A Prediction Model for Unmet Needs of Elders with Dementia and Caregiving Experiences of Family Caregivers (재가치매 환자의 미충족요구와 가족부양자의 돌봄경험 예측모형)

  • Purpose: The purposes of this study were to develop and test a prediction model for caregiving experiences including caregiving satisfaction and burden in dementia family caregivers. Methods: The stress process model and a two factor model were used as the conceptual frameworks. Secondary data analysis was done with 320 family caregivers who were selected from the Seoul Dementia Management Survey (2014) data set. In the hypothesis model, the exogenous variable was patient symptomatology which included cognitive impairment, behavioral problems, dependency in activity of daily living and in instrumental activity of daily living. Endogenous variables were caregiver's perception of dementia patient's unmet needs, caregiving satisfaction and caregiving burden. Data were analysed using SPSS/WINdows and AMOS program. Results: Caregiving burden was explained by patient symptomatology and caregiving satisfaction indicating significant direct effects and significant indirect effect from unmet needs. The proposed model explained 37.8% of the variance. Caregiving satisfaction was explained by patient symptomatology and unmet needs. Mediating effect of unmet needs was significant in the relationship between patient symptomatology and caregiving satisfaction. Conclusion: Results indicate that interventions focusing on relieving caregiving burden and enhancing caregiver satisfaction should be provided to caregivers with high levels of dementia patients' unmet needs and low level of caregiving satisfaction.

A Study on the University Student's Stress and Relationship Satisfaction with their Mother (대학생 자녀의 어미니에 대한 스트레스 및 관계만족도에 관한 연구)

  • The purpose of this research is to examine the university students' stress and relationship satisfaction with their mother. For this purpose , 420 students were selected in Seoul area, and data were analyzed by the statistical methods, such as the Cronbach'a coefficient, Frequency, Percentile, Arithmetic Mean, Standard Deviation, One-Way ANOVA, Peason's Corelation and stepwised Multiple Regression Analysis though SPSS win PC+. The main results are as followed: (1) University students experience stress some extent through all the areas such as communication, dating, choosing occupation, daily activity, or pocket money. But the level of stress shows lower tendency relatively. (2)Students with the lower self-differentiation perceive more stress from mother. When using avoidance coping and personnel dependence coping, the level of stress is high. (3)Several variables(problem-focused coping, self-differentiation of emotional independence, avoidance coping, communication stress, daily activity stress, and sentimental self-differentiation) significiently influence relationship satisfaction with their mother. These variables explain 52% of the satisfaction with mother.

Age and Gender Differences in the Relation of Chronic Diseases to Activity of Daily Living (ADL) Disability for Elderly South Koreans: Based on Representative Data

  • Objectives: This study investigated the gender and age differential effect of major chronic diseases on activity of daily living (ADL) disability. Methods: Surveyfreq and Surveylogistic regression analyses were employed on the 2005 Korean National Health and Nutrition Examination Survey (KNHANES) with a sample of 3,609 persons aged 65 - 89. Results: After adjusting for potential covariates, stroke, among elderly men more so than women, had a 2-3 times greater odds of engendering ADL disability in the 65-69 (p < 0.05) and 70-79 age groups (p < 0.01). In comparison to elderly women, cancer, diabetes, and incontinence in elderly men was associated with a higher risk of ADL disability in the 70 - 79 age group (p < 0.05), and this association was also observed for pulmonary disease in the 80-89 age group. Among elderly women, however, a significant association between incontinence and ADL disability was identified in all three age groups. In addition, this association was found in pulmonary disease and diabetes in elderly women aged 70 - 79 years. Significant gender differences were observed in the association between stroke in the 60 - 79 age group and cancer in the 70 - 79 age group. Conclusions: Age and gender differences were observed in the effect of chronic diseases on ADL disability.

