한정성 효과: 한정성 제약과 비한정성 제약 (Definiteness Effect: Definiteness Restriction and Indefiniteness Restriction)

  • I argue that Definiteness Effect (DE) should include Indefiniteness Restriction (IR) as well as Definiteness Restriction (DR). DR is exhibited by existential constructions, predicate nominals, inalienable possession constructions, and verbs with semantic features like 〔((CAUSE TO) COME TO) EXIST〕. IR is caused by some existence presupposition of aspectual adverbs, aspectual verbs, repetitives, and topic markers. The environments for DR and IR determine the (in)definiteness of Korean bare noun phrases which otherwise can be used either way. The neutralization of DR is also induced by focus which imposes a certain amount of structure on the event quantification. Van der Sandt's (1992) Presuppositions-as- Anaphora-Theory is effectively used to account for those examples from DR, ID, and the neutralization of BR.

Association of DR4 (TRAIL-R1) Polymorphisms with Cancer Risk in Caucasians: an Updated Meta-analysis

  • Death receptor 4 (TRAIL-R1 or DR4) polymorphisms have been associated with cancer risk, but findings have been inconsistent. To estimate the relationship in detail, a meta-analysis was here performed. A search of PubMed was conducted to investigate the association between DR4 C626G, A683C and A1322G polymorphisms and cancer risk, using odds ratios (ORs) with 95% confidence intervals. The results suggested that DR4 C626G and A683C polymorphisms were indeed associated with cancer risk (for C626G, dominant model, OR 0.991, 95%CI 0.866-1.133, p=0.015; for A683C, additive model, OR=1.140, 95%CI: 0.948-1.370, p=0.028; dominant model, OR=1.156, 95%CI: 0.950-1.406, p=0.080) in the Caucasian subgroup. However, the association was not significant between DR4 polymorphism A1322G with cancer risk in Caucasians (For A1322G, additive model: OR 1.085, 95%CI 0.931-1.289, p=0.217; dominant model: OR 1.379, 95%CI 0.934-2.035, p=0.311; recessive model: OR 1.026, 95%CI 0.831-1.268 p=0.429.). In summary, our finding suggests that DR4 polymorphism C626G and A683 rather than A1322G are associated with cancer risk in Caucasians.

  • Progressive facial hemiatrophy, also known as Parry-Romberg syndrome, is a progressive and self-limited deformation of the subcutaneous tissue volume on one side of the face that creates craniofacial asymmetry. We present the case of a patient with a five-year history of progressive right facial hemiatrophy, who underwent facial volumetric restoration using cell-assisted lipotransfer (CAL), which consists of an autologous fat graft enriched with adipose-derived stem cells (ASCs) extracted from the same patient. ASCs have the capacity to differentiate into adipocytes. They also promote angiogenesis, release angiogenic growth factors, and some can survive as stem cells. The use of autologous fat as a filler in soft tissue atrophy has been satisfactory in patients with mild and moderate Parry-Romberg syndrome. Currently, CAL has showed promising results in the long term by decreasing the rate of fat reabsorption. The permanence and stability of the graft in all the injected areas has showed that autologous fat grafts enriched with stem cells could be a promising technique for the correction of defects caused by this syndrome.

Comparative study of finite element analysis and generalized beam theory in prediction of lateral torsional buckling

  • In the construction industry, thin-walled frame elements with very slender open cross-sections and low torsional stiffness are often subjected to a complex loading condition where axial, bending, shear and torsional stresses are present simultaneously. Hence, these often fail in instability even before the yield capacity is reached. One of the most common instability conditions associated with thin-walled structures is Lateral Torsional Buckling (LTB). In this study, a first order Generalized Beam Theory (GBT) formulation and numerical analysis of cold-formed steel lipped channel beams (C80×40×10×1, C90×40×10×1, C100×40×10×1, C80×40×10×1.6, C90×40×10×1.6 and C100×40×10×1.6) subjected to uniform moment is carried out to predict pure Lateral Torsional Buckling (LTB). These results are compared with the Finite Element Analysis of the beams modelled with shell elements using ABAQUS and analytical results based on Euler's buckling formula. The mode wise deformed shape and modal participation factors are obtained for comparison of the responses along with the effect of varying the length of the beam from 2.5 m to 10 m. The deformed shapes of the beam for different modes and GBTUL plots are analyzed for comparative conclusions.

