Ear Acupuncture Treatment for Disaster-Related Trauma: Introduction of the National Acupuncture Detoxification Association (NADA) Protocol (재난트라우마에 대한 이침의 활용 - NADA 프로토콜의 소개)

  • The number of disasters caused by natural or human-made events such as earthquakes, tsunamis, floods, typhoons, industrial disasters, and the recent outbreak of COVID-19 has increased and the damage caused by such disasters is increasing every year. Disasters can cause physical destruction and also have unexpected psychological impacts, such as post-traumatic stress disorder (PTSD). Ear acupuncture is a treatment modality that can be helpful for both physical and mental health problems. Since ear acupuncture is a cost-effective, flexible, and safe treatment tool, it has the potential to provide medical assistance in disasters. The National Acupuncture Detoxification Association (NADA) auricular acupuncture protocol, originally developed for addiction treatment, has been used for mental illness and behavioral symptoms in both community health settings and in a variety of disaster, terror, and refugee settings. In this review, we introduced the NADA protocol, including its history, features, training, clinical evidence, and cases in disaster settings, suggesting that the NADA protocol could be utilized as a disaster medical support model in Korean medicine.

Gliotoxin Protects Trinitrobenzene Sulfonic Acid-Induced Colonic Damage through Induction of Heme Oxygenase-1

  • Background: Crohn's disease is characterized by a chronic relapsing inflammation of the bowel. Gliotoxin has been known to play strong immunosuppressive properties, while mechanisms for its anti-inflammatory actions are not completely understood. Here, we investigated the effects of gliotoxin in trinitrobenzene sulfonic acid (TNBS) induced mouse colitis, an animal model of Crohn's disease. Results: Gliotoxin dramatically improved clinical and histopathological symptoms in accompanied with reduced expression of TNF-$\alpha$, IL-1$\beta$, and ICAM-1 protein levels in TNBS induced colitis. Interestingly Gliotoxin induced Heme oxygenase-1 (HO-1) and the HO-1 inducer cobalt protoporphyrin IX (CoPPIX) completely mimicked the protective effects of gliotoxin in TNBS induced colitis mice. In contrast, the HO-1 inhibitor zinc protoporphyrin IX (ZnPPIX) could reverse the anti-inflammatory effects of gliotoxin and CoPPIX. Conclusions: Gliotoxin is a potential therapeutic agent targeting for the treatment of Crohn's disease by inducing HO-1.

The Early Detection of Recurrence of Malignant Peripheral Nerve Sheath Tumor by Frequent Magnetic Resonance Imaging

  • Surgery has a key role in the treatment of malignant peripheral nerve sheath tumors (MPNSTs), but the resectability of paraspinal MPNSTs is only 20%. Therefore, spinal MPNSTs show frequent recurrence and poor prognosis. Local recurrence is much more common than metastasis for MPNSTs, and surgery still has a key role in the treatment of local recurrence. Therefore, it is important that recurrence must be detected before resectability is lost. However, no evidence-based follow-up protocol has been established for MPNST. The authors performed gross total resection in a 34-year-old woman presented with thoracic MPNST. Adjuvant radiotherapy and chemotherapy were not administered since these adjuvant therapies generally do not improve survival in MPNST and may cause additional neurovascular damage. Instead, the authors monitored the primary site every 3 months using magnetic resonance imaging to detect local recurrence at the earliest opportunity. The tumor recurred locally on two occasions without overt symptoms at 21 and 24 months postoperatively. These recurrences were treated successfully by gross total removal.

Effects of Ammonia on the Sulfur Dioxide Injury in Plants (식물의 아황산 가스 피해에 대한 암모니아 가스의 영향)

  • The experiments were conducted to examine the injuries of $SO_2$, $NH_3$, and $SO_2$ and NH3 mixed gas to the germination and the growth of plants. Six kinds of plants were used as the material. These plants seeds were treated with the gases for five days. Rate of the germination and the growth in height of varying plants were different according to the components of the gases. The critical concentration of the gases for both the germination and the growth were in 5ppm of $SO_2$, 50ppm of $NH_3$, and 50ppm $SO_2$ and 50ppm $NH_3$ mixed gases. When a low concentration of $SO_2$ was treated together with NH3, especially it was reduced to 60 percent of the damage in the germination and the growth. In the treatemnt with $SO_2$, the germination of the seeds which soaked in water for 24 hours reduced the injuries more 40 percent than those which for one hour. It was observed that the seeds with thick coasts or with originally intact coats were suffered but little damaged by the gases, and the external symptoms of an injury were shown, at first, in water pore, and then, in guard cell on the leaves.

