Objectives The purpose of this study was to investigate the pattern characteristics of symptoms for ADHD, the relationship between pattern identifications in TKM and TCM and three presentations of ADHD defined in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5). Methods The pattern characteristics of symptoms for ADHD from the 13 Korean and Chinese literatures were analyzed. ADHD symptoms of each pattern were classified into 3 categories; symptoms of inattention, hyperactivity-impulsivity and others. The characteristics of patterns for ADHD in Traditional Medicine were compared with three presentations of ADHD defined in DSM-5. Results 1. The pattern of dual deficiency of the heart and spleen (心脾兩虛) showed predominant symptoms of inattention and rarely showed those of severe hyperactivity and impulsivity. This characteristic of ADHD can be related to the predominantly inattentive presentation in DSM-5. 2. The pattern of phlegm-fire harassing the heart (痰火擾心), and the pattern of deficiency of liver-yin and kidney-yin (肝腎陰虛), frequently showed symptoms of hyperactivity-impulsivity compared with those of inattention. This characteristic of ADHD can be related to the predominantly hyperactive-impulsive presentation in DSM-5. 3. The pattern of spleen weakness and liver energy preponderance (脾虛肝旺) frequently showed both symptoms of hyperactivity, and this characteristic of ADHD can be related to the combined presentation in DSM-5. Conclusions This study showed the relationship between three presentations of ADHD defined in DSM-5 and the pattern identifications of ADHD in TKM and TCM.
Objectives: Medically unexplained fatigue includes chronic fatigue syndrome (CFS) and idiopathic chronic fatigue (ICF). These disorders quite impair quality of life while no effective therapies exist. Therefore, patients with CFS or ICF frequently choose treatments based on traditional Korean medicine. The study aimed to analyze characteristic of patients visiting an Oriental clinic with CFS or ICF. Methods: Patients who met criteria for CFS or ICF were enrolled in this study, from March 2011 to Jun 2013. Clinical information focused on symptom differentiation and Sasang constitution was analyzed. Pearson's chi-square test was used to compare the frequency among sub-groups. Results: In total, 77 patients (49 male and 28 female) were diagnosed with CFS (17 male and 13 female) or ICF (32 male and 15 female). The symptom differentiation was 37.7%, 26.0%, 13.0%, and 23.3% for "Qi deficiency of both spleen and lung (脾肺氣虛)", "Yin deficiency of both liver and kidney (肝腎陰虛)", "Blood deficiency of both heart and spleen (心脾血虛)" and "Yang deficiency of both spleen and kidney (脾腎陽虛)", while Sasang constitutional distribution was 59.7%, 22.1% and 18.2% Soeumin, Taeumin and Soyangin, respectively. The fatigue severity was notably different between CFS ($63.7{\pm}12.1$) or ICF ($52.2{\pm}15.3$) according to the Chalder fatigue scale (10-point scale for eleven questions). There was statistical significance regarding symptom differentiation and Sasang constitution (p < 0.05). Conclusions: In the process of traditional Korean medicine-based development of therapeutics for patients with chronic fatigue or related conditions, this report can serve as reference data.
Objectives: This survey was done in order to find out how Korean medical doctors derive pattern identification for acupuncture prescriptions in treating low back pain in real clinical practice. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society from March 26th to April 14th in 2009. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results : 1.57 Korean medical doctors selected meridian pattern identification based on the course of the meridians(44.6%), visceral pattern identification(32.1%), pattern identification based on cause of disease(14.3%) as the most commonly used pattern identification methods for acupuncture prescription when treating low back pain patients in real clinical practice. 2. In meridian pattern identification based on the course of the meridians, gallbladder meridian of hypochondriac region(13.0%), bladder meridian of lateral low back region(11.2%), governer vessel of central low back region(11.7%) were selected 3. In visceral pattern identification, yang deficiency of kidney(20.2%), deficiency of kidney(19.3%), liver(16.7%), yin deficiency of kidney(14.0%), violence qi of kidney(8.8%), small intestine(7.9%) were selected. Conclusions : In our e-mail survey, Korean medical doctors answered that Meridian Pattern Identification based on the course of the meridians is the most often used diagnosis method. Visceral pattern identification, pattern identification based on cause of disease, meridian pattern identification based on symptom and pattern identification based on qi-blood-yin-yang theory in order of frequency used, were selected for low back pain diagnosis in real clinical practice.
