Unenhanced Breast MRI With Diffusion-Weighted Imaging for Breast Cancer Detection: Effects of Training on Performance and Agreement of Subspecialty Radiologists

  • Objective: To investigate whether reader training improves the performance and agreement of radiologists in interpreting unenhanced breast magnetic resonance imaging (MRI) scans using diffusion-weighted imaging (DWI). Materials and Methods: A study of 96 breasts (35 cancers, 24 benign, and 37 negative) in 48 asymptomatic women was performed between June 2019 and October 2020. High-resolution DWI with b-values of 0, 800, and 1200 sec/mm2 was performed using a 3.0-T system. Sixteen breast radiologists independently reviewed the DWI, apparent diffusion coefficient maps, and T1-weighted MRI scans and recorded the Breast Imaging Reporting and Data System (BI-RADS) category for each breast. After a 2-h training session and a 5-month washout period, they re-evaluated the BI-RADS categories. A BI-RADS category of 4 (lesions with at least two suspicious criteria) or 5 (more than two suspicious criteria) was considered positive. The per-breast diagnostic performance of each reader was compared between the first and second reviews. Inter-reader agreement was evaluated using a multi-rater κ analysis and intraclass correlation coefficient (ICC). Results: Before training, the mean sensitivity, specificity, and accuracy of the 16 readers were 70.7% (95% confidence interval [CI]: 59.4-79.9), 90.8% (95% CI: 85.6-94.2), and 83.5% (95% CI: 78.6-87.4), respectively. After training, significant improvements in specificity (95.2%; 95% CI: 90.8-97.5; P = 0.001) and accuracy (85.9%; 95% CI: 80.9-89.8; P = 0.01) were observed, but no difference in sensitivity (69.8%; 95% CI: 58.1-79.4; P = 0.58) was observed. Regarding inter-reader agreement, the κ values were 0.57 (95% CI: 0.52-0.63) before training and 0.68 (95% CI: 0.62-0.74) after training, with a difference of 0.11 (95% CI: 0.02-0.18; P = 0.01). The ICC was 0.73 (95% CI: 0.69-0.74) before training and 0.79 (95% CI: 0.76-0.80) after training (P = 0.002). Conclusion: Brief reader training improved the performance and agreement of interpretations by breast radiologists using unenhanced MRI with DWI.

Wavelet-based Watermarking using Correlation Comparison (상관도 비교를 이용한 웨이블릿 기반 워터마킹)

  • This paper presents a wavelet-based digital watermarking scheme for color images. We insert watermarks in the DWI domain using spread-spectrum correlation-based watermarking in luminance component of the color image. The watermark, two pseudorandom patterns, is inserted by modifying the wavelet coefficients at subband HL2, LH2, HH2 according to message bits. To detect watermark, we calculate the correlation between the watermarked image and pseudorandom patterns. As compared with correlations, the watermark detector determines embedding message. Experimental results show that the scheme is not only invisible but also robust to various attacks such as JPEG compression, noise addition and so on.

  • Anisotropic DWI - Mapping of the Proton Diffusion "tensor". In neural ordered tissue, it is thought that water diffusion is mainly influenced by the presence of myelin sheaths and intracellular structures. Perpendicular to the fiber tracts, the cholesterol-laden myelin lipid bilayers might restrict or hinder the spins from diffusing through the normally highly permeable cytomembrane. Diffusion along the fiber is more or less determined by subcellular structures, such as the endoplasmatic reticulum, mitochondria, neuro-filaments and macromolecules. In addition to that, the entire complex of axons and stabilizing tissue (i.e., glia cells, astrocytes) is also assumed to influence diffusion due to the tortuosity of proton translation, but the uniform distribution of such cells throughout the brain might render this notion less important as initially anticipated.

  • Purpose: To evaluate the relationship between reperfusion hyperemia in reversible cerebral ischem and the degree of programmed cell death. Method: We produced the animal models of reversible cerebral ischemia in 10 cats by mean of middle cerebral artery (MCA) occlusion with transorbital approach. MCA was occluded b microvascular clamp for an hour. MR imaging was performed at 0, 1, 2 days after ischemi and reperfusion. Perfusion (PWI) [Contrast enhanced GRE EPI, TR/TE= 1500/40, 40 Phases, 128 matrix, 12 cm FOV] and diffusion (DWI) (SE EPI, b=0, 500, 1000) weighted images were obtained using Philips Intera 1.57 system. rCBV and ADC maps were calculated wi IDL based postprocessing program. Tissue slices were obtained after the last MR imagin TUNEL, Calbin and Acid-Fuscin staining were done for corresponding slices as MR imagin We investigated the differences of degree of apoptosis in the area of reperfusion hyperemia.

