Clinical outcome of conservative treatment of injured inferior alveolar nerve during dental implant placement

  • Objectives: Infererior alveolar nerve (IAN) damage may be one of the distressing complications occurring during implant placement. Because of nature of closed injury, a large proportion is approached non-invasively. The purpose of this study was to analyze the outcomes of conservative management of the injured nerve during dental implant procedure. Materials and Methods: Sixty-four patients of implant related IAN injury, who were managed by medication or observation from January 1997 to March 2007 at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital, were retrospectively investigated. The objective tests and subjective evaluations were performed to evaluate the degree of damage and duration of sensory disturbance recovery. Tests were performed on the day of the first visit and every two months afterward. Patient's initial symptoms, proximity of the implant to the IAN, time interval between implant surgery and the first visit to our clinic, and treatment after implant surgery were analyzed to determine whether these factors affected the final outcomes. Results: Among the 64 patients, 23 had a chief complaint of sensory disturbance and others with dysesthesia. The mean time until first visit to our hospital after the injury was 10.9 months.One year after nerve injury, the sensation was improved in 9 patients, whereas not improved in 38 patients, even 4 patients experienced deterioration. Better prognosis was observed in the group of patients with early visits and with implants placed or managed not too close to the IAN. Conclusion: Nearly 70% of patients with IAN injury during implant placement showed no improvement in sensation or dysesthesia with the conservative management. Earlier decision for active treatment needs to be considered because of possibility of deterioration of symptoms and unsatisfactory recovery.

감마선을 조사한 마우스의 세포 손상과 산화적 스트레스에 대한 나린진과 나린제닌의 방사선방호 효과 (Radioprotective effect of naringin and naringenin against cellular damage and oxidative stress of γ-irradiated mice)

  • 본 연구는 in vitro에서 나린진과 나린제닌의 항산화 활성을 평가하고, in vivo에서 BALB/c 마우스에 나린진과 나린제닌을 7일 동안 경구투여 한 후 6Gy의 감마선을 조사시켜 24시간 뒤 감마선 조사로 인한 세포 손상 및 산화적 스트레스에 대한 방사선 예방효과를 검토하였다. 나린진과 나린제닌의 항산화 활성평가에서 나린제닌이 나린진보다 ABTS 제거 활성 및 FRAP을 유의적으로 증가시켜 아글리콘 형태인 나린제닌이 글리코사이드 형태의 나린진보다 항산화 능력이 뛰어난 것을 확인하였다. 감마선 조사 전 나린제닌을 투여 한 군이 감마선 조사군에 비해 비장 지수, 흉선 지수 및 백혈구 수치가 증가하여 조혈 면역계 손상에 대해 보호효과가 있음을 확인하였다. 나린제닌을 투여 후 감마선을 조사한 군의 ALT와 AST가 감마선 조사군에 비해 유의적으로 감소하여 간세포 손상에 대한 예방효과를 확인하였다. 감마선 조사 전에 나린제닌을 투여한 군의 지방질과산화는 감마선 조사군에 비해 유의적으로 낮게 나타남을 보였으며, XO는 감마선 조사군에 비해 유의적으로 감소하여 방사선에 의한 장해를 감소시킨 것으로 판단할 수 있다. 감마선 조사 전 나린제닌을 투여한 군의 GSH와 항산화 효소의 활성은 감마선 조사군에 비해 유의적으로 증가하여 생체 내에 항산화 활성을 회복시켰다. 이 상의 결과를 통해 방사선 조사 전에 나린제닌의 투여는 방사선에 의한 세포 손상과 산화적 스트레스에 대해 보호 효과가 있어 방사선보호제로 유용하게 사용될 수 있다.

Ethofenprox 처리시기별 콩꼬투리혹파리의 방제효과 (Control Efficacy of Ethofenprox against Soybean Pod Gall Midge, Asphondylia yushimai (Diptera: Cecidomyiidae) at Different Spray Time)

  • 국내 최초로 밀양에서 콩꼬투리혹파리의 발생시기, 콩꼬투리 피해율 및 ethofenprox 20EC의 방제효과를 조사하였다. 콩꼬투리혹파리에 의한 콩꼬투리의 전형적인 피해증상은 피해부위의 갈변 및 기형화로 나타난다. 콩 포장에서 타락법에 의해 조사된 콩꼬투리혹파리 성충은 콩의 개화시기인 7월 5일부터 발생하여 9월 중순까지 그 밀도가 증가하였다. 파종기에 따른 콩꼬투리혹파리에 의한 콩꼬투리의 피해협률은 5월 10일 파종에서 가장 심하였으며, 다음은 6월 11일 및 5월 26일 파종 순 이었다. 콩 생식생장기에 ethofenprox 20EC 1회 살포에 따른 콩꼬투리혹파리의 방제효과는 R2.5에서 92.9%로 가장 높았으며, 다음은 R2에서 87.9%, R3에서 83.6%이었으며, 나머지 처리시기에서는 약 19~66%로 낮았다. 따라서 콩에서 콩꼬투리혹파리의 효과적인 약제방제 시기는 개화성기와 착협시 사이인 R2.5로 여겨진다.

