Ovarian Cancer in Children and Adolescents: Treatment and Reproductive Outcomes

  • Objective: To review ovarian cancer cases in children and adolescents in Siriraj Hospital and assess the prognosis, recurrence of disease, and reproductive outcomes after treatment. Materials and Methods: A retrospective descriptive study was conducted in ovarian cancer patients 21 years and younger who had been treated at Siriraj Hospital between January 1990 and December 2009. Medical records were reviewed and relevant data were recorded. Results: A total of 48 cases met the criteria; their mean age was 16.4 years. Abdominal distension was the major symptom. 91.6% were germ cell tumors and the remaining cases were sex cord-stromal and epithelial tumors. More than half (25/48 cases) presented with stage I disease. The most common used chemotherapy regimen for germ cell tumors was BEP (bloemycin, etoposide, cisplatin). Most of patients had favorable outcomes; 46/48 cases had complete remission and retained their good health at the time of the review. We had only one recurrent case and one dead case. Ten of contacted patients had married and 3 of them had successful full-term pregnancies. Conclusions: Ovarian malignancy in children and adolescents is a rare disease. The authors reported 48 cases in 20 year-period of work. Most of them have favorable outcomes. Return of ovarian function and fertility are the topics of interest.

  • As a consequence of geophysical surveys for large-scale constructions and ground investigations, huge amount of geophysical data are collected. However, due to the lack of the systematic management of such data, most of them are used temporarily and kept in dead storage, which in turn leads the abuse of social properties. To propose the solution of this kind of problem, geophysical data are kept in database using geographical information system (GIS). The introduction of GIS in geophysical data management allows the efficiency in data management and user-friendliness in data access. This data management system is especially appropriate for the many geological companies and the government organizations in systematic data management and sharing so that the time ,md financial expenses are greatly reduced. This system even provides the basic data services for the efficient development and conservation of the surface and underground spaces.

Anti-cancer Activity of Anthricin through Caspase-dependent Apoptosis in Human Hypopharyngeal Squamous Carcinoma Cell

  • Anthricin (Deoxypodophyllotoxin), a naturally occurring flavolignan, has well known anti-cancer properties in several cancer cells, such as prostate cancer, cervical carcinoma and pancreatic cancer. However, the effects of Anthricin are currently unknown in oral cancer. We examined the anticancer effect and mechanism of action of Anthricin in human FaDu hypopharyngeal squamous carcinoma cells. Our data showed that Anthricin inhibits cell viability in a dose- and time-dependent manner ($IC_{50}$ 50 nM) in the MTT assay and Live & Dead assay. In addition, Anthricin treated FaDu cells showed marked apoptosis by DAPI stain and FACS. Furthermore, Anthricin activates anti-apoptotic factors such as caspase-3, -9 and poly (ADP-ribose) polymerase (PARP), suggesting that caspase-mediated pathways are involved in Anthricin- induced apoptosis. Anthricin treatment also leads to accumulation of the pro-apoptotic factor Bax, followed by inhibition of cell growth. Taken together, these results indicate that Anthricn-induced cell death of human FaDu hypopharyngeal squamous carcinoma cells is mediated by mitochondrial-dependent apoptotic pathway. In summary, our findings provide a framework for further exploration on Anthricin as a novel chemotherapeutic drug for human oral cancer.

A study on Development way of 119 drowning rescue (119수난구조대의 현황 및 발전 방안)

  • In Korea, the number of those who were drowned of the dead by 'accident and injury' ranked the third of causes of death is over 2,300 people a year. So the Ministry of Home Affairs arranged '119 drowning rescue team' at frequent occurrence regions of drowning temporarily as a part of drowning rescue measures during every summer season and made perfection more perfect for safety control and rescue services at prevention areas of accidents. However, considering that the number of professional drowning rescuers is small and it is managed temporarily only for summer, the placement of special rescue team equipped with professional education and qualification should be ensured and quick lifesaving and first-aid treatment should be conducted, since much time is required to arrive at accident place after receiving accident report at 119 office, rapid rescue and relief have not been achieved. Therefore, the placement of special rescue team should be increased for reducing the personal damages by considering regional characteristics and in particular placement of more drowning rescue teams in Jeonnam region which has more seas and rivers is needed This study aims at analyzing relief activities and statistics to cope with water accidents and achieve qualitative growth of first-aid services, examining actual conditions of water accident relief team and personnel assignment, providing first-aid services of good quality and establishing improvement methods to increase operation of relief team.

