목 적: 수면 문제는 지속적으로 성인의 자살 위험 요인으로 보고되어왔고 최근에는 청소년을 대상으로도 그 가능성이 보고되고 있다. 본 연구는 과거의 자살 행동 유무에 따라 대상군을 분류하여 이들 사이의 자살위험요인과 수면양상을 비교하고 자살위험요인으로써의 수면의 가능성을 평가해 보았다. 방 법: 고등학생 561명(남 271명, 여 290명)을 대상으로 자살위험행동 체크리스트, Center for Epidemiological Study-Depression(CES-D), Symptom Checklist-90-Revision(SCL-90-R) 불안, 공격성 하위척도, Pittsburg Sleep Quality Index(PSQI)가 시행되었다. 결 과: 46명(8.1%)이 자살시도 목적으로 자기 위해 행동을 한 경험이 있었고, 181명(32.4%)은 자살사고 경험만을 보고했다. 334명(59.5%)은 과거 자살위험행동(사고, 시도)을 보이지 않았다. 자살 위험군은 학교 폭력, 물질남용, 인터넷 중독, CES-D, SCL-90-R, PSQI에서 비자살 위험군에 비해 유의하게 점수가 높았다. 로지스틱 회귀분석의 결과 자살위험행동은 우울, 스트레스와 상관성을 보였고, PSQI에서는 수면 잠복기, 주간기능장애에서 상관성을 보였다. 일원분산분석(ANOVA, Analysis of variance)에서는 자살 시도군에서 수면 잠복기의 연장, PSQI 지수의 증가가 가장 심하게 관찰되었다. 결 론: 수면문제는 청소년군의 자살위험행동과 높은 연관성을 보인다. 특히 수면 잠복기, 주간기능장애가 자살위험행동의 중요한 인자일 것으로 여겨진다. 추후 부모, 교사, 의료인들에 의한 청소년의 수면에 대한 지속적인 평가와 관리를 권장하는 바이다.
Narcolepsy is a central neurologic system disease. It begins early in life with disabling symptoms including excessive daytime sleepiness, cataplexy, sleep paralysis, hypnagogic hallucination and nocturnal sleep fragmentation. Patient with typical symptoms of narcolepsy is diagnosed by objective data from nocturnal polysomnography and multiple sleep latency tests. Narcolepsy is controlled with various medications. Nowadays, modafinil with favorable side effects profiles compared with traditional stimulant is mainly used. Gamma hydroxyl butyrate is effective in cataplexy. Cataplexy is also controlled with antidepressant such as Venlafaxine, SSRI, and TCA. As the knowledge of pathophysiology of narcolepsy expands, new treatment including immunological method, application of hypocretin and histamine systems have been tried.
Objective : This case report aimed to demonstrate the effect of Baekhogainsam-tang on hypersomnolence. Methods : A 33-year-old woman complained of hypersomnolence with fatigue and lack of concentration in the daytime. According to the diagnostic system based on Shanghanlun provisions (DPIDS), the patient was treated with Baekhogainsam-tang decoction. The result was evaluated by the Epworth Sleep Scale. Results : After administration of Baekhogainsam-tang decoction for 30 days, the Epworth Sleep Scale score was decreased from 15 (pathologic sleepiness) to 1 (no clinically significant sleepiness). Conclusions : The patient completely recovered from hypersomnolence and fatigue following treatment with Baekhogainsam-tang decoction according to DPIDS.
Purpose: This study aimed to explore the subjective sleep quality of depressed and non-depressed mothers in the late postpartum period and to determine the relationship with their health promoting behaviors, family functioning, parenting stress. Method: A non-probability sample of 128 mothers completed a self-administered questionnaires at 4-6weeks postpartum. The Edinburgh postnatal Depression Scale (EPDS) and Pittsburgh Sleep Quality Index were used to measure mother's experiences of depression symptoms and sleep. Related factors of sleep quality were measured by the Korean Family Functioning Scale, Health Promoting Lifestyle Profile, and Parenting Stress Index. The data was analysed using t-test, one-way ANOVA, and the Pearson's correlation coefficients. Result: The results indicated that the depressed mothers (EPDS$\geq$ 10) had poorer sleep quality than the non-depressed mothers(EPDS < 10), reported shortened sleep duration, and experienced more daytime dysfunctions. Depressed mothers who had no job, did not drink coffee, and were primipara tended to report poorer sleep quality. There were significant correlation between poorer sleep quality and lower health promoting behaviors, higher family intimacy and lower family communication, and higher parenting stress among depressed mothers. Conclusion: Our findings support the view that depressed mothers' experiences of poor sleep are much higher than non-depressed mothers and multi-faced. Nurse professionals should screen for sleep problems in the depressed mothers with a different biopsychosocial and behavioral aspect from the non-depressed mothers in the late postpartum period.
