대학보건소의 현황과 활성화 방안 연구 - 서울시 여자대학을 중심으로 - (Development of Activation Program through the Evaluation of University Health Center - on the women's university in Seoul -)

  • The purpose of this research is to plan the activation of university health business through the appraisal of the function and role of university health centers. To be more specific: Firstly, analyze the facility, personnel, health service of the health centers, Secondly, appraise the role and function of the health centers, and Thirdly, consider the activation plans of health centers. The research method taken was the study of surveys conducted and written research materials on 5 women's universities in Seoul. These materials where descriptively analyzed by converting the surveys, facility and usage of health center and current personnel status in to percentage. The major results of the research are as follows: Unlike as stated in each university's additional clause on the number of personnel were working. This lack of specialized personnel resulted into lack of specialized and various services. Medicine for external application were stocked well whereas only simple medicine for internal application were on shelf and were issued only with a doctor's prescription. Universities with a full-time doctor had various available equipments. One university conducting dental treatment was equipped with indirect chest camera, dental X-ray, unit, chair(dental treatment chair) and even supersonic, electrocardiogram were available. In the case of D women's university, the number of beds compared to the number of students was lower than that of the other 3 universities, to the total size of the university was smaller than that of the other 3 universities. Among health prevention and care matters, health consultation was the only matter practised by all universities. Uniquely, there was one university that hosted epidemic prevention business. There are various tests given by each university, with each showing many differences, but some universities did not even conduct these tests. Vaccinations were usually being conducted through commission. All universities provided basic treatment, therefore matters concerning treatment at health centers were being conducted well. Concerning the management of equipment and documents, all the matters were being conducted except one university where instead of a student medical record, they were using a daily record. Because these were women's universities, most of the educations were on women's health. The subjects of these educations included: sex, contraception, prevention and control of tuberculosis, obesity, mouth hygiene, alcohol, geriatric disease, mental health and first-aid. The rate of health center usage is growing. Being women's universities, the service and treatment practised were mostly concerning sex education.

보육시설 영유아의 건강사정을 위한 기록지 개발 (A Study on the Development of Children's Health Assessment Tools in Child Care Centers)

  • Purpose: The purpose of this study was to develop child's health assessment tools for children in child care centers. Methods: Based on the literature review and several types of workshops participated with the child health nursing professors, doctoral students, nurses in pediatric units and pediatric psychiatric units from July to December 2006, the standards of child health assessment tools were developed. Graduate students and 4th grade students in nursing were trained for health assessment and used these assessment tools to validate the content and reliability of said tools. Results: Some record forms were newly developed, including demographic characteristics, past history, present health status, behavioral problems, and appropriate developmental screening tests in child care centers were selected. For systematic health care management in child care centers, daily care report, illness log, and referral sheet were also developed. Conclusion: In the face of growing utilization of daycare and their association with increased risk of various diseases, assessment and control measures are indispensable to the promotion of child health. Children's physical and mental health and developmental problems can be assessed using this assessment tools. They can be used for establishing the direction for developing a health care service system for young children.

수분 섭취 및 배설량의 측정방법에 관한 연구 (A Study on Fluid Intake and Output Measurements)

  • The Fluid and electrolytes balance in the body is of critical importance in maintaining good health. When the fluid and electrolyte imbalance is present, patients are in great danger. They must be assessed immediately by a nurse so that appropriate treatment can be started as soon as possible. Patients' fluid intake and output records contain highly important information for the diagnosis and treatment of fluid imbalance, but, these records are often inaccurate and the method of recording the fluid intake is not universal for every hospital. Be-cause they are few quantitative measurements of a patient's hydration, the need to improve the accuracy of fluid intake records is very important. However, very few studies have been done to investigate the accuracy of measurements of patients' fluid intake and output. The purpose of this study was to investigate the methods used for calculation of fluid intake which is most similar to fluid output in normal adults and hospitalized patients. This study focused on three different calculation methods for fluid intake and compared these to fluid output and developed suggestions as to the ideal way to record fluid in-take. Data for 43 hospitalized patients and 37 normal adults were analyzed. The findings of this study are as follows ; 1) In normal adults, the daily intake of water which enteres by the oral route was 2415m1 (the first method of calculation). The daily intake of water in the form of pure water or some other beverage was 1365m1 (the third method of calculation) The daily intake of water including fresh fruits and vegetables, rice, porridges, and Me m which have water content more than 80% were 2186m1 (the second method of calculation). 2) The urine output of the normal adults was 1350m1. This apprroximates the amount of fluid an adult takes in the form of pure water. 3) In patient group, the total intake of water was 2550m1 (the first method of calculation). The in-take of water in the form of pure water or as some other beverage and IV fluid was 1661m1 (the third method of calculation). The daily in-take of water including foods which have high water content was 2356m1 (the second method of calculation). 4) The urine output of the patient's group was 1728m1. This approximates the amount of fluid an adult takes in the form of pure water. 5) Investigation of the method of calculation of the patient fluid intake showed that among the 31 hospitals studied, only eight use the third method of calculation which reflects the most close value to urine output. From the results obtained in this study, it was indicated that the amount of fluid taken in the form of pure water reflects the most close value to urine output. Therefore, it can be suggested that the third method of calculation which includes water in-take only in the form of pure water or beverage should be used as patients' fluid intake record.

