The composite anterolateral thigh flap with vascularized fascia lata has emerged as a workhorse at our institution for complex Achilles defects requiring both tendon and soft tissue reconstruction. Safe elevation of this flap, however, is occasionally challenged by absent or inadequate perforators supplying the anterolateral thigh. When discovered intraoperatively, alternative options derived from the same vascular network can be pursued. We present the case of a 74-year-old male who underwent composite Achilles defect reconstruction using a segmental rectus femoris myofascial free flap. Following graduated rehabilitation, postoperatively, the patient resumed full activity and was able to ambulate on his tip-toes. At 1-year follow-up, active total range of motion of the reconstructed ankle exceeded 85% of the unaffected side, and donor site morbidity was negligible. American Orthopaedic Foot and Ankle Society and Short Form-36 scores improved by 78.8% and 28.8%, respectively, compared to preoperative baseline assessments. Based on our findings, we advocate for use of the combined rectus femoris myofascial free flap as a rescue option for reconstructing composite Achilles tendon/posterior leg defects in the setting of inadequate anterolateral thigh perforators. To our knowledge, this is the first report to describe use of this flap for such an indication.
The reliability of a Bridge management System depends on the quality of visual inspection and the reliable estimation of bridge condition rating. However, the current practices of visual inspection have been identified with several limitations, such as: they are time-consuming, provide incomplete information, and their reliance on inspectors' experience. To overcome such limitations, this paper presents an approach of automating the prediction of condition rating for bridges based on digital image analysis. The proposed methodology encompasses image acquisition, development of 3D visualization model, image processing, and condition rating model. Under this method, scaling defect in concrete bridge components is considered as a candidate defect and the guidelines in the Ontario Structure Inspection Manual (OSIM) have been adopted for developing and testing the proposed method. The automated algorithms for scaling depth prediction and mapping of condition ratings are based on training of back propagation neural networks. The result of developed models showed better prediction capability of condition rating over the existing methods such as, Naïve Bayes Classifiers and Bagged Decision Tree.
Prospective study of lymph node imaging of twenty stomach cancer cases with dissected lymph nodes being injected into the submucosa layer of stomach under surgical field was done. Total dose of 5 mci in 5 cc of volume was injected along the multiple sites of the lesser and greater curvature of stomach and collected lymph nodes within 2 hour of surgical time were placed under gamma camera and lymph node imagings were obtained. Pathological invasion of tumor and correlation of cold defect or hot uptake was compared each other. Tumor invasion of nodes revealed cold defect area which was correlated well with the pathological specimen. Correlation rate was 84.6%. We are planning to extend these procedures and trying endoscopic injection of positive imgaing agents such as Ga-73-3 Ig 2 alpha in future.

Recently, with the development of the fourth industrial revolution that has been achieved through the fusion of information and communication technology (ICT), the technologies of AI, IOT, BIG-DATA, it is increasing utilization rate by industry and research and development of application technologies are being actively carried out. Especially, in the case of unmanned aerial vehicles, the construction market is expected to be one of the most commercialized areas in the world for the next decade. However, research on utilization of unmanned aerial vehicles in the construction field in Korea is insufficient. In this study, We have developed a quantitative building inspection method using the unmanned aerial vehicle and presented the protocol for it. The proposed protocol was verified by applying it to existing old buildings, and defect information could be quantified by calculating length, width, and area for each defect. Through this technical research, the final goal is to contribute to the development of safety diagnosis technology using unmanned aerial vehicle and risk assessment technology of buildings in case of disaster such as earthquake.
A liquid metal reactor (LMR) operated at high temperatures is subjected to both cyclic mechanical loading and thermal loading; thus, creep-fatigue is a major concern to be addressed with regard to maintaining structural integrity. The Korea Advanced Liquid Metal Reactor (KALIMER), which has a normal operating temperature of $545^{\circ}C$ and a total service life time of 60 years, is composed of various cylindrical structures, such as the reactor vessel and the reactor baffle. This study focuses on the creepfatigue crack initiation for a cylindrical Y-junction structure made of 316 stainless steel (SS), which is subjected to cyclic axial tensile loading and thermal loading at a high-temperature hold time of $545^{\circ}C$. The evaluation of the considered creep-fatigue crack initiation was carried out utilizing the ${\sigma}_d$ approach of the RCC-MR A16 guide, which is the high-temperature defect assessment procedure. This procedure is based on the total accumulated strain during the service time. To confirm the evaluated result, a high-temperature creep-fatigue structural test was performed. The test model had a circumferential through wall defect at the center of the model. The defect front of the test model was investigated after the $100^{th}$ cycle of the testing by utilizing a metallurgical inspection technique with an optical microscope, after which the test result was compared with the evaluation result. This study shows how creep-fatigue crack initiation for a high-temperature structure can be predicted with conservatism per the RCC-MR A16 guide.