Studies on the Liver Toxicities with different Dosage of Wild Aconiti Tuber Decoction (임상투여용량에서 초오전탕액의 흰쥐에 대한 간독성연구)

  • Objective : This study was carried out to evaluate the liver toxicities of Wild Aconiti Tuber decoction. Methods : The amounts of aconitine in the methanol extract of Wild Aconiti Tuber was measured by HPLC. Safeties was studied by LD50 in mice. Liver toxicities were evaluated histologically and by CBC, blood chemistry after 2 weeks of 0.4g/kg/day clinical dosage oral administrations in rat. Results : 1. The amounts of aconitine in the methanol extract of Wild Aconiti Tuber is $1.697{\pm}0.052mg/g$. But aconitine was not detected in the water decoction of Wild Aconiti Tuber. 2. To evaluate LD50 and safeties of Wild Aconiti Tuber decoction, ICR mice were given high dose of 2, 5, 10g/kg for single time and were observed for 2 weeks. There were no dead animal and abnormal clinical sign and no abnormalities at the autopsy. So, LD50 was admitted to higher than 10g/kg. 3. After 2 weeks of 0.4g/kg/day clinical dosage oral administrations in rat, there was no significant change in the CBC and blood chemistry. 4. In the liver tissues of clinical dosage, mitotic figures, apoptosis and individual cell death were observed, but clear liver toxicities like fatty liver or necrosis were not observed. the liver tissues of high dose in mice, hydropic changes were getting severe as dose grows. Conclusions : According to the results, though aconitine was not detected in the Wild Aconiti Tuber decoction, 0.4g/kg/day 2 weeks p. o (clinical dosage) group showed weak changes in the liver tissues and high dose group showed liver toxicities like hydropic changes.

Electron Microscopic Alterations in Pediculus humanus capitis Exposed to Some Pediculicidal Plant Extracts

  • Head lice, Pediculus humanus capitis, infestation is an important public health problem in Egypt. Inadequate application of topical pediculicides and the increasing resistance to the commonly used pediculicides made the urgent need for the development of new agents able to induce irreversible changes in the exposed lice leading to their mortality. The aim of the present work is to evaluate pediculicidal efficacy of some natural products such as olive oil, tea tree oil, lemon juice, and ivermectin separately in comparison with tetramethrin-piperonyl butoxide (licid), as a standard pediculicide commonly used in Egypt. The effects of these products were evaluated by direct observation using dissecting and scanning electron microscopes (SEM). Results showed that after 1 hr exposure time in vitro, absolute (100%) mortalities were recorded after exposure to 1% ivermectin and fresh concentrate lemon juice. The mortalities were decreased to 96.7% after exposure to tea tree oil. Very low percentage of mortality (23.3%) was recorded after 1 hr of exposure to extra virgin olive oil. On the other hand, the reference pediculicide (licid) revealed only mortality rate of 93.3%. On the contrary, no mortalities were recorded in the control group exposed to distilled water. By SEM examination, control lice preserved outer smooth architecture, eyes, antenna, respiratory spiracles, sensory hairs, and legs with hook-like claws. In contrast, dead lice which had been exposed to pediculicidal products showed damage of outer smooth architecture, sensory hairs, respiratory spiracles and/or clinching claws according to pediculicidal products used.

  • To improve the current waveform of diode rectifiers, we propose a new operating principle for the voltage -doubler diode rectifiers. In the conventional voltage-doubler rectifier circuit, relatively large capacitors are used to boost the output voltage, while the proposed circuit uses smaller ones and a small reactor not to boost the output voltage but improve the input current waveform. A high input power factor of 97[%] and an efficiency of 98[%] are also obtained. The harmonic guide lines of proposed rectifier is no interfered with inverter switching, resulting in a simple, reliable and low-cost ac-to dc converters in comparison with the boost-type current-improving circuits.It compared conventional pulse-widthmodulated(PWM)inverter with half pulse-widthmodulated (HPWM) inverter. Proposed HPWM inverter eliminated dead-time by lowering switchingloss and holding over-shooting.

