A case of direct restore using 4-META/MMA-TBB resin containing organic filler in patients with severe occlusal surface wear and enamel fracture (심한 교합면 마모 및 교두 파절 환자에서 유기필러를 함유한 4-META/MMA-TBB 레진을 활용한 직접수복 증례)

  • Attrition is the loss of tooth hard tissue due to contact between teeth, and in severe cases, dentin is exposed, accompanied by selective corrosion and excessive wear of teeth, which is called cupping. If these lesions are left untreated, the size of the lesion gradually increases, breaking the unsupported enamel, resulting in a decrease in aesthetics and chewing function. In this case report, patients with cupping and enamel fracture due to severe attrition were directly restored using a resin with soft properties containing organic fillers. In the follow-up observation six years later, most of the filling of the occlusal surface was eliminated, but the filling on the buccal surfaces remained relatively intact, and it was confirmed that this type of resin was suitable for the area where the occlusal force was relatively weak rather than the area where the occlusal force was greatly applied.

Analysis of the factors of dental hygiene plans influencing patients of the dental hygiene program based on dental hygiene process (치위생과정에 근거한 구강건강관리프로그램 대상자의 치위생계획의 영향요인 분석)

  • Objectives: This study aims to recognize the importance of dental hygiene process diagnosis of dental hygiene process which can comprehensively grasp the patient's problem and to use it as a basis for establishing the patient's preventive treatment plan. Methods: This study did survey to 443 patients who received treatment based on the oral health care program from a dental clinic in Busan from January 2015 to January 2017. Data analysis was performed using IBM SPSS Statistics (Version 21.0), and statistical significance level was set at ${\alpha}=0.05$. Binary logistic regression analysis was performed to the dental hygiene problems affecting the dental hygiene plan. Results: There were significant differences in dental hygiene problems between male and female respondents on various dental problems such as dental plaque deposition, attrition, stain, dental fear, possibility of jaw joint disorder, food pressing, possibility of malocclusion. There were also significant differences in dental hygiene plans between male and female respondents in air-Jet, non-smoking education, and sealant. The most common dental hygiene plan was scaling, The problem of stain showed that the scaling plan was 0.20 times less. The explanatory power of the model was 43.5%, and the Hosmer and Lemeshow tests were 0.345. Conclusions: Therefore, if we continue to study the factors affecting the dental hygiene problems and the plan, we can reduce the burden of the dental hygienists applying the dental hygiene process in the dental clinic. And, it is expected that the oral health care program using the dental hygiene process will spread to the dental clinic as an excellent oral preventive program.

Full mouth rehabilitation of the elderly patient on anticoagulant medication with loss of vertical dimension due to severely worn dentition (전반적인 치아 마모로 수직 고경이 상실된 항응고제 복용 고령 환자의 전악 수복 증례)

  • Severe dental attrition causes pathological changes of the tooth, collapsed occlusion, and functional and aesthetic complications and can also result in a decrease in occlusal vertical dimension. Before increasing the vertical dimension with full-mouth rehabilitation, it is important to determine the amount of vertical dimension through accurate diagnosis. In this case, a 77 year old elderly male patient on anticoagulant medication with generalized attrition and fracture of teeth was treated with full-mouth rehabilitation in order to recover vertical dimension and aesthetics. Accurate clinical and radiographic examination, diagnostic, wax-up, and occlusal vertical dimension evaluation were step by step performed considering pre-medical history and old age. Patient adaptability was evaluated using an occlusal splint and interim restoration. After 3 months of stabilization with interim restoration, definitive prostheses were fabricated. Satisfactory functional and esthetic outcomes are observed after 6 months of follow up.

A Study on the Life Expectancy of the Dental Prosthetic Restorations (치과보철물의 평균수명에 관한 연구)

  • This study was performed to investigate the mean life expectancy of dental prosthetic restorations. The author has examined 352 dental prosthesis clinically and radiologically, and decided the success(survival) and failure(mortality) of the dental prosthesis. The dental prosthesis which had been treated in the Seoul National University Dental Hospital, two private clinics in Seoul, one university dental hospital, and two private clinics in local province were included in this study. The survival analysis using product limit estimator was used and the mean life expectancy of each type of dental prosthesis was calculated. The results were as follows : 1. The life expectancies were 10.5 years in gold crown and bridge, 8.5 years in porcelain fused to metal crown and bridge, 8.3 years in nonprecious metal crown and bridge, 8.1 years in removal partial denture, and 7.7 years in full denture. 2. The causes of mortality were in the order of dental caries(24.6%), fracture of dental prosthesis(19.2%), periodontal problems(18.6%), chronic chewing difficulty and dysfunction due to dental prosthesis(15.0%), excessive exposure of abutments due to the marginal defect of dental prosthesis(14.4%), abnormal occlusion due to severe attrition of artificial teeth in dentures(3.0%), periapical problems(2.4%), perforation of dental prosthesis(1.8%), and loose contacts with neighboring tooth(1.2%). 3. Among survival cases, 66.5% showed normal chewing ability and 31.9% showed partial chewing ability. However, 1.6% of them complained loss of chewing ability. 4. Among failure cases, 6.6% showed normal chewing ability and 38.9% showed partial chewing ability. However, 54.5% of them complained loss of chewing ability.

