  • Dental implant has recently become one of the most viable treatment options for regaining the oral function and dental aesthetics compared with conventional fixed or removable dentures. Dental implants vary in material, dimensions, geometries, surface properties, and interface geometries. It has been reported that there has been a proliferation of manufacturers who produce implants using various materials and surface treatments, and the dentist needs to select from over 2,000 different dental implants and abutments in a specific treatment situation. Unfortunately, however, no metrics have been specifically identified for the purpose of quality assessment and selection of an appropriate dental implant. This study aims to provide practical guidelines for quality assessment of dental implant based on clinical data. Like other medical devices and materials, the superiority related to specific characteristics of the dental implant needs to be verified through extensive clinical studies. The procedures of clinical monitoring for dental implants have been proposed along with a case study to exemplify the usefulness of clinical monitoring for the purpose of continuous quality improvement in medical industry.

  • The aim of this study was to clarify the barriers of dental treatment for special needs patients felt by dentists and to determine the dentist-related factors contributing to the obstacles in treatment planning and decision making. Questionnaires were distributed and responded by dentists working at five public-based special care clinics in South Korea. Factors divided into three parts (dentist demographics, clinical factors, and educational and administrational factors) were assessed and analyzed for correlations between dentist-related factors and dentist-felt burdens for special care treatment. Fisher's exact test and Chi-square test were used at the level of 0.05. A total of 34 dentists responded to the questionnaires. Almost all dentists had obstacles in the treatment of special needs patients in terms of the patients' lack of cooperation (94.1%), proxy communication with caregivers (94.1%), payment reward system (63.6%), deficient workforce (67.7%), and others. The longer dentists had been practicing for special needs patients, the more they were dissatisfied with the reward system and a longer time was spent for communication with patients and their caregivers (p<0.05). For specialists, more obstacles were experienced in treatment planning due to a deficiency in the clinical information obtained from their patients compared to general practitioners (p<0.05). A total of 82.4% of the respondents approved of mandatory educational programs for special care dentists. There were practitioner-based factors related to the amount of obstacles felt by special care dentists. To overcome the treatment barrier of special needs patients, in-depth education and training are required in special care dentistry.

  • Williams syndrome is a rare congenital disorder with various physical abnormalities and characterized by facial, oral, and dental features. Individuals with Williams syndrome typically have eating disorders in the early childhood, which lead to prolonged night feeding. Prolonged night feeding is a risk factor for rampant dental caries. Williams syndrome is caused by the microdeletion of chromosome 7, resulting in elastin deficiency. Elastin is integral to cardiovascular health. Many patients with Williams syndrome have complex cardiovascular abnormalities that must be considered a part of dental management. Complications related to cardiovascular diseases may induce adverse effects such as dangerously elevated blood pressure. This may occur in patients during stressful dental treatment. In addition, characteristics of auditory hyperalgesia and anxiety disorders among patients with William syndrome, complicate receiving routine dental management. Therefore, dental treatment under sedation or general anesthesia may be preferable for patients with Williams syndrome; in particular, patients who are very uncooperative and/or needs extensive dental treatment. A thorough evaluation of each patient's physical condition is required before making decisions regarding dental treatment. Careful monitoring and preparation for emergencies are very important during and shortly after dental treatment under general anesthesia or sedation. Monitoring is critical until vital signs have stabilized and return to normal. A 28-month-old man diagnosed as having Williams syndrome, visited the Dental Hospital of ${\bigcirc}{\bigcirc}$ University for the management of rampant dental caries. We reported on the management of this patient who had peripheral pulmonic stenosis, and received dental treatment under general anesthesia. We also reviewed the characteristics of Williams syndrome and discussed considerations for dental treatment under general anesthesia.

