups with a positive experience through word - of - mouth who had a perception about the quality of dental clinic service (p<.001) and customer satisfaction (p<.001) were higher than other groups who had no comments on their dental treatment and clinics. The results showed that there was a correlation(r=. 852, p<.001) between the positive comments by word-of- mouthabout the quality of dental clinics service and customer satisfaction. The perception of dental clinic service and patients' satisfaction with positive comments by word of mouth was highly recognized by older and more experienced patients of dental treatment. Therefore, the results suggest that empirical information on important factors about strategic marketing lead to positive comments by word of mouth experience for patients in dental clinics.

A Survey on Mothers' Awareness of Nursing Bottle Caries according to Infants' Age (유아의 연령에 따른 어머니들의 우유병우식증에 대한 인식도 조사)

  • Dental caries in infants is a representative oral disease causing the malocclusion of permanent dentition and developmental variations in the permanent teeth, so it is very important to manage caries in infants. Thus, in order to survey mothers who visited pediatric dentistry concerning their awareness of nursing bottle caries and to obtain data necessary for developing education materials to prevent nursing bottle caries, the present study conducted a survey using a self reporting questionnaire with 205 mothers who visited the Pediatric Dentistry of a dental hospital in Cheonan with a child aged below 5 during the period from June 7 to August 2 in 2008, and obtained results as follows. 1. The awareness of dental caries was highest in the group aged over 37 months 96.8%, and next in order of age 13~24 months 96.5%, 25~36 months 94.5%, and younger than 13 months 83.3%. 2. The awareness of nursing bottle caries was highest in infants aged over 37 months 58.7%, and next, in 13~24 months 57.9% and in 25~36 months 54.8%, and lowest in infants younger than 13 months 33.3%, suggesting the necessity of education on nursing bottle caries(p=0.423). 3. As to the time of the first visit to dental clinic, 50.0% of infants younger than 13 months, 52.6% of those aged 13~24 months, and 49.3% of those aged 25~36 months replied the completion of the eruption of primary teeth, and 36.5% of those aged over 37 months replied the occurrence of dental caries. Thus, the motives for visiting dental clinic were different among the age groups, and the differences were statistically significant(p=0.005). 4. The age of toothbrushing to be started by mother's opinion was 6 months in 0.5%, 12 months in 68.8%, 18 months in 22.9%, 24 months in 5.9%, and 25 months in 1.0%, and no statistically significant difference was observed according to age. 5. The number of toothbrushing was usually once a day after meal. and increased by in age was shown statistically significantly difference(p=0.035). 6. The most frequent method of preventing nursing bottle caries was regular toothbrushing in all the age groups, and next restriction of sugar intake, regular dental consultation, and fluoridation in order The most effective prevention of dental caries was regular visit to dental clinic and next, toothbrushing after meal.

A Study on the Dental Hygienists' Reactions to Noise When Occurred in Dental Clinic (치과병원에서 발생하는 소음에 대한 치과위생사의 반응)

  • The purposes of this research were to evaluate the relationships of between characteristics of noise and annoyance of dental hygienist by noise in dental clinic. To investigate the dental clinic workers' reactions to noise when occurred in dental clinic, the noise level test in dental clinic and questionnaire were taken. As a result of noise evaluation, It shows that the range of noise level was 67.7~78.3dB(A) and frequency was very high (more than 4KHz). It's seem to be begins occurrence of stamina-loss, contraction of peripheral blood vessel, decrease of adrenocortical hormones. Most of respondents were affected by noise: 67% of respondents were nervous about noise and the rest of respondents were bearable. Analysis by NR-curve showed that it was exceed the noise permit level in working space. As a result of correlation - test, the more exposed dental hygienist to noise, the more felt the unpleasantness and fatigue. It's hard to sufficient explanation to patients about the dental treatment. So it's thoughts that insufficient explanation will negative impact on the patients' satisfaction and increase competitiveness in dental clinics. To remedy a unpleasantness and fatigue of noise in dental hygienist, it's considered that making an offer the ear protection and choosing the low noise-vib. equipment and using the masking effect. Therefore, It can be provide a pleasant working environment with dental hygienist and It will have a great advantage to dental clinics to improve their competitiveness.

An Influence on Dental Care Satisfaction in College Women of Some Areas (일부지역 여대생의 치과의료 서비스 만족도에 미치는 영향)

  • The purpose of this study was to examine factors related to the satisfaction of dental service in college women. This survey was conducted between March 14, 2014 and April 21, 2014 to investigate the influence on dental care satisfaction in college women. Data were obtained from 412 college women of some areas. At the conclusion of this investigation, as for the correlation analysis between variables affecting college women satisfaction, every variables showed significantly positive relation, and the relation between satisfaction of dental hygienist and satisfaction of hospital environment had the strongest positive relation (p<0.01). To increase the satisfaction in dental clinic, it is necessary to provide the continuing education programs for dental service to dental clinic worker.

A survey of dental treatment under general anesthesia in a Korean university hospital pediatric dental clinic

  • Background: In South Korea, the number of cases of dental treatment for the disabled is gradually increasing, primarily at regional dental clinics for the disabled. This study investigated pediatric patients at a treatment clinic for the disabled within a university hospital who received dental treatment under general anesthesia. This data could assist those that provide dental treatment for the disabled and guide future treatment directions and new policies. Methods: This study was a retrospective analysis of 263 cases in which patients received dental treatment under general anesthesia from January 2011 to May 2016. The variables examined were gender, age, reason for anesthesia, type of disability, time under anesthesia, duration of treatment, type of procedure, treatment details, and annual trends in the use of general anesthesia. Results: Among pediatric patients with disabilities who received dental treatment under general anesthesia, the most prevalent age group was 5-8 years old (124 patients, 47.1%), and the primary reason for administering anesthesia was dental anxiety or phobia. The mean time under anesthesia was $132.7{\pm}77.6min$, and the mean duration of treatment was $101.9{\pm}71.2min$. The most common type of treatment was restoration, accounting for 158 of the 380 treatments performed. Conclusions: Due to increasing demand, the number of cases of dental treatment performed under general anesthesia is expected to continue increasing, and it can be a useful method of treatment in patients with dental anxiety or phobia.
