Enhanced visualization of the root canal morphology using a chitosan-based endo-radiopaque solution

  • Objectives: This study aimed to investigate the efficacy of ionic and non-ionic-based contrast media (in vitro study) and the combinatorial effect of chitosan-based endo-radiopaque solution (CERS) (in vivo study) for visualization of the root canal anatomy. Materials and Methods: In vitro study (120 teeth): The root canal of maxillary premolars and molars (in vitro group 1 and 2 respectively, n = 60 each) were analyzed using 4 different contrast media (subgroups: Omnipaque 350, Iopamidol, Xenetix 350, and Urografin 76; n = 15 each) in combination with 5.25% sodium hypochlorite (NaOCl). Based on the results of the in vitro study, in vivo study (80 teeth) was done to compare Xenetix 350 + 5.25% NaOCl with CERS (in vivo group 1 and 2 respectively, n = 40 each) on maxillary and mandibular premolars and molars. Two endodontists used radiovisiography to assess the depth of ingress and identify the aberrant root anatomy after access cavity preparation, and after initial cleaning and shaping of canals. Kruskal-Wallis test was used for in vitro comparison (p < 0.05), and Wilcoxon signed-rank test and Mann-Whitney U test for in vivo analysis (p < 0.01). Results: In vitro study, Xenetix 350 + 5.25% NaOCl facilitated a significant higher visualization (p < 0.05). For in vivo study, CERS had a statistically significant depth of ingress (p < 0.01), and was efficient in identifying the aberrant root canal anatomy of premolars and molars. Conclusions: CERS facilitates better visualization of the root canal anatomy of human premolars and molars.

Bone repair in defects filled with AH Plus sealer and different concentrations of MTA: a study in rat tibiae

  • Objectives: This study aimed to evaluate the effects on bone repair of different concentrations of mineral trioxide aggregate (MTA) added to AH Plus. Materials and Methods: Bone tissue reactions were evaluated in 30 rats (Rattus norvegicus) after 7 and 30 days. In the AH + MTA10, AH + MTA20, and AH + MTA30 groups, defects in the tibiae were filled with AH Plus with MTA in proportions of 10%, 20% and 30%, respectively; in the MTA-FILL group, MTA Fillapex was used; and in the control group, no sealer was used. The samples were histologically analyzed to assess bone union and maturation. The Kruskal-Wallis and Mann-Whitney tests were performed for multiple pairwise comparisons (p ≤ 0.05). Results: At the 7-day time point, AH + MTA10 was superior to MTA-FILL with respect to bone union, and AH + MTA20 was superior to MTA-FILL with respect to bone maturity (p < 0.05). At the 30-day time point, both the AH + MTA10 and AH + MTA20 experimental sealers were superior not only to MTA-FILL, but also to AH + MTA30 with respect to both parameters (p < 0.05). The results of the AH + MTA10 and AH + MTA20 groups were superior to those of the control group for both parameters and experimental time points (p < 0.05). Conclusions: The results suggest the potential benefit of using a combination of these materials in situations requiring bone repair.

투명표본에 의한 상악견치의 근관형태에 관한 연구 (A STUDY ON THE ROOT CANAL MORPHOLOGY OF HUMAN MAXILLARY CANINE WITH TRANSPARENT SPECIMENS)

  • Eighty-two maxillary canine were injected with china ink, decalcified, cleared and used in a study, in vitro, to determine the number of root canals, the frequency and location of lateral canals, the location of apical foramens, and the curvature of root canals. The results were as follows; 1.All of the maxillary canines showed the single canals. 2. Of the 82 canals studied, 20.7% of the canals were found to have lateral canals and most ramification of them were located in the apical third of the root. 3. In the case of the curvature, the mesial curvature of the canals (40.3%) usually showed more than the distal curvature of them (13.4%) 4. 41.2% of the apical foramen were located directly on the root apex and 58.5% of them laterally.

투명표본에 의한 하악중절치의 근관형태에 관한 연구 (A STUDY ON THE ROOT CANAL MORPHOLOGY OF HUMAN MANDIBULAR INCISORS WITH TRANSPARENT SPECIMENS)

  • Sixty six mandibular incisor were injected with China ink, decalcified, cleared and used to study the number of root canals, the frequency and location of lateral canals, the location of apical foramens, the frequency of apical deltas, and the curvature of root canals. The results were as follows: 1. Most of the teeth demonstrated single canal, but 19.7% of the teeth were found to have bifurcated canals. 2. 12% of the teeth were found to have lateral canals and these ramifications were located in the apical third of the root. 3. 25 apical foramens were located directly on the root apex and 41 foramens laterally. 4. 43 canals showed straight, 7 canals distal curvature, 7 labial curvature, 3 mesial curvature.

