  • 골다공증은 45세 이상의 여성 50% 이상이 갖고 있는 질환이다. 더욱이 질병의 증상을 환자가 느끼지 못하여 병을 심화시키기 때문에 조기 진단으로 골다공증을 예방하는 것이 중요하다. 이를 위해 골밀도(Bone Mineral Density)의 효과적인 측정을 위한 다양한 연구가 국내외적으로 수행되고 있다. 골밀도 측정을 위해서는 X-선 영상이 활용되고 있으며, 이 중에서 이중 에너지 X 선 흡수법(DEXA)을 이용하여 골밀도를 측정하는 방법은 많은 관심을 받고 있는 분야이다. DEXA 영상은 서로 다른 두 에너지준위의 X선으로부터 영상을 획득함으로써 생체조직의 중첩에 의한 잡음을 효과적으로 줄여 진단효과를 높이는데 유용하다. 하지만, DEXA 영상에서 골영역을 추출하기 위해서는 실험적으로 계수를 결정해야 하는 문제점이 있다. 본 논문에서는 두 에너지 흡수 영상에서 골영역을 추출하기 위해 실험적으로 결정하는 변수를 X-선 영상의 물리적 의미와 각 에너지에 반응하는 생체조직들의 감쇠상수 특성을 기반으로 분석하여 골영역을 효과적으로 검출하는 방법을 제안한다. 더불어 다양한 영상을 적용한 실험으로 제안한 알고리즘의 효율성을 확인하였다.

  • In this experiment, how DEXA(Dual-energy X-ray Absorptiometry) bone mineral density was measured using the equipment. In order to maintain the same measurement conditions, bone mineral density measurements of 10 cm thick phantom, with an actual patient at a point when examining the same conditions(100 kVp, 1 mA) and then out to the five doses of radiation and its average was calculated by dividing measured. X-ray dose rate measured at the Research Institute, Sword of the gamma survey meters calibrated MEDCOM Ltd. (Inspector GM counter tube) was used, calibration factor is 1.15. On a horizontal plane around the patient, depending on the distance was significantly reduced dose rate. In addition, orientation $0^{\circ}$ head end was higher in the direction of the highest dose rate, $0^{\circ}$ $180^{\circ}$ direction from the direction towards the higher dose rate reduced to some extent in the direction of all the $120^{\circ}$ were able to identify.

  • Corticosteroids are commonly used for pain control in rotator cuff tear. Deregulated $NF-{\kappa}B$ activation is a hallmark of chronic inflammatory diseases and has been responsible for the pathogenesis of rotator cuff tear. The Dexamethasone(DEXA) is a synthetic corticosteroid. The purpose of this study was to examine the exact effect of dexamethasone on $NF-{\kappa}B$ signaling in rotator cuff tear. We measured $NF-{\kappa}B$ expression in four groups: control, $TNF-{\alpha}$-treated, DEXA-treated, and combined treatment with $TNF-{\alpha}$ and DEXA. Tenocytes were isolated from patients with rotator cuff tears and pre-incubated with $TNF-{\alpha}$ (10 ng/ml), DEXA ($1{\mu}M$), or both of them for 10 min, 1 h, and 2 h. Expression of p65, p50, and p52 in the nuclei and cytosol was analyzed by western blotting and immunofluorescence imaging using confocal microscopy. We also evaluated nucleus/cytosol (N/C) ratios of p65, p50, and p52. In our study, the combined treatment with DEXA and $TNF-{\alpha}$ showed increased N/C ratios of p65, p50, and p52 compared with those in the $TNF-{\alpha}$ group at all time points. Additionally, in the DEXA group, N/C ratios of p65, p50, and p52 gradually increased from 10 min to 2 h. In conclusion, DEXA promoted the nuclear localization of p65, p50, and p52, but was not effective in inhibiting the inflammatory response of $TNF-{\alpha}$-stimulated rotator cuff tear.

치조 골소주 모폴로지 특성과 골다공증의 연관관계에 대한 연구 (Relationship between the morphologic features of alveolar trabecular bone and systemic osteoporosis)

  • Purpose : The purpose of this study was to investigate the preliminary use of morphologic operation (MO) in analyzing trabecular pattern of alveolar bone for the predicting systemic osteoporosis. Materials and Methods : Study subjects consisted of 35 females (average age 48.5 years) and 25 males (average age 25.8 years). Bone mineral density BMD $(grams/cm^2)$ of lumbar spine and proximal femur of these subjects were measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized periapical radiographs of subjects' posterior jaw. A custom computer program processed morphology operations of ROIs. We compared mean values of 11 MO variables according to the osteoporotic group divided by the T-scores of DEXA. We also studied correlation between radiographic density and these MO variables. Results : The mean radiographic densities insignificantly correlated with MO variables. There were statistically significant differences among the values of 9 MO variables according to the osteoporotic group. Conclusion Morphologic operation can be effective in analyzing trabecular pattern of alveolar bone for the predicting osteoporosis.

