체계적 문헌고찰을 통한 국내 치과공포 연구 동향분석 (2007-2017) (A systematic review of Korean research trends in dental fear between 2007-2017)

  • Objectives: The purpose of this study was to investigate the overall research trends and factors influencing dental fears in the last 10 years (2007-2017) and provide recommendations for future research. Methods: The literature review focused on dental fear research in Korea. Inclusion criteria of selected studies were as follows: studies that examined fear in middle school students or older, studies undertaken between 2007-2017, and studies that examined trends and factors relating to dental fears. Results: Findings from this literature review showed that the most commonly used tool for measuring dental fear was the Dental Fear Survey (DFS). A lack of trust towards the dentist increased dental fear among patients. Women managed fear better than men. Past pain experiences increased dental fear. The most frequent intervention for reducing fear was sound (41.7%). Most of the intervention studies demonstrated a reduction of fear, with the exception of interventions using ear plugs. Fear was increased in studies involving ear plugs. Conclusions: Based on the results of the study, specific measures should be taken to alleviate past pain experiences, such as the fear of anesthetic injections and sensations of the dental drill. Continuous research is needed to reduce dental fear.

Retrospective Study on the Flow and Characteristics of Dental Emergency Patients in Chosun University Hospital

  • Purpose: The aim of the present study is to assess the importance of proper treatment timing for dental emergency patients by characterizing current patient care in the emergency room. Materials and Methods: A retrospective chart review of 3,211 patients who visited the Chosun University Hospital's dental emergency department (Gwangju, Korea) was conducted from January 1, 2011 to May 31, 2014. Information regarding age, gender, onset date, main causes, and diagnoses were collected and analyzed. The main causes were divided into six categories: assault, household/play, sports, traffic, work, and others. Result: Emergency visits were more common for men (69%), and the ratio of males to females was 2.2:1 On average, the major cause was household/play (49.8%), followed by others (18.9%), traffic (16.6%), assault (9.1%), sports (2.9%), and work (2.6%). The most frequent diagnosis on average was dental trauma with 82.4%, followed by infection (10.7%), others (4.7%), and bleeding (2.2%). Conclusion: The main reasons for visits to the dental emergency department are dental trauma, dental infection, bleeding, and others. The most frequent reason for dental emergency patients to visit the emergency department was dental trauma (82.4%).

The cumulative survival rate of dental implants with micro-threads: a long-term retrospective study

  • Purpose: This study aimed to evaluate the long-term cumulative survival rate (CSR) of dental implants with micro-threads in the neck over a 10-year follow-up period and to examine the factors influencing the survival rate of dental implants. Methods: This retrospective study was based on radiographic and dental records. In total, 151 patients received 490 Oneplant® dental implants with an implant neck micro-thread design during 2006-2010 in the Department of Periodontology of Seoul National University Dental Hospital. Implant survival was evaluated using Kaplan-Meier analysis. Cox proportional hazard regression analysis was used to identify the factors influencing implant failure. Results: Ten out of 490 implants (2.04%) failed due to fixture fracture. The CSR of the implants was 97.9%, and no significant difference was observed in the CSR between external-and internal-implant types (98.2% and 97.6%, respectively, P=0.670). In Cox regression analysis, 2-stage surgery significantly increased the risk of implant failure (hazard ratio: 4.769, P=0.039). There were no significant differences in influencing factors, including sex, age, implant diameter, length, fixture type, location, surgical procedure, bone grafting, and restoration type. Conclusions: Within the limitations of this retrospective study, the micro-thread design of the implant neck was found to be favorable for implant survival, with stable clinical outcomes.

  • The advent and wide use of antibiotics have decreased the incidence of deep neck infection. When a deep neck infection does occur, however, it can be the cause of significant morbidity and death, resulting in airway obstruction, mediastinitis, pericarditis, epidural abscesses, and major vessel erosion. In our clinic, a patient with diffuse chronic osteomyelitis of mandible and fascial space abscess and necrotic fasciitis due to odontogenic infection at the time of first visit came. We successfully treated the patient by early diagnosis using contrast-enhanced CT and follow up dressing through the appropriate use of radiographic images.

  • In the case of failed fixed implant prosthesis accompanied by abutment screw fracture, fractured screw fragment must be removed to use the existing implant fixtures. A 61-year-old male patient, who had a failed maxillary fixed implant prosthesis accompanied by three abutment screw fracture, hoped to reconstruct the maxillary implant prosthesis, while maintaining the existing implant fixtures. To use the existing implant fixtures, fractured screw fragments were removed. A maxillary implant overdenture using available existing implants was planned. Bar-attachment with Locator was used for implant splinting, denture stability, and retention. Final impression was taken after treatment of peri-implantitis. Jaw relation registration was taken to evaluate available interarch space for bar-attachment. After fabricating bar-attachment, centric relation was taken. Implant overdenture using bar-attachment with Locator was delivered after wax-denture evaluation. This case report showed that a satisfactory clinical result was achieved by implant overdenture using existing implant fixtures in a maxillary edentulous patient.

  • Burkitt lymphoma is a highly aggressive type of hematopoietic malignancy that is comparatively common in children and young people. It is important that Burkitt lymphoma be diagnosed as early as possible for prompt intervention due to its rapidly progressive, high-grade malignant nature. Dentists, especially maxillofacial surgeons, can play a life-saving role in patients with such unknown malignancy as the first clinical or radiological manifestation might occur in the oral region.

  • Bone is a complex tissue in which resorption and formation continue throughout life. The bone tissue contains various types of cells, of which the bone forming osteoblasts and bone resorbing osteoclasts are mainly responsible for bone remodeling. Periodontal disease represents example of abnormal bone remodeling. Osteoclasts are multinucleated cells present only in bone. It is believed that osteoclast progenitors are hematopoietic origin, and they are recruited from hematopoietic tissues such as bone marrow and circulating blood to bone. Cells present in the osteoclast microenvironment include marrow stromal cells, osteoblasts, macrophages, T-lymphocytes, and marrow cells. These cells produce cytokines that can affect osteoclast formation. In vitro model systems using bone marrow cultures have demonstrated that $IL-l{\beta},\;IL-3,\;TNF-{\alpha},$ bFGF can stimulate the formation of osteoclasts. In contrast, IL-4 inhibits osteoclast formation. Knowledge of cytokines and bFGF that affect osteoclast formation and their capacity to modulate the bone-resorbing process should provide critical insights into normal calcium homeostasis and disorders of bone turnover such as periodontal disease, osteoporosis and Paget's disease.

일부 학교구강보건실 운영 초등학생들의 구강보건지식도 및 실천에 관한 조사 (Oral health knowledge and practice of elementary school students in school dental clinics)

  • Objectives : The purpose of this study was to evaluate the overall oral health knowledge and toothbrushing pattern of school dental clinic students in school dental clinics and to provide the basic data for the oral health. Methods : The subjects were 290 elementary school students visiting school dental clinics in Busan Metropolitan city. The data were collected from July 3, 2012 to July 19, 2013. Results : The visit rate of school dental clinics was 40.3%. Girl students brushing three times a day accounted for 47.3% and boy students brushing twice a day accounted for 58.5%(p<0.05). Students using rolling brushing method accounted for 35.4% and 8% of the students did not know the rolling method. Brushing method response consisted of circular motion(21.6%) and rolling method(30.8%). Brushing more than 3 minutes accounted for 46.5%(p<0.001) and 30.8% of the students chose rotation brushing method(p<0.001). Conclusions : It is very important for the elementary school students to learn the right oral health education and oral health knowledge.
