Full mouth rehabilitation of fully edentulous patient using implant hybrid prosthesis (완전무치악 환자에서 임플란트 하이브리드보철물을 이용한 전악 수복 증례)

  • Loss of teeth may not only imply impaired oral function and loss of alveolar bone but is also often accompanied by reduced self-confidence. This results in a larger problem with the fully edentulous patient. The patient introduced in this study showed multiple missing teeth and mobility of remaining teeth and wanted to have fixed dental prosthesis using implants. Remaining teeth were extracted because of periodontally bad prognosis. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using implant hybrid prosthesis in fully edentulous patient.

  • Individuals with cognitive disabilities face many barriers to oral care, often suffering from partial or complete edentulism. While the use of implant reconstruction is becoming more common in general population, such care is still being used infrequently in individuals with intellectual impairment. Because of the patients'oral hygiene practice is poor, special need is required for prosthetic design. These two cases were restored by prostheses using implants. Prosthetic modalities were different but the implants were fully osseointegrated, presenting satisfactory functional and esthetic conditions without clinical or radiographic signs of alterations or pathologies. We report that it is good for cognitive disabilities to give prosthetic treatment using implants.

  • Even if we can recover the function of mastication, pronunciation and esthetic using the fixed or removable prosthesis in the loss of teeth or hard tissue in the oral cavity, we use several kinds of implants in order to solve the problem which can be occured when we can't install the denture because of excessive bone resolution or psychlogical affairs. At present Titanium implant plays a major role in this field and has osseointegrated. And the study on the modern material is going on, that result in developing and using the implant which is coated with HA, bone induced material. In this, I studied histologically the change of the bone tissue which is occured when three kinds of HA coated implants. Such as Integral, Sustain, Biovent implanted into mandible of dog and got a 8-weeks healing period. I got the conclusion as follows: 1. Most of the implant which is covered with serveral kinds of HA coating implant in bone after 8 weeks being implanted and has osseointegrated, partially converted into the connective tissue. 2. Biovent formed the connective tissue in the perforated area of inferior alveolar canal and has osseointegrated.

Treatment of a Huge Odontogenic Myxoma in the Mandible with Surgical Resection and Reconstruction Using a Vascularized Fibular Free Flap: Case Report (하악골에 발생한 거대한 치성 점액종의 절제 및 비골 혈관화 유리 피판을 이용한 재건: 증례보고)

  • The odontogenic myxoma is a relatively rare, benign tumor that occurs on the jaw. A 41 year-old man visited Seoul National University Dental Hospital because of swelling of the mandible. Clinical and radiographic evaluation showed a huge mass invading most of the mandible. After biopsy, he was diagnosed with odontogenic myxoma. For resection of the lesion, partial mandibulectomy and reconstruction with a vascularized fibular free flap was done. The result showed successful removal of the lesion. Reconstruction resulted in satisfactory functional and esthetic outcomes. We conclude that huge benign neoplasms such as odontogenic myxomas can be successfully treated by using a wide margin of resection followed by vascularized fibular free flap reconstruction.

A literature review on the survival rate of single implant-supported restorations (단일 임플란트 지지에 의한 보철물의 생존율에 관한 문헌 연구)

  • Implant material, implant design, surface quality, status of the bone, surgical technique, and implant loading conditions were regarded as prerequisites for osseointegration which is a prime condition for implant success. The aim of this review paper was to investigate the survival rate of single implants in relation to the prerequisites for osseointegration. Fifty-eight papers reporting survival rates of single implants were selected by use of the 'PubMed' and hand searching. The survival rate of single implants were assessed with reference to factors influencing osseointegration. The results showed that single implants in general showed a high survival rate except a few failures in certain extreme conditions and early stages. Those failures and complications such as screw loosening and esthetic problem were almost solved with the development of implant components and surgical techniques and a better understanding of biology around a single implant. Single-tooth implant-replacement is now considered as a reliable and predictable treatment option for a single missing tooth and its application seems to expand to compromised situations which were previously thought to be impossible for single implant therapy.

  • Transferring condylar and anterior guidance on an articulator is essential to the diagnosis of a patient for full mouth reconstruction. In this clinical report, ARCUS digma I system was used to measure inherent condylar guidance of a patient requiring full mouth reconstruction in preoperate treatment, and the patients was given provisional restoration based on a functional anterior guidance. Then, ARCUS digma II system was used to mount the final casting model on an articulator, and the definitive prosthesis was placed in the patient. An esthetic and functionally proper clinical result regarding inherent condylar path of the patient was observed, and results from comparison of the two systems are given in this case.

  • In clinic, esthetic restoration of a defective natural tooth with composite resin is challenging procedure and needs complete understanding of the color of tooth itself and materials used. The optical characteristics of the composites are different because the chemical compositions and microstructures are not same. This review provided basic knowledge of the color and the color measurement devices, and analyze the color of the natural tooth. Further, the accuracy of the shade tab, color of the composite resins before and after curing, effect of the water, food and bleaching agent, and translucency, opalescence, and fluorescence effects were evaluated.

  • Excessive gingival display is an esthetic issue that is commonly managed by different procedures. Lip repositioning is a modality to address concerns of affected patients. The aim of this review was to investigate the scientific evidence on outcomes and long-term stability of lip repositioning surgery with or without myotomy. The electronic search was conducted in three databases: MEDLINE, Embase, and the Cochrane Library up to October 2019. No publication status, language, or time restrictions were applied. The electronic search was complemented by a manual search of the reference lists. Three hundred thirty-eight studies were screened by title, and 16 articles remained for data extraction. The included studies assessed the lip repositioning procedure in 144 patients aged between 15-59 years (134 females and 10 males). Based on the available data, lip repositioning with myotomy/muscle containment can be a successful treatment for minor discrepancies in gingival display in selected cases. However, further well-organized controlled clinical trials are recommended to derive a conclusion about the long-term stability compared with other alternatives.

  • Compared to conventional method, if metal framework of removable partial denture is fabricated by selective laser melting, various laboratory works are omitted, saving time and simplifying the process. In addition, metal framework with homogeneous density can be obtained, expecting excellent mechanical properties, especially resistance to fatigue fracture. In these cases, impression were taken using conventional methods in partial edentulous patients, master casts were fabricated and scanned to obtain digital data. After designing the metal frameworks on the scanned data, removable partial dentures were fabricated using selective laser melting methods. Through these procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

  • Black staining of the teeth in children and adolescents does not cause pain or serious illness, but it can be socially debilitating for esthetic reasons. Black staining is easily removed through periodic professional mechanical tooth cleaning and ultrasonic scaling, but it can easily recur within few months. Using essential oil-containing mouth rinses diluted at 50% twice per day could prevent the black staining from returning after it is removed, reducing the need for mechanical treatments and improving esthetics.
