How to Understand Sleep and Sleep Problems in Patients with Prader-Willi Syndrome?

  • Sleep problems occur frequently among patients with Prader-Willi syndrome (PWS). The most common problem is excessive daytime sleepiness (EDS) that are closely related to of sleep-related breathing disorder (SRBD) such as obstructive sleep apnea (OSA) and congenital hypoventilation syndrome. Obesity, craniofacial dysmorphism and muscular hypotonia of patients with PWS may increase the risk of SRBD. Sleep apneas can interrupt the continuity of sleep, and these disruptions result in a decrease in both the quality and quantity of sleep. In addition to SRBD, other sleep disorders have been reported, such as hypersomnia, a primary abnormality of the rapid eye movement (REM) sleep and narcolepsy traits at sleep onset REM sleep. Patients with PWS have intrinsic abnormalities of sleep-wake cycles due to hypothalamic dysfunction. The treatment of EDS and other sleep disorders in PWS are similar to standard treatments. Correction of sleep hygiene such as sufficient amount of sleep, maintenance of regular sleep-wake rhythm, and planned naps are important. After comprehensive evaluation of sleep disturbances, CPAP or surgery should be recommended for treatment of SRBD. Remaining EDS or narcolepsy-like syndrome are controlled by stimulant medication. Bright light therapy might be beneficial for disturbed circadian sleep-wake rhythm caused by hypothalamic dysfunction.

일개 지역 영아의 수면 및 포유양상에 관한 연구 (Sleep and Feeding Patterns in Infancy)

  • This study was conducted to ascertain infant's sleep and feeding patterns for the purpose of building up the positive parent-child relationship and setting up a preliminary parenting education program. The subjects were 16 infants who were cared for in their houses. The data was collected using NCASA developed by Barnard et al. from October 1st to December 3lth, 1999. The analysis of data was done with SPSSWIN for descriptive statistics and t-test, ANOVA, Correlation Analysis. The result were as follows: 1. The average day sleep of the infant was 9.51 hours, night sleep was 6.27 hours, and total sleep was 15.84 hours. The longest period of day sleep was 3.02 hours, and the longest period of night sleep was 3.72 hours. The regularity of day sleep was 34.78%, night sleep was 61.72% and total was 43.76%. 2. The frequency of daytime feeding was 5.13 times, nighttime was 2.44 times and total was 7.38 times. The regularity of feeding was 52.14%. 3. In the correlation between sleep and feeding patterns, night sleep was negatively correlated with feeding method(r=-.52, P<0.05), the longest period of day sleep was negatively correlated with feeding method(r=-.73, P<0.001), feeding frequency(r=-.60, P<0.05), the regularity of feeding(r=-.70, P<0.001). The longest period of night sleep was negatively correlated with feeding frequency (r=-.53, P<0.05). Feeding method was positively correlated with feeding frequency(r=.71, P<0.001), the regularity of feeding(r=.57, P<0.05). Day sleep was positively correlated with total sleep(r=.93, P<0.001), the regularity of total sleep was positively correlated with the longest period of day sleep(r=.54, P<0.05), total sleep(r=.65, P<0.001). The regularity of feeding was positively correlated with feeding method(r=.57, P<0.05), total feeding frequency(r=.50, P<0.05), the frequency of daytime feeding(r=.61, P<0.05). Feeding method was positively correlated with total feeding frequency(r=.71, P<0.001). In conclusion, the study showed that infants slept more in the nighttime than in the daytime, but after 3 months the pattern changed and showed that nighttime sleep was increased. Also the regularity of night sleep was increased with the infant's age. Total feeding frequency was decreased with the infant's age, and the regularity of feeding was increased in infants who slept shorter periods in the daytime than in the nighttime.

수면과 알코올 (Alcohol and Sleep)

  • Alcohol has extensive effects on sleep and daytime sleepiness. Alcohol has a sleep inducing effect and the effect of increased non-REM sleep and suppressed REM sleep during the first half portion of night sleep, but alcohol induces the effect of decreased non-REM sleep and increased light sleep and frequent awakenings and REM rebound during the second half portion of night sleep. Alcohol provokes chronobiological change such as the changes of amplitude or the phase shifts of hormones or core body temperature. The sleep disruption resulting from alcohol drinking may lead to daytime fatigue and sleepiness. The elderly are at particular in the increased risk of alcohol-related sleep disorders because they achieve higher levels of alcohol in the blood and brain than do younger adults after consuming an equivalent dose. Bedtime alcohol consumption among older adults may lead to unsteadiness if walking is attempted during the night, with increased risk of falls and injuries. Continued alcohol use for sleep induction often induces aggravation of insomnia, alcoholism or sleep related breathing disorders such as obstructive sleep apnea. Alcohol should not be used as substitution of sleep pill because of the dependence and tolerance for sleep inducing effect, and the sleep disruption produced by alcohol withdrawal.

