노인치위생학 교육과정에 따른 교육효과 분석을 위한 예비연구 (Preliminary Study of the Educational Effects of the Geriatric Dental Hygiene Curriculum)

  • 연구배경: 고령화가 심화되면서 증가하는 노인인구에 대한 적절한 구강관리 중재자로서 치과위생사의 역할이 중요해지고 있다. 본 연구는 치위생학과 학생들의 노인구강보건전문가로서의 역량을 키울 수 있는 효과적인 교육과정 개발을 위해 예비연구를 수행하였다. 연구방법: 노인치과학과 노인치위생학 및 실습으로 개설된 교과목을 수강한 학생을 대상으로 수업 마지막 주차에 설문을 수행하였다. 이론위주의 문제해결기반(PBL)교육과정과 이론과 실습을 병행한 PBL교육과정의 교육효과차이를 확인하기 위해 학기 말에 학생들의 전공역량달성도, 노인치위생 과정에 대한 인식, 수업만족도, 연령차별의 차이를 분석하였다. 결과: 전공역량달성도의 경우 이론 위주의 PBL교육보다 이론과 실습을 병행한 PBL교육에서 '의사소통역량'이 유의미하게 높았다(p=0.038). 연령주의의 경우 이론과 실습을 병행한 PBL교육에서 '노인을 기피하는 경향'이 유의미하게 낮았다(p=0.040). 수업만족도의 경우 이론과 실습을 병행한 PBL교육에서 '수업분위기'가 '매우 높음'으로 응답한 비율이 유의미하게 높았다(p=0.025). 결론: PBL 교수법은 노인치위생 교육과정으로 유용한 교수법이며, 이론위주의 교육보다는 노인치위생 실습교육을 함께 운영하는 교육과정이 더 효과적일 수 있다.

포괄치위생관리(CDHC) 과정 적용에 따른 스케일링 경험 대상자의 구강건강신념과 만족도 (Oral health belief and satisfaction after scaling experience with comprehensive dental hygiene care)

  • 연구목적: 본 연구는 포괄치위생관리(Comprehensive Dental Hygiene Care; CDHC) 과정 적용에 따른 스케일링 경험 대상자의 구강건강신념과 만족도를 확인하고자 수행하였다. 연구방법: 2020년 7월 1일부터 2020년 9월 20일까지 치과 병·의원에서 스케일링을 받은 대상자 182명의 자료를 수집하여 분석하였다. 자료는 SPSS Statistics 22.0을 이용하여 빈도분석, chi-square, t-test, One way ANOVA, Pearson's correlation analysis를 실시하였다. 연구결과: 구강건강행태는 CDHC 적용 그룹에서 칫솔, 치약 외 구강관리용품 사용 92.4%(p<0.001), 구강관리를 위한 정기적인 치과 방문이 67.4%였다(p<0.001). 일반적 특성에 따른 구강건강신념은 CDHC 적용 그룹에서 연령 '50-65세'가 2.41로 가장 높았고, 사후검정 결과 '20-29세'와 유의한 차이가 있었다(p <0.001). CDHC 적용 그룹에서 구강건강행태에 따른 구강건강신념은 스케일링 주기 '3개월'이 가장 높았다(p<0.05). CDHC 적용 그룹의 구강건강신념 하위 요소인 유익성(p <0.01)과 중요성(p<0.05)이 높을수록 만족도와 높은 상관관계를 보였다. 결론: CDHC 적용이 스케일링 경험 대상자들의 구강건강신념과 만족도에 긍정적인 영향을 주는 것을 알 수 있었다. CDHC 과정 적용은 구강건강신념과 만족도를 높이는 매개로써 임상에 더욱 확산하여야 할 것이다.

임상 치과위생사의 직무만족 경험과 일의 의미에 대한 질적 연구 (A qualitative study on experiences of job satisfaction and meaning of work among clinical dental hygienists)

  • Objectives: This study explored job satisfaction experience and meaning of work in dental hygienists. Methods: In-depth interviews were conducted with 13 clinical dental hygienists who had been working in dental clinics for more than 10 years. The analysis method used was developed by Giorgi, including all four steps. Results: The results of the interview were categorized into five central meanings: recognition and trust (including patient's trust, trust in the boss, recognition in the hospital, and intimacy), growth (growth of the hospital, growth in jobs, and problem-solving skills), professional pride (self-esteem, extension of work and changes of awareness, and being able to help others), reward (financial compensation and psychological rewards), and meaning of life given by work (the process of maturing, self-representation, and vitality of life). Conclusions: Based on this study, we need to develop tools to measure dental hygienists' level of job satisfaction as well as follow-up research on ways to improve it.