피복처리가 월동배추 동해 발생에 미치는 영향 (Influence of covering treatment on the incidence of frost injury in chinese cabbage during winter season)

  • The average temperatures for year and winter season have been risen by $0.7{\circ}C$ and $1.4{\circ}C$, respectively, during the last 30 years. Recently abnormal climate phenomena occurred frequently results in severe loss of vegetable crops grown in Korea. Specially, Chinese cabbages grown in the southern area of Korea are often significantly affected by sudden cold waves during winter season before harvest. This experiment was conducted to find out a potential role of covering materials on the protection of frost damage of 'Bularm' chinese cabbage in the winter season. The lowest temperature was $-15.8^{\circ}C$ in non-covering, $-8.1^{\circ}C$ in the PE film covering and $-4.6^{\circ}C$ in the non-woven fabric covering with PE film, respectively. The cumulative times below $4.0^{\circ}C$ were 145.5 hours for the non-covering treatment, 94 hours in the PE film covering and 14.5 hours in the non-woven fabric covering with PE film, respectively. The symptoms of frost damage were severe at non-covering chinese cabbages compared to polyethylene film (PE) non-woven fabric with PE covering ones. Microscopic studies showed the normal anatomical structure of palisade and spongy tissue of cabbage leaves covered with non-woven fabric with PE film. Leaf cells, however, were slightly damaged in cabbages covered with PE film alone, and both palisade and spongy cells were were completely collapsed in uncovered cabbages. The result of this study suggests that chinese cabbages is required to be covered with non-woven fabric with PE film to minimize the frost damage by sudden cold wave below $-7^{\circ}C$.

디지털트윈 시스템 적용을 위한 공동구 복합재난 시나리오 구축 (Establishment of Complex Disaster Scenario on the Utility Tunnel Study for Digital Twin System Application)

  • 연구목적: 본 연구에서는 공동구에서 발생 가능한 다양한 재난 상황을 종합적으로 고려할 수 있는 복합재난 시나리오를 구축하는 데 목적을 두었다. 연구방법: 재난 간 상관관계를 포괄적으로 고려하기 위해 재난 원인요소와 피해요소의 조합으로 복합재난 시나리오를 도출했고, 위험성 평가 기반의 시나리오 우선순위를 도출했다. 연구결과: 공동구 재난사례를 기반으로 피해 중심의 복합재난 시나리오 방안을 마련하였다. 화재, 침수, 지진 등 단일재난요소 및 다중재난요소들을 준정량 평가 방법을 활용하여 복합재난 시나리오를 구체화했다. 결론: 본 연구에서 도출된 복합재난 시나리오는 재난의 전조증상이 감지된다면 피해 예방·대비에 활용될 수 있다. 또한, 본 연구에서의 결과물은 긴급 재난 발생 시 신속한 대응·복구를 유도할 수 있도록 전략 방안 마련과 복합재난 대응 기술 마련의 기초자료로써 활용이 기대된다.

All about pain pharmacology: what pain physicians should know

  • From the perspective of the definition of pain, pain can be divided into emotional and sensory components, which originate from potential and actual tissue damage, respectively. The pharmacologic treatment of the emotional pain component includes antianxiety drugs, antidepressants, and antipsychotics. The anti-anxiety drugs have anti-anxious, sedative, and somnolent effects. The antipsychotics are effective in patients with positive symptoms of psychosis. On the other hand, the sensory pain component can be divided into nociceptive and neuropathic pain. Non-steroidal anti-inflammatory drugs (NSAIDs) and opioids are usually applied for somatic and visceral nociceptive pain, respectively; anticonvulsants and antidepressants are administered for the treatment of neuropathic pain with positive and negative symptoms, respectively. The NSAIDs, which inhibit the cyclo-oxygenase pathway, exhibit anti-inflammatory, antipyretic, and analgesic effects; however, they have a therapeutic ceiling. The adverse reactions (ADRs) of the NSAIDs include gastrointestinal problems, generalized edema, and increased bleeding tendency. The opioids, which bind to the opioid receptors, present an analgesic effect only, without anti-inflammatory, antipyretic, or ceiling effects. The ADRs of the opioids start from itching and nausea/vomiting to cardiovascular and respiratory depression, as well as constipation. The anticonvulsants include carbamazepine, related to sodium channel blockade, and gabapentin and pregabalin, related to calcium blockade. The antidepressants show their analgesic actions mainly through inhibiting the reuptake of serotonin or norepinephrine. Most drugs, except NSAIDs, need an updose titration period. The principle of polypharmacy for analgesia in case of mixed components of pain is increasing therapeutic effects while reducing ADRs, based on the origin of the pain.

