과잉치 치수유래 줄기세포의 분화제 처리 기간에 따른 상아모세포 발현 특성 (Characterization of Differentiation of the Supernumerary Dental Pulp Stem Cells toward the Odontoblast by Application Period of Additives)

  • 본 연구의 목적은 치과분야에서 줄기세포 공급원으로써 과잉치의 활용 가능성을 알아보고자 Real Time Quantitative Reverse Transcription Polymerase Chain Reaction (Real Time qRT-PCR)법을 이용하여 발치된 과잉치 치수 유래 줄기세포 (supernumerary dental pulp stem cells, sDPSCs)로부터 상아모세포로의 분화여부를 관찰해 보는 것이었다. 이를 위해 상아모세포의 대표적인 발현인자로 알려진 alkaline phosphatase (ALP), osteocalcin (OC), osteonectin (ON), dentin matrix acidic phosphoprotein 1 (DMP-1) 그리고 dentin sialophosphoprotein(DSPP)의 발현을 분화제 처리 후 0일, 8일 그리고 14일째에 각각 Real Time qRT-PCR 법을 통해 상대적인 mRNA의 발현 양을 비교하여 변화 양상을 알아보았다. 또한 Alizarin-red solution 의 염색을 통해 sDPSCs가 분화제 처리 7일, 14일, 21일 그리고 28일째에 석회화 결절을 형성하는 정도를 시각적으로 확인해 보았다. Real Time qRT-PCR 결과 분화제 처리 8일째에 가장 높은 발현 양을 보이다가 14일째에 감소하는 추세를 나타내었으며, Alizarin-red solution 염색 결과는 7일째부터 흐리게 나타나다가 14일째에는 배지 전반에 걸쳐 진하게 염색되는 소견을 보였다. 따라서, sDPSCs를 이용한 연구에서 Real Time qRT-PCR법을 위한 분화제의 처리 기간은 8일 정도가 적절하며, Alizarin-red solution 염색은 14일이 적당한 것으로 사료된다.

과잉치 치수 세포의 계대별 석회화 비교 (Comparison of Mineralization in Each Passage of Dental Pulp Stem Cells from Supernumerary Tooth)

  • 본 연구는 상악 정중과잉치에서 얻은 치수유래 줄기세포(human dental pulp stem cells from supernumerary tooth, sDPSCs)를 대상으로 분화능의 변화 특성을 알아보고자 하였다. 전신 병력이 없는 6세 남자아이 상악 중절치와 측절치 사이에 매복 된 과잉치를 발치하여 sDPSCs를 얻었다. 세포들을 16계대까지 배양하였고, 1 - 8계대는 Young군으로 9 - 16계대는 Old군으로 나누었다. 각 계대 배양에 소요된 시간은 Young군에서는 $2.25{\pm}0.46$일, Old군에서는 $3.25{\pm}0.46$일을 보였으며, 이는 통계적으로 유의한 차이를 보였다(p < 0.05). 각 계대 별로 세포 형태를 관찰하고 분화를 유도한 후 Alizarin-red solution 염색을 통해 골모세포(odontoblast)로 분화되는 정도를 관찰하였다. 1, 8, 9계대에서 분화제를 처리하지 않은 세포의 형태에서는 방추형의 섬유모세포와 유사한 길쭉한 형태로 과립(nodule)이 적었지만, 16계대에서는 세포의 크기가 커지고 넓적한 형태로 변하고 과립도 많아졌다. 1계대는 분화 7일부터 과립이 관찰되며, 8계대에서는 14일 동안 분화제를 처리한 후 과립이 명확히 관찰되었다. 그러나 9계대 이후에서는 과립의 빈도가 상당히 감소되었다. ARS 염색에서는 1, 8계대는 진한 붉은색으로 염색되었으나, 9, 16계대는 염색이 옅게 되었다. 이를 통해 sDPSCs는 8계대 이전의 세포를 줄기세포의 원천으로 우선 고려하는 것이 좋다고 사료된다.

