Dental Service System and Oral Health Providers in Correctional Institutions (구금시설의 치과진료체계 및 구강보건의료인력 현황 조사)

  • The purpose of this study is to analyze the dental service system in correctional institutions and 10 find the factors for improving inmates' oral health. This study is comprised of document review, telephone and questionnaire survey. The subjects of questionnaire survey are public health dentists and doctors in correctional institutions. They responded to questionnaire and the survey was collected from previous research and selected information about the dental service system. The findings of the study were as follow : Documentary survey 1. According to 2004's study, there are 42 dental offices in 46 all correctional institutions. 2. Criminals who took an health examination occupied 69.0% when committed to a jail in 2002's study. Majorities of them(81.5%) responded that they didn't take any oral examination. Telephone & Questionnaire survey 1. Full-time public health dentists are 26 in 2009. There is no correctional institution having oral health providers in 26 correctional institutions surveyed. 2 About 10 patients use the dental services in a day. Part-time dentists visit 4 times a month as average in 80% of institutions. 40% of institutions responded dental treatments can't be progressed conveniently because of the lack of oral health providers. 3. 80% of respondents answered that it is hard to cure prisoners, and that's because they are forbidden to get out of the institutions. 4. Only 20% of correctional institutions offered the oral hygiene instructions. There is no regular oral hygiene education for all inmates. 5. They need to increase the number of oral health providers.

Analysis of distribution trend among students of dental hygiene departments and active hygienists by region (지역별 치위생(학)과 학생 및 활동 치과위생사 분포의 추세 분석)

  • Objectives: This study compared the number of graduates in each region for the past 6 years and the number of dental hygienists working in dental clinics by region to evaluate the trend of dental hygienists moving to work areas after graduation. Methods: Health care big data open system_medical manpower statistics, resident population and household status data by year, and education statistics service were used to calculate the number of dentists and dental hygienists, admission status by region, number of dental hygienists per 100,000 population, number of dental hygienists per number of dentists, and distribution of dental hygienists by region. Results: Although the number of active dental hygienists increased in the metropolitan area, the ratio of dental hygienists to dentists did not improve significantly. In addition, the number of students enrolled in provincial universities decreased, and there were fewer active dental hygienists than graduates in provincial areas. Conclusions: Although the number of active dental hygienists increased due to increase in the number of dental hygiene departments, it was found that rural areas did not have a significant impact on the availability of dental hygienists as the graduates moved to the metropolitan area.

  • The purpose of this study is to analyze the data on the medical personnel education system and license scheme and provide the basic material for an enhanced qualification system. In China, dental education reform has been underway with a view to providing an inclusive basic medical service package to the whole country by 2020. It is also estimated that the number of Korean dental medicine students in China would be fewer. And most of them desire to get a job in China after acquiring the Chinese license, suggesting little prospect of a massive inflow into Korea in the near future. In Japan, students are required to complete a clinical training program for over 1 year after receiving licenses to become an independent practitioner. But they can apply for the Korean preliminary examination without this post-graduation process. For this reason, the quality issue in license effectiveness review has been seriously discussed. It is deemed desirable to limit the Korean accreditation to Japanese graduates from the dental schools certified by the Japanese dental accreditation body.

A study on Knowledge and Compliance among dental personnels in dental implants (치과종사자들의 치과 Implant에 대한 지식 및 이행실태 - 대전광역시 치과 병·의원을 중심으로 -)

  • It follows in increase of the old age population and the loss of teeth increases, also the supplement prosthetics treatment which is caused by loss of teeth is various and it develops and the dentistry implant demand is increasing. This study enforced a self-administered survey with 197 dental personnels employed in dental hospitals, dental clinics from August 15, 2007, to September 15, in the area of Daejeon. It's ultimately intended to serve as a basis for the preparation of more effective, appropriate dental implants education programs for personnels, who take a crucial part in dental implants operation, provide better dental services to patients who are in need of dental implants operation. As the result, the study got the following conclusion. 1. The population sociological feature of dental hospital and dental clinic showed that significant differences of dental service career. Dental service career of dental personnels shows; below 3 years 43.1%, 4~6 years 35.3%, more than 7 years 21.6% in dental hospitals, below 3 years 29.5%, 4~6 years 28.1%, more than 7 years 42.5% in dental clinics(p=0.027). 2. The average score of personnels knowledge in dental implants was 3.67point, from analyzing the knowledge on dental implants of dental personnels from dental health-care settings, dental personnels employed in dental hospitals scored relatively higher than in dental clinics(p=0.129). Dental personnels with 4~6 years of experience scored the highest(p=0.002). 3. The average score of dental personnels compliance in dental implants was 3.92point, from analyzing the compliance of dental implants of dental personnels from dental health-care settings, dental personnels employed in dental hospitals scored relatively higher than in dental clinics(p=0.006). Dental personnels with 4~6 years of experience scored the highest(p=0.707). 4. The contingency coefficient between dental implants general knowledge and the general compliance(r=0.233, p=0.001), operation knowledge and operation compliance(r=0.332, p=0.000), maintenance knowledge and maintenance compliance(r=0.236, p=0.001). 5. Recently dental implants is emerging as one of the important medical services in the dental treatment sector. From analyzing the compliance of dental implants of dental personnels in dental health-care settings, dental personnels employed in dental hospitals scored relatively higher than in dental clinics. Consequently, the effort of the dentist and the dental personnels demanded to be earnestly to improves the difference of the knowledge and compliance against the dental implants of the dental personnels in dental health-care settings, it is thought that with reinforcement of effective role share and professionalism to success of dental implants, more system and the specialty dental implants education which is standardized continuously must be provided to all dental personnels.

