Correlation Analysis between Cognitive function and Praxis tasks in the Elderly

  • The purpose of this study was to identify differences in cognitive function according to the presence or absence of apraxia and the tasks most relevant to the cognitive function among the various types of tasks in the apraxia test. The subjects were 42 community residents who participated in a cognitive rehabilitation program related to dementia in a Chungbuk area. MMSE-K and BCoS(Birmingham Cognitive Screen) apraxia test were administered to all subjects. The apraxia test includes three types of tasks, gesture production tasks that make meaningful movements according to verbal instructions, gesture recognition tasks that display behavior after make sense of meanings, and meaningless imitation task. Apraxia group(n=30, MMSE-K mean score: 25) showed lower cognitive function than group without apraxia(n=12, MMSE-K mean score: 28). All tasks in the apraxia test showed a significant correlation with cognitive function, but the meaningless imitation task had a negligible correlation. The apraxia test is a good way to assess cognitive function, and it may be more effective to use meaningful behavior to replace cognitive testing.

  • The purpose of this study was to identify the variables which affect BMI and the relationships among BMI and socio-demographic variables and health-related behaviors among women in Pajoo city. A self-report questionnaire was used covering questions regarding health, eating habits and health related behaviors such as smoking and drinking behavior, exercise, weight management, etc. Test of significance for differences between the groups were performed with $X^2$-test and hierarchial regression. The level of BMI has a positive relationship with age, married status, and weight management among Pajoo citizens in Korea. This study may be utilized in preparation and planning for dementia prevention programs and policies of older people. Also it can be useful for developing and enhancing various health programs and policies and policies when applied appropriately.

  • The purpose of this study is to provide a basic data for architectural planning on the wards and special quality of physical remedy spaces of geriatrics hospital. This research, to analyse dependency degree of inpatients in geriatric hospital, shows relativeness between remedy space and wards, and to show behavior of each patients reaction to there space. Result of interviewed inpatients who were sorted by age, sex, dependency degree, and days in hospital, suitable space programming is to need for physical and mental state. On the bases of survey and analysis, the basic data for planning and design the hospital for the elderly with dementia has been proposed.

Development and Analysis of the Effects of Caregiver Training Program on Aggressive Behavior m Elders with Cognitive Impairment

  • Purpose. The purposes of this study were to 1) describe the type and frequency of aggressive behavior of cognitively impaired nursing home resident, 2) develop a caregiver training program on prevention and management of aggressive behavior, 3) examine the effects of caregiver training program on the incidence of aggressive behavior of cognitively impaired nursing home resident, and 4) examine the effects of caregiver training program on nursing staff's aggressive behavior management skills. Methods. One-group, time series, quasi-experimental design with a pre-test and two post- tests was used. Data were collected from cognitively impaired home residents (N = 32) and nursing staff (N = 36) in a proprietary nursing home using Ryden Aggression Scale I, II, and Aggressive Behavior Management Scale. Data were entered and analyzed by descriptive statistics and repeated measures ANOVA. Results. Incidence of aggressive behavior was high with a mean score of 3.09 (SD = 3.11) at baseline. Caregiver training program was developed based on Progressively Lowered Stress Threshold (PLST) model and gerontological and psychiatric literature. The mean scores of aggressive behavior at baseline, Post I, and II did not differ significantly although the difference approached to the significant level (F = 2.925, p = .066). Nursing staff's aggressive behavior management skills increased at Post I, and at Post II when compared to baseline, and the difference was significant (F=12.736, p=<.00l). Conclusion. Caregiver training program showed potential impact on reduction of aggressive behavior in elders with cognitive impairment and was effective in increasing nursing staff's aggressive behavior management skills.

A Study on the Usage Behavior of Elderly Welfare Facilities for the Elderly with Moderate Dementia: Focusing on Satisfaction with Memory School Users (경증치매노인 노인복지시설의 이용행태에 관한 융합연구: 기억학교 이용자만족도를 중심으로)

  • The purpose of this study is to provide basic data to confirm the necessity and effectiveness of the memory school by investigating the perception of users and caregivers about the services and programs provided by the memory school, a social welfare institution for the elderly with minor dementia. For this purpose, 363 students and students in Daegu City were surveyed about the satisfaction and attitude of using memory school. As a result of the analysis, it was found that the use of the memory school is very helpful to the users, and at the same time, it is very meaningful for the decrease of the caregiver burden of the caregiver. Thus, it can be seen that the memory school has the effect of appropriately matching the installation purpose of the facility. In particular, it was found that the degree of satisfaction with the program level, type of service, and future sustainability of the memory school exceeded 90%. Therefore, this study aims to provide policy implications for the social welfare response for the elderly with mild dementia and for the basic data on the memory school for this purpose.

Factors Influencing Job Satisfaction of Nurses Working in Long-term Care Hospitals: Focused on Burnout and Burden for Caring Problematic Behaviors in Dementia (요양병원 간호사의 직무만족도 영향요인: 치매 문제행동 간호에 대한 부담, 소진을 중심으로)

  • This study was to investigate the factors influencing job satisfaction of nurses in long-term care hospitals focusing on burnout and burden. The participants were 206 nurses recruited from long-term care hospitals in B metropolitan city and C city. Data were collected from January to March, 2014 and analyzed using SPSS WIN 18.0 program. The frequency of experience was $1.65{\pm}0.42$, which is 4~5 times per month and once or twice per week in the problematic behavior in dementia. The frequency of burden was $1.46{\pm}0.48$, which means one nurse can solve the problematic behaviors in dementia. Burnout was $2.53{\pm}0.61$, and job satisfaction was $2.87{\pm}0.45$. Negative correlations were found between burden and job satisfaction (r=-.256, p=.003), and between burnout and job satisfaction (r=-.718, p<.001). A factor influencing job satisfaction was burnout (${\beta}$=-.735), which explained 57%. It is suggested that Intervention programs should be developed to reduce burnout and burden, and to increase job satisfaction for nurses.

