Purpose: Rehabilitation of the incomplete dentition by means of osseointegrated dental implants represents a highly predictable and widespread therapy; however, little is known about potential risk factors that may impair long-term implant success. Methods: From 2004 to 2012, a total of 13,147 implants were placed in 4,316 patients at the Academy for Oral Implantology in Vienna. The survival rates after 8 years of follow-up were computed using the Kaplan-Meier method, and the impact of patient- and implant-related risk factors was assessed. Results: Overall implant survival was 97% and was not associated with implant length (P=0.930), implant diameter (P=0.704), jaw location (P=0.545), implant position (P=0.450), local bone quality (P=0.398), previous bone augmentation surgery (P=0.617), or patient-related factors including osteoporosis (P=0.661), age (P=0.575), or diabetes mellitus (P=0.928). However, smoking increased the risk of implant failure by 3 folds (P<0.001) and a positive history of periodontal disease doubled the failure risk (P=0.001). Conclusions: Summing up the long-term results of well over 10,000 implants at the Academy for Oral Implantology in Vienna it can be concluded that there is only a limited number of patients that do not qualify for implant therapy and may thus not benefit from improved quality of life associated with fixed implant-retained prostheses.
Purpose: The aim of this retrospective study was to investigate the cumulative success rate, the implant survival rate, and the occurrence of biological complications in implants supporting full-arch immediately loaded rehabilitations supported by upright and tilted implants. Methods: The clinical records and periapical radiographs of patients who attended follow-up visits were collected, and information was recorded regarding marginal bone loss resorption, the occurrence of peri-implant infectious diseases, and the implant survival rate. Implants were classified as successful or not successful according to two distinct classifications for implant success. Results: A total of 53 maxillary and mandibular restorations including 212 implants were analysed, of which 56 implants were studied over the full five-year follow-up period. After five years, the cumulative success rate was 76.04% according to the Misch classification and 56.34% according to the Albrektsson classification. The cumulative implant survival rate was 100%, although one implant was found to be affected by peri-implantitis at the second follow-up visit. Conclusions: The cumulative success rate of the implants dropped over time, corresponding to the progression of marginal bone resorption. The prevalence of peri-implantitis was very low, and the implant survival rate was not found to be related to the cumulative success rate.

Because sublingual region is well-vascularized and sublingual artery is passed throughout this region, it should be careful not to perforate lingual cortex when placing dental implant on mandible. A 83-years-old male complained severe sublingual hematoma, hemorrhage and dyspnea came our outpatient department. He had received dental implant placement in the same day. He needed hemostasis and airway control. If soft tissue of sublingual region and the artery are injured, it may result in life-threatening excessive hemorrhage. In dental implant surgery, especially mandible, we should recognize the accurate shape of mandible and anatomy of sublingual region. It is important to stop anticoagulant agent before surgery. When a patient has airway obstruction, the operator should manage airway quickly.
PURPOSE. Patient-reported outcomes are increasingly commonly used as a method of evaluating treatments. This cross-sectional study aimed to evaluate implant treatment from the perspective of patient-reported outcomes. MATERIALS AND METHODS. Subjects were 804 patients who visited the Department of Oral Implantology at Osaka Dental University. The participants were categorized into a pre-implant group and a post-implant group. They were further categorized into five subgroups based on the number of occlusal supports provided by the remaining teeth according to the Eichner classification. The participants answered a basic questionnaire and the General Oral Health Assessment Index (GOHAI) questionnaire, an oral health-related quality of life (QOL) scale. GOHAI scores were compared according to the number of occlusal supports within each group and between the two groups. RESULTS. The results revealed a significant difference in terms of the number of occlusal supports within the pre-implant group; GOHAI scores decreased as the number of occlusal supports decreased (P<.001). However, no significant difference was observed in GOHAI scores in terms of the number of occlusal supports in the post-implant group (P>.05). GOHAI scores significantly improved in both pre- and post-implant groups in all occlusal support subgroups (P<.001). CONCLUSION. GOHAI scores decrease as occlusal support is lost. However, implant treatment performed in areas of loss improves the GOHAI score when occlusal support is restored.
Purpose: Optical coherence tomography (OCT) has been investigated as a novel diagnostic imaging tool. The utilisation of this equipment has been evaluated through several studies in the field of dentistry. The aim of this preliminary study was to determine through basic experiments the effectiveness of OCT in implant dentistry. Materials and Methods: To assess detection ability, we captured OCT images of implants in each of the following situations: (1) implants covered with mucosae of various thicknesses that were harvested from the mandibles of pigs; (2) implants installed in the mandibles of pigs; and (3) implants with abutments and crowns fixed with temporary cement. The OCT images were captured before cementation, after cementation, and after removing the excess submucosal cement. Results: If the thickness of the mucosa covering the implant body was less than 1 mm, the images of the implants were clearly detected by OCT. In the implants were installed in pigs' mandibles, it was difficult to capture clear images of the implant and alveolar bone in most of the samples. Remnants of excess cement around the implants were visible in most samples that had a mucosa thickness of less than 3 mm. Conclusion: Currently, OCT imaging of implants is limited. Cement remnants at the submucosal area can be detected in some cases, which can be helpful in preventing peri-implant diseases. Still, though there are some restrictions to its application, OCT could have potential as an effective diagnostic instrument in the field of implant dentistry as well.
