인체 치간부위 치조골 결손에 사용된 합성골의 효과에 관한 연구 (THE EFFECTS OF POROUS HYDROXYAPATITE AND NATURAL CORAL ON HUMAN PERIODONTAL DEFECTS)

  • Various alloplastic materials have been used on the periodontally diseased ossous defects. Hydroxyapatite, which is used the most common alloplastic material is a non-resorbable form of calcium phosphate and natural coral which is a biodegradable by carbonic anhydrase in osteoclast was introduced recently. The purpose of the present study was to evaluate the clinical effects of porous hydoxyapatite and natural coral on the human periodontal defects. Four males and three females who had adult periodontitis were selected for this study. The teeth that had similar bone loss radiographically and periodontal pocket deeper than 5mm were selected. Gingival recession, pocket depth, plaque index(Silness & Loe), sulcus bleeding index and tooth mobility (measured by Periotest$^{(r)}$) were examined before graft. Before insertion of alloplastic materials, the depth from CEJ to bone crest and from CEJ to base of the osseous defect was recorded. Porous particulate hydroxyapatite(Interpore 200$^{(r)}$, A group) was place on the defect and natural coral(Biocoral$^{(r)}$, B group) was placed on the defect of the opposing tooth. Six months post-surgically the same parameters were recorded by reentry procedures. A and B group showed 0.6mm of mean recession. Mean reduction of pocket depth were 5mm for A group and 4.9mm of B group. Reduced SBI and tooth mobility were recorded. Osseous defect fills of the original defects were 2.9mm for A and 3mm for B group. Percentage defect fills were 71% for A and 59% for B group. The difference of defect fill between pre- and post-insertion was statstically significant(p<0.05). But the difference between the two groups was not significant statistically(p<0.05). The clinical impression at 6 month re-entry and the numerical date indicate that natural coral as well as porous particulate hydoxyapatite has a definite potential as an alloplastic implant in the treatment of periodontal osseous defects.

Fe와 galvanic couple된 알루미늄의 내식성에 미치는 마그네슘의 영향 (Effects of Mg on corrosion resistance of Al galvanically coupled to Fe)

  • Effects of magnesium and pH on corrosion of aluminum galvanically coupled to iron have studied by using potentio- dynamic and static tests for polarization curves, Mott-Schottky test for analysis of semiconductor property, and GD-AES and XPS for film analysis. Pitting potential was sensitive to magnesium as an alloying element but not to pH, while passive current was sensitive to pH but not to magnesium. It was explained with, instead of point defect model (PDM), surface charge model describing that the ingression of chloride depends on the state of surface charge and passive film at film/solution interface is affected by pH. In addition, galvanic current of aluminum electrically coupled to iron was not affected by magnesium in pH 8.4, 0.2M citrate solution but was increased by magnesium at the solution of pH 9.1. The galvanic current at pH 9.1 increased with time at the initial stage and after the exposure of about 200 minute, decreased and stabilized. The behavior of the galvanic current was related with the concentration of magnesium at the surface. It agreed with the depletion of magnesium at the oxide surface by using glow discharge atomic emission spectroscopy (GD-AES). In addition, pitting potential of pure aluminum was reduced in neutral pH solution where chloride ion maybe are competitively adsorbed on pure aluminum. It was confirmed by the exponential decrease of pitting potential with log of [$Cl^-$] around 0.025 M of [$Cl^-$] and linear decrease of the pitting potential. From the above results, unlike magnesium, alloying elements with higher electron negativity, lowering isoelectric point (ISE), are recommended to be added to improve pitting corrosion resistance of aluminum and its alloys in neutral solutions as well as their galvanic corrosion resistance in weakly basic solutions.

