본 연구는 청소년들이 느끼는 치과에 대한 공포와 불안감에 대해 알아보고 치과이용 실태와 과거 통증 경험 등이 어떤 영향을 미치는지에 관하여 알아보고자 경기 지역 일부 고등학교 352명을 대상으로 설문조사를 실시하여 다음과 같은 결과를 얻었다. 주사바늘이 살을 찌를 때($3.19{\pm}1.43$)와 마취용 주사바늘을 볼 때의 두려움($3.14{\pm}1.44$)이 높게 나타났고, 치아삭제용 드릴 소리($2.82{\pm}1.38$)와 치아삭제용 드릴을 볼 때의 두려움($2.74{\pm}1.36$), 치과 내원 시 근육 긴장($2.51{\pm}1.34$), 치료 대기 시 두려움($2.45{\pm}1.37$), 치아삭제 시 두려움($2.43{\pm}1.31$) 등의 순으로 나타났다. 치과치료 실태에 따라 치과 공포와 불안 수준에 차이가 있는지를 살펴본 결과, 충치치료를 받기위해 방문한 집단($2.75{\pm}0.94$)이 스켈링을 위해 방문한 집단($1.50{\pm}0.65$)에 비해 치과에 대한 공포와 불안감을 많이 느끼는 것으로 나타났다(p<0.01). 치과 공포와 불안에 영향을 미칠 것으로 예측되는 과거 치과 치료 시 통증 경험 수준은 3.45로 비교적 높게 나타났고, 가족이나 친구의 치과치료에 대한 통증을 들은 경험은 3.26점, 마취 미비 상태에서의 치과치료 경험은 2.47점으로 높지 않은 것으로 나타났다. 과거 치과 이용 시 통증 경험과 치과 공포 불안간의 관련성을 살펴보기 위하여 상관분석을 실시한 분석 결과 치과 치료 시 통증 경험과 마취 미비 상태에서의 치료 경험, 그리고 가족/친구의 치과치료 통증을 들은 경험 등의 변인 모두 치과에 대한 공포와 불안감과 유의미한 정(+)적 상관관계를 보였다. 학생의 성별과 현재의 치아 건강상태, 흡연 여부, 그리고, 치과치료 시 통증 경험, 마취 미비상태에서의 치료 경험, 가족/친구의 치과치료 통증을 들은 경험 등 과거의 치과 통증 경험 등의 변인이 치과 공포와 불안에 영향을 미치는 중요 예측 변인이었다.
Objectives: This study aimed to evaluate the effects of 5% lidocaine and 2.5% lidocaine/2.5% prilocaine topical anesthetic on pain during needle insertion and infiltration injection in the labial mucosa of anterior maxillary teeth, and to assess the relationship between patients' anxiety and pain scores. Materials and Methods: The Modified Dental Anxiety Scale questionnaire was applied and recorded. Patients were randomly divided into 4 groups (n = 30), as follows: G1 group: 5% lidocaine and placebo for 1 minute, G2 group: 2.5% lidocaine/2.5% prilocaine and placebo for 1 minute, G3 group: 5% lidocaine and placebo for 3 minutes, and G4 group: 2.5% lidocaine/2.5% prilocaine and placebo for 3 minutes. Before the application of topical anesthesia, one side was randomly selected as the topical anesthesia and the contralateral side as the placebo. The pain levels were measured with Visual Analog Scale (VAS) immediately after needle insertion and injection and were compared. The correlation between anxiety and pain scores was analyzed. Results: Administration of 5% lidocaine for 1 minute had significantly higher pain scores for both insertion and infiltration injection than the other groups (p < 0.05). There was a significant moderate positive correlation between dental anxiety and the injection-induced VAS pain score in the placebo side in all groups (p < 0.05). Conclusions: Topical anesthetics significantly reduced the pain caused by both needle insertion and injection pain in comparison to the placebo side. The pain scores of patients with dental anxiety were lower on the topical anesthesia compared to the placebo side.
