PURPOSE. The purpose of this study is to evaluate and compare the shear bond strength of the gingiva-colored composite resin and the tooth-colored composite resin to porcelain, metal and zirconia. MATERIALS AND METHODS. Sixty cylindrical specimens were fabricated and divided into the following 6 groups (Group 1-W: tooth-colored composite bonded to porcelain, Group 1-P: gingiva-colored composite bonded to porcelain, Group 2-W: tooth-colored composite bonded to base metal, Group 2-P: gingiva-colored composite bonded to base metal, Group 3-W: tooth-colored composite bonded to zirconia, Group 3-P: gingiva-colored composite bonded to zirconia). The shear bond strength was measured with a universal testing machine after thermocycling and the failure mode was noted. All data were analyzed using the two-way analysis of variance test and the Bonferroni post-hoc test at a significance level of 0.05. RESULTS. The mean shear bond strength values in MPa were 12.39, 13.42, 8.78, 7.98, 4.64 and 3.74 for Group 1-W, 1-P, 2-W, 2-P, 3-W and 3-P, respectively. The difference between the two kinds of composite resin was not significant. The shear bond strength of Group 1 was the highest and that of Group 3 was the lowest. The differences among Group 1, 2 and 3 were all significant (P<.05). CONCLUSION. The shear bond strength of the gingiva-colored composite was not less than that of the tooth-colored composite. Thus, repairing or fabricating ceramic restorations using the gingiva-colored composite resin can be regarded as a practical method. Especially, the prognosis would be fine when applied on porcelain surfaces.
환자의 심미적 요구가 증가함에 따라 전치뿐만 아니라 구치부에서도 자연치아와 유사한 색조와 외형을 재현한 보철물에 대한 필요성이 증가하고 있다. 단일 지르코니아와 CAD/CAM 시스템을 통해 이러한 환자의 요구를 만족시키는 보철물 제작이 가능해졌다. 본 증례는 70세 여자환자로 전치부 보철물이 깨져서 보기 싫고 하악 구치부가 없어서 저작이 불편하다는 주소로 내원하였다. 하악 구치의 상실로 교합평면이 붕괴되었으며 과도한 치아 마모 및 수직고경 상실이 관찰되어 치아 상실 부위에 임플란트를 식립하고 수직고경 증가와 함께 지르코니아 고정성 보철물을 이용한 전악 수복 치료를 진행하였다. 치료 후 3년 간 이상적인 교합이 잘 유지되고 기능적, 심미적으로 만족한 결과를 얻었기에 이를 보고하는 바이다.
PURPOSE. To describe and characterize the surface topography and cleanliness of CAD/CAM manufactured zirconia abutments after steaming and ultrasonic cleaning. MATERIALS AND METHODS. A total of 12 ceramic CAD/CAM implant abutments of various manufacturers were produced and randomly divided into two groups of six samples each (control and test group). Four two-piece hybrid abutments and two one-piece abutments made of zirconium-dioxide were assessed per each group. In the control group, cleaning by steam was performed. The test group underwent an ultrasonic cleaning procedure with acetone, ethyl alcohol and antibacterial solution. Groups were subjected to scanning electron microscope (SEM) analysis and Energy-dispersive X-ray spectroscopy (EDX) to verify and characterize contaminant chemical characterization non- quantitatively. RESULTS. All zirconia CAD/CAM abutments in the present study displayed production-induced wear particles, debris as well as organic and inorganic contaminants. The abutments of the test group showed reduction of surface contamination after undergoing an ultrasonic cleaning procedure. However, an absolute removal of pollutants could not be achieved. CONCLUSION. The presence of debris on the transmucosal surface of CAD/CAM zirconia abutments of various manufacturers was confirmed. Within the limits of the study design, the results suggest that a defined ultrasonic cleaning process can be advantageously employed to reduce such debris, thus, supposedly enhancing soft tissue healing. Although the adverse long-term influence of abutment contamination on the biological stability of peri-implant tissues has been evidenced, a standardized and validated polishing and cleaning protocol still has to be implemented.