  • Today, maxillary sinus graft is considered to be the most prognostic of bone augmentation surgery as a preprosthetic treatment. Implant survival rates of more than 95% can be expected if appropriate decisions are made on the basis of implants, implant surface morphology, and use of a shield over the maxillary sinus front-wall. In addition, maxillary sinus grafty has a low rate of complications, and even if complications occur during or after maxillary sinus graft, most are localized and easily recovered.

  • This study was undertaken to find out the characteristic craniofacial morphology of Class II Division 2 malocclusions in children by means of roentgenocephalometry. The subjects consisted of twelve boys and thirteen girls with Class Ii, Division 2, thirty seven boys and fifty three girls with Class II, Division 1, and forty six boys and eighty one girls with normal occlusion, ranged from 10 years old to 18 years old. The following results were obtained; 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 2 malocclusion similar to the normal occlusion, but the mandible was the posterior position in th the cranial anatomy. 2. There were no significant differences in the anteroposterior relationship of the maxilla and the mandible between Class II, Division 1 and Class II, Division 2 malocclusions. 3. In Class II, Division 1 the axes of maxillary incisors showed labial inclination, but lingual inclination in Class II, Division 2 malocclusions. 4. Overbite was prominent one in Class II, Division 2, on the other hand overjet was distinguished in Class II, Division 1.

임상가를 위한 특집 1 - 큰광대근과 작은광대근의 해부학적 연구 (An anatomic study of the zygomaticus major and minor muscles)

  • The aim of this study was to clarify the arrangement of the zygomaticus major muscle, and to describe morphology of zygomaticus minor muscle. After a detailed dissection, the zygomaticus muscles were observed in 66 embalmed cadavers. It was found that the insertion of zygomaticus major was divided into superficial and deep bands(42/70, 60%). Zygomaticus minor was inserted not only upper lip also alar portion(5/54, 9.2%). The arrangement and insertion patterns of the zygomaticus muscles in this study are expected to provide critical information for understanding or smile pattern and treatment or fold.

  • In this study, precipitation behaviors of hydroxyapatite on highly ordered nanotubular Ti-35Ta-xNb alloy surface were researched. Ta and Nb additions to Ti increased corrosion resistance. The surface characteristics of anodized alloy depended on the nanotube formed voltage and alloy element. The HA precipitation morphology was influenced by nanorubular structure of alloys.

근관 치료의 숨겨진 실패 원인- Coronal leakage (Coronal leakage- The hidden cause of endodontic failure)

  • Perhaps the most frustrating scenario in dentistry lies in the apparently well-instrumented and well-obturated tooth that, becomes symptomatic after treatment of shows a periapical lesion on recall examination. Conventional root canal treatment demands stringent adherence to asepsis, awareness of canal morphology, and mastery of a variety of clinical techniques. But the risk of endodontic failure does not end with canal obturation. The quality of the coronal restoration may have a greater bearing on the ultimate success of a case than the quality of the root canal treatment itself. This review of the endodontic literature will define coronal leakage, describe its influence on endodontic treatment failures, and make specific recommendations on prevention.

Dento-maxillofacial Abnormalities Caused by Radiotherapy and Chemotherapy

  • A case of dento-maxillofacial abnormality involving a 10-year-old male patient with a history of esthesioneuro-blastoma is presented. This patient had been treated with 54 Gy /sup 60/Co-gamma-radiation to the nasal cavity for 6 weeks and 6 cycles of combination chemotherapy of Cyclophosphamide, Cisplatin, Adriamycin, VM-26 (Tenipo-side), and DTIC (Dacarbazine) when he was 16 months of age. Five years after cessation of cancer therapy, he was disease free and transferred for extensive dental care to Kyung Hee University Dental Hospital. A clinical and radiologic follow-up over last 4 years showed root stunting, premature closure of the root apices, microdontia, developmental arrest, small crowns, and partial anodontia. Maxillofacial morphology evaluated by cephalometric analysis showed deficiency of maxillary development.