Void Formation Induced by the Divergence of the Diffusive Ionic Fluxes in Metal Oxides Under Chemical Potential Gradients

  • When metal oxides are exposed to chemical potential gradients, ions are driven to diffusive mass transport. During this transport process, the divergence of ionic fluxes offers the formation/annihilation of oxides. Therefore, the divergence of ionic flux may play an important role in the void formation in oxides. Kinetic equations were derived for describing chemical potential distribution, ionic fluxes and their divergence in oxides. The divergence was found to be the measure of void formation. Defect chemistry in scales is directly related to the sign of divergence and gives an indication of the void formation behavior. The quantitative estimation on the void formation was successfully applied to a growing magnetite scale in high temperature oxidation of iron at 823 K.

  • Corrosion and lassitude are very important in the metal construction Currently, the shot peening is used much for removing the defect on the surface, improving the fatigue strength on surface. In this study. the influence of shot peening to the corrosion property were investigated in 3.5% NaCl, 10% HNO3 + 3% HF. 6% FeC13. The immersion test was performed on two kind of specimen(Removed corrosion product. RCP and Continuous immersed specimen: CIS). The immersion periods was 3600hours. Corrosion potential. weight loss, and fatigue crack growth characteristics were investigated. The result, shot peening in corrosion potential showed stable potential. Also, shot peening displayed small Weight loss than un peening. And Fatigue crack resistivity of shot peening in corrosion solution's fatigue crack appeared greatly than un peening.

랫드 두개골 결손부에서 단삼 수용성 추출물의 골형성 효과 (Effect of water extract of Danshen on bone regeneration of rat calvarial defect model)

  • The purpose of this study was to evaluate the osteogenic capacity of water extract of danshen (Salvia miltiorrhiza Bunge). We have established in rat critical-sized calvarial defect model using the combination with collagen scaffold and danshen hydrophilic extract. All rats were extinguished at 8 weeks after bone graft surgery, and the bone regeneration ability of bone grafting sides was evaluated by plain radiography and micro-CT. These results revealed water extract of danshen had the potential to promote osteogenesis especially continuous oral administration with local treatment compared to one-shot local treatment. This compound may provide a new alternative agent for growth factors to promote bone healing and bone regeneration. In conclusion, these results suggest that danshen hydrophilic extract have the potential to promote osteogenesis in bone defects. Further studies about fusion technology with salvianolic acid B, peptides, growth factors, and scaffolds using of the combination of tissue engineering, cell engineering and mechanical engineering are needed.

  • Background: Bone grafting has been considered the gold standard for hard tissue reconstructive surgery and is widely used for large mandibular defect reconstruction. However, the midface encompasses delicate structures that are surrounded by a complex bone architecture, which makes bone grafting using traditional methods very challenging. Three-dimensional (3D) bioprinting is a developing technology that is derived from the evolution of additive manufacturing. It enables precise development of a scaffold from different available biomaterials that mimic the shape, size, and dimension of a defect without relying only on the surgeon's skills and capabilities, and subsequently, may enhance surgical outcomes and, in turn, patient satisfaction and quality of life. Review: This review summarizes different biomaterial classes that can be used in 3D bioprinters as bioinks to fabricate bone scaffolds, including polymers, bioceramics, and composites. It also describes the advantages and limitations of the three currently used 3D bioprinting technologies: inkjet bioprinting, micro-extrusion, and laserassisted bioprinting. Conclusions: Although 3D bioprinting technology is still in its infancy and requires further development and optimization both in biomaterials and techniques, it offers great promise and potential for facial reconstruction with improved outcome.

3D 유한요소법을 이용한 원전 매설배관 부식결함 탐상기술 개발 (Technology for the Detection of Corrosion Defects in Buried Pipes of Nuclear Power Plants with 3D FEM)

  • The modeling of 3D finite elements based on CAD data has been used to detect sites of corrosion defects in buried pipes. The results generated sophisticated profiles of electrolytic potential and vectors of current distributions on the earth surface. To identify the location of defects in buried pipes, the current distribution on the earth surface was projected to a plane of incidence that was identical to the pipe locations. The locations of minimum electrolytic potential value were found. The results show adequate match between the locations of real and expected defects based on modeling. In addition, the defect size can be calculated by integrating the current density curve. The results show that the defect sizes were $0.74m^2$ and $0.69m^2$, respectively. This technology may represent a breakthrough in the detection of indirect damage in various cases involving multiple defects in size and shape, complex/cross pipe systems, multiple anodes and stray current.