A study of the multicomponent therapeutic recreation function intervention strategy by analysis on the operating condition of the cognitive rehabilitation program in dementia care center

  • This study was conducted with 50 elderly people each (5) participating in the cognitive rehabilitation treatment program at the Dementia Care Center in D City to derive the development direction and contents of a multidimensional therapeutic recreation program and a revitalization plan through analysis of the current status and actual conditions of the cognitive rehabilitation program at the Dementia Care Center. aperture) was selected, and 9 people were selected as the subject of expert group opinion collection. The collected data was SPSS ver. Using the 18.0 statistical program, descriptive statistics and the importance and priority of each component were analyzed by hierarchical structure analysis. First, unlike the needs of users, the cognitive rehabilitation support programs currently being provided are not sufficient and require considerable experience. It was found to be low, and the areas for improvement were the expansion of care and protection facilities and the development of various programs to meet the needs of users. Second, the importance and priority of each component of therapeutic recreation were categorized into 6 major categories: exercise therapy , middle category (16 items) behavior-centered approach to exercise therapy, small category (47 items) strength and brain gymnastics, and silver health gymnastics were the highest. This result shows that a multidimensional program plan that considers the priorities of each area must be made when developing a therapeutic recreation program.

A Study on Aggressive Behavior Among Nursing Home Residents with Cognitive Impairment

  • Purpose. With a sample of cognitively impaired nursing home residents and nursing staff, the following were examined 1) the proportion and nature of aggressive behavior, 2) the frequency and types of aggressive behavior, 3) the difference between the residents who demonstrate aggressive behavior and those who do not demonstrate aggressive behavior (age, mental status, functional status, and pain, length of nursing home stay), and 4) nursing staff responses to aggressive behavior by residents. Methods. A cross-sectional descriptive study design was used. Data were collected from cognitively impaired nursing home residents (N=205) and nursing staff (N=60) at two nursing homes using Ryden Aggression Scale I and II, Mini-Mental State Exam, Modified Barthel Index, Verbal Descriptor Scale, and aggressive behavior management questionnaire. Data were analyzed using descriptive statistics including t-test. Results. About $62.9\%$ residents were found to be aggressive and $38.5\%$ were both physically and verbally aggressive. Pushing, making threatening gestures, hitting, slapping, cursing/obscene/vulgar languages, making verbal threats were occurred frequently. Aggressive residents were significantly older, had more cognitive impairment, had more pain, and stayed longer in the nursing home when compared with non-aggressive residents. Considerable proportion of nursing staff responded to aggressive behaviors inadequately. Conclusion. Aggressive behavior among cognitively impaired nursing home residents is prevalent thus needs to be prevented and reduced. Along with environmental modification, educational programs for nursing staff and family caregivers need to be developed and implemented so that they can have extensive knowledge and skills to manage aggressive behaviors.

An analysis of risk factors for falls in the elderly by gender (노인의 성별 낙상관련 요인)

  • Objectives: The objective of this study is to identify the gender differences of risk factors for falls among the elderly in community dwellings. Methods: We analyzed the data on 3,278(male 1,255, female 2,023) persons, including 497 persons who have experienced falls, drawn from the 2004 National Elderly Survey. We conducted a cross-tabulation analysis, $X^2$-test and hierarchical regression analysis of the impact of the socio-economical characteristics, environmental characteristics, the number of chronic diseases, usage of supplementary devices, activities of daily living, dementia, and the severity of problem behavior. Results: For the entire sample of the elderly, gender, age, the size of the cities of residence, the number of chronic diseases, and the severity of problem behavior were identified as risk factors for falls. The number of chronic diseases and the severity of problem behavior were found to be significant for the male subsample, while age, the size of cities of residence, dwelling types, and the number of chronic diseases were found to be significant for the female subsample. Conclusion: The number of chronic diseases was identified as a common risk factor for falls in the male and female elderly. Chronic diseases were also found to aggravate the risk for falls when they concur with other diseases.

The Relationship between Dementia and Oral Health in Some Elderly in Daejeon (대전 일부 노인의 치매와 구강건강의 관계)

  • The elderly population with dementia is rapidly growing in South Korea. The aim of this study was to investigate the relationship between dementia and oral health in 197 subjects aged ${\geq}65$ years. The questionnaire included questions on subjective health status, subjective health concern, subjective oral health status and behavior, mastication ability, Oral Health Impact Profile-14 (OHIP-14), and Korean Dementia Screening Questionnaires (KDSQ). Oral examination was conducted by a single dentist to evaluate upper or lower denture use, and determine the numbers of remaining and functioning teeth, including implant and fixed prosthesis. The subjects who required a dementia test (KDSQ-C [cognition] of ${\geq}6$) had significant differences in systemic disease prevalence rate, subjective health status, subjective health concern, KDSQ-V (vascular disease) score, KDSQ-D (depression) score, subjective oral treatment need, key food mastication ability and OHIP-14 score compared to the healthy subjects. The proportion of denture wearers, total remaining teeth, total functioning teeth, toothbrushing frequency, oral pain severity, symptoms of periodontal disease, subjective oral health status, and subjective oral health concern showed no significant differences between the two groups. KDSQ-C and OHIP-14 scores showed a strong positive relationship, while KDSQ-C score and total remaining teeth or key food mastication ability showed a weak negative relationship. In the multiple regression analysis, the KDSQ-D, KDSQ-V, and OHIP-14 scores influenced the KDSQ-C scores. We suggest a relationship between oral health and cognitive impairment.