초음파 영상에서 동적영역과 주파수 방식의 설정에 따른 효과 (Effects of Settings in Dynamic Ranges and Frequency Modes on Ultrasonic Images)

  • 임상에서 정확한 진단을 위해 양질의 초음파 영상을 얻는 것은 중요하다. 본 연구에서는 초음파 진단기의 주파수와 동적 영역(dynamic range, DR) 설정 및 프로브 유형에 따른 초음파 영상의 변화를 관찰하였다. 실험에서 고형 및 낭성 종괴 병변과 유사한 초음파 평가 팬텀(539,551, ATS, USA)의 6가지 LCS(low contrast sensitivity) 표적(-15, -6, -3, +3, +6, +15 dB)에 대한 영상을 대상으로 평가하였다. 초음파 영상은 볼록형(convex, C3-7IM) 및 선형(linear probe, L5-12IM) 두 가지 탐촉자(Probe)에 대해 SA-9900(Medison Ltd, Korea)을 이용하여 얻었다. 주파수는 4가지 방식(gen, pen, res, harmonic), 동적영역은 $40{\sim}100\;dB$ 범위에서 설정을 변화하면서 영상을 취득하였다. 취득한 영상의 질은 표적의 명목상 LCS 값과 영상에서 측정된 LCS을 비교하여 평가 하였다. 실험 결과 볼록형 탐촉자의 경우, 각 LCS 표적에 대해 주파수 방식 별로 동적영역 값을 40, 60, 80, 100 dB로 변화할 때, 초음파 영상의 질은 유의한 변화가 없었다. 그러나 낭종성 병변에 가까운 LCS 표적 -15 dB에서는 동적영역 60 dB과 하모닉 주파수 방식에서 LCS 값이 높게 나타났다. 선형 프로브에서는 -15 dB LCS 표적에서 동적영역 40 dB, 하모닉 방식에서 명목상 값에 근접하였다. LCS관점에서 정량화된 영상의 평가의 한계와 주관적인 평가 즉 심리적인 관점에서의 ROC(Receiver Operating Characteristic) 평가의 필요성에 대해 토의하였다.

BASINS/HSPF 모형을 이용한 수질오염총량관리 유달율 산정방법 연구 (Estimation of Delivery Ratio Based on BASINS/HSPF Model for Total Maximum Daily Load)

  • In this study Window interface to Hydrological Simulation Program-FORTRAN (HSPF) developed by the United States Environmental Protection Agency (EPA) was applied to the upstream of Namgang watershed to estimate its applicability for estimating Delivery Ratio (DR) of water pollutants for Total Maximum Daily Load (TMDL). BASINS/HSPF which is selected in this study, is found to be appropriate for simulation of daily flow and water quality in target basins. DR was estimated utilizing discharge loads of unobserved sub-basin and delivery load of unobserved locations obtained not by actual evaluation but by simulation through validation and verification. Annual average DR of BOD, TN and TP were 0.97 ~ 1.50, 2.23 ~ 3.21, and 0.81 ~ 1.09 respectively. Net DR of dependent basins excluding influence of upstream basin was 1.50 ~ 1.70, 0.55 ~ 0.69, and 0.24 ~ 0.31, all of which are lower than those of independent basins area. Utilizing the model selected by this research, DR and Net DR of unobserved basins will be estimated, which will help determine priorities in management of basin areas.

악성 종양 환자에 대한 DR-$70^{TM}$ 면역 분석법의 의의: Validation Study (Meaning of the DR-$70^{TM}$ Immunoassay for Patients with the Malignant Tumor)

  • Background: The DR-$70^{TM}$ immunoassay is a newly developed cancer diagnostic test which quantifies the serum fibrin degradation products (FDP), produced during fibrinolysis, by antibody reaction. The purpose of this study was to evaluate the potential of DR-$70^{TM}$ Immunoassay in screening malignant tumor. Methods: Sample subjects were 4,169 adults, both male and female, who visited the health promotion center of a general hospital from March 2004 to April 2005 and underwent the DR-$70^{TM}$ immunoassay test and other tests for cancer diagnosis. The patient group was defined as 42 adults out of the sample subjects who were newly diagnosed with cancer during the same time period when the DR-$70^{TM}$ immunoassay test was performed. Final confirmation of a malignant tumor was made by pathological analysis. Results: The mean DR-$70^{TM}$ level was $0.83{\pm}0.65{\mu}g/ml$ (range: 0.00 (0.0001)${\sim}7.42{\mu}g/ml)$ in the control group (n=4,127) as opposed to $2.70{\pm}2.33{\mu}g/ml$ (range: $0.12{\sim}9.30{\mu}g/ml)$ in the cancer group (n=42), and statistical significance was established (p<0.0001, Student t-test). When categorized by the type of malignant tumor, all cancer patients with the exception of the subgroups of colon and rectal cancer showed significantly higher mean DR-$70^{TM}$ levels compared with the control group (p<0.0001, Kruscal-Wallis test). The receiver operating characteristic (ROC) curve analysis revealed ${\geq}1.091{\mu}g/ml$ as the best cut-off value. Using this cut-off value, the DR-$70^{TM}$ immunoassay produced a sensitivity of 71.4%, a specificity of 70.1%, a positive predictability of 69.4%, and a negative predictability of 69.2% (1). Conclusion: A significant increase in the mean DR-$70^{TM}$ value was observed in the cancer group (thyroidal, gastric, breast, hepatic and ovarian) com pared with the control group. In particular, the specificity and sensitivity of the DR-$70^{TM}$ immunoassay was relatively high in the subgroups of breast, gastric, and thyroidal cancer patients. There is need for further studies on a large number of malignant tumor patients to see how the DR-$70^{TM}$ level might be changed according to the differentiation grade and postoperative prognosis of the malignant tumor.