  • Cerebral palsy is a non-progressive disorder resulting from central nervous system damage caused by multiple factors. Almost all cerebral palsy patients have a movement disorder that makes dental treatment difficult. Oral hygiene management is difficult and the risks for periodontitis, dental caries and loss of multiple teeth are high. Placement of dental implants for multiple missing teeth in cerebral palsy patients needs multiple rounds of general anesthesia, and the prognosis is poor despite the expense. Therefore, making the decision to perform multiple dental implant treatments on cerebral palsy patients is difficult. A 33-year-old female patient with cerebral palsy and mental retardation was scheduled for multiple implant treatments. She underwent computed tomography (CT) under sedation and the operation of nine dental implants under general anesthesia. Implant-supported fixed prosthesis treatment was completed. During follow-up, she had the anterior incisors extracted and underwent the surgery of 3 additional dental implants, completing the prosthetic treatment. Although oral parafunctions existed due to cerebral palsy, no implant failure was observed 9 years after the first implant surgery.

  • Purpose: To evaluate compliance by analyzing and comparing treatment duration, degree of improvement after treatment and treatment response of oral lichen planus (OLP) patients according to characteristics of them and the severity of the lesion. Methods: According to treatment process, 132 subjects with OLP who first visited the Department of Oral Medicine at the Pusan National University Dental Hospital from January 2017 to December 2020were classified into three groups: Treatment completed (CT) group, Under treatment (UT) group, and Dropped out during follow-up (DT) group. The reticulation/keratosis, erythema, and ulceration (REU) scoring system was used to assess the severity of OLP. The degree of improvement after treatment was evaluated in CT group. Results: There were 53 (40.15%) CT, 27 (20.45%) UT and 52 (39.39%) DT. In CT group, according to initial REU score there was a statistical difference in the degree of improvement, but not in the length of time to complete treatment. There was no statistical difference between the days it took for patients to feel symptom relief, and the days of entire treatment among three groups. However, there was a positive correlation between the REU score of gingiva and duration of treatment in DT group. In the CT and DT groups, there was a correlation between the length of time taken to relieve symptoms and the duration of treatment. Conclusions: The severity of the gingival lesion and the initial response to treatment have a large effect on the entire treatment period and prognosis, so it should be considered when explaining the disease prognosis and treatment period to patients, and the clinician needs to focus on initial symptom relief.

  • The author studied the quality and change of the effective demand for dental treatment of the cutpatients of the Wonkwang dental clinic from the March of 1982 to the August of 1983. The results were compared with the previous reports to find any changes that might come from the effect of dental insurance system. After discussing the results, the author concluded as follows: 1. The twenties were the most in the age groups, but it is desirable that age groups of mixed dentition be the most to enhance the dental health status of community. 2. The effective demand for dental treatment increased especially in the areas such as intraoral radiograph taking, amalgam filling, temporary filling, endodontic treatments, and drug application on the soft tissue, but decreased in the areas such as prosthodontic treatments, and there were no demand for the orthodontic treatment. 3. As compared with the absolute needs, the effective demand was relatively high for teeth extraction and amalgam filling, but it was generally very low. 4. So, the present extent of dental insurance system should be expanded, and the dental care delivery system should be established within the community. Finally, the economic and social status of Iri community should be enhance by political and educational plannings.

