Usefulness of Predictors for Hepatotoxicity in Acetaminophen Poisoning Patient (아세트아미노펜 중독 환자에서 간독성 발생 예측인자들의 유용성)

  • Purpose: The purpose of this study was to determine whether hepatotoxicity could be predicted early using biochemical markers in patients with acetaminophen (AAP) poisoning and to assess the usefulness of predictive factors for acute liver injury or hepatotoxicity. Methods: This study was a retrospective observational study involving a medical records review. The participants were patients who were admitted to the emergency department (ED) with AAP overdose at two hospitals over a 10-year period. Demographic data, age, time from ingestion to visit, initial AAP level, initial hepatic aminotransferases, and initial prothrombin time were recorded. Acute liver injury was defined as a peak serum ALT >50 U/L or double the admission value, and hepatotoxicity was defined as a peak ALT >1,000 U/L. Receiver operating characteristic curve analyses were performed to compare the prognostic performance among variables. Results: A total of 97 patients were admitted to the ED with AAP overdose, of whom 26 had acute liver injury and 6 had hepatotoxicity. Acute liver injury was associated with the time interval after taking the drug, and hepatotoxicity was associated with the initial PT and the ALT level. The scoring system proposed by the authors has a significant ability to predict both acute liver injury and hepatotoxicity. Conclusion: To predict the prognosis of AAP poisoning patients, the time interval after taking AAP was important, and initial prothrombin time and ALT level were useful tests. Also a scoring system combining variables may be useful.

Medication Adherence for Hypertensive Patients in Korea (우리나라 고혈압 환자의 투약 순응도 연구)

  • Background: Medication adherence in hypertension is the most important to control blood pressure and prevent major complications. The purpose of this study was to identify factors affecting medication adherence and to examine the relationship between medication adherence and blood pressure control in Korea. Methods: This study used data from the 7th Korea national health and nutrition examination survey (2016-2018) of the Korea Disease Control and Prevention Agency. We selected 4,063 hypertensive patients from the data. And we choose socio-demographic, health behavior, healthcare utilization, and severity characteristics as hypertensive patient characteristics. Results: Of the patients with hypertension, 92.3% had shown adherence to medication as of 2016-2018 and shows variation according to the characteristic of patients. The cases with male, under 50 years old, urban area, single household, unmet medical services, less than 5 years of hypertension duration, no comorbidities (diabetes mellitus, myocardial infarction) showed significantly low medication adherence. After adjusting for confounders, adherent patients tended to have lower current systolic blood pressure (β=-10.846, p<0.001) and diastolic blood pressure (β=-5.018, p<0.001) than nonadherent patients. And, adherent patients increased the control odds of blood pressure compared with nonadherent patients (odds ratio, 3.02; 95% confidence interval, 2.21-4.12). Conclusion: This study confirmed that adherence to antihypertensive drugs was effective in controlling blood pressure. In order to more actively manage hypertensive patients at the national level, it is necessary to make an effort to improve the medication compliance of nonadherent groups, such as early-diagnosis patients, young patients under 50 years of age, and patients living alone.

An Exploratory Analysis of Contributors to Smart TV Usage (스마트TV 이용에 영향을 미치는 요인에 관한 연구)

  • Smart TV is rapidly growing with the provision of OTT services, and has become an alternative to pay-TV services. Yet smart TV in Korea has been diffused slowly, and thus few research has paid attention to smart TV. With the global growth trend of smart TV, research on smartTV, focusing on users' characteristic and their behaviors, is critical to explain smart TV's usage. To employ the Mediapanel dataset, this study explores the impact of smart TV users' demographic factors and their pay-tv usage on smart TV usage. The results suggested that age, the type of household, household income, and market size are associated with smart TV usage. Pay-TV usage, specifically IPTV subscription and VOD expenses, have impacts on smart TV usage. Lastly the interaction effect between IPTV subscription and movie VOD expenses, has negatively associated with smart TV usage. This study may bring to re-attention to smart TV and to give insights into the industry field.

Exploring News Sharers' Characteristics and Factors Affecting News Sharing Behavior (온라인 뉴스 공유자의 특성 및 뉴스 공유에 미치는 요인 탐색)

  • The present study aims to explore news sharers' characteristic. Specifically, it aims to look at news sharers' demographic characteristics, old media news usage and new media news usage. Besides, it also explores factors affecting news sharing behavior. The study used the second data of Korea Press Foundation. Findings from surveys suggest that first, news sharers are younger and have higher education than not news sharers. Second, news sharers use less news through old media while more news through new media. Third, political orientation, portal, SNS and online video platform new usage, messenger news reliability have positive effects on news sharing, while age and portal news reliability have negative effects on it. Based on these findings, implication, limitations, and topics for future research are discussed.