PURPOSE. Limited data is available regarding the differences for possible microleakage problems and fitting accuracy of zirconia versus titanium abutments with various connection designs. The purpose of this in vitro study was to investigate the effect of connection design and abutment material on the sealing capability and fitting accuracy of abutments. MATERIALS AND METHODS. A total of 42 abutments with different connection designs [internal conical (IC), internal tri-channel (IT), and external hexagonal (EH)] and abutment materials [titanium (Ti) and zirconia (Zr)] were evaluated. The inner parts of implants were inoculated with $0.7{\mu}L$ of polymicrobial culture (P. gingivalis, T. forsythia, T. denticola and F. nucleatum) and connected with their respective abutments under sterile conditions. The penetration of bacteria into the surrounding media was assessed by the visual evaluation of turbidity at each time point and the number of colony forming units (CFUs) was counted. The marginal gap at the implant- abutment interface (IAI) was measured by scanning electron microscope. The data sets were statistically analyzed using Kruskal-Wallis followed by Mann-Whitney U tests with the Bonferroni-Holm correction (${\alpha}=.05$). RESULTS. Statistically significant difference was found among the groups based on the results of leaked colonies (P<.05). The EH-Ti group characterized by an external hexagonal connection were less resistant to bacterial leakage than the groups EH-Zr, IT-Zr, IT-Ti, IC-Zr, and IC-Ti (P<.05). The marginal misfit (in ${\mu}m$) of the groups were in the range of 2.7-4.0 (IC-Zr), 1.8-5.3 (IC-Ti), 6.5-17.1 (IT-Zr), 5.4-12.0 (IT-Ti), 16.8-22.7 (EH-Zr), and 10.3-15.4 (EH-Ti). CONCLUSION. The sealing capability and marginal fit of abutments were affected by the type of abutment material and connection design.
Purpose: The purpose of this study was to evaluate the effects of various zirconia surface treatment methods on shear bond strength with resin cements. Methods: We prepared 120 cylindrical zirconia specimens (⌀10 mm×10 mm) using computer-aided design/computer-aided manufacturing (CAD/CAM). Each specimen was randomly subjected to one of four surface treatment conditions: (1) no treatment (control), (2) airborne-particle abrasion with 50 ㎛ of Al2O3 (A50), (3) airborne-particle abrasion with 125 ㎛ of Al2O3 (A125), and (4) ZrO2 slurry (ZA). Using a polytetrafluoroethylene mold (⌀6 mm×3 mm), we applied three resin cements (Panavia F 2.0, Super-Bond C&B, and Variolink N) to each specimen. The shear bond strength tests were performed in a universal testing machine. The surfaces of representative specimens of each group were evaluated under scanning electron microscope. We used one-way analysis of variance (ANOVA), two-way ANOVA, and post hoc Tukey honest significant difference test to analyze the data. Results: In the surface treatment method, the A50 group showed the highest bond strength, followed by A125, ZA, and control groups; however, no significant difference was observed between A50 and A125, A125 and ZA, and ZA and control (p>0.05). Among the resin cements, Super-Bond C&B showed the highest shear bond strength, followed by Panavia F 2.0 and Variolink N (p<0.05). Conclusion: Within the limitations of this study, application of airborne-particle abrasion and ZrO2 slurry improved the shear bond strength of resin cement on zirconia.
Purpose: The purpose of this study is to measure and compare the removal torques of different cements applied in attachments of zirconia restorations on titanium (Ti) abutments fitted with retrievable cement-type slot (RCS) on the lingual side for the better retrievablity by use of a slot driver. Materials and Methods: Three types of cements were used in the experiment: two permanent cements in $RelyX^{TM}$ U200 (RU) (3M ESPE) which is a resin cement and $FujiCem^{TM}$ (FC) (GC) which is a resin-modified glass ionomer cement, and a temporary cement in $Freegenol^{TM}$ temporary cement (TC) (GC). Measurements of removal torques were conducted as follows; an attached sample was fixed on the equipment customized for the experiment; a slot driver was connected to a MGT12 (Mark-10 Corp.), a torque measurement instrument; the sample had the driver fitted to its RCS and then was rotated until the it was removed; and finally, the maximum torque value was recorded. Result: As for the removal torque measurement results, the average values were $47.9{\pm}2.6Ncm$ for RU, $43.4{\pm}1.5Ncm$ for FC, and $20.9{\pm}1.0Ncm$ for TC. The statistical analysis using Kruskal-Wallis test yielded the significance probability of P<0.05 (P=0.002), which confirmed the presence of significant differences between the three groups. Conclusion: All three cements exhibit clinically acceptable levels of removal torque when applied to an upper zirconia implant restoration fitted with a lingual slot, with RU and FC, the two permanent cements, having the significantly higher values than that of TC, the temporary cement.
