Severe Diarrhea-induced Acute Kidney Injury and Its Consequence in an Elderly

  • Methods: This study presents a comprehensive case study of an elderly male diagnosed with acute kidney injury (AKI) resulting from severe dehydration, supported by an extended follow-up with laboratory findings. Results: An 83-year-old male patient experienced severe diarrhea overnight, leading to hospitalization due to symptoms of dehydration and hypotension. His laboratory results displayed a typical AKI pattern, including a significant increase in creatinine levels (5.19 mg/dL) and the presence of hyperkalemia and hyponatremia. Following general treatments, including the administration of an herbal drug (Bulhwangeumjeonggi-san), the estimated glomerular filtration rate (eGFR) improved from 10 ml/min (Stage 5) to 34 ml/min (Stage 3) within five days when he was discharged. Although subsequent eGFR tests, conducted one and two months later as an outpatient, revealed an improvement of 42 ml/min, the patient still experienced mild chronic dysfunction as a consequence. Conclusion: This study presents a noteworthy case of acute kidney injury attributed to severe dehydration, emphasizing the importance of medical awareness regarding diarrhea-induced kidney function impairment, especially in the elderly population.

Severe hypernatremic dehydration in a breast-fed neonate (모유 수유 환아에서 발생한 심한 고나트륨혈성 탈수)

  • Although it is a rare condition, breast-feeding may result in hypernatremic dehydration. However, incidences might be increasing with more mothers breast-feeding. Although the early detection and management of hypernatremic dehydration from breast-feeding is important, its prevention is even more important on account of its serious complications. In order to prevent hypernatremic dehydration secondary to breast-feeding, it is essential to educate mothers in successful breast-feeding methods. An early follow-up after discharge is recommended. We report a case of hypernatremic dehydration secondary to breast-feeding in a full-term newborn that was corrected without any complications.

  • It was studied to prepare anhydrous magnesium chloride which could used as the raw material of a fused salt electrolysis of magnesium by dehydration of magnesium chloride hydrate. The dehydration was carried out in a tube furnace at $350{\sim}580^{\circ}C$. It was confirmed that magnesium chloride hydrate was oxdized to magnesia through the dehydration in ambient atmosphere, but anhydrous magnesium chloride could be obtained in hydrogen chloride gas atmosphere. And the crystallity of the product increased with increasing temperature and time of dehydration. All of the un-reacted hydrogen chloride gases which were generated during the dehydration in hydrogen chloride gas atmosphere could be recovered as hydrochloric solution, and it could be reused for chlorination of magnesia to prepare magnesium chloride hydrate.

  • The raw surf clam meat was pretreated with BHA, EDTA or $NaHSO_3$. The pretreated meat was chopped with chopper, and spread the chopped meat on nylon net, and then dehydrated with cabinet type hot air dryer. In the surf clam meat flake process, the pretreating and the copping steps prior to final dehydration improved the pigment retention and texture of the products. The chopping steps prior to dehydration reduced the dehydration time of surf clam meat. The BHA treatment prior to dehydration of mashed surf clam meat had an outstanding effect on the pigment retention during a process of dehydration and storage. The surf dam meat flakes reabsorbed water more rapidly about two times than the dehydrated natural surf clam meat products. The surf clam meat flakes stored for three months in the dark place showed less pigment loss than the exposed ones, and the former reabsorbed water more rapidly than the latter.

Photosynthetic Responses to Dehydration in Green Pepper(Capsicum annuum L.)Leaves

