Composite skull defects in patients with severe head injuries are very challenging to manage. The dilemma when deciding whether to perform a definitive reconstruction is how long to wait for physiological recovery before an intervention complicates the situation. The inability of such patients to tolerate prolonged anesthetic exposure is a driving factor for performing the minimal intervention necessary to facilitate recovery. Herein, we present a case involving the successful immediate reconstructive treatment of a severely head-injured adolescent with a composite scalp defect secondary to trauma. A 14-year-old boy sustained a severe head injury from a motor vehicle accident with a composite scalp defect in the right fronto-parietal region. The frontal lobe was exposed, and the right eye was crushed and devitalized. The patient was deeply unconscious for 3 days, without any significant improvements before reconstructive surgery was proposed due to fear of possible meningitis resulting from the exposure of brain structures. We successfully managed the patient with a fronto-parieto-occipital flap, after which the patient promptly recovered consciousness.
This report is a analysis of 72 cases of mediastinal tumors which are originated in mediastinum or probably metastasized from other organs, and classified on the basis of histopathological types. And all are experienced in the Depart. Thoracic and cardiovascular Surgery, National Medical Center from 1959 to April 1976. Among these cases, 58 cases were confirmed by histopathologically, and remained 14 cases were considered as mediastinal tumor by clinically and roentgenologically. In this series, dermoid cyst and teratoma was most frequent tumor among histopathologically confirmed cases(27.6%), and thymoma 20.7%, neurogenic tumors 17.2%, carcinoma 19%, lymphoid tumor 8.6% and others was 5.9%. The cases of not verified histopathologically were 14 cases. The main clinical symptoms were dyspnea on exertion or orthopnea (62.55%), productive or irritative cough (59.77%), decreased breathing sound (43. 09%). S.V.C syndromes was seen in 25.02%, and there were no definitive symptom in 4.2% of all cases. The main treatment method was surgical removal and irradiation therapy.
Is a multiple number of sputum specimens necessary for the diagnosis of pulmonary tuberculosis? To answer this question, 6844 respiratory specimens obtained from previously untreated patients suspected of having pulmonary tuberculosis between 1998 and 2001 were evaluated retrospectively. All of the specimens were evaluated by acid fast bacilli smear and BACTEC 460 TB culture system. A total of 785 (11%) specimens from 353 patients were positive for Mycobacterium tuberculosis complex. For 76% (270/353) of these patients the organism was detected from sputum specimens collected sequentially for daily basis. Mycobacterium tuberculosis was isolated in the first, second and third samples of the majority (98%,195/199) of patients who had three or more sputum samples sent to the laboratory. Our results indicate that, we could carry out Mycobacterium tuberculosis isolation in the first, second and third sputum samples of the overwhelming majority of the patients and the diagnostic value of four or more sputum specimens submitted to the laboratory was very low (2%). We recommend that, for definitive and cost-effective diagnosis of pulmonary tuberculosis at least three sequential sputum specimens be collected for all patients suspected pulmonary tuberculosis.

SAR interferometry (InSAR) using the space-borne Synthetic Aperture Radar (SAR) have recently become one of the most effective tools monitoring surface changes caused by landslides, earthquakes, subsidences or volcanic eruption. This study focuses on examining the feasibility of InSAR using the RADARSAT data. Although the RABARSAT SAR with its high resolution and variable incidence angle has several advantages for repeat-pass InSAR, it has two key limitations: first, the orbit is not precisely known; and second, RADARSAT's 24-day repeat pass interval is not very favourable for retaining useful coherence. In this study, two pairs of RADARSAT data in the Nahanni area, NWT, Canada have been tested. We will discuss about the special consideration required on the interferometric processing steps specifically for RADARSAT data including image co-registration, spectral filtering in both azimuth and range, estimation of the interferometric baseline, and correction of the interferogram with respect to the "flat earth" phase contribution. Preliminary results can be summarized as: i) the properly designed azimuth filter based upon the antenna characteristic improves coherence considerably if difference in Doppler centroid of the two images is relatively large; ii) the co-registration process combined by fringe spectrum and amplitude cross-correlation techniques results in optimal matching; iii) the baseline is not always possible to be estimated from the definitive orbit information.