수면다원검사를 적용한 호흡관련 수면장애 한약 치료 3례 증례 보고 (Clinical Report of 3 Cases of Herbal Medicine Treatment Applied with Polysomnography for Breathing-Related Sleep Disorders)

  • Objectives: This case report intended to examine how herbal medicine treatment for breathingrelated sleep disorders applied with polysomnography affected the structure of sleep. Methods: From January to November 2019, we retrospectively reviewed the progress records and polysomnography reports of patients who visited Korean medical hospitals in Daejeon and Cheonan. Results: In case 1, the patient complained of daytime sleepiness and snoring and was diagnosed with obstructive sleep apnea disorder and treated with Ukgan-san-gami. There were significant changes in the polysomnographic parameters, such as sleep efficiency, waking after sleep onset, sleep latency, the total arousal index, and the apnea-hypopnea index after herbal treatment. In case 2, the patient complained of frequent waking after sleep onset, daytime sleepiness, and snoring, and was diagnosed with obstructive sleep apnea disorder and treated with Yeoldahanso-tang-gami. There were significant changes in the polysomnographic parameters, such as the total arousal index and the apnea-hypopnea index after herbal treatment. In case 3, the patient complained of daytime sleepiness and was diagnosed with obstructive sleep apnea disorder and treated with Bangpungtongsung-san. There was no significant change in the polysomnographic parameters after herbal treatment. Conclusions: It is meaningful to note that objectively identifying the outcome of treatment for breathing-related sleep disorders is necessary in the clinical practice of Korean medicine.

일차성 불면증 환자에서의 주간 졸림증과 연관된 수면변인에 대한 연구 (Relatively Decreased Level of Subjective Daytime Sleepiness and Its Associated Factors in Patients with Primary Insomnia)

  • 목 적:본 연구에서는 일차성 불면증으로 진단받은 환자들과 대조군 사이의 주간 졸림증 정도를 비교하고 불면증 환자에서의 주간 졸림증과 야간 수면다원검사에서 산출한 수면 변인 사이의 상관관계를 평가해 보고자 하였다. 방 법:서울대학교병원 수면 클리닉에서 일차성 불면증으로 진단받은 환자 87명과 88명의 대조군을 대상으로 하였다. 한국판 엡워스 졸림증 척도로 두 군의 주관적인 주간 졸림증을 평가하여 비교하였고 불면증 환자군에서 야간수면다원검사상의 수면변인과 엡워스 졸림증 척도 사이의 상관관계를 평가하였다. 결 과:불면증 환자군(남자 43명, 여자 44명; 평균연령 $50.7{\pm}11.9$)의 엡워스 졸림증 척도가 대조군(남자 43명, 여자 45명; 평균연령 $52.0{\pm}14.0$)에 비해 유의하게 낮았다. 연령을 통제하였을 때, 불면증 환자군의 엡워스 졸림증척도 점수는 수면변인 중 총 수면시간(total sleep time, min), 수면 효율(sleep efficiency%), 2단계 수면 분율(stage 2 sleep time%)과 유의한 음의 상관관계를 보였다(각각 r=-0.232, p=0.034; r=-0.215, p=0.049; r=-0.349, p=0.001). 또한, 수면 중 각성 횟수(number of awakenings), 수면 중 2분 이상 각성한 횟수(number of awakenings>2 mins), 수면 중 각성시간(wake after sleep onset, min)과는 유의한 양의 상관관계를 보였다(각각 r=0.412, p=0.000; r=0.473, p=0.000; r=0.367, p=0.001). 불면증 환자군의 엡워스 졸림증 척도는 수면 잠복기(sleep latency, min), 1단계 수면 분율(stage 1 sleep time%), 서파 수면분율(slow wave sleep time%), 렘수면 분율(REM sleep time%)과 유의한 상관관계를 보이지 않았다. 결 론:본 연구에서 불면증 환자는 대조군에 비해 주간 졸림증 정도가 더 낮았으며, 불면증 환자의 주간 졸림증은 수면의 양적 질적 저하와 연관되어 있었다.

주간졸음증 환자로부터 기면증 1예 보고 (A Case of Narcolepsy by Excessive Daytime Sleepiness)

  • We report a case of narcolepsy. A 25-year-old man has had excessive daytime sleepiness of about 10 years durations. He awakens daily feeling exhausted and continually falls asleep during the day while engaged in such situation like reading and watching television. He has exhibited cataplexy, a sudden loss of muscular tone, brought on by emotion, usually laughter. Polysomnogram revealed increased sleep stage 1, 2 and decreased deep sleep. Multiple sleep latency test (MSLT) showed that sleep latency was 1.33 minutes and there were 3 noted sleep onset rapid eye movement (SOREM) on 5 trials. The epworth sleepiness scale (ESS) was 17/24. Typing of HLA haplotype that was positive for the $DQB1^{\ast}0602$ allele, and hypocretin-1 (orexin A) could not be detected in cerebrospinal fluid (CSF). Brain MRI showed normal image. We diagnosed his case as narcolepsy based on history of cataplexy, and three occurances of SOREM, and positive of HLA haplotype.