  • Photosynthetic responses to dehydration were examined by the simulataneous measurement of O2 evolution and chlorophyll (Chl) fluorescence in green pepper leaves. Dehydration was induced by immersing the plant roots directly in the Hoagland solution containing varying concentration (2-30%) of polyethylene glycol(PEG-6000) . Water potential of the leaf was decreased time-and concentation -dependently by PEG-treatment. The decrease in water potential of leaf was correlated with the decrease in both the maximal photosynthesis (Pmax) and quantum yield of O2 evolution, but Pmax dropped more rapidly than quantum yield at all water deficit conditions tested. However, Chl fluorescence parameters were not affected much. Dehydration did not change the initial fluorescence (Fo) and maximum photochemical efficiency(Fv/Fm) of photosystem(PS) II. Both the photochemical quenching (qP) and non-photochemical quenching(NPQ) were not changed by dehydration under low PFR(50 $\mu$mols m-2s-1 ). In contrast, under high PFR(270$\mu$mols m-2s-1)qP was slightly decreased while NPQ was greatly increased. The fast induction kinetics of Chl fluroecence showed no change in Chl fluorescence pattern by dehydration at high PFR (640 $\mu$mols m-2s-1 ), but exhibited a significant drop in peak level(Fp)at low PRFR (70$\mu$mols m-2s-1 ). PS I oxidation and reduction kinetics revealed normal reduction but delayed oxidation to P-700+, suggesting no lesionin electron flow from PSII to PSI , but impaired electron transport to NADP+,These results suggest that water stress caused by PEG-treatment results in the reduction of photosynthesis, promarily due to the reducted electron trasport from PSI to NADP+ or hampered subsequent steps involving Calvin Cycle.

Exploratory Study of the Prevalence of Major Chronic Disease According to Daily Water Intake and Physiological Parameters related to Dehydration in Korean Elderly: Based on the Sixth Korea National Health and Nutrition Examination Survey (2015) (한국 노인의 일일 수분섭취 상태에 따른 주요 만성질환 유병률 및 탈수 예측 생리적 지표 탐색 연구: 2015년 국민건강영양조사 결과를 바탕으로)

  • Purpose: The aim of this study was to explore the prevalence of major chronic diseases related to daily oral water intake and to identify the physiological parameters related to dehydration in Korean elderly. Methods: The data were collected from the sixth Korea Health and Nutrition Examination Survey (KHANES), which was a nationwide and cross-sectional survey in 2015. We analyzed 1,392 participants using t-test and logistic regression. All participants were divided into the adequate water intake (AWI) group and the non-adequate water intake (NAWI) group based on the dietary reference intakes for Koreans. Results: There was a significant difference in the water intake between the AWI (6.8 cups in a day) and NAWI (2.8 cups) groups (p< .001). There was no statistically significant association between the level of water intake and any of the major chronic diseases. Blood urea nitrogen (BUN) and BUN/Creatinine (Cr) ratio were significantly higher in the NAWI group. Especially, BUN/Cr ratio shows that the NAWI group reached dehydration status. Older age (adjusted odd ratio, OR= 1.07, 95% confidence interval, CI [1.04-1.10]), female gender (adjusted OR= 1.56, 95% CI [1.05-2.33]), lower body mass index (BMI) (adjusted OR = 1.00, 95% CI [0.92-1.00]), higher BUN (adjusted OR = 1.04, 95% CI [1.01-1.08]), and higher urine specific gravity (USG) (adjusted OR= 1.56, 95% CI [1.19-2.05]) were factors associated with the NAWI group. Conclusion: The findings suggest that the level of water intake needs to be considered in relation to age, gender, BMI, BUN, and USG. These are sensitive physiological parameters used for predicting dehydration of the elderly according to their daily oral water intake. It would be helpful to develop strategies to prevent dehydration in elderly individuals and enhance their water intake.

Correlation of Dehydration with Body Mass Index and Blood Lipid Levels (탈수와 체질량지수 및 혈중지질 농도와의 관련성)

  • Maintaining adequate fluid balance is essential for all biological functions in the body. The purpose of this study was to evaluate vulnerability to dehydration by analyzing age, gender, body mass index (BMI), and blood lipid parameters in health checkup examinees who visited Sahmyook Seoul Hospital for comprehensive health checkups. In a binary logistic regression analysis stratified by age and body mass index the odd ratio for dehydration was as high as 3.317 (95% CI: 1.666~6.605) in the 50s age group, 4.224 (95% CI: 2.038~ 8.755) in the 60s age group, and 4.610 (95% CI: 1.943~10.940) in the above 70s age group compared to 20s reference age group. Aged females showed greater vulnerability to dehydration with significance levels of P<0.01 and P<0.001. Compared to a normal weight (BMI: 18.5~22.9) the odd ratio was higher in males with an under weight (BMI: less than 18.5) (5.130 [95% CI: 1.631~16.132]) and in females with an over weight (BMI: 23.0~24.9) (1.500 [95% CI: 1.065~2.114]). In conclusion, our results showed that vulnerability to dehydration increased with age and was higher in under weight males and over weight females than that in normal weight.