Solitary plasmacytoma is a rare malignant neoplasm that originate in immunoglobulin-producing plasma cell. Solitary bone plasmacytoma can be found at any site throughout the skeleton and in most cases eventually progresses to multiple myeloma, of which it is thought to be an unusual presentation. On the other hand, extramedullary plasmacytoma has a tendency to occur in the head and neck region, mainly in association with the upper air passages. Incisional biopsy is the primary approach to make a definitive diagnosis, and immunohistochemical staining can be very helpful in understanding the nature of these tumors. We report a 66 years old patient with solitary bone plasmacytoma and a 36 years old patient with solitary extramedullary plasmacytoma with literatures review.
Sensorineural hearing loss(SNHL) is defined as a sudden hearing impairment which was develope over a period of hours to days. The definitive cause of SNHL is unknown in most cases. Disturbance of the blood flow of the inner ear is a main causal hypotheses. Most symptomatic treatment is focusing to improving the blood flow of the inner ear. At our hospital, most patients are recommended to bed rest, vasodilators(nicotinic acid, antihistamines) and stellate ganglion block(SGB) for two weeks. We experienced two cases of recurrence immediately after recovery from SNHL. They discontinued SGB after initial recovery. due to associated pain with operation and or psychologic stress. One patient resumed SGB and medication treatment, but the second who gave up treatment was not recovered. Therefore, we recommend to continuance of treatment of stellate ganglion blocks to improve recovery rate of SNHL and avoid further trauma and psychologic stress during the recovery period of SNHL.
A traumatically intruded tooth is one that is forcefully and abruptly dispaced from its position into the surrounding alveolar bone. Although intrusion of permanent teeth is infrequent, the sequelae compromise the longevity of the tooth and often include pulp necrosis, internal and external root resorption, rupture of periodontal ligament and loss of marginal bone. The purpose of this study was to examine three common management techniques for traumatic intrusion, observation for re-eruption, surgical repositioning & fixation and orthodontic extrusion. In the recent, the accepted treatment was to allow the permanent teeth to reerupt spontaneously for 6-8 weeks. If this did not occur, orthodontic traction was applied. The pulpal status of the teeth was monitored and either calcium hydroxide therapy or conventional endodontics was instituted following pulpal necrosis depending on the maturity of the root end. Pulpectomy and a calcium hydroxide filling were also the treatment of choice if there was evidence of internal or external root resorption. This will reduce the chance of root resorption and provide a period of monitoring prior to a definitive root canal filling.
This study analyze the architectural space in the aspect of the 'Raumplan' and 'Plan Libre'. These concepts based on the 'Raumplan versus Plan Libre' exhibition in Delft University, Netherlands. It is generally agreed that the concept of 'Plan Libre' in connection with Le Corbusie architecture and 'Raumplan' related to Adolf Loos's works. that exhibition contents was focused an extent of modem architecture. But, These concepts continually influence contemporary architecture and offer diverse vision in architecture. Therefore, this study focused on the analysis that look for common element in the space through the case study and space element connection in change of the values, technical growth. This Paper reveals that Adolf Loos's Raumplan is similar to Louis I. Kahn's 'Room' concept and Ando Tadao's centrifugal space composition method In the dwelling architecture. And the concept 'Plan Libre': non-definitive formal system and the elements : column, free form screening wall is revived as formal application of 3 dimension composition in architecture. and transfer lots, void, and linear space in Rem Koolhaas's urban design projects. These aspects is so called topology. This topological concept is an attempt in view of the space connection state rather than formative viewpoint.
Helicobacter pylori is the definitive carcinogen for stomach cancer and is known to induce proinflammatory cytokines, such as tumor necrosis factor-$\alpha$ (TNF-$\alpha$) and interleukin-1(IL-1) in the stomach. Based on our findings that TNF-$\alpha$ is an endogenous tumor promoter, we identified the TNF-$\alpha$ inducing protein (Tip$\alpha$) gene family, and confirmed Tip$\alpha$ and HP-MP1 as new carcinogenic proteins of H. pylori. Tip$\alpha$ protein is unique to H. pylori, and this paper shows the strong tumor promoting activity of Tip$\alpha$ gene family, in cooperation with Ras protein and its mechanisms of action in relation to NF-${\kappa}B$ activation, and discusses the carcinogenic role of Tip$\alpha$ in stomach cancer. Our recent finding showing that penicillin-binding proteins of other bacteria are weak homologues of Tip$\alpha$ is also discussed.