인간치수세포에서 상아모세포의 분화과정 동안 Lysyl Oxidase Family의 역할 (Role of Lysyl Oxidase Family during Odontoblastic Differentiation of Human Dental Pulp Cells Induced with Odontogenic Supplement)

  • LOX는 결합조직의 세포외기질에 존재하여 교원섬유와 탄성섬유의 교차결합을 촉진시키는 핵심적인 효소이다. 뼈세포에서 세포분화 및 세포이동과 관련된 LOX의 역할은 보고되었으나, 상아모세포 분화에서 LOX에 대한 직접적인 효과는 거의 알려지지 않았다. 이 연구에서는 인간치수세포에서 상아모세의 분화과정 동안 LOX의 역할에 대해 실험하였다. 치수세포에 저 혈청 분화유도 배지를 처리하여 0, 1, 3, 7, 14일 동안 배양하였다. 세포성장은 세포계수, 분화와 관련된 유전자들은 RT-PCR, 광물화는 alrizarin red S 염색으로 각각 실험하였다. LOX 유전자 발현은 RT-PCR로 측정하였고, LOX 효소활성은 고감도 형광분석을 시행하였다. 1. 세포성장은 저 혈청 분화유도 배지에서 시간 의존적으로 증가하였다. 2. 분화표지자인 ALP, OPN, OCN, DMP1, DSPP는 시간 의존적으로 발현되었으나 초기, 중기, 말기에 대한 차별화는 이루어지지 않았다. 3. 광물화는 3일부터 관찰되기 시작하여 14일까지 증가되었다. 4. LOX와 LOXL은 mRNA 발현이 7일까지 증가되었고, 14일에서 큰 감소를 보였다. 하지만, LOXL3와 LOXL4 mRNA는 낮은 발현을 보였고, LOXL2 mRNA는 발현되지않았다. 5. Collagen type I mRNA는 7일까지 증가하다가 14일에서 의미있게 감소를 보였고, collagen type IV는 낮은 mRNA 발현을 보였다. 6. LOX 효소 활성은 분화유도기간 중 7일에서 가장 많은 발현을 보였고, 교원질 1형 기질이 교원질 IV형 기질보다 두드러진 발현을 보였다. 이상의 결과를 종합해 볼 때, LOX isoform의 발현과 LOX 효소 활성은 인간치수세포에서 상아모세포 분화과정 동안 중요한 조절자로 사료된다.

  • After orthognathic surgery, postoperative complications are studied by many clinician. The complications include sensory disturbance, jaw fracture, excessive bleeding, condylar positional changes and loss of pulp vitality. Few surgical procedures are as satisfying for the surgeon and patient as a well-done orthognathic surgery. On the other hand, the patient is more satisfied with the result than who are treated with only orthodontic treatment especially in severe deformity case. There are problems that patient overcome but it is not serious complications. One of these, the problem about loss of pulp vitality can't influence function but give a lot of discomfort to the patient. From September 1997 to January 1998, 7 patients who are treated for dentofacial deformity via Le Fort I osteotomy or anterior segmental osteotomy were examined pulp sensitivity using digital pulp tester. This preliminary study have a focus on the investigation of recovery of pulp vitality. The electric pulpal test were used at preoperative, postoperative, at intervals. And we report some results acquired from this study. Follwing result are obtained 1. In anterior segmental ostetomy case (1 case), total 12 teeth were examined. Postoperative 8 weeks, 1 tooth are positive reaction 2. In Le Fort I osteotomy case (6 case), total 71 teeth were examined. Postoperative 8 weeks, 5 teeth are positive reaction

치과용 레이저 조사가 배양 치수 섬유모세포에 미치는 영향에 관한 연구 (The EFFECTS OF DENTAL LASER ON PULP FIBROBLAST IN VITRO)