Acellular Dermal Matrix as a Core Strut for Projection in Nipple Reconstruction: Approaches for Three Different Methods of Breast Reconstruction

  • Background The objective of this paper was to describe a novel technique for improving the maintenance of nipple projection in primary nipple reconstruction by using acellular dermal matrix as a strut in one of three different configurations, according to the method of prior breast reconstruction. The struts were designed to best fill the different types of dead spaces in nipple reconstruction depending on the breast reconstruction method. Methods A total of 50 primary nipple reconstructions were performed between May 2012 and May 2015. The prior breast reconstruction methods were latissimus dorsi (LD) flap (28 cases), transverse rectus abdominis myocutaneous (TRAM) flap (10 cases), or tissue expander/implant (12 cases). The nipple reconstruction technique involved the use of local flaps, including the C-V flap or star flap. A $1{\times}2-cm$ acellular dermal matrix was placed into the core with O-, I-, and L-shaped struts for prior LD, TRAM, and expander/implant methods, respectively. The projection of the reconstructed nipple was measured at the time of surgery and at 3, 6, and 9 months postoperatively. Results The nine-month average maintenance of nipple projection was $73.0%{\pm}9.67%$ for the LD flap group using an O-strut, $72.0%{\pm}11.53%$ for the TRAM flap group using an I-strut, and $69.0%{\pm}10.82%$ for the tissue expander/implant group using an L-strut. There were no cases of infection, wound dehiscence, or flap necrosis. Conclusions The application of an acellular dermal matrix with a different kind of strut for each of 3 breast reconstruction methods is an effective addition to current techniques for improving the maintenance of long-term projection in primary nipple reconstruction.

A Study on Landscape of Naeap Village in Andong interpreted with CPTED Principles (범죄예방환경설계(CPTED) 원리로 해석한 안동 내앞마을 경관 연구)

  • The purpose of this study is to interpret Landscape of Naeap Village which still preserves the Confucian culture and the traditional clans among the ideally settled villages mentioned in Joong Hwan Lee's "Taengniji" and "Joseon's Fengsui" from the time of Japanese Imperial Rule, from CPTED principles. The following are the findings of this study: First, in terms of access control and zoning, Naeap Village was controlling access from the outside with the natural environment of Banbyeoncheon River and the hills surrounding the village, the artificial environment of human-scale walls around the head house and the dead ends, and the reformation by the Confucian ideologies. Naeap Village, in particular, is prominently configured by the hierarchy of zones; the Gaehosong pine trees, Gyeongpodae, and Naeapssu by the entrance to the village are considered the village itself and the landscapes and valleys are managed by the head house. Second, the Confucian culture across the village, the traditional vegetation method that does not hide the visibility, and the workers in the farms allow natural monitoring. The surrounding visibility is also applied to the spirit of mutual cooperation in the farming society, the culture of commoners at the common well and laundry site, and the culture of ruling class at the towers and pavilions. Third, Traditional villages show the efforts to preserve and maintain the villages with the village rules, the organizational decrees of the clan, and active response to national disasters.

Analysis of Treatment Efficiency according to Open-water in Constructed Wetland (인공습지 내 개방수역 조성에 따른 처리효율분석)

  • The field scale experiment which is constructed with four sets (0.88 ha for each set) of wetland (0.8 ha) and pond (0.08 ha) systems was performed to examine the effect of plant coverage on the constructed wetland performance and to recommend the optimum development and management of macrophyte communities. After six growing seasons of wetlands, plant coverage was about 100%. And the concentration of DO showed low value (1.0~5.4 mg/L). This is caused by a blighted plant consumed dissolved oxygen with decay in water column. As the result, water column went to be anaerobic conditions and T-N removal rate are 58~67%. Dead vegetation increased nitrogen removal during winter because it is a source of organic carbon which is an essential parameter in denitrification. However, wetland released phosphorus caused by a blighted plant and accumulation, the removal rate of phosphorus might be decreased. To rise of DO concentration, the three open-waters were constructed in cell 3 and 4. Cell 3 has two open-waters (width 10 m, depth 1.8 m) and cell 4 has one open-water (width 20 m, depth 1.8 m). As the result, DO concentration and treatment efficiency of nutrient and BOD were improved. In case that constructed wetland is operated for a long time, physical circulation structure such as open water help continuous circulation of aerobic and anaerobic conditions. Through the constructed open-water, treatment efficiency of phosphorus and nitrogen in wetland could be improved effectively.