Purpose: The study was aimed at investigating changes in periodontal parameters and superoxide dismutase activity triggered by root surface debridement with and without micronutrient supplementation in postmenopausal women. Methods: Forty-three postmenopausal chronic periodontitis patients were divided into two groups: group 1 (n=22) were provided periodontal treatment in the form of scaling and root planing (SRP) and group 2 (n=21) patients received SRP along with systemic administration of micronutrient antioxidants. Patients in both groups were subjected to root surface debridement. Group 2 patients also received adjunctive micronutrient antioxidant supplementation. Serum and salivary superoxide dismutase (SOD) activity along with periodontal parameters were recorded at baseline and 3 months after therapy. Results: Salivary and serum SOD values significantly (P<0.05) improved with periodontal treatment. Improvement in systemic enzymatic antioxidant status along with reduction in gingival inflammation and bleeding on probing (%) sites was significantly greater in group 2 as compared to group 1. Conclusions: Adjunctive micronutrient supplements reduce periodontal inflammation and improve the status of systemic enzymatic antioxidants in postmenopausal women.
Objectives. The standardization of connection between fixture and abutment has not been defined. The success of dental implants was not always depends on connection. However, the connection mechanism is one of the most important things for dental implant treatment success. Most implant systems are very comparable in their design and engineering. They share many common characteristics and have similar strengths and weaknesses. Their significant weaknesses are connection, microgap and the resulting micromovement allowing bacterial contamination and bone loss. In the present study, we investigated the clinical performance of Ankylos implant (conical connection implant) Patients and Methods. The clinical performance of conical connection implant was studied under well-controlled clinical conditions. A total of 133 conical connection implants were placed in 50 patients from April 2005 to March 2006. The mean follow-up loading period of implants which was considered successful was 220$\pm$29 days. We recorded the age, sex, installation site, reason of edentulous region, bone density of installation site, diameter and length of dental implants and periods from installation to uncovering surgery using patients medical chart. Results Four Ankylos implants were lost during pre-loading period. 129 implants provided excellent clinical performance during 220$\pm$29 days on an average. The short-term success rate of this conical connection implant system was 96.99%.
Purpose: The aim of this work is to evaluate a surgical technique for mobilization of mal posed dental implant in anterior area. Methods: A 38-year-old patient consulted our unit for esthetic dissatisfaction with the implant treatment of a central incisor. An implant was observed in 11 and 21, where 11 was 3 mm above the ideal limit, with excessive vestibular angulation. The choice was made to perform a segmental osteotomy and mobilize the bone block and the implant down and forward; a bone block extracted from the mandibular ramus was installed between the implant block and the bed to stabilize the segment. Results: After 4 months, a conventional fixed prosthesis was created and the esthetic result achieved was close to what the patient wanted, with no need for further surgery. The surgical condition was stabilized and maintained for the long-time and no complications how necrosis, infection or bone defects was present. Conclusions: It was concluded that the procedure is efficient, and the biological arguments in favor of the procedure are discussed.
Purpose: The purpose of this study was to analyze the cumulative survival rate of dental implants installed with guided bone regeneration (GBR), and also elucidate the factors related with the survival of dental implants. Material and Methods: This retrospective study was conducted on 148 dental implants installed in 76 patients by one specialist (Y.K.) at the Department of Periodontology and Implant Center, Seoul National University Dental Hospital from 2001 to 2010. The cumulative survival rates were obtained by the Kaplan-Meier method. The correlations between various factors and dental implant survival were analyzed by using the log-rank test and Cox proportional hazards model. Results: Among 148 dental implants installed in 76 patients, 8 implants in 7 patients were lost and the cumulative survival rates up to 5-years and 10-years were 97% and 89%, respectively. Gender, smoking status and location of implant were significantly associated with the cumulative survival rate of implants (p < 0.05). Age, history of hypertension and diabetes were not significantly associated with the cumulative survival rate of implants (p > 0.05). Conclusion: The dental implants installed with guided bone regeneration is predictable technique according to the results of cumulative survival rate over 10 years.
This case report is to report an extraordinary case of the spontaneous discharge of a dental implant. A 45-year-old woman was referred to our department complaining of symptoms of acute Maxillary sinusitis. She underwent a computed tomography scan that revealed a dental implant dislocated in the maxillary sinus. A preoperative medical treatment [Amoxicillin - Clavulanate (375 mg) and Streptokinase-Streptodornase (12500IU)] three times a day for two weeks was administered in order to prepare her for surgery. Unexpectedly, she reported that she had discharged the implant from mouth in the early morning. Mucociliary clearance in combination with a local osteolytic inflammatory process and mucolytics therapy are the likely causes of this unusual discharge.
