This study addresses the effectiveness of a VR model that is designed to relieve distress and improve communication during pediatric dental treatment for 3-9 year-old young patients. This is due to the challenging nature of the dental treatment for both young patients and pediatric dentists. The proposed model is designed to alleviate the unpleasant experience a young patient might go through during the dental treatment through providing a VR content to be displayed in on a head-mounted device during the treatment session. In addition, the model is designed to help decrease the frequent movement of the young patient during the dental treatment session by directing the patient's head to the immersive VR content. Furthermore, the model could solve the communication difficulties that might occur between pediatric dentists and young patients during the treatment session by showing live instructions related to adjusting body postures
Background: Sevoflurane sedation in pediatric and disabled patients has the advantage of faster induction and recovery compared to general anesthesia, as well as minimum influence on the respiratory and cardiovascular functions, and airway protective reflexes. This study aimed to evaluate the clinical efficacy of sevoflurane sedation used in dental treatment at one provincial dental clinic center for the disabled. Methods: We investigated patients' gender, age, reasons for undergoing sedation, medication history prior to treatment, duration of anesthesia, treatment length, type of treatment, and yearly patterns, for 387 cases of dental treatment performed using sevoflurane sedation from January 2013 to October 2016. Results: We analyzed 387 cases (215 male patients, 172 female patients). Male patients aged 20 year or older accounted for 39.0% of all patients, marking the highest proportion. Patient's lack of cooperation was the most common reason for performing dental sedation. Prosthetic treatment was the most frequently practiced, accounting for 174 treatment cases. The mean lengths of the entire treatment and of the dental procedure were 55.2 min and 39.8 min, respectively. Conclusions: Sevoflurane sedation has the advantage of fast anesthesia induction and recovery compared to general anesthesia; therefore, it can be used efficiently to induce anesthesia in pediatric and disabled patients during short dental procedures, enabling stable treatment of these patients.
Modern endodontics has essentially changed following the introduction of the dental microscope since 1990's. One of main advantage of using dental microscope in nonsurgical endodontic treatment is enhancing clinician's ability and quality of treatment through illumination and magnification. Scopes of dental microscope in nonsurgical endodontics are finding a missed or additional root canal and a tooth crack, management of procedural errors, and others. These improvements in technology will result in greater confidence in treatment and better success in clinical practice.
The purpose of this study is to survey the status of oral care of adult patients with disabilities and satisfaction degree of outpatient general anesthesia for dental treatment. 30 adult patients who underwent general anesthesia for dental treatment at clinic for the disabled in Seoul National University Dental Hospital enrolled for telephone survey. Many patients (83.3%) had mental problems to do social communication. And someone of family members (mainly mother) had cared for the patient but there was only one patient who got regular oral care. The satisfaction level of outpatient general anesthesia and nursing care for dental treatment was very high(100%).
The effectiveness of dental implants in patients with disability, who are non-compliant during treatment, is controversial because of their poor oral health. Thus, oral health-care and management in such patients is concerning. Moreover, limited information is available on prognosis after implant placement. Herein, we describe a patient with schizophrenia who underwent dental implantation under multiple inductions of general anesthesia (5 times) and required conservative treatment and tooth extraction for multiple dental caries and retained roots because of inadequate oral health-care. Postoperatively, fracture of the prosthodontics and progression of dental caries were observed, and with 3 additional inductions of general anesthesia, conservative treatment, implant surgery, and prosthesis implantation were conducted. Postoperative 12-month follow-up since the last prosthesis implantation showed successful results. For patients with schizophrenia, multiple implantation can reduce horizontal bone loss and achieve aesthetic results compared to treatment with removable prosthodontics and could serve as an alternative treatment modality.
Korean dental hygienists perform more tasks in dental clinics than are described in the law. The purpose of this study was to investigate the actual work of Korean dental hygienists in dental clinics using an observational study design. Two dental hygienists observed and recorded the work performed in 1 day from the time of commencement to the time of leaving work by 18 dental hygienists working in the dental clinic in a single area. Data management, prevention of infection, counseling, calculus removal, radiography, assistance of fixed orthodontic treatment, assistance of fixed prosthetic treatment, and preparation for implant surgery were recorded. Among the tasks that take more than 1 min, the most time-consuming tasks were document management, infection control, consultation, calculus removal, preparation for implant surgery, assistance of orthodontic treatment, and assistance of fixed prosthetic treatment. Korean dental hygienists are performing various tasks, such as infection control, dental hospital/clinic management, oral disease prevention, and dental treatment assistants.
Pregnancy induces significant anatomical and physiological changes in the mother. Many pregnant women need dental treatment due to poor oral hygiene related to pregnancy. However, most dentists are reluctant to provide, and most pregnant women are reluctant to receive, dental treatment during pregnancy. Theoretically, maternally administered drugs are transferred to the fetus. Depending on the types of drugs and the stage of pregnancy, the effects of drugs on the mother, as well as the fetus, may vary. Local anesthetics are the most widely used in dental treatment. It is, therefore, important to understand the potential effects of local anesthetics during pregnancy. In this review, we will focus on the maternal and fetal effects of local anesthetics widely used in dental treatment with consideration of the use of local anesthetics during pregnancy.
Eisenmenger syndrome (ES) is characterized by pulmonary arterial hypertension and right-to-left shunting. The signs and symptoms of ES include cyanosis, shortness of breath, fatigue, hemoptysis, and sudden death. In patients with ES, it is important that the systemic and pulmonary circulations be properly distributed and maintained. General dental treatment is not known to be particularly dangerous. To control pain and anxiety, local anesthetics without epinephrine are usually recommended. However, in cases of difficulty of cooperation, general anesthesia for dental treatment makes the condition worse. In the present case, intravenous deep sedation with propofol and remifentanil was administered for behavioral management during dental treatment successfully.
Dental treatment under general anesthesia is considered for behavioral control of disabled patients who have severe anxiety or involuntary movement. However, in case of simple treatment, inhalation or intravenous sedation, which has earlier onset and recovery, is preferred. Conventionally, nitric oxide is used for inhalation sedation, nowadays sevoflurane can also be used due to easily titratable for controllable effect and less complications. In this case report, two patients with mental retardation required simple dental treatment. Deep sedation with inhaled sevoflurane were successfully employed and patients were discharged without any complications.
Objectives: A study investigated the effect that a dental orthodontics treatment has on the social life and self-worth perceived by the patient. The study was conducted to collect information on patients worried about dental orthodontics in later life. Methods: A survey was conducted on patients with dental orthodontics living in Seoul and Gyeonggi-do, from December, 2016. Of the collected 200 aquatic samples, 182 were used for the analysis, corresponding to 91% of the collected data, while excluding 18 insufficient responses. Results: The difference in self-worth and perceived social life before and after dental orthodontic treatment were investigated. Self-worth increased by 0.53 points after dental orthodontics treatment. Perceived social life increased by 0.81 points after dental orthodontics treatment. Conclusions: The relationship between self-worth and perceived social life after dental orthodontics treatment was found to be closely correlated. The study provides information on people hoping for treatment in accordance with the insurance benefits that should be offered.