A study on the performance and necessity of dental hygienists for oral health promotion activities for the elderly according to their learning experiences and performance experiences (노인 구강건강증진 활동에 대한 치과위생사의 학습경험과 수행경험에 따른 수행가능성 및 필요성 조사)

  • Objectives: The purpose of this study was to investigate the awareness of the performance and necessity of oral health promotion activities and seek ways to revitalize the professional role of dental hygienists in oral healthcare for the efficient oral care of the elderly. Methods: Eighty-five dental hygienists in charge of oral health promotion projects at public health centers and 38 dental hygienists in network dental clinical trials were investigated for their learning experience, performance experience, feasibility, and necessity of dental hygienists for general and oral health service items for the elderly. The collected data were analyzed using frequency analysis, chi-square test, and Mann-Whitney U test. Results: The degree of performance possibility according to the learning experience and performance experience of the dental hygienist for the whole body and oral health promotion activity items for the elderly showed that the degree of performance possibility was higher among those with experience compared to the non-experienced person, and it was statistically significantly higher (p<0.05). Conclusions: The dental hygienist's professional oral health service is a necessary system to improve practical knowledge and skills and to provide a wide range of professional oral health services for the elderly.

PRESENT STATE AND PROSPECT OF PUBLIC DENTAL HEALTH SERVICE FOR CHILDREN AND ADOLESCENTS IN KOREA (우리나라 어린이와 청소년의 구강건강을 위한 공공서비스의 현황과 전망)

  • To achieve the dental health of children and adolescents which is the objective of the pediatric dentistry, the role of the public service should be increased. The basis of the public service is established by many laws of which the Dental Health Law is most important. The percentages of primary schools that had the school dental health clinic, that implemented the fluoride rinsing program, and that implemented the group toothbrushing after lunch were 7.2%, 57.5%, and 46.9%, respectively, and the percentages of primary school children that received the dental health education and that received the fissure sealing were 48.0% and 12.4%, respectively(2006). About 42% of infants and preschool children from 0 to 6 years received preventive dental care in the last one year, at the nursery or kindergarten(18%) or at the health center(1%)(2005). The percentage of the health centers that implemented water fluoridation was 11.3%, and the percentage of the population who drank the fluoridated water was 5.7%(2006). It was suggested that the school dental health administration should be unified, that dental health teachers should be employed, and that the comprehensive dental health care should be supplied to all the children and adolescents through the circuit school dentists and the school dental hospitals in the long term. Also, the dentist in charge system for the children and adolescent was suggested.

A Study on Satisfaction Factors with Dental Implant Patients (치과용 임플란트 보철물적용환자의 만족요인)

  • This research has been attempt to examine closely factors that high cost and value to overall satisfaction state for dental implant prosthesis, re-utilization intention, inducement intention of the general hospital dental service user and provide basis data necessary to establish competitive general hospital dentistry management strategy. Collected data using own recording way questionnaire from April 17, 2004 to May 15, 2004 choosing 142 people that agree on this research and question of 361 people that dental implant prosthetic treatment finished from March 1, 1999 to March 1, 2004 for this. Major analyzation consequences are as follows : First, general satisfaction, re-utilization intention, inducement intention for implant prosthesis that highly correlation variables are kindness, explanation, medical treatment level and appreciation of the aesthetic. Second, correlation is high relatively between re-utilization intention, inducement intention in the 3 variables such as general satisfaction re-utilization intention, inducement intention. Third, the result of multiple regression analysis showed that most significant effective factors are satisfaction with explanation of dental implant prosthesis between the 3 dependant variables such as general satisfaction, re-utilization intention, inducement intention. In looking into these consequence, how to improve that raise satisfaction about dental implant prosthetic treatment and to establish patient focused care service system for dental implant prosthesis, it is consider that explain enough about implant prosthesis and raise quality of medical examination and treatment level, including satisfied with fabrication of good esthetic dental implant prosthesis.