Effects of Scalp Acupuncture on Short-term NIHSS and MBI in Stroke Patients (두침이 중풍환자의 단기간 NIHSS와 MBI에 미치는 영향)

  • Objective: The aim of this study was to investigate the effect of scalp acupuncture on recovery from motor disorders in stroke patients. Methods: Twenty-two patients with post-stroke hemiplegia were randomized into two groups. Ten patients (test group) were treated by 2 methods: scalp acupuncture and general acupuncture. The other twelve patients (control group) were treated only with general acupuncture. The activity of daily living was measured with a National Institutes of Health stroke scale (NIHSS) and Modified Barthel Index (MBI). The therapy was performed once a day for 2 weeks. Results: In terms of NIHSS score, the test group showed statistically meaningful decrease after 2 weeks treatment, while the control group showed statistically meaningful decrease after 1 week (p<0.05). And in terms of score of MBI, the test group did not show statistically meaningful increase but the control group showed statistically meaningful increase after 1 week (p<0.05). There was no statistically meaningful difference after 1 and 2 weeks treatment between the groups. Conclusions: These results support that the test group has almost same the effectiveness compared with the control group in improvement of the activity of daily living of post-stroke hemiplegic patients.

Effects of Levels of Crude Fiber on Growth Performances and Intestinal Carbohydrases of Domestic Goslings

  • This study was conducted to examine the effects of dietary crude fiber levels on the growth and intestinal carbohydrases of goslings. Thirty-two, 2 week old female White Roman goslings were divided into four groups of 4 goslings with 2 replicates. Diets were isocaloric and isonitrogenous, containing 40, 80, 120 or 160 g/kg crude fiber. At the end of the 4 weeks of the experimental period, all goslings were sacrificed and the carbohydrases activities were measured. Feed intake was significantly higher in the 120 and 160 g/kg crude fiber groups over that in the 40 and 80 g/kg groups (p<0.05). The average daily gosling weight gain significantly increased with increasing crude fiber levels from 40 to 120 g/kg. However, both the daily gain and feed conversion of the 160 g/kg crude fiber diet group decreased significantly. Amylase, maltase and ${\alpha}-glucosidase$ activities in the duodenum significantly decreased as the dietary crude fiber level increased. The maltase and ${\alpha}-glucosidase$ activities in the jejunum-ileum showed a similar trend with those in the duodenum. By increasing the levels of crude fiber, cellulase activity in the caecum content significantly increased. There was, however, an adverse effect on the amylase activity.

The Effects on Home Visiting Nursing Service for Community Based Disabled Persons (지역사회 지체장애인을 위한 방문간호사업 효과 평가 - 재가 뇌병변장애인을 중심으로 -)

  • To evaluate the effects of planned visiting nursing services for homebound disabled persons in the community who have had a cerebrovascular accident Methods: One group pre-test and post-test research deign was applied to 61 disabled persons who were undergoing rehabilitation with visiting nursing services to evaluate the effects on the health status (SF-36) and activity daily living (ADL/IADL) from March to August, 2001. Results: The health status score by SF-36 of homebound disabled people was significantly improved; the average score was 28.56$\pm$21.24 before service, 34.29$\pm$22.30 at 3 months after, and 40.84$\pm$=27.41 at 6 months after. The activity daily living score by OASIS II was also significantly increased (z= -6.09, p=.000; z= -6.04, p= .000) at 3 months and 6 months after home visiting nursing services. Conclusion: The strategy plan for developing a visiting nursing service in health centers should be prepared to develop community based rehabilitation (CBR) programs as well as to improve the level of health status and ADL/IADL for homebound disabled people in the community.

Effects of Activity of Daily Living and Depression on Quality of Life among Elderly Women in Rural Areas (농촌 여성 노인의 일상생활수행능력과 우울이 삶의 질에 미치는 영향)

  • Purpose: This study was done to examine factors affecting quality of life among community-dwelling elderly women who live in rural areas. Methods: The design of this research was cross-sectional descriptive study. The participants were 92 community-dwelling women aged 65 or older. Data were collected from November 1 to 15, 2013. ADL (Kart's Index), GDS-SF (Geriatric Depression Scale Short Form) and GQOL (Geriatric Quality of Life Scale) were used to measure variables. Data were collected using self-administered questionnaires and analyzed using descriptive statistics, Spearman's coefficient and stepwise multiple regression. Results: The participants' mean age was 77.5, and 85.9% had elementary school graduation or less education, and for 64.1%, their economic status was low. Spearman's rho coefficient analysis found that QOL was significantly associated with depression (r=-.72, p<.001), perceived health (r=.58, p<.001), regular exercise (r=.47, p<.001), education level (r=.29, p=.005), and ADL (r=-.21, p=.043). Multiple regression analysis showed that 65.9% of their QOL was explained by depression (${\beta}=-.72$), perceived health (${\beta}=.24$), ADL (${\beta}=-.16$), exercise (${\beta}=.22$) and number of diseases (${\beta}=.19$). Conclusion: These results indicate that older women who live rural areas need support for ADL, and prevention of depression to improve their quality of life.