Surgical treatment of chronic constrictive pericarditis: Report of 23 cases (만성 교약성 심낭염 (23례 수술 보고))

  • In the 10 years from 1958 through 1968, 23 patients have undergone 24 times of operation for constrictive pericarditis at this department. Follow-up data were available for periods varying several months to almost 10 years from examination at this department or follow-up letters. There were 21 males and 2 females in this series. Range of the age varied from 2 years to 53 years. Seven cases were below 15 years of age. There were two hospital death, one expired two weeks and another four weeks after the pericardiectomy. In both of them, myocardial damage by disease process seemed to be major contributing factors. Clinical and histological study showed tuberculous origin in 12 cases, non-specific chronic infiammatory changes in 8 cases and in 3 cases previous pyogenic pericardial infection by staphylococcus preceded to the constriction. In 11 cases bone tissue was noted microscopically in the pericardium: Sixteen patients[70 %]had pleural effusions, five cases had bilateral, 6 right, and 5 left. Calcification was seen along cardiac border in 9 cases. In 15 catheterized patients, 6 showed pressure gradients between vena cava and right atrium, ranging from 6 mmHg to 10 mmHg. One case was reoperated 3 months after the original operation resulting in marked improvement. In this reoperated case the original pericardiectomy seemed to be incomplete. In 21 cases. excluding 2 hospital death, symptoms and physical findings improved markedly in every patient

Influence of Herbicide Quinclorac Application on Growth of Tobacco (Quinclorac제제 제초제가 연초생육에 미치는 영향)

  • This study was aimed at finding out the effect of herbicide quinclorac (3, 7-dichloro-8- quinoline carbxylic acid) on tobacco culture in paddy soil. Herbicides quinclorac(Pozol, Pulta and Tomata) was randomly applied to subdivided paddy soil in Sep. 1992, and tobacco seedlings were transplanted to the field when seedling averaged 2cm in height in April 1993. Experiment plots were divided into non - treatment, standard(3kg/10a) and two times(6kg/10a) in amounts for each three herbicides. The symptoms of damage from herbicide were sighted when the plants had developed 8-9 leaves at 30 day after transplanting in standard amount plots, and when the plants had developed 4-5 leaves at 20 days after transplanting in two times treatments. The new leaves from damaged plants were bended out, and leaf color changed from green to dark - green, and then gradually advanced to abnormal narrow leaves. Standard treatments of herbicides showed a decrease of 18% in price per kg, 18% in yield and 33% in value per 10a than those of non - treatment, while two times treatments showed a decrease of 33%, 29%, 52% compared with non - treatment, respectively.

  • Amiodarone is an effective antiarrhythmic agent because of its vasodilator actions. Nowadays it is mostly used to treat patients with severe cardiomyopathy or coronary artery disease complicated by disturbances in the supraventricular or ventricular rhythm. But, some doctors are reluctant to prescribe it because of its many side effects. These include impairment of liver and thyroid fuction and, rarely, damage to the lungs. Most of all, its most serious side effect is amiodarone-induced pulmonary toxicity, which can occur in up to 10% of patients, with mortality rates as high as 50%. We recently experienced one case of the patient with the Amiodarone-induced pulmonary toxicity. The clinical manifestations of the patient was cough, painful breathing, fever, presence of rales, decreased breath sounds, and sputum. We report the change of the patient's symptoms through both western medical treatment and oriental medical treatment.

  • Guillain-Barre Syndrome is a disorder caused by nerve inflammation. The inflammation damages portions of the nerve cells, resulting in pain, numbness, muscle weakness or paralysis and sensory loss. The damage can also leads to denervation (killing the axon part of the nerve cell), which stops nerve function entirely. Without the axon, messages cannot be transferred from one nerve cell to another, but the causes and mechanism of this syndrome are unknown. This is a clinical report about two patients diagnosed with Guillain-Barre Syndrome. The patients, a 54-year-old woman and a 37-years-old man, had pain, and weakness in both legs and arms. After about 4 weeks of Korean medicine and acupuncture treatment, most of their symptoms improved. Therefore, Korean traditional therapy has potential for treatment of Guillain-Barre Syndrome.

  • Penetrating neck trauma by gunshot injury involving tracheobronchial tree is rare in Korea. Extensive tissue damage by cavitation, tissue fragmentation and shock wave transmission of high-velocity projectile along with associated organ injury renders high rate of mortality and morbidity. A 28 year old man in military service with gunshot wound in left cervical area presented initial symptoms of severe dyspnea and subcutaneous emphysema. Computed tomography of chest and cervical region as well as bronchoscopic evaluation was performed to confirm highly suspected injury to cervical trachea. Surgical exposure was established through a low collar incision; the damaged segment of 3.5 cm length including 2-4th tracheal rings was resected out and end-to-end anastomosis was performed. Bleeding from lacerated anterior jugular vein was controlled by ligation of both ends and a K2 bulllet was found upon inner border of body of first rib, medial to right carotid sheath and removed out. Cervical esophagus, carotid artery, internal jugular vein and recurrent laryngeal nerve were spared. Extubation was done on the first postoperative day and postoperative course until discharge on nineth postoperative day remained uneventful.

  • Musculoskeletal disorders show symptoms of pain and dysfunction caused by accumulation of minute damage on nerves and blood vessels which are related to specific body parts. This is more likely to occur in the elderly. In Korea, 40.3% of the agricultural population is over 65 years old. In these cases, the portion of musculoskeletal disorders is more than 80% in acute/chronic diseases related to agricultural work. Therefore, measures for physical health care and safety prevention of elderly farmers are needed. In this study, we intend to propose a method to measure musculoskeletal health condition through the motion recognition device and VR contents. For this, we analyze the agricultural work causing musculoskeletal disorders of farmers, and suggest a motion analysis algorithm that can measure the health condition of the musculoskeletal system. In addition, we are going to propose a training method customized for farmers on the basis of VR contents in order to prevent musculoskeletal disorders.