Predictors of the Subjective Well-Being of Middle-Aged Single Mothers (중년기 여성 한부모의 주관적 안녕감 예측 변인)

  • This study aims to investigate predictors of the subjective well-being of middle-aged single mothers who experienced divorce or bereavement. The study involved 244 divorced or widowed women from the 7th Korean Longitudinal Survey of Women and Families aged 40-60 who had children and were employed. Descriptive statistics and multiple regression analyses were conducted. The research results showed that the socio-demographic characteristic (marital status and health), person-related variables (satisfaction with leisure activity and stress), and work-related variables (job satisfaction and work-family enrichment) were significantly related to happiness of middle-aged single mothers. Current economic status and stress were associated with depression in middle-aged single mothers. Increasing middle-aged single mothers' participation in healthy leisure activities, reducing their stress, and improving their satisfaction with work were found to be important to enhancing their subjective well-being. Regarding the foregoing, policy directions were discussed for middle-aged single mothers' leisure activities, stress management, and improved satisfaction with work.

Prediction of intensive care unit admission using machine learning in patients with odontogenic infection

  • Objectives: This study aimed to develop and validate a model to predict the need for intensive care unit (ICU) admission in patients with dental infections using an automated machine learning (ML) program called H2O-AutoML. Materials and Methods: Two models were created using only the information available at the initial examination. Model 1 was parameterized with only clinical symptoms and blood tests, excluding contrast-enhanced multi-detector computed tomography (MDCT) images available at the initial visit, whereas model 2 was created with the addition of the MDCT information to the model 1 parameters. Although model 2 was expected to be superior to model 1, we wanted to independently determine this conclusion. A total of 210 patients who visited the Department of Oral and Maxillofacial Surgery at the Dankook University Dental Hospital from March 2013 to August 2023 was included in this study. The patients' demographic characteristics (sex, age, and place of residence), systemic factors (hypertension, diabetes mellitus [DM], kidney disease, liver disease, heart disease, anticoagulation therapy, and osteoporosis), local factors (smoking status, site of infection, postoperative wound infection, dysphagia, odynophagia, and trismus), and factors known from initial blood tests were obtained from their medical charts and retrospectively reviewed. Results: The generalized linear model algorithm provided the best diagnostic accuracy, with an area under the receiver operating characteristic values of 0.8289 in model 1 and 0.8415 in model 2. In both models, the C-reactive protein level was the most important variable, followed by DM. Conclusion: This study provides unprecedented data on the use of ML for successful prediction of ICU admission based on initial examination results. These findings will considerably contribute to the development of the field of dentistry, especially oral and maxillofacial surgery.

Characteristics of Smartphone User in Application Usage and Implications for Applications Business Model (스마트폰 사용자들의 앱 이용 특성과 앱 비즈니스 모델에의 시사)

  • As the smartphone market grows, the needs for its new business model are also increased. However, most previous researches on smartphone applications focused on Technology Acceptance Model(TAM) and Rogers' Diffusion of Innovation Theory so that there was lack of researches on characteristics for actual smartphone users. In this research, we divided the smartphone applications into five category functions (Call & Text/Music & Video/Information Search/Game/Social Network Service (SNS)). We analyzed characteristic differences of users who used the each application category and found that the differences were statistically significant in both demographic and smartphone usage characteristics (frequency of downloading applications, and download experience of paid applications). Additionally, the smartphone usage characteristic is closely related to the usage duration. The representative result is that the characteristics of people used Music & Video function actively were women in their 20s who downloaded applications more than three times per week, and had a download experience of paid applications. It is positive result for players in the application markets, because it means the users are willing to pay for downloading the paid applications. However, large companies already occupied most of the market share in music applications so that small and medium-sized players should develop an innovative and distinguishable business model in order to success. We believe this research result would provide significant implications for the players in planning the successful business model and developing an user-specific application product.