Although titanium-ceramic systems have gained substantial interests in dental prosthetic field, bonding problem between porcelain and titanium has not been solved. Main obstacle in titanium-porcelain bonding is excessive oxidation of titanium during porcelain firing. The effects of several coating materials on the bonding strength of titanium-porcelain system were investigated in this study. RF sputtering and electroplating of platinum significantly increased the bonding strength of porcelain-titanium specimen. However, coatings of Ni-Au, Ir, and ceramics(zirconia and hydroxyapatite) did not showed a significant effect on bonding strength. Platinum might be a promising material for the protective layer of excessive oxidation of titanium during porcelain firing, resulting in increase in the bonding strength.
연구 목적: 본 연구는 다양한 CAD/CAM 시스템에 의해 제작된 지르코니아를 이용한 3-unit 브릿지 코어의 적합도를 "Replica Technique"으로 측정하여 기존 제품들의 내면 및 변연 적합도를 평가해 각 시스템의 적합 특성을 알아보고 임상 적용에 참고 자료로 활용하고자 시행하였다. 연구 재료 및 방법: 상악 좌측 제1소구치를 결손치로 하고 상악 좌측 견치, 상악 좌측 제2소구치를 지대치로 하는 3-unit 고정성 가공의치를 구현한 티타늄 모형을 제작하고, 40개의 실험 모형을 제작하였다. 각 모형을 10개씩 분류하고 Cerasys$^{(R)}$ (C군), Dentaim$^{(R)}$ (D군), KaVo Everest$^{(R)}$ (K군), $Lava^{TM}$ (L군)시스템으로 각각 지르코니아 코어를 제작하였다. 제작된 코어는 각 지대치마다 내면에 16개의 점을 선택하고 "Replica Technique"을 이용하여 각 포인트의 간극을 측정하였고 부위별 측정치를 비교, 평가하였다. Two-way ANOVA와 Dunnett T3 analysis를 이용하여 통계 분석하였다(${\alpha}$=.05). 결과: 모든 시스템에서 변연의 외부보다 내부에서 조금 더 큰 간극을 보이는 특징을 보였다. K군의 경우 전반적인 적합도는 우수하였으나 절단부위의 간극이 아주 크게 나타나는 특징을 보였고, C군의 경우 변연부위의 간극은 K군와 비교하여 크지만(P<.05) 전반적인 내면 간극이 다른 시스템에 비해 균일한 특징을 보였다. 축벽의 간극에서는 모든 시스템, 모든 절단부위에서 $100\;{\mu}m$ 미만의 우수한 적합도를 보였다. L군와 C군의 경우 변연의 외측부와 내측부의 간극차이가 적었으나 KaVo Everest$^{(R)}$와 Dentaim$^{(R)}$의 경우 마진 내측이 외측보다 간극이 큰 특징을 보였다(P<.05). 변연과 내면적합도로 나누어 분석하였을 때 K군과 C군이 D군과 L군보다 상대적으로 더 좋은 변연 및 내면 적합도를 보였고(P<.05) 두 지대치 중 견치가 제2소구치보다 더 좋은 적합도를 보였다(P<.05). 결론: 대부분의 시스템에서 변연 부위의 적합도는 임상적으로 허용 가능한 범위에 있었다. 변연 간극과 내면 간극 모두에서 지대치 중에 견치에서보다 제2소구치에서 더 큰 간극을 나타내었다. 또한 대부분의 시스템에서 축벽보다는 교합면 쪽 간극이 상대적으로 크게 형성되었다.