증기발생기 전열관 2차측 응력부식균열의 실험실적 모사 방법 (Laboratorial technique for fabrication of outer diameter stress corrosion cracking on steam generator tubing)

  • In this work, it is aimed to develop the fabrication method of axial stress corrosion cracking (SCC) defects having various sizes, on the outer diameter surface of the steam generator (SG) tubings. To control the length of the artificial SCC defect, the specific area of the SG tubing samples was exposed to an acidic solution after a sensitization heat treatment. During the exposure to an acidic solution, a direct current potential drop (DCPD) method was adopted to monitor the crack depth. The size of the SCC defect was first evaluated by an eddy current test (ECT), and then confirmed by a destructive examination. From the comparison, it was found that the actual crack length was well controlled to be similar to the length of the surface exposed to an acidic solution (5, 10, 20 or 30 mm in this work) with small standard deviation. From in-situ monitoring of the crack depth using the DCPD method, it was possible to distinguish a non-through wall crack from a through wall crack, even though the depth of the non-through wall crack was not able to be precisely controlled. The fabrication method established in this work was useful to simulate the SCC defect having similar size and ECT signals as compared to the field cracks in the SG tubings of the operating Korean PWRs.

Updated Surgical Techniques and Expanded Indications of Free Vascularized Fibular Graft

  • Free or pedicled vascularized fibular grafts (VFG) are useful for the reconstruction of large skeletal defects, particularly in cases of scarred or avascular beds, or in patients with combined bone and soft tissue defects. Compared to non-VFG, VEG, which contains living osteocytes and osteoblasts, maintains its own viability and serves as good osteoconductive and osteoinductive graft. Due to its many structural and biological advantages, the free fibular osteo- or osteocutaneous graft is considered the most suitable autograft for the reconstruction of long bone defects in the injured extremity. The traditional indication of VFG is the long bone and soft tissue defect, which cannot be reconstructed using a conventional operative method. Recently, the indications have been widely expanded not only for defects of midtibia, humerus, forearm, distal femur, and proximal tibia, but also for the arthrodesis of shoulder and knee joints. Because of its potential to allow further bone growth, free fibular epiphyseal transfer can be used for the hip or for distal radius defects caused by the radical resection of a tumor. The basic anatomy and surgical techniques for harvesting the VFG are well known; however, the condition of the recipient site is different in each case. Therefore, careful preoperative surgical planning should be customized in every patient. In this review, recently expanded surgical indications of VFG and surgical tips based on the author's experiences in the issues of fixation method, one or two staged reconstruction, size mismatching, overcoming the stress fracture, and arthrodesis of shoulder and knee joint using VFG are discussed with the review of literature.

두개골의 결손과 동반된 재발성 세균성 뇌막염 2례 (Recurrent Bacterial Meningitis Secondary to Cochlear Aplasia, Right and Acquired Cribriform Plate Defect due to Trauma)

  • Recurrent meningitis in children is not only a potentially life threatening condition, but often involves the child in the trauma though repeated hospital admissions and multiple invasive investigations to find the underlying causes. Symptoms and signs of CSF rhinorrhea or otorrhea are infrequent in these patients and difficult to diagnose in young children. All young children treated for meningitis should then be administered an evoked potential audiometry as a post-treatment test. If sensorineural hearing loss is identified, the clinician should be alerted to the possibility of CSF leakage as the cause of the meningitis. Radiologic studies should be performed to rule out preexisting congenital, or acquired, abnormalities requiring surgical exploration. Two young children with recurrent meningitis due to a right cochlear aplasia and a cribriform plate defect caused by trauma are presented to illustrate the problems of diagnosis and management. A review of literatures will also be presented briefly.