우리나라 농촌과 서울 아동의 Na 섭취 및 짠맛에 대한 기호도 비교 연구 (Sodium Intake and preference for Salty Taste in elementary School Children Residing in Rural and Urban Areas of Korea)

  • To compare Na intake and excretion in elementary school children in rural and urban areas of Korea, dietary intake record, urinary Na and K excretion and preference for salty taste were measured in 39 children in 4th grade in Seoul and 36 children in 4th and 5the grades in Kochang-gun)farming area) and their mothers. mean daily urinary Na excretions were 48.8mEq in children and 62.3mEq in mothers. Mothers showed significantly higher preference for salty taste compared to children(X2=16.09, p<0.01). Among the children, those in Seoul had higher preference for salty taste than those in Kochang-gun(X2=8.32, p<0.05). Since the children showed significantly lower preference for salty taste than mothers, more researches and efforts should be directed to prevent the shifts of the taste of the children in order to decrease their Na intake and the risk of hypertension later in life.

한국형 재활환자분류체계 버전 1.0 개발 (The Development of Korean Rehabilitation Patient Group Version 1.0)

  • Background: Rehabilitations in subacute phase are different from acute treatments regarding the characteristics and required resource consumption of the treatments. Lack of accuracy and validity of the Korean Diagnosis Related Group and Korean Out-Patient Group for the acute patients as the case-mix and payment tool for rehabilitation inpatients have been problematic issues. The objective of the study was to develop the Korean Rehabilitation Patient Group (KRPG) reflecting the characteristics of rehabilitation inpatients. Methods: As a retrospective medical record survey regarding rehabilitation inpatients, 4,207 episodes were collected through 42 hospitals. Considering the opinions of clinical experts and the decision-tree analysis, the variables for the KRPG system demonstrating the characteristics of rehabilitation inpatients were derived, and the splitting standards of the relevant variables were also set. Using the derived variables, we have drawn the rehabilitation inpatient classification model reflecting the clinical situation of Korea. The performance evaluation was conducted on the KRPG system. Results: The KRPG was targeted at the inpatients with brain or spinal cord injury. The etiologic disease, functional status (cognitive function, activity of daily living, muscle strength, spasticity, level and grade of spinal cord injury), and the patient's age were the variables in the rehabilitation patients. The algorithm of KRPG system after applying the derived variables and total 204 rehabilitation patient groups were developed. The KRPG explained 11.8% of variance in charge for rehabilitation inpatients. It also explained 13.8% of variance in length of stay for them. Conclusion: The KRPG version 1.0 reflecting the clinical characteristics of rehabilitation inpatients was classified as 204 groups.

Searching for Ways to Improve Visiting Oral Health Care Services in Korea through Comparison with Japanese System in Long-Term Care Insurance

  • Background: Legal regulations and fees have been established in Korea to provide visiting oral health care services to individuals with long-term care insurance (LTCI). However, beneficiaries of this service are very limited. Therefore, to improve the Korean system we propose a comparative analysis with the Japanese system. Methods: This study is a descriptive analysis based on secondary data, such as statistics, laws, and service record forms from Korea and Japan. The most recent institutional documents were obtained through a Google search. The variables investigated were financial resources of LTCI, co-payment structure, monthly limit of LTCI benefits, care levels of LTCI, service providers, service costs, contents of service, and the number of cases of service. Results: In both Korea and Japan, LTCI is financed through a combination of taxes and insurance premiums. However, the monthly limit for receiving LTCI services in Japan is about 2.4 times higher than in Korea. Visiting medical and dental treatment is also possible in Japan. Furthermore, nursing staff can provide daily oral health care services according to dental hygienists' instruction unlike Korea. Oral health care services in Korea are focused on oral hygiene and prevention of oral diseases, while Japan additionally provides oral function screening, patient education for oral health management, and training for nursing staff to enhance oral function, eating, and swallowing of the patients. Conclusion: We concluded that the possibility of visiting dental treatment, differences in monthly limit of LTCI benefits, oral function assessment and guidance, as well as collaboration with other healthcare professionals contributed to the difference in the frequency of utilization of visiting oral health care services between Korea and Japan.