Objective : This study was to evaluate the clinical improvement of Panic attack and Anticipatory anxiety on Panic Disorder patients after Oriental medical treatment. In this paper, to find out how different the treatment of special individuals named 'old people' is with that of ordinary cases, Dongeuibogam(東醫寶鑑) was chosen as the study material and some parts in it where the symptoms of old people in the same disease category were treated in different ways were excerpted to analyze the tendency of pathological symptoms and prescriptions in the physiological perspective. As the result of analysis on the parts in Dongeuibogam(東醫寶鑑) where the old people were treated in different ways, it turned out that 65 prescriptions were used in 24 pathological symptoms. The 24 symptoms are included mainly in chapters of 'inner view(內景)' and 'outer form(外形)'. They are including the pathological symptoms which had been presented as general geriatric symptoms and also seems to have added other symptoms which should be clinically reflected in the specialty of treatment for geriatric diseases. The 65 prescriptions are also recorded mainly in chapters of 'inner view(內景)' and 'outer form(外形)'. The herbs used for them were sweet and warm herbs such as Rehmannia glutinosa (Gaertner) Libosch(熟地黃), Angelica gigas Nakai(當歸), Panax ginseng C. A. Mey(人蔘) and Atractylodes macrocephala Koidz(白朮) that can support the essence blood of liver and kidney and the energy of spleen, lung and stomach. Those herbs could be added or subtracted according to the symptom. It seems to have resulted from the treatment method that old people was applied to old people in consideration of their physiological features. When the 24 symptoms and 65 prescriptions for geriatric diseases different from ordinary ones in Dongeuibogam(東醫寶鑑) were considered in the physiological view point, it could be seen that each pathological symptom was manifested as a exhaustion of kidney qi(腎氣枯渴) which was a characteristic physiologic state of old people. Through this, it could be seen that the use of prescriptions was also made to mitigate the body fluid deficiency(津液不足), the kidney yin inner heat(腎陰內熱), the heart spirit void and loss(心神虛損), the kidney yang deficiency(腎陽不足), the spleen failing in transportation(脾失健運), the spleen yang deficiency(脾陽不足), the liver fire inner movement(肝火內動) and the lung energy void and loss(肺氣虛寒).

The score of PSQI has positive correlation with sc10 of deficiency of qi(氣虛) and zp of heart(心), and the score of IQ has positive correlation with sc10 of liver(肝) in male patients. 6. The duration from onset in the group of 22-59years are longer than the group of 60-80 years and the duration of using hypnotics have positive correlation with total sleep time in 22-59years group. 7. The score of PSQI has positive correlation with zp of liver(肝) in 22-59years group. 8. The score of PSQI has positive correlation with the score of BDI in 60-80years group. 9. The score of BDI has positive correlation with sc10 of deficiency of qi(氣虛) and zp of blood-deficiency(血虛), and the score of IQ has positive correlation with zp of coldness(寒) in 60-80years group. 10. The score of IQ has positive correlation with sc10 and zp of dryness(燥) in below 6 Months Group. 11. The score of PSQI has positive correlation with the score of BDI in over 6 Months Group. 12. The score of PSQI has positive correlation with zp of liver(肝) in over 6 Months Group. 13. The score of IQ has positive correlation with sc10 of dryness(燥) in BDI 2nd Grade Group. 14. The score of BDI has positive correlation with sc10 of kidney(腎) and the age has positive correlation with zp of heart(心) in BDI 3rd Grade Group. 15. The age has positive correlation with sc10 of damp(濕) in BDI 4th Grade Group. Conclusions : This study provides insights into the complicated associations of the pattern of insomnia with depression and Diagnos system of oriental medicine. And especially this study showed apparent correlation between insomnia and depression in 60-80years group and over 6 months group.
Objectives: The purpose of this study is to find out the characteristics of dysmenorrhea patients with the Inbody test results by Sasang constitutions. Methods: The data from the 541 participants were collected using a structured measurement of menstrual pain. Based on the survey responses, we had 329 women with dysmenorrhea as the test group and 212 women without dysmenorrhea as the control group. The clinical trials subjects were asked to respond to another questionnaire for identifying their constitutional types and undergo Inbody test. Results: The result of a comparison of the test and control groups showed that the DSOM test showed that the scores of blood deficiency, qi stagnation, blood stasis and phlegm were significantly higher in dysmenorrhea. The result of the taeumin's test groups showed that the DSOM test showed that the scores of heat were significantly higher. For the Sasang constitution, there is a difference on the cause of the outbreak. Taeumin from blood deficiency, blood stasis, dampness, heart, kidney, phlegm and lung is associated with dysmenorrhea. Soeumin from blood deficiency, qi stagnation, blood stasis, liver, heart, spleen and phlegm is associated with dysmenorrhea. The ratio of overweight of taeumin was low in blood deficiency and yin deficiency. The ratio of lowweight of soeumin was high in heat. Conclusions: The DSOM test showed that the scores of blood deficiency, qi stagnation, blood stasis and phlegm were significantly higher in dysmenorrhea.