수면무호흡증의 진단과 치료 (Diagnosis and Treatment of Sleep Apnea)

  • Sleep apnea syndrome is a common clinical disorder characterized by intermittent cessation of airflow at nose and mouth during sleep. The clinical significance of this syndrome is that it is one of the most common causes of excessive daytime sleepiness. It can also cause neuropsychiatric, cardiovascular, and cerebrovascular complications. The standard for diagnosis of sleep apnea syndrome is nocturnal polysomnography. Because polysomnography is a time-consuming and expensive test, many efforts have been made to replace polysomnography with a simpler system of monitoring, but no method has yet been approved as a definitive investigation method. The goals of treatment for this syndrome are to eliminate excessive daytime sleepiness and to reduce the risk of possible cardiovascular complications. Continuous positive airway pressure is the most definite and widely accepted treatment for achieving these goals. Other treatments such as surgical treatment, oral appliances, and behavioral therapy may be useful for selected patients who are mildly affected.

  • Objectives : The purpose of this study is to report periodic limb movements child in sleep. Methods : We report of 5-year-old male patient with periodic limb movements in sleep. The patient suffered from limb movements in sleep, fatigue in the daytime, nasal discharge, nasal obstruction and intermittent cough. We treated him with Kamibojungikki-tang(加味補中益氣湯) and acupuncture Results : After this treatments, the symptoms(limb movements in sleep, fatigue in the daytime, nasal discharge, nasal obstruction, intermittent cough,) are improved. Conclusion : We had good effects in oriental medical treatment on periodic limb movements in sleep. And the more study about this diseases is needed.

An Evaluation of the Quality of Sleep Before and After Surgical Treatment of Patients with Cervical Disc Herniation

  • Objective : It has been reported in recent studies that 50-80% of patients with cervical disc hernia have concomitant sleep disorders. The aim of this study was to evaluate the quality of sleep before and after surgical treatment in patients with cervical disc hernia and to assess the effects on treatment. Methods : The study included 32 patients performed discectomy and fusion with an intervertebral cage through the right anterior cervical approach. Oswestry Disability Index (ODI), Visual Analog Scale (VAS) and Pittsburgh Sleep Quality Index (PSQI) were applied to all patients preoperatively and at one month postoperatively. Results : The postoperative PSQI total points and all the PSQI subscale points, the ODI and VAS scores were significantly reduced compared to the preoperative values. A positive correlation was determined between the preoperative ODI points and the PSQI total points and sleep duration, sleep latency and daytime functional loss subscale points. A positive correlation was also found between preoperative ODI points and VAS points. A positive correlation was determined between the preoperative VAS points and and the PSQI total points and sleep duration, and sleep latency subscale points. A negative correlation was determined between the postoperative ODI and the daytime functional loss subscale points. Conclusion : The results of the study showed that in patients with cervical disc hernia, sleep quality and daytime functionality were negatively affected by severity of pain that limited daily activities. Bringing the pain under control with surgical treatment was observed to increase sleep quality. It can be concluded that when planning treatment for these patients, it should be taken into consideration that there could be a sleep disorder in addition to the complaints and symptoms such as pain, hypoestesia and loss of strength.

간호 대학생의 실습 전, 후 임상실습 스트레스, 수면의 질 및 주간 졸림증의 차이 (Differences in Clinical Practice Stress, Sleep Quality, and Daytime Sleepiness before and after Nursing Students)

  • 간호 대학생의 실습 전, 후 임상실습 스트레스, 수면의 질, 주간 졸림증 차이와 관계를 확인하는데 목적이 있다. 연구대상은 간호학과 3학년 학생 130명이다. 연구결과 연구 대상자의 임상실습 스트레스는 총점 5점 만점 중 평균점수 3.15점으로 나타났다. 임상실습 스트레스의 외부조정 요인 중 과제물에 대한 점수가 3.64로 가장 높았고, 성별(t=-3.88, p<.001), 주관적 건강상태(F=5.78, p=.004), 거주형태(F=3.20, p=.026)에 유의한 차이가 있는 것으로 나타났다. 간호대학생은 수면의 질이 낮고(실습 전 80.0%, 실습 후 70.0%), 주간 졸림증이 있는 대상이(실습 전 26.9%, 실습 후 42.3%) 일반 대학생에 비해 많음을 확인 할 수 있었다. 임상실습 전, 후 수면의 질은(χ2=4.06, p=.004), 주간 졸림증은(χ2=6.08, p=.009)로 유의한 차이가 있었다. 또한 주간 졸림증과 수면의 질은 음의 상관관계(r=-.259, p<0.01)가 있는 것으로 나타났다. 따라서 간호 대학생들의 신체적 건강 및 정신적 건강을 위하여 임상실습 스트레스를 줄이고 수면의 질을 높일 수 있는 전략 개발이 필요하며, 변화하는 간호 교육현장에서 임상실습 적응에 도움이 되도록 하여 추후 간호의 질을 향상시킬 수 있도록 해야겠다.