Relationship between the values of blood parameters and physical status in Korean native calves with diarrhea

  • Dehydration, electrolyte disturbance, and acid-base imbalance are the most significant consequences of diarrhea in calves. We aimed to determine blood gas, hematological, electrolyte, and biochemical values and investigate the relationship between the physical status and blood parameters in Korean native calves (KNCs) with diarrhea. One hundred eighty KNCs with diarrhea (age < 75 days) were investigated. Blood samples were collected from the external jugular vein and analyzed using a portable clinical blood gas analyzer. The measured parameters were statistically compared according to the status of physical activity, dehydration, or prognosis. The mean values of parameters in the Calves with diarrhea showed metabolic acidosis, hyponatremia, and azotemia. The mean values of potassium, chloride, hematocrit, and hemoglobin were in the upper limit of their reference ranges. More than 75% of the calves had metabolic acidosis caused by bicarbonate loss, and 63.6% had high blood urea nitrogen (BUN) values. Moreover, BUN showed the highest correlation with the physical activity status and dehydration. pH, base excess of the extracellular fluid (BE), anion gap, potassium, hematocrit, bicarbonate, and hemoglobin were closely correlated with physical deterioration and dehydration (p < 0.001). BUN, pH, BE, and anion gap were closely correlated with physical deterioration and dehydration. These correlations between clinical symptoms and blood gas parameters can be clinically relevant in predicting the status of parameters according to clinical symptoms.

Effects of Dehydration Methods on Physical Properties of Reconstituted Instant Rice (건조방법이 복원된 즉석밥의 물리적 성질에 미치는 영향)

  • The physical properties of reconstituted instant rice produced by three different dehydration methods were evaluated. The rehydration ratios of reconstituted instant rice produced by air dehydration at 9$0^{\circ}C$ (process A) had higher values than those produced by freeze dehydration (process C) during entire range of rehydration. After 4min of rehydration time at 95$^{\circ}C$ , the moisture contents of reconstituted instant rice produced by process A and C were higher values than those of control(151.47%, dry basis) which was cooked by electric cooker. Regardless of dehydration method, the sizes of reconstituted instant rice (6min, 95$^{\circ}C$) had larger values than those of control. The values of hardness (H), stickiness (S) and S/H ratio of reconstituted instant rice (6min, 95$^{\circ}C$) produced by high temperature (7min, 15$0^{\circ}C$) air dehydration (process B) as the first stage prior to air dehydration at 9$0^{\circ}C$ were almost the same as those of control. The H of reconstituted instant rice decreased, while S and S/H ratio increased with increasing rehydration time. The higher values of whiteness(L) and lower values of yellowness (b) represented in reconstituted instant rice (6min, 95$^{\circ}C$) than those of control.

Physical and Sensory Properties of Peeled Ginkgo Nuts Prepared under the Different Dehydration Conditions (내피제거 은행의 물리적 및 관능적 품질 특성)

  • The effects of different dehydration conditions for the removal of ginkgo nut inner coat on peeled ginkgo nuts were examined using physical and sensory methods. Dehydration of ginkgo nuts at $90^{\circ}C$ for 120 min resulted in greater hardness, lightness (L), redness (a), yellowness (b), and firmness. Higher roasted flavor and larger holes were observed with dehydration at $150^{\circ}C$ for 30 min and greater cooked ginkgo flavor and green color with dehydration at $120^{\circ}C$ for 50 min. Based on these findings, dehydration at $120^{\circ}C$ for 50 min was determined as the adequate hot air dehydration condition for the removal of ginkgo nut inner coat. Ginkgo nuts stored at $25^{\circ}C$ maintained overall desirability up to 4 weeks. In ginkgo nuts stored at $4^{\circ}C$, all sensory desirabilities evaluated were reasonably high until 60 days of storage; desirability of appearance decreased moderately at 80 days of storage, and appearance became very undesirable at 180 days. Ginkgo nuts stored at $-18^{\circ}C$ showed little changes until 10 months of storage, but slight decreases in overall desirability, flavor, and texture were noted at 12 months.