From the 24 kinds of literature on the Consumptive disease, it can be concluded as follows. 1. The consumptive disease is the Imparement of deficiency type due to overstrain. it is a general term for these all symptom such as and Deficiency of primordial Qi and Essence of life and blood. 2. The excessive fire due to Yin-Deficiency and the injury of spleen-stomach is accounted much of the cause of Consumptive disease. 3. The main cause of the Hepatic asthenia are the Anger, Consumption and over-thinking. 4. The symptoms of the Consumptive disease are mainly caused by the functional disorder of Liver taking charge of tendons. storing and regulating blood, Heart being in charge of blood circulation. taking charge of mental activities. Spleen taking charge of muscles, transforting and transforming nutrients. Lung taking charge of skins and hairs, taking charge of respirations, Kidney taking charge of bones, storing essence of life. 5. The main symptoms of Hepatic asthenia are flaccidity of muscles and temeons which causes limited movement caused by muscular atonia and the loss of bightness of eyes. 6. The main treatments of Consumptive disease are the invigorating the Spleen-stomach and the invigorating the Kidney and storing essence of life. 7. The treatments of Hepatic asthenia are the moderating the middle and the nourishing the muscles and tendons.
Objectives: This study was a methodological study to verify the reliability and validity and to make a diagnosis of a diagnostic tool for climacteric and postmenopausal syndrome pattern identification (CaPSPI). Methods: This study was conducted from June 1, 2018 to October 18, 2018 with ${{\bigcirc}{\bigcirc}}$ University Korean Medicine Hospital IRB's approval (2018-3). To make a diagnosis using CaPSPI, we decided the cut-points for the tool. Three professors of ${{\bigcirc}{\bigcirc}}$Korean Medical University conducted pattern identification diagnosis. The result is marked from 0 to 3, 0 is 'No', 1 is 'Slightly Yes', 2 is 'Yes' and 3 is 'Very Yes'. And if two or more professors' diagnoses are the same, we took the diagnoses as a diagnostic criteria. The decision of pattern by three experts converted to 0-1 scores in two ways. In "method 1", if the diagnosis was zero points, the score was 0 (have no such identification), and the rest was 1 (have such identification). In "method 2", if the diagnosis was zero or 1 point, the score was 0, and the rest was 1. After that, intraclass correlation was calculated for experts agreement. And logistic regression was conducted. A response variable was the results of the experts' diagnosis and an explanatory variable was the results of the pattern identification diagnostic tool. Results: The diagnosis of the three experts showed excellent concordance of more than 0.794 and showed a significant correlation with the diagnostic tool. Both 'Method 1' and 'Method 2' showed statistically significant effects with the diagnosis of 3 experts and the results of the diagnostic tool. The frequency of cumulative pattern identification diagnosis in 'Method 1' and 'Method 2' were found to be 578 occasions and 203 occasions, respectively. The average number of pattern held by participants in 'Method 1' and 'Method 2' were found to be 5.26 and 1.85, respectively. In both "Method 1" and "Method 2," the yield frequency of liver qi depression was the highest, and the frequency of kidney yin deficiency and liver-kidney yin deficiency was relatively high. Conclusions: Based on the above results, it is thought that, in diagnosis using CaPSPI of menopausal women, 'method 1' could be used for the health diagnosis and prevention, 'method 2' could be used for the pattern diagnosis. On the conclusion, CaPSPI is thought to be available for pattern diagnosis of menopause women.
Objectives The aim of this study was to develop a standard tool of pattern identification for Knee Osteoarthritis, which will be applied to clinical research. Methods The advisor committee for this study was organized by 11 panel of experts (Korean Rehabilitation Medicine professors, Acupuncture and Moxibustion professors belonging to Korean Medicine colleges, Principal Researcher of Korea Institute of Oriental Medicine, Doctor of Korean medicine). The pattern identifications and symptoms for this tool were extracted from published Korean and Chinese literature. Through the discussion among internal experts and consultation from advisors, the Instrument on Pattern Identifications for Knee Osteoarthritis was developed. Results 1) Five pattern identifications (The Wind, Chill, and Moisture, The Moist-Heat, Blood Stasis, Yang Deficiency of Spleen and Kidney, Yin Deficiency of Liver and Kidney) were set for the tool. 2) The mean weights which represent the importance of each symptom and scored on a hundred-point scale was obtained. 3) The Instrument on Pattern Identifications for Knee Osteoarthritis was designed in the self-reporting format composed of 46 questions. Conclusions The Instrument on Pattern Identifications for Knee Osteoarthritis was created through this study. Though this study is not proved about validity, reliability, the instrument of pattern identification for Knee Osteoarthritis is meaningful and expected to be applied to the subsequent.