목적: 본 연구의 목적은 지르코니아 전장관과 금전장관으로 수복된 임플란트 지지 고정성 보철물의 장착 후 초기 6개월간의 마모를 비교 분석하는 것이다. 연구 재료 및 방법: 2015년 1월부터 2016년 1월까지, 대구치 부위에 임플란트 지지 고정성 보철 수복을 요하는 환자를 대상으로 하였다. 총 47개에 대하여 임플란트 지지 고정성 보철물과 대합치에 대해 조사를 시행하였으며, 보철물 장착 1주 및 6개월 후 보철물 및 대합치를 인상채득 하여 교합면의 형태를 인기 하였다. 인상체를 스캔하여 보철물 및 대합치의 교합면 형태를 비교하여 마모 분석을 시행하였다. SPSS (Version 23.0; SPSS, IBM Corporation, Armonk, USA) 프로그램을 이용하여 정규성 분석 후 Mann-Whitney 분석을 시행하였다. 결과: 임플란트 지지 고정성 보철물에서 지르코니아 전장관과 금전장관 보철물의 6개월간 평균 수직거리 차의 중앙값은 각각 $50.84{\mu}m$, $42.84{\mu}m$로 통계적 유의차가 없었다(P > 0.05). 대합치가 자연치인 경우, 지르코니아 전장관 및 금전장관 보철물의 6개월간 평균 수직거리 차의 중앙값은 각각 $47.72{\mu}m$, $41.97{\mu}m$였으며, 대합치 법랑질의 평균 수직거리 차의 중앙값은 각각 $47.26{\mu}m$, $44.59{\mu}m$이었고, 통계적 유의차는 없었다(P > 0.05). 결론: 조사 기간이 짧았고, 실험군의 개수가 적었다는 단점이 있지만, 임플란트 지지 고정성 보철물 장착 후 초기 6개월간의 마모 비교에서 지르코니아 전장관과 금전장관은 유의한 차이가 없었고, 대합되는 자연치 법랑질의 마모도 유의한 차이는 없었다.
연구 목적: 본 연구는 지르코니아, 유리침투알루미나 및 금속도재소부용치관의 파절강도를 비교하고자 하였다. 연구 재료 및 방법: 지르코니아, 유리침투알루미나 및 금속도재소부용합금으로 각각 15개의 치관을 제작한 후, 제작된 치관을 장축에 $30^{\circ}$ 경사지게 제작된 지그에 장착하여 만능시험기로 파절강도를 측정하였으며, 전단결합강도 시험을 위하여 지르코니아, 유리침투알루미나 및 금속도재소부용합금을 $5.5({\phi}){\times}2.5mm$ 크기로 제작하고 포세린 분말을 $3.5({\phi}){\times}2.5mm$ 크기로 축성한 후 제조사의 설명서에 따라 전기로(Ceramco 7, Dentsply, USA)에서 소결하여 총10개의 시편을 제작하였다. 결과: 전부관 형태로 제작한 시편의 파절강도는 금속-도재 시스템이 $569.1{\pm}61.8N$, 알루미나-도재 시스템이 $551.0{\pm}76.5N$ 및 지르코니아-도재 시스템이 $588.3{\pm}49.6N$으로 측정되었으며, 각 실험군간 유의한 차이는 없었다(P>.05). 전단결합강도는 금속-도재 시스템이 $38.9{\pm}5.0MPa$, 알루미나-도재 시스템이 $39.4{\pm}5.1MPa$, 지르코니아-도재 시스템이 $25.5{\pm}5.6MPa$로 지르코니아-도재 시스템이 다른 두 시스템 보다 유의하게 낮은 값을 보였다(P<.05). 금속-도재 시스템, 알루미나-도재 시스템 및 지르코니아-도재 시스템의 각 결합계면을 SEM/EDS로 분석한 결과 각 시스템에서 상이한 결합 양상이 관찰되었다. 결론: 본 연구 결과 지르코니아-도재 시스템의 파절강도가 제일 높았으며, 금속-도재 시스템, 알루미나-도재 시스템 순서로 나타났으며, 전단결합강도는 알루미나-도재 시스템이 제일 높았고 금속-도재 시스템, 지르코니아-도재 시스템 순서으로 나타났다.