일부 노인의 양생 수준에 영향을 미치는 요인 연구 (A Study on Factors Affecting the Level of Yangseng(養生) of Certain Elderly People)

  • Objectives : To explore the causes that affect the level of Yangseng(養生) certain elderly people.(Level of Yanseng; The oriental medicine tool to measure Yanseng developed to fit the situation of korea) Methods : For this purpose, characteristics, Life Satisfaction and Activities of Daily Living were adopted as independent variables and analyzed statistically. Out of questionnaires distributed to the old people aged 65 or more living in Jeonbuk province, 824 replies were collected from April to June 2004 and processed for an analysis. Results : The analysis revealed that the higher points old persons tend to score in Total of Yangseng, the more they are inclined to be Life satisfaction, the higher K-ADL they get and the longer they have leisure. Rather old men than women or rather those who live with family than those who do not have more possibility to record higher scores of Total of Yangseng(R Square=0.365). There was greater possibility that the higher points persons can get in Morality Yangseng(R Square=0.200), Mind Yangseng(R Square=0.173), Diet Yangseng(R Square=0.139), Exercise Yangseng(R Square=0.353), Sleep Yangseng(R Square=0.187), Seasonal Yangseng(R Square=0.107), Sex life Yangseng(R Square=0.199), the more they tend to be Life satisfaction. It was revealed from the analysis that the better care old people pay to Activities & Rest Yangseng, the higher K-ADL they get(R Square=0.186). Conclusions : Front the above results, it may be concluded that approaches of Oriental medicine can contribute to maintaining and promoting health as well as preventing and controlling disease for old people who are in greater needs for proper health care in the aged period.

뇌졸중 후 집단 자가 운동프로그램이 삶의 질과 운동기능에 미치는 영향 (Effects of a Group Self Exercise Program on Quality of Life and Motor Functions After Stroke)

  • The purpose of this study was to assess the effect of a group self exercise program in improving the quality of life regarding depression and the activities of daily living (ADL) of chronic stroke survivors, as well as the motor functions such as the 3 meter round walk, upper extremity function, and static balance. The subjects were 12 post-stroke ambulatory community center participants. All subjects participated in one 90 minute session per week for 7 weeks and received a home exercise program in every session. They had to record and submit an exercise check list. Quality of life was measured with the Beck depression inventory and the 8-Item Short-Form (SF-8). Motor functions were measured with the manual function test (MF'T), the kinesthetic ability trainer (KAT 3000), and the modified Barthel index. The level of depression decreased somewhat, but there were no significant differences after intervention. However, quality of life related health (SF-8) improved significant1y. There were significant improvements in the time for the 3 meter round walk, the functions of the affected upper extremities, and static balance and ADL (p<.05). The findings of this study suggest that a group self exercise program can improve quality of life related health and motor functions in stroke survivors.

Analysis of habitat characteristics of mosquitoes in Danwongu, Ansan city, Korea, based on civil complaint data

  • Climate change due to global warming and changes in land use increase the development time and distribution, as well as the abundance, of mosquitoes, thereby negatively affecting human life and health. In this study, we investigated the habitat characteristics of mosquito occurrence sites in Danwon-gu, Ansan city, Korea, based on a daily record of civil pest complaints lodged at the Danwon-gu Community Health Center. We considered two types of factors (natural and artificial) known to affect mosquito occurrence. We confirmed a total of 554 cases of mosquito occurrence from March 2014 to August 2017. The total study area was $49.11km^2$, with urban areas constituting the largest proportion. Additionally, we investigated habitat preferences of mosquitoes in terms of terrain factors; generally, mosquitoes occurred at low elevations and on low slopes. Regarding the preference of oviposition sites, mosquitoes occurred at higher rates near streams and crops lands. The rate of mosquito occurrence tended to increase in areas with dense human populations. Regarding climatic factors, the rate of mosquito occurrence increased in areas with high temperature and low precipitation.

Development of a Social Contact Survey Instrument Relevant to the Spread of Infectious Disease and Its Application in a Pilot Study Among Korean Adults

  • Objectives: This study aimed to develop a valid social contact survey instrument and to verify its feasibility for use among Korean adults. Methods: The Delphi technique was used to develop an instrument to assess social contacts, which was then applied in a cross-sectional pilot study. A panel of 15 medical professionals reviewed the feasibility and validity of each item. The minimum content validity ratio was 0.49. Thirty participants used the developed measure to record contacts during a 24-hour period. Results: After a systematic review, the survey instrument (parts I and II) was developed. Part I assessed social contact patterns over a 24-hour period, and part II assessed perceptions of contacts in daily life and preventive behaviors (hand hygiene and coughing etiquette). High validity and feasibility were found. In the pilot study, the 30 participants had a combined total of 198 contacts (mean, 6.6 daily contacts per person). The participants' age (p=0.012), occupation (p<0.001), household size (p<0.001), education (p<0.001), personal income (p=0.003), and household income (p<0.001) were significantly associated with the number of contacts. Contacts at home, of long duration, and of daily frequency were relatively likely to be physical. Assortative mixing was observed between individuals in their 20s and 50s. Contact type differed by location, duration, and frequency (p<0.001). Conclusions: The developed social contact survey instrument demonstrated high validity and feasibility, suggesting that it is viable for implementation.