Effects of Periodontal Disease on Cardio-Cerebrovascular Disease: A Focus on Personal Income and Social Deprivation (치주질환이 심뇌혈관질환 발생에 미치는 영향: 지역결핍과 개인소득을 중심으로)

  • The study aimed to investigate the effects of differences between personal income and social deprivation on the association between cardio-cerebrovascular disease (hypertension, stroke) and periodontal disease. This study used 12 years of cohort data from the Korea National Health Insurance Service covering the years 2002 to 2013. Among the patients aged more than 40 years who had received treatment for periodontal disease 224,067 and 284,730 who had not received treatment for hypertension and stroke, respectively, were included in the analysis. The Kaplan-Meier analysis revealed differences in the rate of treatment for cardio-cerebrovascular disease (hypertension, stroke) according to regional differences, the rate of treatment increased as the composite deprivation index value increased. The difference in treatment rates for cardio-cerebrovascular disease (hypertension, stroke) according to income was found to be higher in the treatment group with low income. This study empirically proved that the association between systemic disease and periodontal disease varies depending on personal income and the regional socioeconomic deprivation level. This shows that the clinical influence of periodontal illness on systemic disease differs according to the personal socio-demographic characteristic and residential area and that an individual's characteristic (income and the regional) needs to be considered along with the patient's clinical intervention in the disease treatment process.

Risk Factors of Arthritis in Middle-aged Women in Korea (중년여성의 관절염 발생 영향요인 분석)

  • Purpose: To investigate the risk factors of arthritis, this study was carried out with 1,829 Korean women aged between 40-64 who responded to a health behavior survey. Methods: Data were extracted from the results of the 2001 National Survey on Health and Nutrition conducted by the Ministry of Health and Welfare. Results: 1 Financial difficulty and continuous anxiety about health were directly associated with arthritis. The odd ratios of these two factors of arthritis were 1.43 and 1.82 respectively. 2. With regard to the presence or absence of arthritis according to demographic characteristic, the incidence of arthritis was higher in the age between 50-64, in the group whose education level was middle school or lower, and in those who experienced the death of spouse, divorce or separation. Nevertheless, it was not significantly different according to financial status and employment status. 3. Concerning the presence or absence of arthritis according to psychological characteristic, the incidence of arthritis was found to be higher in those who were not healthy by subjective health assessment, in those who were always anxious about their health, in those who were always depressed and sad, and in those who felt heavy stress. 4. With regard to the presence or absence of arthritis according to the health behavior, the incidence of arthritis was higher in postmenopausal cases and cases who were having hormone therapy, and the incidence of arthritis was higher in those who did not drink alcohol, those with BMI higher than 25, and those who slept shorter than 7 hours a day. Conclusion: Based on the above results, it is required to provide a means applicable to communities by developing preventive education that can prevent the incidence of arthritis in middle-aged women as well as intervention programs.

A Study to Compare between Groups Glassified by Demographic Characteristic into Effects of Word of Mouth and Methods of Sales Promotion in Intention of Watching Movies (개봉 전 후 영화의 구전효과와 판촉방식에 따른 인구통계학적 집단 간의 차이에 관한 연구)

  • It's important to analyse effects of word of mouth for making its impact higher in performance of motions pictures. And it's required to combine variety sales activities like free gift, promotion goods and price discount with word of mouth for the box office of film. The purpose of this study is to compare between groups classified by demographic characteristic into effects from word of mouth and methods of sales promotion in intention of watching film. On the other hand existing studies on sales activity and word of mouth were one-sided in theoretical background, a meaning of this study is theorizing a social phenomenon about sales promotion of movie giving actual examples that currently are effected by production company, Movie theaters, distribution company and affiliated company. For this purpose, it conducted a survey targeting 500 students in B university in Seoul city and 379 answers got received, and it proceeded this study with 369 answers except 10 inaccurate ones. Creating questionnaires with Likert 5 point scale, it decided that case of substantial inclination was 5 points and inverse one is 1 point. Doing analysis T and ANOVA according to male and female, kinds of major study and number of average monthly watching movie, it analysed the test results after comparison analysis between classified group. The results are summarized as follows: First, offering premiums is more effective by masculine than feminine, but situation of free gift is an opposite result. Second, there are no differences of effects word of mouth and methods of sales promotion by majority departments. Third, there are much differences between groups classified by average number of watching film in a month into effects from word of mouth and methods of sales promotion. Group of watching film more 3 times in a month is more effective than the other groups in intension of watching film by word of mouth. Fourth, word of mouth is great factor to increase intention of watching film and second one is discount on the price.