  • 두개하악장애는 저작근, 측두하악관절 및 그와 관련된 구조물의 많은 임상적인 문제를 포함하는 포괄적인 용어이다. 두개하악장애의 원인은 다양하고 종종 다인성이며 위험을 증가시키는 여러가지 소인들이 존재한다. 이갈이나 이악물기 같은 이상기능습관은 흔하며 또한 임상적으로 많은 관련이 있는 것으로 사료된다. 그러나 이전의 연구에서는 연령증가에 따른 기능적 마모를 고려한 치아교모에 관한 연구가 시행되지 않고 단순한 교모도의 평가비교만이 시행되었다. 본 연구는 법치의학적 연령감정에서 널리 사용되고 있느 치아교모도를 이용한 연령감정법을 이용하여 두개하악장애환자의 치아교모도에 관한 연구를 시행하였다. 연령은 기능적마모에 따른 정도를 평가하기 위해서 고려되었다. 단국대학교 치과병원 구강내과에서 두개하악장애로 진단된 환자18명과 대조군으로 외래환자 14명을 대상으로 하였다. 연령차는 연령감정법의 추정연령과 환자의 실제나이의 차이를 구해서 비교분석하였다. 이상의 결과로 두개하악장애환자의 중절치, 측절치, 제1소구치, 제2소구치의 치아교모도가 정상군보다 유의성 있게 높았다.

A Comparative Sudy on Accuracy of Occlusal Plane Angle on Mounting The Hanau 96-H2 Articulator (Hanau 96-H₂교합기에 상악 모형 부착시 교합면 경사각에 대한 비교 연구)

  • The Purpose of this study was to know the differences between the occlusal plane angles formed by F-H plane on cephalograms and the occlusal plane angles by the upper margin of the articulator, and to ind the contributing factors to the difference of occlusal angles. for this study, 39 young adults (20 men, 19 women) were selected who had normal occlusion, no severe attrition, no missing tooth, not been under orthodontic treatment and occlusal equilibration and no temporomandibular disorders. The maxillary casts were mounted with Hanau 159-1 ear piece type face-bow and measured the occlusal plane angle with prototype occlusal plane projector. The following results were obtained; 1. The mean value of the ear rod F-H plane occlusal angle was 14.75˚, articulator occlusal angle was 9.26˚ and strong positive correlation between these angles, 2. It was almost same angle between the ear rod F-H plane angle (14.75˚) and the beyron point to infraorbital rim-6mm. occlusal angle (14.46˚) 3. The mean distance from the ear rod porion to the Byron point was 5.67mm. 4. It was no significant correlation between the distances Po-Beyron point, Or-infraorbital rim and articulator occlusal angle.

Stress Analysis of the Occlusal Force on the Mandibular First Premolar

  • The occlusal force of the tooth leads to loss of tooth tissue owing to attrition and abrasion, and may cause abfraction and pathological change of the dentin. Thus, we developed finite element models, examined them by applying ordinary occlusal force, and analyzed the stress distribution. Specimens used were mandibular first premolars from 15 Korean males and 13 females and were made into finite element models from medical images that were obtained using a Micro-CT. We have found that the irregular feature of the tooth is not only useful to masticating and pronouncing as well known, but it is also suitable for protecting inner tissue by dispersing stress and delivering proper pressure to periodontal tissue to continue a physiological action. Also, image analysis could let us know the factor that is the cause of a disorder due to stress concentration in the cervical line. These results are expected to support the field of dental treatment planning, operating procedure and clinical trial, and the advance of technical expertise to develop implants and dentures.

Full mouth rehabilitation of a patient with occlusal plane discrepancy with milling machine for clinic (진료실용 밀링머신을 이용한 교합평면 부조화 환자의 완전 구강 회복술)

  • Recently, digital technology has become increasingly prevalent in the dental clinic. Using a milling machine for clinic, it is possible to produce provisional restoration inside the clinic. This can promote large clinical cases such as full mouth rehabilitation with the help of a tabletop scanner, which is capable of semi-adjustable articulator equipment, and a powerful dental CAD software with excellent user convenience. In this case report, a full-mouth rehabilitation was done with digital technology to a 55 year-old female patient, who has lost vertical dimension through the attrition, and has got inclined occlusal plane with unplanned and repeated dental reconstruction. Through the design and milling of the provisional restoration in the clinic and the duplication of these provisionals by double scanning technique, a good functional and esthetic result could be achieved.

  • Tooth wear is a multifactorial condition, leading to the loss of dental hard tissues. While for many years, tooth wear was a condition of little interest in daily clinical practice, nowadays this is changed. Tooth wear is becoming increasingly significant in maintaining the long-term health of the dentition. This becomes especially important when the dentition is kept relatively intact in the contemporary ageing population, and edentulism is decreasing. There is also evidence that the prevalence of tooth wear is growing. It is a natural consequence of the unidirectionality of tooth wear that it is frequently used as a means of individual age estimation. Tooth wear was very serious in ancient populations up to the medieval period. Tooth wear is thus studied in a wide variety of areas. The purpose of this paper is to summarize and analyze the many issues surrounding tooth wear through recent studies in various fields and to search for future research direction.