  • 확산텐서영상(Diffusion Tensor Imaging)의 개념은 1980년대 중반 확산강조영상(Diffusion Weighted Imaging)의 개념이 도입되면서 거의 동시에 도입되었지만 MR 장비의 기계적 한계등으로 인해 최근에야 임상적으로 사용되기 시작했다. 확산텐서영상(DTI)과 확산강조영상(DWI)의 방법론적인 차이점은 단순히 확산강조영상의 경우 세 개의 다른방향 (X-, Y-, Z-축)으로 확산측정용 경사자장을 가하는 반면 확산텐서영상의 경우 이보다 많은 방향 (최소한 6 방향)으로 확산측정용 경사자장을 가한다는 점이다. 이러한 차이로부터 금방 알 수 있는 점은 확산강조영상은 확산텐서영상의 일부라는 점이다. 즉, 확산텐서영상의 경우 더 많은 방향으로 확산측정용 경사자장을 가했으므로 더 많은 정보를 획득할 수 있고 이중 세 개의 방향 (주로 X,Y,Z)에 대한 정보만을 이용하게 되면 확산강조영상이다. 이러한 이유로 확산텐서영상을 획득하면 확산강조영상에서 얻게 되는 방향별 확산강조영상, 등방성(isotropic) 확산강조영상, ADC (Apparent Diffusion Coefficient) map등도 기본적으로 얻게 되므로 추가로 확산강조영상을 획득할 필요가 없다. 본 강의에서는 이러한 확산(텐서)영상의 물리적 원리를 복잡한 수학적 설명보다는 개념 위주로 설명해 보고자 한다.

3T DWIs with Different b-Values in Brain Tumors

  • Purpose: It is known that diffusion-weighted MR imaging (DWI) is helpful in the evaluation of malignancy grading in brain tumor. This study was to evaluate the DWls with different b-values of various brain tumors in order to determine optimal b-values on 3T MR unit. Method: On a 3T MR unit, DWls with b-values of 1, 000, 3, 000 and 5, 000 s/mm2 were obtained in 20 patients of pathologically-proven brain tumors (7 metastases, 4 high grade gliomas, 2 Iymphomas, 2 low grade gliomas, 2 germinomas, and one each of germinoma, meningioma, hemangioblastoma and central neurocytoma. The overall image quality, contrast between normal brain parenchyma and tumor and signal intensities of solid and cystic components were comparatively evaluated among DWls with different b-values by visual inspection.

Male-Silkmoth-Inspired Routing Algorithm for Large-Scale Wireless Mesh Networks

  • This paper proposes an insect behavior-inspired routing algorithm for large-scale wireless mesh networks. The proposed algorithm is adapted from the behavior of an insect called Bombyx mori, a male silkmoth. Its unique behavior is its flying technique to find the source of pheromones. The algorithm consists of two steps: the shortest-path algorithm and the zigzag-path algorithm. First, the shortest-path algorithm is employed to transmit data. After half of the total hops, the zigzag-path algorithm, which is based on the movement of the male B. mori, is applied. In order to adapt the biological behavior to large-scale wireless mesh networks, we use a mesh topology for implementing the algorithm. Simulation results show that the total energy used and the decision time for routing of the proposed algorithm are improved under certain conditions.

  • Cineam gold deposit is one of an epithermal gold deposit of Low Sulfidation type, which is located in the Sunda-Banda magmatic Arc Zone. Its existence in this region has led to a new concept or few point on the Sunda - Banda Magmatic Arc. Even the deposit is classified as a small scale in mine, however, mineralogically, fluid inclusion and isotopes studies are very interesting to be studied. (omitted)

  • This study presents a case of diffuse large B cell lymphoma (DLBCL) in a 58-year-old man showing unusual manifestations mimicking chronic osteomyelitis. In this case review, we describe the imaging findings of DLBCL which mimics chronic osteomyelitis and review existing reports regarding the differential diagnosis of bone involvement of lymphoma and osteomyelitis through imaging and laboratory findings and diffusion-weighted magnetic resonance imaging (DWI) such as the advanced MRI sequence.

The Pharmacological Properties of Silymarin and Its Constituents

  • Silymarin is a standardized extract obtained from the seeds and fruits of Silybum marianum L., or commonly called milk thistle, a member of Carduus marianum family that contains mix of flavonolignans. Some epidemiological and preclinical studies revealed that S. marianum L. has been used for herbal remedies for centuries for its pharmacological activity. In this review, pharmacological studies in vitro and in vivo of silymarin are discussed thoroughly stressing on antioxidant, antimicrobial, antiviral, and anti-carcinogenic aspects of silymarin. In addition, the protective influences of silymarin on some organs such as heart, liver, bone, and neuron tissue are reviewed as well. This review would be useful for further study regarding the potential of natural plant, notably silymarin, and its therapeutic potential in the prevention and treatment of diseases.