A Factor Analysis on Job Stress Level in Dental Hygienists (치과위생사의 직무스트레스에 관한 분석요인)

  • This study was carried out in order to offer medical service in good quality by enhancing dental hygienists' job satisfaction and to arrange basic data necessary for individual development and health promotion in dental hygienists through grasping the job stress level and job satisfaction of being experienced by dental hygienists, and the correlation. The self-administered questionnaire was surveyed and collected from August 1-12 of 2011 targeting 210 dental hygienists of general hospitals, dental hospitals, and dental clinics nationwide(Seoul, Gyeonggi, Daegu, Pohang).The analysis was made on 205 people. The collected data was analyzed by using SPSS WIN 15.0 program. As a result, out of job stress, the average was indicated to be the highest with 2.71 in 'job demand.' As a result of examining about sphere-based relationship between health perception level and job stress related to building in dental hygienists, the central nervous system manifestation showed statistically significant positive correlation with physical environment(r=.281, p<.001), job demand(r=.281, p<.001), relational conflict(r=.163, p<.05), organizational system(r=.166, p<.05), compensation inappropriateness(r=.267, p<.001), and job stress(r=.311, p<.001). Overall, the building-related health perception level showed statistically significant correlation with physical environment(r=.259, p<.001), job demand(r=.311, p<.001), relational conflict(r=.172, p<.05), organizational system(r=.158, p<.05), compensation inappropriateness(r=.197, p<.01), and job stress(r=.275, p<.001). They need to improve their quality of life and life and mental health by developing proper air conditioning system and using one in their work place. if so, they could contribute to better quality of oral cavity service for everyone.

  • Objectives: The purpose of this study is to look into the health insurance systems and application in Korea and Japan in order to improve elderly people's quality of life. Their quality of life can be improved by upgrading the long term care systems; and extending treatment and prevention. Methods: This study is to examine long term care systems articles through content analysis in the National Health Insurance Service. Data were collected from the printed Internet and analyzed. Results: A part-time dentist system in elderly care facilities has trouble in taking care of old people's oral health due to both lack of time and operational difficulties. It is urgent to arrange dental experts who can permanently stay in care facilities and to build systems which can be managed periodically and continually. Conclusions: As having staff training for the efficiency and using the manpower in care facilities (care workers), it is necessary to solve the unequal medical service in oral health care for the elderly in Korea.

Knowledge on the chemical disinfectants among dental workers in some Daejeon region (대전 일부 지역 치과종사자의 화학소독제에 관한 지식도 연구)

  • The usage of appropriate disinfectants is essential for infection control in dental hospitals, dental clinics. Inadequate use of disinfectants is the cause of human or environmental toxicity and is a waste of cost. This study was aimed to assess the level of knowledge on the disinfection and chemical disinfectants among dental workers in dental hospitals, dental clinics. It's ultimately intended to serve as a basis for the preparation of more effective, appropriate educational programs on the disinfection and manuals on the use of disinfectants for dental workers. This study enforced a questionnaire with 184 dental workers employed in dental hospitals, dental clinics from 15 Aug. to 15 Sep. 2007, in the area of Daejeon. The obtained result were as follows; 1. The population sociological feature of dental hospital and dental clinic showed that significant differences of dental service career(pM0.039). Dental service career of dental workers shows; below 3 years 42.9%, 4~6 years 34.7%, more than 7 years 22.4% in dental hospitals, below 3 years 30.4%, 4~6 years 26.7%, more than 7 years 43.0% in dental clinics(pM0.039). 2. The average score of dental workers knowledge in 'Critical item soaks in high-level disinfectants for 20minutes was 2.73V0.49 point, got from knowledge of dental instrument is appropriate to immerse before sterilization in the dental device disinfection(pM0.002). 3. In the general disinfection which it follows in education experience of chemical disinfectants direction for use, 'Direction for use by Spaulding process classification' responded that the correct answer was the education experience dental workers 60.0%, the education non-experience dental workers 39.5%(p=0.026). 4. In the dental device disinfection which it follows in education experience of chemical disinfectants direction for use. 'High level disinfection is not applied for the non-critical items and equipment' responded that the education experience dental workers 49.2%, the education non-experience dental workers 31.9%(pM0.045), 'Semi-critical items is applied same method in presence of the infection disease which it acts responded that the education experience dental workers 44.6% answer back, the education non-experience dental workers 24.4%(pM0.017). 5. 'A hand disinfectants of anticeptics have effect' the education experience dental workers 78.5% answer back, the education non-experience dental workers 52.9%(pM0.003). 1t uses with hand disinfectants when the instruments which be imbrued and patient contact', the education experience dental workers 78.5% answer back, the education non-experience dental workers 62.2%(pM0.026), 'Boric acid solution uses for the skin disinfectants the education experience dental workers 52.3% answer back, the education non-experience dental workers is 37.0%(pM0.016), 'Gluconate have effective difference which it follows in chemical disinfectant consistency and the solution type' education experience dental workers 72.3% answer back, education non-experience dental workers 47.9%(pM0.004). 6. The education experience dental workers were appeared higher than the education non-experience dental workers in knowledge of the disinfection and chemical disinfectants. Consequently system and the specialty education which is standardized continuously must be provided to all dental workers.