연세대학교 치과대학병원에서 1997년 7월부터 2000년 7월까지 외래 전신마취하에 치과치료를 시행한 235명의 치과 장애인 환자를 대상으로 연령 분포, 전신마취 선택이유, 소요시간, 마취시간, 치료내용과 합병증 등을 분석하고, 우편 설문 조사에 응답한 109명을 대상으로 보호자의 연령과 교육정도, 보호자의 전신마취 경험 유무, 환자의 연령, 치과치료 경험의 유무 등에 따른 보호자의 불안정도와 만족도를 조사하여 다음과 같은 결과를 얻었다. 1. 아동의 전신마취시 보호자가 느끼는 불안정도는 아버지보다 어머니가 더 높았으며, 보호자의 학력이 높을수록 불안정도는 감소하는 경향을 보였다.(P<0.05) 2. 보호자의 학력이 높을수록 전신마취 치료의 만족도는 높았다.(P=0.05) 필요하다면 다시 전신마취하에 치과치료를 받겠냐는 설문에 95명(87.2%)이 받겠다고 응답하였다. 이상의 결과로 볼 때 외래 전신마취하의 치과치료는 환자 보호자에게 통상의 치과치료보다 불안을 야기하지만 치료에 대한 만족도는 높았으며 행동조절의 어려움으로 인해 치과 치료받기가 어려운 환자에게 양질의 치과 서비스를 제공할 수 있는 방안임을 알 수 있었다.
치과에 대한 거부감은 치과 치료의 역사만큼이나 오래되었다. 역사와 함께한 치과 치료 기술은 절개와 절삭을 포함하는 효과적인 치료와 함께 통증을 최소화하도록 지속적으로 발전하였으나 치과에 대한 부정적 인식은 기술의 발전만큼 감소하지는 못하였다. 특히 소아치과 환자들은 외부 자극에 대한 저항성, 정신적 발육, 인내심 등의 부족과 함께 부모의 영향을 포함하는 양육 방식의 변화로 인하여 DFA(dental fear and anxiety) 및 DBMP(dental behavior management problem)을 경험할 수 있다. 치과에 내원하는 환아가 경험하는 다양한 자극들 가운데 심리에 영향을 주는 요소들로는 치과의사, 치과 진료 기구들이 있으며 치과의사와 관계를 형성하는데 영향을 주는 사회적인 요인이 존재한다. 특히 최근에는 스마트폰과 관련한 중독이 사회문제로 대두되고 있으며 스마트폰의 사용이 구강에 미치는 영향과 치과에서 보이는 행동에 미치는 영향에 대해 알아본다.

Dental sedation plays a pivotal role in alleviating patient anxiety during various procedures. Remimazolam, a benzodiazepine derivative, stands out for its distinctive attributes, particularly its rapid onset of sedation coupled with a brief duration, making it an invaluable option for dental applications. The patient was admitted for the extraction of impacted third molars via patient-controlled sedation and not only demonstrated stable vital signs but also expressed a high level of satisfaction with the procedure. An in-depth analysis of plasma remimazolam concentrations and changes in the Patient State Index revealed negative correlation patterns, highlighting the inherent potential of remimazolam in achieving effective sedation. This expanded research scope aims to provide a more nuanced understanding of the pharmacological responses to remimazolam in dental sedation scenarios. This case report offers valuable insights into the evolving landscape of dental sedation methodologies and paves the way for a more informed and evidence-based approach to the use of remimazolam in patient-controlled sedation.