치위생과 학생의 치근단 촬영법 인식에 관한 조사 연구 (A survey study on recognition of periapical radiography in dental hygiene students)

  • Objectives : This study was carried out in order to obtain basic data for students' efficient acquirement and instruction of radiography technology in the future by surveying dental hygiene students' recognition of periapical radiography. Methods : This study carried out a questionnaire survey targeting dental hygiene students from December 2009 to December 2010, and obtained the following results. Results : 1. As a result of examining recognition on periapical radiography, the bisecting angle technique was indicated to be averagely $3.84{\pm}0.566$ points. The paralleling technique was indicated to be $2.66{\pm}0.701$ points. 2. As a result of examining about problems given the bisecting angle technique, what had been most difficult given the bisecting angle technique was indicated to be the highest in cone positioning with 34.2%. The most difficulty given deciding on the X-ray vertical-angel irradiation direction was indicated to be the highest with 66.9% in adjusting the cone direction on the virtual bisector. 3. As a result of examining about problems given the paralleling technique, what had been most difficult in the process of the paralleling technique was indicated to be the highest with 56.7% in fixing the film immobilization device inside the mouth. Conclusions : Examining the above results, it is considered that there is a need of understanding morphological and anatomical structure inside the mouth in order to reduce the mistake rate given the periapical radiography, and that it is important to increase skill level by repetitively shooting several times with having enough time.

Cultural Competence and Influencing Factors of Dental Hygiene Students

  • The purpose of this study was to investigate the level of cultural competence and its influencing factors among 530 dental hygiene students. The analysis was performed using PASW Statistics ver. 18.0, and the following conclusions were drawn. Cultural knowledge levels showed a significant difference according to general characteristics in four components of cultural competence levels. Cultural competence and its four components were higher in subjects who have a religion (p<0.05). The differences in cultural competence levels according to culture-related characteristics are as follows. In individuals with experience of taking foreign culture-related classes, intention to work overseas, or fluency in a foreign language, cultural competence levels and its four components such as cultural comfort, knowledge, awareness, and behavior intent were higher (p<0.05). Intention of overseas employment was the factor most strongly influencing the level of cultural competence of the subjects. The more intention the subjects have to work overseas, the more they have experienced foreign culture-related lessons, the more fluently they speak a foreign language, and the higher level of cultural competency they reached (p<0.05). As the research results show, it is necessary to establish a continuing plan to understand and communicate with individuals of other cultures through foreign language and foreign culture classes in schools in order to improve the level of cultural competence. This education would reduce the fear and stress that subjects might make a mistake in meeting with people from other cultures, and it could be a preparatory process to actively carry out dental hygiene practice with foreigners.

Caries management by risk assessment (CAMBRA)의 발전; CRA 업데이트 (The development of CAMBRA; Updated CRA)

  • The aim of this study was to investigate the caries management by risk assessment (CAMBRA) development process and the latest updated caries risk assessment for the domestic settlement of the CAMBRA program and the activation of dental clinics. A system for CAMBRA was published in California Dental Association (CDA) in 2007. To investigate whether CAMBRA can be successfully implemented outside a university setting, a practice-based research network (PBRN) was created in the San Francisco Bay Area. Based on the CAMBRA-PRAN clinical studies, the caries risk assessment (CRA) tool was updated for patients aged 6 through adults in 2019. The CAMBRA system is expected to be widely and actively used in the dental field in Korea to contribute to oral health care.

치위생과정(ADPIE)의 임상적용 가능성과 실천방안 모색 (A Study on Clinical Feasibility and Practical Strategies for Dental Hygiene Process (ADPIE))

  • 치위생과정을 임상에서 활용하고 정착시키기 위한 방안을 모색하고자 SEM을 이용하여 치과위생사 업무 자율성과 치위생과정의 임상적용 기대효과 및 장애요인이 임상적용 의향에 미치는 효과를 분석하고, 치위생과정에 대한 태도가 임상적용 의향에 미치는 매개효과를 검증한 결과는 다음과 같다. 가설적 모형은 ${\chi}^2=421.67$, GFI=0.858, CFI=0.915, TLI=0.896, RMR=0.039, RMSEA=0.099로 적합한 것으로 나타났다. SEM을 검증한 결과, 치과위생사 업무 자율성(${\beta}=0.398$, p<0.05)과 치위생과정 기대효과(${\beta}=0.363$, p<0.01)는 치위생과정 태도에 정(+)의 방향으로, 치위생과정 태도(${\beta}=0.678$, p<0.05)는 임상적용 의향에 정(+)의 방향으로 영향을 미치는 것으로 나타났다. 치과위생사 업무 자율성(${\beta}=0.269$, p<0.05)과 기대효과(${\beta}=0.246$, p<0.01)는 치위생과정 태도를 매개로 임상적용 의향에 간접적인 영향을 미치는 것으로 확인되었다. 위의 결과로 치과위생사의 업무 자율성, 치위생과정에 대한 기대효과와 태도는 임상적용 의향에 영향을 미치는 중요한 요인임을 확인하였다. 따라서 치위생과정을 임상에 정착시키기 위해서는 치과위생사 역량을 제고하여 업무 자율성을 확보해야 하며, 치위생과정에 대한 경영적 효과와 가치에 대한 객관적인 검증이 이루어져야 한다.