기면병은 과도한 주간 졸림, 탈력발작, 수면 분절화, 입면환각의 특징을 가진 수면 질환이다. 기면병의 증상은 내과적, 신경과적 질환으로부터 생길 수도 있다. 17세의 고등학생 남자 환자가 3개월 전부터 발생한 참을 수 없는 과도한 주간 졸림으로 본원 수면클리닉을 통해 입원하였다. 내원 이후 측정된 체질량지수는 30.4 kg/$m^2$였고 맥박은 분당 70~90회, 혈압은 150/100~120/70 mmHg로 관찰되었다. 갑상선기능 검사에서 T3 391.2 ng/dL(60~181), free T4 4.38 ng/dL(0.89~1.76), TSH(thyroid stimulating hormone) <0.01 ${\mu}IU$/mL(0.35~5.5)로 갑상선 중독증이 시사되었다. 수면다원검사가 실시되었고, 수면 자세의 변환은 시간당 81 회로 매우 많은 편이었다. 입면 잠복기는 33.5분, 수면 효율은 47.9%, 입면에서 렘수면 입면시간은 153.6분으로 지연되었고 렘수면은 27.1%로 증가하였다. 주기성 사지 운동지수는 13.4/h로 나타났다. 수면잠복기반복검사에서 평균 입면잠복시간은 24초, SOREMP(sleep onset REM period)은 3회에서 관찰되었다. actogram상 수면-각성의 경계가 불분명하였고, HLA typing에서 DQB1 $^*0602$는 음성이었다. 환자의 갑상선중독증은 대해 3개월간 methimazole 30 mg, propranolol 40 mg이 투약되며 갑상선 기능이 호전되었다. 탈력발작은 venlafaxine 75 mg으로 조절되었고, 야간 수면 유지와 주기적 사지운동증을 치료 하기 위해 clonaze-pam 0.5 mg이 사용되었고 주관적인 야간 수면의 질은 향상되었다. 과도한 주간 졸림에 대해서는 3개월간 modafinil 200~400 mg이 투여되었고 부분적이긴 하지만 다소의 호전을 보이고 있다. 본 증례는 기면병으로 최종 진단된 환자에 있어 병력과 검사상 발견된 갑상선 중독증, 그리고 수면 효율 감소 등이 과도한 주간 졸림을 평가하는데 혼란 변수로 작용한 경우라 하겠다. 다만, 주간 과다 졸림이 modafinil에 부분적인 효과를 보이는 것, HLA DQB1 $^*0602$ 음성의 결과를 보인 환자에게 나타났던 탈력발작에 대해서도 더 설명이 필요할 것으로 보인다. 향후 CSF hypocretin level을 추가로 측정하고 수면 문제의 추이를 관찰하면서 추가적인 PSG와 MSLT가 필요할 것으로 사료된다.
연구배경 : 본 연구는 정상 단태 임부에서 임신 경과에 따른 수면양상의 변화 및 그 원인을 알아보고자 하였다. 방법 : 임신 제1 삼분기에 수면설문지를 작성한 임부를 대상으로, 임신 제2 및 제3 삼분기에도 동일한 수면 설문지를 가지고 추적조사를 하였다. 조사내용은 각 임신 시기에서의 수면 잠복기, 총 수면시간, 수면중 깨는 횟수, 각성시 회복감, 주간의 상태, 수면 자세, 수면 양상변화의 원인 등이었다. 결과 : 다음은 총 3회의 설문조사를 성실히 수행했던 26명의 임부에 대한자료분석의 결과이다. 1. 각 임신 시기와 임신전과의 비교 결과, 수면 잠복기는 임신 제1 및 제2 삼분기에서 감소하였고, 총 수면시간은 임신 제1, 제2 및 제3 삼분기에서 모두 증가하였다. 임신 제3 삼분기에는 수면 중 깨는 횟수가 증가하였고, 각성시 회복감이 감소하였다. 임신 제1, 제2 및 제3 삼분기 모두에서 주간에 피곤감과 졸리움이 증가하였다. 2. 각 임신 시기간의 비교 결과, 임신 제3 삼분기에는 수면 잠복기 및 수면중 깨는 횟수가 임신 제1 및 제2 삼분기보다 증가하였다. 또한 임신 제3 삼분 기에는 제1 및 제2 삼분기보다 각성시 회복감이 더 적었고, 주간의 졸리움도 더 심하였다. 3. 수면 자세는 임신 제1 및 제2 삼분기에 복와위 또는 앙와위를 취했던 임부도 제3 삼분기에는 모든 임부가 측와위를 취했다. 4. 수면양상변화의 원인으로서 임신제1 삼분기에는 오심과 구토, 속쓰림과 소화불량 및 빈뇨, 제2 삼분기에는 빈뇨, 태아의 움직임 및 둔부의 통증, 제3 삼분기에서는 빈뇨, 태아의 움직임, 하지의 경련과 요통순으로 많았다. 결론 : 이상의 결과는 정상 단태임신과 관련한 수면 변화의 자료로서, 향후 임부에서 수면위생의 향상과 수면장애의 치료를 위한 기초적 자료로 활용할 가치가 있다고 생각된다.