  • The responses of human pulp fibroblastic cells to Ga-As Semi-Conductor-Dens-Bio Laser (Frequency: 5 Hz~10,000 Hz Model: SD-101A RCA, U.SA)) were examined in vitro using pulp fibroblastic cells obtained from the pulp tissue of human tooth. The mitogenic effect of soft laser was assessed by measuring the MTT assay. The morphologic effect for soft laser showed under the scanning and transmission electron microscopy. The results as follows; 1. The mitogenic response of the soft laser was not observed until 4th time of radiation, while the mitogenic response at 4th time increased mitogenic effect by as much as 1.7 fold compared to the control value. 2. The mitogenic response of the soft laser on pulp fibroblast differ from the mitogenic response on other fibroblasts. 3. In scanning electron microscopic study, The microvilli of cell surface increased gradually with width and length after laser radiation, it demonstrate that development of microvilli have close connection with differentiation of cells. 4. Under the transmission electron microscope, The laser-treated cells maintained their elongated shape and a high degree of cellular polarization. The large cell body containing a well developed Golgi complex, a large number of profiles of rough endoplasmic reticulum, and great numbers of mitochondria. 5. The laser-treated cells maintained the long straight bundles of closely apposed microfilaments or individual filaments forming a cross-linked network. These findings suggest that the laser may have important roles in promotion of pulp healing and consequently may be useful for clinical application in pulp regenerative procedures.

광활성 유무가 치아미백과 치아표면 및 치수 내 온도에 미치는 영향 (Tooth bleaching effect by light activation on the tooth surface and intra-pulpal temperature: an in vitro study)

  • Objectives : The purpose of this study was to investigate of the color change, tooth surface and intra-pulpal temperature of tooth bleaching by light activation Methods : Forty-eight extracted bovine teeth were immersed into a tea solution for 24 hours. The specimens were randomly divided into four groups(n=15):(G1) 15% HP + without light activation, (G2) 15% HP + light activation, (G3) 25% HP + without light activation, (G4) 25% HP + light activation. All specimens were bleached for 15 minutes three times. The spectrophotometer (CM-2600d, Konica Minolta, Osaka, Japan) was used including before bleaching, immediately after bleaching, 1 week, 1 and 3 months after the end of bleaching. The temperature rise were measured in the pulpal chamber and tooth surface with a digital thermocouple thermometer(Termopar Digital Multimeter, Tektronix DMM916, USA). Between the tested time points, the specimens were stored in distilled water. The data were analyzed by ANOVA, t-test and Tukey's post hoc test set at 0.05. Results : There was no significant color change by the use of light after the bleaching treatment(p>0.05). The dental bleaching treatments of teeth with 15% HP and 25% HP did not seem to be more effective when light source was used. There was no difference in color stability between groups within three month(p>0.05). There was an increase in tooth surface and pulp temperature, but it was not sufficient to cause damage to the pulp. Conclusions :The use of light activation has no obvious effective impact on the tooth bleaching effect.

Biocompatibility of two experimental scaffolds for regenerative endodontics

  • Objectives: The biocompatibility of two experimental scaffolds for potential use in revascularization or pulp regeneration was evaluated. Materials and Methods: One resilient lyophilized collagen scaffold (COLL), releasing metronidazole and clindamycin, was compared to an experimental injectable poly(lactic-co-glycolic) acid scaffold (PLGA), releasing clindamycin. Human dental pulp stem cells (hDPSCs) were seeded at densities of $1.0{\times}10^4$, $2.5{\times}10^4$, and $5.0{\times}10^4$. The cells were investigated by light microscopy (cell morphology), MTT assay (cell proliferation) and a cytokine (IL-8) ELISA test (biocompatibility). Results: Under microscope, the morphology of cells coincubated for 7 days with the scaffolds appeared healthy with COLL. Cells in contact with PLGA showed signs of degeneration and apoptosis. MTT assay showed that at $5.0{\times}10^4$ hDPSCs, COLL demonstrated significantly higher cell proliferation rates than cells in media only (control, p < 0.01) or cells co-incubated with PLGA (p < 0.01). In ELISA test, no significant differences were observed between cells with media only and COLL at 1, 3, and 6 days. Cells incubated with PLGA expressed significantly higher IL-8 than the control at all time points (p < 0.01) and compared to COLL after 1 and 3 days (p < 0.01). Conclusions: The COLL showed superior biocompatibility and thus may be suitable for endodontic regeneration purposes.