  • The Status of oral health and dental prosthetic treatment in Daegu and Gyoung Buk area was examined. To investigate the correlation between factors such as demographic and socioeconomic characteristics and status of oral health and knowledge about oral preventive measures, a survey was performed. After analyzing the effects of knowledge levels about dental prosthetic treatment on the oral health status and preventive measures, following results were obtained. A Total of 625 people participated in the survey for three months from October 2007 to January 2008. The responses from the survey were then analyzed to assess whether those with less knowledge of preventive measures tended to have more dental prosthetic work. Of those that took part in the survey: ${\cdot}$ 30.2% were in their twenties ${\cdot}$ 56.2% were married ${\cdot}$ 51.4% of respondents had at least a college degree ${\cdot}$ 42.4% had monthly incomes below one million Won ${\cdot}$ 24.3% were students ${\cdot}$ 55.8% were from urban areas After studying the relationship between oral health and various demographics, researchers concluded that respondents' sex, income, job and place of residence had no effect on the overall status of their oral health. However, those that were divorcees or widows (1.95${_+}/{_-}$ 0.32), over the age of 70 (1.67${_+}/{_-}$0.31), or had little education (0.82${_+}/{_-}$0.28), all demonstrated a statistically significant effect on their oral health with a significance level of 0.05.Cross analysis revealed a p-value of 0.000. The correlation between knowledge of oral preventive measures and other factors was examined. No difference was found between men and women but people who were divorced or lost their spouse, people over the age of 60, and people with no education, the results of knowledge of oral preventive measure was poor as 1.74$\pm$0.44, 1.85$\pm$0.40, 1.85$\pm$0.44,and 1.60$\pm$0.47 separately. Cross analysis showed that p-value was 0.000 and correlation between knowledge of oral preventive measures and those factors were statistically significant at significance level of 0.05. The status of dental prosthetic treatment was investigated. Women, people with higher income, and married people had more dental prosthetic treatment and it was related to education, income, residential area, and the status of dental prosthetic treatment. The returns of those factors was 32.7%. Respondents$^{\circ}{\emptyset}$ oral hygiene status and knowledge of oral preventive measures was related to demographic or socioeconomic factors. Therefore, a preventive program for oral health care needs to be developed in advanced countries. Knowledge of oral health is below the international average and more research and effort needs to be put in to develop public knowledge of dental prosthetic treatment. Government intervention such as enacting an oral health initiative or dental insurance that covers prosthetic treatments is urgently needed.

  • This study endeavors to provide basic data for healthcare policy development by identifying factors that influence the unmet healthcare needs of the elderly with chronic diseases. The study utilized data from the 2017 Korean National Survey on the Elderly, and analyzed 9,117 elders afflicted with more than 1 chronic disease. We determined that 851 (9.3%) elders experienced unmet healthcare needs from hospital & clinical treatment, and 1,469 (16.1%) elders experienced unmet dental treatment. 'Economic difficulty' was the most common reason expressed by 425 (49.9%) elders for unmet healthcare needs from hospital and clinical treatment, and by 1,066 (72.6%) elders for dental treatment. Furthermore, unmet healthcare needs in hospital and clinical treatment were affected by various factors, including spouse, education, self-rated health status and exercise, whereas unmet healthcare needs in dental treatment were affected by factors such as spouse, education, medical aids, self-rated health status, smoking, and exercise. Based on the research results achieved, we propose a necessity to establish healthcare policies that consider the influencing factors, and help resolve the unmet healthcare needs of the elderly with chronic diseases.

  • The purpose of this study was to examine what types of experiences dental hygienists underwent with patients who were unapproachable in an effort to find out the latter group's needs and expectations, explore how to treat them of different personality type, and provide better dental services in response to their needs. To attain the purpose, Q-methodology was employed, which made a subjective and systematic assessment of human subjectivity. There were three types of subjectivity among dental hygienists in conjunction with their experiences with patients who were hard to please. Each group whose subjectivity was different also had a different preference for patients, which was not exclusive to one another but unique. Type 1 was "avoiding patients who showed off". Dental hygienists of this type found it unpleasant to treat patients who boasted of their background, position or relations with the head of hospital and wanted to be given special treatment. They avoided those patients, since giving special treatment to specific patients was likely to do damage to others. They believed that better medical services could be provided through mutual concern and good manners between medical personnels and patients. Dental hygienists of type 2 considered it hard to treat patients who were picky and looked at treatment or its outcome negatively. Those who had to be separated from others on account of possible cross-infection or who called for special decontamination methods of dental instruments were also difficult to deal with. Dental hygienists of this type could be said to "avoid picky patients", as they preferred to fare with patients by offering good, faithful treatment rather than by giving special treatment. Dental hygienists of type 3 believed that smooth and successful treatment hinged on mutual trust, confidence and collaboration between medical personnels and patients. According to them, patients who choose a specific hospital or a particular medical team at their own option have to cooperate if necessary, listen carefully to medical personnels and treat them without any hostility or bias. Therefore, they could be said to "avoid patients who were not cooperative".