Background: Dental fear is usually associated with hemodynamic changes. Fear of pain during the surgical removal of a lower impacted third molar might cause patients anxiety, thereby leading to avoidance of any future dental therapy. This study aimed to determine the effect of experiencing a surgical impacted-tooth removal on the pain perception-related anxiety and hemodynamic status. Method: Twenty-seven healthy patients aged 15-30 years (mean age, 24 years), for whom surgical removal of bilateral lower third molars was advised, were included. This prospective, randomized, controlled, split-mouth study involved operations on both sides of the mandibular arch, with a 1-month washout period in between. Blood pressure and heart rate were measured before the surgical procedure, during and after the injection, preoperatively, and postoperatively. Pain perception was evaluated using a 100-mm visual analog scale during the injection, preoperatively, and postoperatively after the numbness disappeared. Differences in the blood pressure, heart rate, and pain perception between the two appointments were analyzed using the paired t-test. For all statistical analyses, SPSS version 11.5 was used. Results: The mean pain perception values during the injection and preoperatively showed no significant differences between the two appointments (P > 0.05); however, significant differences in the blood pressure and heart rate were noted before the surgical procedure; preoperatively, the blood pressure alone showed a significant difference (P < 0.05). Conclusion: There was a significant decrease in the blood pressure and heart rate preoperatively; hence, experiencing a surgical impacted-tooth removal can reduce the subsequent preoperative anxiety in healthy patients.
Objectives : This study was progressed a research with the aim of using it as basic data necessary for a plan in order to increase qualitatively medical level for patients who feel anxiety about dental treatment, and of offering basic data for improving the whole medical services by grasping conditions and satisfaction level with laser therapy in patients who visited dental clinic. Methods : The statistical analysis was carried out t-test and ANOVA by using SPSS(PASW statistics) 18.0. When there is statistically significant difference in analysis of variance, post-test was carried out by using Duncan multiple range test. Results : In therapy kinds of using laser, gingival treatment was the largest with 37.0%. It was the highest in likely seeming to be not painful with 32.9% as for a decisive factor, which came to receive laser therapy, and in having been enough to endure even if having been slightly tingling as for pain level. Conclusions : This study can be considered to have significance in changing recognition of medical cure by surveying patients satisfaction with using laser as a method for slightly solving elements of fear and anxiety about dental treatment and to have its value as basic data for seeking improvement plan for diverse medical services in the future.
This study was done as a pilot study to develop a color test measuring children's dental anxiety. The subjects of study were 511 preschool children aged from 3 to 6. Caries experience was examined. Children's drawings of the dentist were collected and analyzed by the method of human figure drawing test modified by Sonnenberg and Venham. Colors of high dental anxiety level were black, brown, purple, red, yellow, pink, green, orange, blue, gray, yellow ochre, and white in that order. Colors of high frequency of use were yellow ochre, green, blue, red, orange, pink, black & purple, yellow, brown, gray, and white in that order. As the age increased, white & gray, yellow ochre, and blue increased and black, purple, and pink decreased. Boys liked green and blue, and girls liked orange and yellow. Filled teeth rate was highest in yellow ochre group and very low in black group. Children who had an educational dental visit before the test used green color more than the rest of children.
Purpose: This study aims to induce interest in the future society that could change with the advance of the 4th Industrial Revolution. It also intends to identify the direction of university education by investigating the perception of university students majoring in dental laboratory technology about the latest technology used in this era. Methods: The study recruited 240 college students majoring in dental laboratory technology at K and D universities in Gangwon-do and Daejeon, respectively. Online and face-to-face surveys were conducted from March 20, 2022 to April 10, 2022. Results: Although their interest in the use of emerging technologies during the 4th Industrial Revolution was above average 3.85, the dental laboratory technology students exhibited relatively high levels of anxiety 3.19 and reluctance 2.96. Second, their interest in emerging technologies demonstrated positive correlations with major satisfaction and understanding of concepts and readiness, whereas reluctance and anxiety pointed to negative correlations with understanding of the concepts of the 4th Industrial Revolution. Conclusion: Formulating guidelines is necessary to help students increase their understanding of the 4th Industrial Revolution and prepare them appropriately by setting an educational direction that can flexibly respond to the rapidly changing industrial environment.