Systematic Literature Review on Coronavirus Infectious Disease-19 and Dental Masks

  • Background: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is transmitted primarily through droplets, and dental practitioners are at risk of occupational exposure to SARS-CoV-2 infection owing to direct contact with the patient's mouth, aerosols from dental procedures, and saliva. Wearing a mask is believed to be the best method of protection against infection, and a systematic literature review was conducted on whether the dental masks used in dentistry are effective in preventing Coronavirus Infectious Disease-19 (COVID-19). Methods: We used PubMed, Google Scholar, DBpia, NDSL, and KISS databases for this study. Of the 917 documents narrowed down by the search terms "Coronavirus, COVID-19, and dental," 83 documents were collected and studied. Ultimately, 42 of these papers were selected for analysis after considering duplication from the flow chart of the literature selection process. Results: While dental masks are often used when treating patients with unknown COVID-19 status, the Ministry of Food and Drug Safety stated that the use of dental masks is insufficient in preventing cross-transmission of COVID-19; instead, it was recommended that N95 masks, which correspond to KF94 masks, should be worn daily. On the other hand, wearing a dental mask and following precautions such as hand hygiene may not pose a significantly higher risk of infection than wearing an N95 mask when treating COVID-19 patients. Conclusion: There is an ongoing discussion regarding the use of dental masks when treating dental patients, and many argue that different types of masks should be selected according to the degree of infection and the individual's respiratory condition. By considering the safety and efficacy of dental masks in preventing infection, improvements can be made in the management of COVID-19 and dental-related infections.

요인별 치과 의료서비스 질이 환자만족과 서비스 가치에 미치는 영향 (The effects of the quality of dental care service by factors on the patient satisfaction and service value)

  • The purpose of this study is to forecast patient satisfaction and service value through the staged regression analysis of the relation between each factor and its satisfaction and service value, following the measurement of the quality of dental care service that patients recognize. And a self-administered survey was used for this study. The subjects of this survey were 394 outpatients of dental clinics located in K city in the period from Dec. 5, 2004 to Feb. 19, 2005, the questionnaire was composed of a total of 31 questions to measure the quality of overall dental care service(22 questions), the patient satisfaction(6 questions) and the service value(3 questions). The 7 points Likert scale ranging from "very poor"(1-point) to "very high"(7-point) was used for these questions The results of study could be summarized as follows: 1. The most useful index to predict patient satisfaction turned out to be "internal environment", followed by "treatment process and waiting time", "dentist", "external environment", and "dental hygienist". 2. The service value had a significant effect on the "internal environment", "dentist", "treatment process and waiting time". 3. The most useful index to predict the service value turned out to be "treatment process and waiting time", followed by "internal environment" and "dentist". Today, the quality of dental care service is becoming a prime concern since it is directly connected to a matter of survival of medical service organizations. Dental clinics desperately need patient-oriented marketing strategies in order to actively cope with changing medical environment. They also need to thoroughly examine possible measures to maximize the service value by establishing a variety of service strategies which can promote service quality that patients recognize.

Motivational Interviewing Training Using Role Play in Dental Hygiene Students

  • The purpose of this study was to develop a motivational interviewing (MI) training program to improve competency in communication and to effect the evaluated changes that would occur in dental students' counseling techniques as result of the training in the program. The study proceeded through the program via role-play practice, which was explained effectively during the MI program training process. A convenience sample of 43 fourth-year dental hygiene students was recruited. Twenty-two students were randomly assigned to the intervention group and 21 to the control group in the order of recruitment. The theoretical lecture was delivered over 3 hours and the practice was done in parallel by applying cases occurring in clinic settings. The practice was technical training, partner practice, and small group practice. The intervention group received three hour MI lecture while the control group received no lecture. Evaluation through role-play practice was separated according to "researcher's viewpoint," "counselor's role viewpoint," and "patient's role viewpoint." The results of the analysis showed that the intervention group had higher MI skills and ability than the control group. Furthermore, program participants showed positive impressions to the MI training program. The use of coaching sessions improved the MI techniques and counseling skills of dental hygiene students learning MI. The effect of the application of the MI training program was that the MI training improved counseling skills and interviewing abilities. Moreover, continuous training and feedback enhanced MI techniques and core skills. Training using lectures in parallel with practice rather than education through theory alone improved students' techniques. Application of role play through the combined method of the MI program was confirmed by an effective training method.