체적골밀도 측정법 동향: 수술부위 골밀도 분석 (Volumetric Bone Mineral Density Measurement: for Surgery Specific Bone Volumes)

  • CT영상을 기반으로 한 QCT는 CT장비와 컴퓨터영상처리기술의 발전과 함께 적은 방사선 투과양으로도 체적골밀도를 측정할 수 있는 방향으로 크게 발전하고 있다. 방사선 의료영상을 대표적인 골밀도 측정법인 이중방사선흡수계수법(DXA)은 척추나 근위대퇴골과 다른 뼈와의 상관성이 떨어지는 경우가 많고질량효과에 의한 부정확성의 근본 원인을 가지고 있기 때문에 체적골밀도측정법인 단일에너지CT영상을 이용한 QCT법이 가장 보편적인 정확한 골밀도 측정법이다. 이중에너지CT의 사용은 단단한 뼈와 동시에 종양이나 연부조직의 이상을 같이 볼 수 있는 영상을 얻을 수 있게 되어 QCT 골밀도와 함께 특정연부조직이나 장기를 검사할 수 있어 그 사용이 더욱 늘어날 것으로 예상된다. 임상수술이나 추적검사가 필요한 위치에 사용자가 해부학적으로 특정한 형상체적에 대한 국부골밀도측정 방법도 병원의 임상현장에서 활용할 기술로 대두되고 있다.

요추관 협착증과 골밀도와의 상관성에 관한 임상례 보고 (Correlation between lumbar spinal stenosis and bone mineral density : a clinical survey of 9 cases)

  • Objective : The aim of this study was to evaluate association between lumbar spinal stenosis, one of degenerative diseases, and bone mineral density. Methods : We monitored 9 lumbar spinal stenosis patients in women above 50 years, visiting outpatient clinic of the Jaseng Oriental Medicine Hospital between January 5 2006 and March 31, 2006. They were diagnosed by radiologist ist after taking Magnetic Resonance Imaging(MRI). The cases were Investigated the bone mineral density using Dual Energy X-ray Absorptiomerty(DEXA). and then Picture Archiving Communication System(PACS) were used to assess correlation between lumbar spinal stenosis and bone mineral density. Results : 1. In comparison of the spinal canal area and lumbar spine 2 level bone mineral density, the data showed a significant result 2. The data, between spinal canal area and lumbar spine 1-2 level bone mineral density, indicated a significant result. 3. Also, the result of comparison between spinal canal area and the lowest value of bone mineral density showed significance. Conclusions : It showed that there is a statistically significant correlation between lumbar spinal stenosis and bone mineral density.

정상 및 골다공증 여성의 파노라마 방사선사진 계측치 비교 (Comparison of panorama radiomorphometric indices of the mandible in normal and osteoporotic women)

  • Purpose : This study was conducted to compare the radiomorphometric indices of the mandible on panoramic radiographs among three groups of normal, osteopenia and osteoporosis and to determine whether panoramic indices have validity in predicting osteoporotic risk. Materials and Methods: Ninty-four postmenupausal women aged 42-72 years (mean 62±7.5) were examined using dual energy X-ray absorptionmetry (DEXA) of the lumbar and classified into three groups by WHO classification. Panoramic mandibular index (PMI), mandibular cortical thickness (Mental index (MI), Antegonial index (AI), Gonial index (G!)) and the mandibular cortical index (MCI) were measured bilaterally on panoramic radiographs and analyzed. Results: There were signigicant differences among normal, osteopenic and osteoporoti groups in PMI (I), PMI (S), MI, AI, GI and MCI. The sensitivities of the MCI (93%) and the combination index of MCI and MI (90%) in diagnosing osteopenic/osteoporotic cases were high. Conclusion: The widths and shapes of the mandibular cortical bone on panoramic radiography may highly reflect the systemic osteopenic/osteoporotic conditions of the patients.