Purpose: The purpose of this study was to compare the quality of sleep with the serum lipid profile in patients who have restless legs syndrome (RLS). Methods: The data were obtained from 116 patients with RLS through questionnaires and blood sampling. Results: The results of this study showed correlations between lower quality of sleep and serum lipid profile (LDL Cholesterol) in patients with RLS (r=.19, p=.040). There were correlations for scores of quality of sleep from the, Pittsburgh Sleep Quality Index (PSQI) sub-region between lower subjective sleep quality and serum lipid profile (LDL Cholesterol) (r=.20, p=.026), between fewer hours of sleep duration and serum lipid profile (Total Cholesterol) (r=-.21, p=.024), and, between higher daytime dysfunction and serum lipid profile (LDL Cholesterol) (r=.42, p<.001) of patients with RLS. Conclusion: Pati-ents with RLS have sleep disorders with lower quality of sleep and changes in the serum lipid profile for total cholesterol and LDL cholesterol. That is, patients with RLS have lower quality of sleep and dyslipidemia compared to persons without RLS. Further research is needed to monitor serum the lipid profile in early stage symptoms of midlife adult patients with RLS and especially older women.
Purpose: The purpose of this study was to identify the quality of sleep and its influencing factors on the institutionalized elderly. Methods: The respondents were 145 institutionalized elderly in Daegu and a structured questionnaire was used for data collection, which was conducted from February 10 to 28, 2007. The questionnaire was the Yi(2004) quality of sleep scale. All the information was collected through face-to-face interviews, using the questionnaire. The data were analyzed by SPSS/WIN 12.0. Results: The results of this research were as follows: First, the mean for the quality of sleep in the institutionalized elderly was 83.92. Second, in the subscales of the quality of sleep scale, the mean for daytime dysfunction was 40.35; for restoration after sleep it was 10.43; for difficulty in falling asleep it was 11.00; for difficulty in getting up it was 8.88; for satisfaction with sleep it was 8.62; for difficulty in maintaining sleep it was 4.75. Third, the factors influencing the quality of sleep were economic status, perceived health status, and pain. Conclusion: Based upon the findings above, this study provides useful information that could assist in improving the quality of sleep among institutionalized elderly, and indicates that nursing intervention is needed.
Sleep is an essential process maintaining the life cycle of the human. In parallel with physiological, cognitive, subjective, and behavioral changes that take place during the sleep, there are remarkable changes in the electroencephalogram (EEG) that reflect the underlying electro-physiological activity of the brain. However, analyzing EEG and relating the results to clinical observations is often very hard due to the complexity and a huge data amount. In this article, I introduce several linear and non-linear tools, developed to analyze a huge time series data in many scientific researches, and apply them to EEG to characterize various sleep states. In particular, the spectral analysis, detrended fluctuation analysis (DFA), and synchrony analysis are administered to EEG recorded during nocturnal polysomnography (NPSG) processes and daytime multiple sleep latency tests (MSLT). I report that 1) sleep stages could be differentiated by the spectral analysis and the DFA ; 2) the gradual transition from Wake to Sleep during the sleep onset could be illustrated by the spectral analysis and the DFA ; 3) electrophysiological properties of narcolepsy could be characterized by the DFA ; 4) hypnic jerks (sleep starts) could be quantified by the synchrony analysis.
Obstructive sleep apnea (OSA) has been definitively shown to be a risk factor for the development of cardiovascular disease and mortality. Recent reports have indicated that obstructive sleep apnea is associated with insulin resistance and impaired glucose metabolism, also have type 2 diabetes. The potential mechanisms leading to the development of type 2 diabetes in OSA patients are likely to be various. Reduced physical activity resulting from daytime somnolence, sympathetic nervous system activation, intermittent hypoxia, sleep fragmentation and sleep loss, dysregulation of the hypothalamic-pituitary axis, alteration in adipokine profiles, and activation of inflammatory pathways have been proposed. Based on the current evidence, clinicians should assess the risk of OSA in patients with type 2 diabetes and, conversely, consider that possibility of glucose intolerance in patients with OSA. Further large-scale and long-term follow-up studies in patient populations with selected by reliable but inexpensive diagnostic measures, controlled for potential confounder factor, are needed.