감마나이프 방사선수술 후 발생한 급속 순환성 이차성 조증 1예 (A Case of Rapid Cycling Secondary Mania after Gamma-knife Radiosurgery)

  • A case of rapid cycling mania secondary to gamma-knife radiosurgery for the treatment of refractory epilepsy was reported. A 21-year old woman who had a gamma-knife radiosurgical operation for the treatment of refractory seizure two years ago was admitted because of manic episodes. Although seizure was relieved, manic symptoms like decreased need for sleep, elated mood, unprovoked laughing, grandiose delusion and bizarre behaviors were developed 11 months after the operation. These symptoms recurred lour rimes for eight months. There were no past personal and family history of mood disorders. Laboratory examinations including electroencephalogram ana endocrinological study did not show any abnormal findings. The rapid cycling secondary manic was relieved by lithium. She was then discharged after 5 months. Mood change was not significant during follow-up while hypomania emerged by dose reduction. The secondary mania seemed to be caused or triggered by the right temporal lobe damage induced by gamma-knife radiosurgery.

실음(失音)의 병인(病因) 병기(病機)에 대(對)한 문헌적(文獻的) 고찰(考察) (A Literatual Study on the Dysphonia)

  • In the Literatual Study on the Dysphonia, the results were as follows. 1. The causes of dysphonia are exogenous pathogenic factors,(specially cold evil)internal damage and meridian in The Yellow Emperor's Canon of Interal Medicine, since then endogenous pathogenic factors are lung-asthenia and deficiency of lung-yin etc. The main causes are disease caused by exogenous evils, general body weakness, emotional stimulation and excess of high voice rescently. 2. The pathogenesis of dysphonia originated from two factors; The first internal damages are consumption of body fluid with the formation of dryness evil resulting from the insufficienty of lung-yin and lung-collaterals damaged by heat-evil caused by deficiency of lung and kidney-yin. The second disease caused by exogenous evils is sluggishness of lung-energy caused by exogenous pathogenic factors. 3. The main relative organ are heart, lung and kidney etc. 4. The prescriptions of wind-cold symptoms are Samyoutang(三拗湯) and Hangsosan(杏蘇散), in the prescriptions of phlegm-heat symptom is Chenginyongphetang(淸咽寧肺湯), in the prescriptions of depressive syndrome due to disorder of vital energy are Sogangkitang(小降氣湯) and Shihochenggantang(柴胡淸肝湯加減), in the prescriptions of consumption of body fluid with the formation of dryness evil resulting from the insufficiency of lung-yin symptoms are Sanghangtang(桑杏湯) and Chenginguphetang(淸咽救肺湯, in the prescriptions of deficiency of lung and kidney-yin are Baekhabgokumtang(百合固金湯) and Maekmigiwhangtang(麥味地黃湯). 5. The treatment of acupunctures are used by LI-4(合谷), H-7(湧泉), Liv-3(太衝), K-3(太谿), Sp-6(三陰交), H-5(通里), GV-15(아門), CV-23(廉泉), S-40(農隆), K-6(照海), L-7(列缺), S-36(足三里) etc.

  • 고추, 들깨, 가지 등 3개 작물에 pH 2.0, 2.7, 3.0 및 6.0(대조구)의 인공산성비를 10mm씩 2일 간격으로 10회 처리한 후 잎의 피해율과 조직의 변화 그리고 효소 활성의 변화를 살펴본 결과를 요약하면 다음과 같다. 1. 산성비 처리로 작물에 나타난 가시적피해는 고추>들깨>가지의 순 이었으며, 작물의 산성비에 대한 일반적인 피해증상은 잎표면에 나타나는 갈색 또는 백색의 반점이다. 2. pH 3.0의 인공산성비를 처리하여 작물의 잎표면을 전자현미경(SEM)으로 관찰한 결과, 가지를 제외한 고추와 들깨 잎표면의 모용(Trichome)이 변형되었다. 3. pH 2.7 인공산성비 처리로 기공(Stoamta)의 피해가 나타났다. 4. 산성비 처리에 따른 작물의 생리적 특성 변화에서 Peroxidase 활성의 차이를 보였으며, 들깨에서는 1회 처리로 급격한 활성증가를 보였다.

  • Amyotrophic lateral sclerosis (ALS) is a progressive disorder that causes degeneration of motor neurons of the brain and spinal cord. ALS is a progressive, fatal neuromuscular disease characterized by loss of motor neurons leading to muscle weakness. Sensation and mental function stay intact during the course of the disease. ALS is characterized by both upper and lower motor neuron damage. Diagnosis includes magnetic response imaging (MRI) electromyogram (EMG), muscle biopsy, and blood test. There is no cure for ALS. We recently observed three cases of ALS. The patients were diagnosis with ALS by EMG and symptoms. This report was conducted to evaluate how oriental medical treatment can affect ALS. We report the change of their symptoms through oriental medical treatment compared with taking riluzole.