The objectives of this study were to classify the first permanent molars showing molar-incisor hypomineralization (MIH) on the basis of defect size and to assess differences in the prognoses of restorations performed with different materials. The first permanent molars with MIH and posteruptive breakdown were categorized into MIH class I, II, and III. After performing restorations, retreatment frequencies were examined according to the defect class and initial restorative material used. Sealants, composite resins, and stainless steel crowns showed the highest survival rates in cases of MIH class I, II, and III defects, respectively.
The purpose of this study is to prevent the unstable fracture at the FCA butt weldment of hatch coaming deck in the ultra large containership during service life. In order to do it, the behavior of the embedded crack at the weldment under design loading conditions was evaluated in accordance with BS7910. Here, the level of primary stress induced by ship motion was evaluated by the design code of classification society and FEA. The level of residual stress as secondary stress was calculated in consideration of the restraint degree of weldment and welding heat input by using the predictive equation proposed by authors in the previous study. The fatigue crack growth rate at the weldment was evaluated using CT specimen in accordance with ASTM E647. According to the results, although the allowable defect for embedded crack specified in the classification society exists at the weldment, the occurrence possibility of unstable fracture at the weldment could not be negligible, regardless of CTOD value given in this study. So, in this study, the effect of initial defect size, welding heat input, restraint degree and CTOD on the fracture mechanical characteristics of embedded crack at the weldment was evaluated by the comprehensive fracture assessment. Based on the results, the design criteria including allowable defect, residual stress level and CTOD value was established to prevent the unstable fracture at the FCA butt weldment of hatch coaming deck in an ultra large containership during service life of 20years.

Background: Recent studies have reported high rates of recurrence of shoulder instability in patients with glenoid bone defects greater than 20% after capsulolabral reconstruction. The purpose of the present study was to evaluate the failure rate of arthroscopic capsulolabral reconstruction for the treatment of anterior instability in the presence of glenoid bone deficits >20%. Methods: Retrospective analyses were conducted among cases with anterior shoulder instability and glenoid bone defects of >20% that were treated by arthroscopic capsulolabral reconstruction with a minimum 2-year follow-up (30 cases). We included the following variables: age, bone defect size, instability severity index score (ISIS), on-/off-track assessment, incidence recurrent instability, and return to sports. Results: The mean glenoid bone defect size was 25.8%±4.2% (range, 20.4%-37.2%), and 18 cases (60%) had defects of >25%. Bony Bankart lesions were identified in 11 cases (36.7%). Eleven cases (36.7%) had ISIS scores >6 points and 21 cases (70%) had off-track lesions. No cases of recurrent instability were identified over a mean follow-up of 39.9 months (range, 24-86 months), but a sense of subluxation was reported by three patients. Return to sports at the preinjury level was possible in 24 cases (80%), and the average satisfaction rating was 92%. Conclusions: Arthroscopic soft tissue reconstruction was successful for treating anterior shoulder instability among patients with glenoid bone defects >20%, even enabling return to sports. Future studies should focus on determining the range of bone defect sizes that can be successfully managed by soft tissue repair.
Purpose: Various methods have been proposed to achieve the nearly complete decontamination of the surface of implants affected by peri-implantitis. We investigated the in vitro debridement efficiency of multiple decontamination methods (Gracey curettes [GC], glycine air-polishing [G-Air], erythritol air-polishing [E-Air] and titanium brushes [TiB]) using a novel spectrophotometric ink-model in 3 different bone defect settings (30°, 60°, and 90°). Methods: Forty-five dental implants were stained with indelible ink and mounted in resin models, which simulated standardised peri-implantitis defects with different bone defect angulations (30°, 60°, and 90°). After each run of instrumentation, the implants were removed from the resin model, and the ink was dissolved in ethanol (97%). A spectrophotometric analysis was performed to detect colour remnants in order to measure the cumulative uncleaned surface area of the implants. Scanning electron microscopy images were taken to assess micromorphological surface changes. Results: Generally, the 60° bone defects were the easiest to debride, and the 30° defects were the most difficult (ink absorption peak: 0.26±0.04 for 60° defects; 0.32±0.06 for 30° defects; 0.27±0.04 for 90° defects). The most effective debridement method was TiB, independently of the bone defect type (TiB vs. GC: P<0.0001; TiB vs. G-Air: P=0.0017; TiB vs. GE-Air: P=0.0007). GE-Air appeared to be the least efficient method for biofilm debridement. Conclusions: T-brushes seem to be a promising decontamination method compared to the other techniques, whereas G-Air was less aggressive on the implant surface. The use of a spectrophotometric model was shown to be a novel but promising assessment method for in vitro ink studies.