Comparison of dental radiography and computed tomography: measurement of dentoalveolar structures in healthy, small-sized dogs and cats

  • Background: Dental diseases are common in dogs and cats, and accurate measurements of dentoalveolar structure are important for planning of treatment. The information that the comparison computed tomography (CT) with dental radiography (DTR) is not yet reported in veterinary medicine. Objectives: The purpose of this study was to compare the DTR with CT of dentoalveolar structures in healthy dogs and cats, and to evaluate the CT images of 2 different slice thicknesses (0.5 and 1.0 mm). Methods: We included 6 dogs (2 Maltese and 1 Spitz, Beagle, Pomeranian, mixed, 1 to 8 years, 4 castrated males, and 2 spayed female) and 6 cats (6 domestic short hair, 8 months to 3 years, 4 castrated male, and 2 spayed female) in this study. We measured the pulp cavity to tooth width ratio (P/T ratio) and periodontal space of maxillary and mandibular canine teeth, maxillary fourth premolar, mandibular first molar, maxillary third premolar and mandibular fourth premolar. Results: P/T ratio and periodontal space in the overall dentition of both dogs and cats were smaller in DTR compared to CT. In addition, CT images at 1.0 mm slice thickness was generally measured to be greater than the images at 0.5 mm slice thickness. Conclusions: The results indicate that CT with thin slice thickness provides more accurate information on the dentoalveolar structures. Additional DTR, therefore, may not be required for evaluating dental structure in small-sized dogs and cats.

Does the presence and amount of epinephrine in 2% lidocaine affect its anesthetic efficacy in the management of symptomatic maxillary molars with irreversible pulpitis?

  • Background: This was a randomized controlled clinical trial that aimed to evaluate the anesthetic efficacy of 2% lidocaine combined with different concentrations of epinephrine (plain, 1:200,000 and 1:80,000) during endodontic treatment of maxillary molars with symptomatic irreversible pulpitis. Methods: The trial included 144 adult patients who were randomly allocated to three treatment groups. All patients received buccal-plus-palatal infiltration. After 10 min, pulp sensibility testing was performed using an electric pulp test (EPT). If a tooth responded positively, anesthesia was considered to have failed. In the case of a negative EPT response, endodontic access was initiated under rubber dam isolation. The success of anesthesia was defined as having a pain score less than 55 on the Heft Parker visual analog scale (HP VAS), which was categorized as 'no pain' or 'faint/weak/mild' pain on the HP VAS. Baseline pre-injection and post-injection maximum heart rates were recorded. The Pearson chi-square test was used to analyze the anesthetic success rates at 5% significance. Results: Plain 2% lidocaine and 2% lidocaine with 1:200,000 epinephrine and 1:80,000 epinephrine had anesthetic success rates of 18.75%, 72.9%, and 82.3%, respectively. Statistical analysis indicated significant differences between the groups (P < 0.001, 𝛘2 = 47.5, df = 2). The maximum heart rate increase was seen with 2% lidocaine solution with epinephrine. Conclusion: Adding epinephrine to 2% lidocaine significantly improves its anesthetic success rates during the root canal treatment of maxillary molars with symptomatic irreversible pulpitis.

비도시지역 학교인구의 구강보건진료소비실태에 관한 조사연구 (A study on the school dental health care in rural area)

  • In order to develop the school dental health care in rural area, the author collected data about the population of all 6-17 year students living in Young dong-gun county, and Surveyed their dental health cares during one year of 1982. From the collected data, several dental health indices such as percentage of students of all population, percentage of students who visited dentists once or more during one year, average annual dentist visit and average annual dental treatment case were calculated and discussed. The obtained results were as follows; 1. The percentage of students of all population in Young dong-gun county was 29.65%. 2. The percentage of students who visited dentists once or more during one year was 4.67%. 3. The average annual dentist visit per student was 0.11. 4. The average annual dental treatment case per student was 0.16. 5. The oral examination case was 0.05, intraoral radiograph 0.01, oral prophylaxis 0.00, filling of dental carious lesion 0.02, pulp treatment 0.02, extraction of teeth 0.04, and others 0.02 annually in the average. In comparison with detectable need for dental treatment cases, oral prophylaxis was not supplied at all, filling of dental carious lesion was supplied about 1% and extraction of teeth was supplied about 10% of detectable need. 6. It was recommended that school incremental dental care project should be developed for school dental health programme in order to supply all of the detectable need for dental treatment.