The Effect of Swiss Ball Exercise on Lower Extremity Function of Elderly Women with Mild Cognitive Impairment (스위스볼 운동이 경도인지장애 여성노인의 하지기능에 미치는 효과)

  • Purpose: The purpose of this study was to confirm whether the Swiss Ball exercise program is effective to improve lower extremity function and activity of daily living of elderly women with mild cognitive impairment (MCI). Methods: This study was a double blind control study. Subjects participated 34 female elderly women who were assigned to exercise group (n=17) and control group (n=17). The Swiss Ball exercise program was performed 2 times a week during the 12 weeks. Pre-and post-exercise various measurements were made: lower extremity function (OLS, TUG, STS). Results: In exercise group, There showed statically significance improve between pre-and post-exercise in OLS (7.29${\pm}$1.10 and 7.64${\pm}$1.32), TUG (10.47${\pm}$2.03 and 10.05${\pm}$2.04), STS (7.71${\pm}$1.04 and 7.94${\pm}$.82)(p<.05). ADL also showed statically significance improve between pre-and post-exercise (13.76${\pm}$3.54 and 12.82${\pm}$3.39, p<.05). There showed statically significance difference between two groups (p<.05). Conclusion: The Swiss Ball exercise program improved lower extremity function and ADL of elderly women with MCI. Further studies are required to examine the significance of the assessment of motor function of lower extremities.

Inhibitory Effects of Glycine on Morphine-Induced Hyperactivity, Reverse Tolerance and Postsynaptic Dopamine Receptor Supersensitivity in Mice

  • The effects of glycine on morphine-induced hyperactivity, reverse tolerance and postsynaptic dopamine receptor supersensitivity in mice was examined. A single administration of morphine (10 mg/kg, s.c.) induced hyperactivity as measured in mice. The morphine-induced hyperactivity was inhibited by pretreatment with glycine (100, 200 and 400 mg/kg, i.p.). In addition, it was found repeated administration of morphine (10 mg/kg, s.c.) to mice daily for 6 days caused an increase in motor activity which could be induced by a subsequent morphine dose, an effect known as reverse tolerance or sensitization. Glycine (100, 200 and 400 rng/kg, i.p.) also inhibited morphine-induced reverse tolerance. Mice that had received 7 daily repeated administrations of morphine also developed postsynaptic dopamine receptor supersensitivity, as shown by enhanced ambulatory activity after administration of apomorphine (2 mg/kg, s.c.). Glycine inhibited the development of postsynaptic dopamine receptor supersensitivity induced by repeated administration of morphine. It is suggested that the inhibitory effects of glycine might be mediated by dopaminergic (DAergic) transmission. Accordingly, the inhibition by glycine of the morphine-induced hyperactivity, reverse tolerance and dopamine receptor supersensitivity suggests that glycine might be useful for the treatment of morphine addiction.

Knowledge and Learning Needs of Coronary Artery Bypass Graft Patients on Cardiac Rehabilitation (관상동맥 우회술(CABG)환자의 심장재활에 대한 지식과 교육 요구도 조사)

  • Purpose: The purpose of this study was to explore the knowledge and learning needs on cardiac rehabilitation of coronary artery bypass graft(CABG) patients. Method: The subjects consisted of 100 CABG patients at A hospital in Seoul. Data were collected by the two different kind of questionnaires which measure knowledge and learning needs on cardiac rehabilitation of CABG patients. The subjects responded the questionnaire on knowledge before CABG and that on learning needs before their discharge. Result: The mean score of knowledge on cardiac rehabilitation was 68.54. Knowledge on risk factor, nature of disease, diet, daily activity, medication, post operative care were great in order. The mean score of learning needs on cardiac rehabilitation was 4.28. Learning needs on diet, medication, nature of disease, post operative care, daily activity, risk factor were great in order. There were significant differences in knowledge according to occupation, economic status and family history(p=.021, p=.017, p=.023). There was a positive correlation between knowledge and learning needs(r=.3009, p=.002). Conclusion: Level of knowledge on cardiac rehabilitation of CABG patients is low and knowledge on postoperative care is the lowest, and learning needs are great in ail categories.