CT 영상 기반 근감소증 진단을 위한 AI 영상분할 모델 개발 및 검증 (Development and Validation of AI Image Segmentation Model for CT Image-Based Sarcopenia Diagnosis)

  • 근감소증은 국내는 2021년 질병으로 분류되었을 만큼 잘 알려져 있지 않지만 고령화사회에 진입한 선진국에서는 사회적 문제로 인식하고 있다. 근감소증 진단은 유럽노인근감소증 진단그룹(EWGSOP)과 아시아근감소증진단그룹(AWGS)에서 제시하는 국제표준지침을 따른다. 최근 진단방법으로 절대적 근육량 이외에 신체수행평가로 보행속도 측정과 일어서기 검사 등을 통하여 근육 기능을 함께 측정할 것을 권고하고 있다. 근육량을 측정하기 위한 대표적인 방법으로 DEXA를 이용한 체성분 분석 방법이 임상에서 정식으로 실시하고 있다. 또한 MRI 또는 CT의 복부 영상을 이용하여 근육량을 측정하는 다양한 연구가 활발하게 진행되고 있다. 따라서 본 논문에서는 근감소증 진단을 위해서 비교적 짧은 촬영시간을 갖는 CT의 복부영상기반으로 AI 영상 분할 모델을 개발하고 다기관 검증한 내용을 기술한다. 우리는 CT 영상 중에 요추의 L3 영역을 분류하여 피하지방, 내장지방, 근육을 자동으로 분할할 수 있는 인공지능 모델을 U-Net 모델을 사용하여 개발하였다. 또한 모델의 성능평가를 위해서 분할영역의 IOU(Intersection over Union)를 계산하여 내부검증을 진행했으며, 타 병원의 데이터를 활용하여 동일한 IOU 방법으로 외부검증을 진행한 결과를 보인다. 검증 결과를 토대로 문제점과 해결방안에 대해서 검증하고 보완하고자 했다.

비만관련 생활습관병 위험인자 예측을 위한 다중 주파수 기반의 분할 체임피던스 측정법 (The Novel Method of Segmental Bio-Impedance Measurement Based on Multi-Frequency for a Prediction of risk Factors Life-Style Disease of Obesity)

  • The purpose of this study is to determine whether there is a correlation between the segmental bio-impedance measurement with the frequency modulations and the life-style disease of obesity. An obesity is not simply the factor for estimating the life-style disease of obesity, but also the risk factor occurring. There are many methods (BMI, WHR, Waist, CT, DEXA, BIA, etc.) for measuring a degree of obesity; the bio-impedance measurement is more economic and more effective than others. The physical examination, the blood test, the medical imaging diagnosis and the bio-impedancemeasurementswithmultiple frequencies for each body parts have been conducted for 77 people. The estimated value has been calculated through a segmental bio-impedance model based on multi-frequency that was created to reflect the highest correlation by analyzing correlation with linear regression analysis method for the measured bio-impedance and the risk factors. Then we compared with the clinical diagnosis. In case of high level cholesterol, low HDL-C and high LDL-C for life-style disease, the sensitivity is 80~100%and the specificity is 83~100%. This study has shown conclusively that bio-impedance can be a possible predictor to analyze the disease risk rate of population and individual health maintenance. And also the multi-frequency segmental bio-impedance can be used as early predictor to estimate the life-style disease of obesity.

연령과 골소주 특성의 골량에 대한 연관관계 (Relationship of bony trabecular characteristics and age to bone mass)

  • Purpose : Bony strength is dependent on bone mass and bony structure. So this study was designed to investigate the relationship between the bone mass and bony trabecular characteristics. Subjects and Methods : Study subjects were 51 females (average age 68.6 years) and 20 males (average age 66.4 years). Bone mineral density (BMD, $grams/cm^2$) of proximal femur was measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized radiographs of proximal femur. A customized computer program processed morphologic operations (MO) of ROIs. 44 skeletal variables of MO were calculated from ROIs on the Ward's triangle and greater trochanter of femur. WHO BMD classes were predicted by MO variables of the same ROI. Classification and Regression Tree analysis was used for calculating weighted kappa values, sensitivity and specificity of MO. Results : The discriminating factors of morphologic operation were branch point, branch point [per cm sq]. Age also played important role in distinguishing osteoporotic classes. The sensitivity of MO at Ward's triangle and Greater Trochanter was 91.8%, 65.6%, respectively. The specificity of MO was 100% at Ward's triangle and Greater Trochanter. Conclusion : Bony trabecular characteristics obtained using radiological bone morphometric analysis seem to be related to bone mass.