조류인플루엔자 바이러스의 양-반응 모형 (Dose-Response Relationship of Avian Influenza Virus Based on Feeding Trials in Humans and Chickens)

  • This study aimed to determine dose-response (DR) curve of avian influenza (AI) virus to predict the probability of illness or adverse health effects that may result from exposure to a pathogenic microorganism in a quantitative microbial risk assessment. To determine the parametric DR relationship of several strains of AI virus, 7 feeding trial data sets challenging humans (5 sets) and chickens (2 sets) for strains of H3N2 (4 sets), H5N1 (2 sets) and H1N1 (1 set) from the published literatures. Except for one data set (study with intra-tracheal inoculation for data set no. 6), all were obtained from the studies with intranasal inoculation. The data were analyzed using three types of DR model as the basis of heterogeneity in infectivity of AI strains in humans and chickens: exponential, beta-binomial and beta-Poisson. We fitted to the data using maximum likelihood estimation to get the parameter estimates of each model. The alpha and beta values of the beta-Poisson DR model ranged 0.06-0.19 and 1.7-48.8, respectively for H3N2 strain. Corresponding values for H5N1 ranged 0.464-0.563 and 97.3-99.4, respectively. For H1N1 the parameter values were 0.103 and 12.7, respectively. Using the exponential model, r (infectivity parameter) ranged from $1.6{\times}10^{-8}$ to $1.2{\times}10^{-5}$ for H3N2 and from $7.5{\times}10^{-3}$ to $4.0{\times}10^{-2}$ for H5N1, while the value was $1.6{\times}10^{-8}$ for H1N1. The beta-Poisson DR model provided the best fit to five of 7 data sets tested, and the estimated parameter values in betabinomial model were very close to those of beta-Poisson. Our study indicated that beta-binomial or beta-Poisson model could be the choice for DR modeling of AI, even though DR relationship varied depending on the virus strains studied, as indicated in prior studies. Further DR modeling should be conducted to quantify the differences among AI virus strains.

Which One is More Effective, Filgrastim or Lenograstim, During Febrile Neutropenia Attack in Hospitalized Patients with Solid Tumors?

  • Background: Chemotherapy-induced febrile neutropenia (FN) with solid tumors causes mortality and morbidity at a significant rate. The purpose of this study was to compare the effects of filgastrim and lenograstim started with the first dose of antibiotics in hospitalized patients diagnosed with FN. Materials and Methods: Between February 2009 and May 2012, 151 patients diagnosed with FN were evaluated, retrospectively. In those considered appropriate for hospitalization, convenient antibiotic therapy with granulocyte colony stimulating factors was started within first 30 minutes by completing necessary examinations in accordance with FEN guide recommendations. Results: In this study, 175 febrile neutropenia attacks in 151 patients were examined. Seventy three of the patients were male and 78 were female. The average age was 53.6 and 53.6, respectively. The most common solid tumor was breast carcinoma in 38 (25%). One hundred and five FN patients (58%) were those who received granulocyte colony stimulating factors as primary prophylaxis. Conclusions: While studies comparing both drugs generally involve treatments started for prophylaxis, this study compared the treatment given during the febrile neutropenia attack. Compared to lenograstim, filgastrim shortens the duration of hospitalization during febrile neutropenia attack by facilitating faster recovery with solid tumors.

Effect of Supplementation of Zinc, Glutamine, Fiber, and Prebiotics in Presumed Healthy Indonesian Children Aged 1-3 Years

  • Purpose: Impaired intestinal mucosal integrity may affect the gastrointestinal function, especially in relation to nutrition, absorption, and barrier function. The purpose of this study was to measure the prevalence of impaired intestinal mucosal integrity in presumed healthy children aged 1-3 years and assess the effects of zinc, glutamine, fiber, and prebiotic supplementation in them. Methods: A cross-sectional study was conducted in 200 children aged 1-3 years in Pasar Minggu, South Jakarta, Indonesia. A randomized double-blind parallel group method clinical trial was then performed to assess the effects of zinc, glutamine, fiber, and prebiotic supplementation. Results: Elevated calprotectin was found in 91/200 subjects (45.5%) at the onset of the study. After 10 months, 144 subjects completed the study: 72 subjects received the trial formula, whereas the other 72 received the standard formula. A transitory decrease in fecal calprotectin (FC) was observed after 6 months in the subgroup with normal FC levels, who were fed the test formula (p=0.012). Conclusion: The prevalence of impaired intestinal mucosal integrity in this group of Indonesian children aged 1-3 years was high. Supplementation with zinc, glutamine, fiber, and prebiotics during 6 months reduced FC only in those who had low levels at baseline but not in those with impaired integrity.
