Relationship of Glucose Control and Wound Infection in Diabetics after Lumbar Spine Surgery

  • Objective: The purpose of this study is to investigate the elevated blood glucose levels in the postoperative period are associated with an increased risk of deep wound infection in diabetic individuals undergoing lumbar spine surgery. Methods: Of 2896 patients who underwent lumbar spine operations by one surgeon between 1993 and 2002, 329(11.4%) were diabetics. The rate of deep wound infections in diabetic patients was 6.4%, versus 3.2% for nondiabetics. 152 patients had their operation before implementation of the protocol and 177 after implementation. Charts of the diabetic patients were reviewed. Mean blood glucose levels were calculated from documented results of finger-stick glucometer testing. Results: Twenty-one diabetic patients suffered deep wound infection. Infected diabetic patients had a higher mean blood glucose level through the first 2 postoperative days than noninfected patients($230{\pm}6.9$ versus $175{\pm}3.8mg/dL$; p<0.003) and had a long operation time($216{\pm}57.9$ versus $167.5{\pm}42.2$ minute; p<0.05). Multivariable logistic regression showed that mean blood glucose level for the first 2 postoperative days, long operation time, and use of the instrumentation(p<0.02) were all related predictiors of deep wound infection. Institution of a protocol of postoperative continuous intravenous insulin to maintain blood glucose level less than 200mg/dL was began in september 1997. This protocol resulted in a decrease in blood glucose levels for the first 2 postoperative days and a concomitant decrease in the proportion of patients with deep wound infection, from 8.3%(11/132) to 5.1%(10/195) (p<0.02). Conclusion: The incidence of deep wound infection in diabetic patients is reduced after implementation of a protocol to maintain mean blood glucose level less than 200mg/dL in the immediate postoperative period.

세 가지 방식의 근적외선 분광분석기를 이용한 글루코오스 수용액의 측정 (Aqueous Glucose Solution Measurement by Three Types NIR Spectrometer)

  • A method is described for measuring clinically relevant levels of glucose in a pH 7.4 phosphate buffer by nearinfrared (NIR) absorption spectroscopy. Three types of NIR spectrometer, dispersive type, photo-diode array (PDA) type, and fourier transform (FT) type spectrometer were used and the performance was compared. Spectra were collected with a cuvette cell or quartz liquid fiber of 1 mm or 2 mm optical pathlength as transmittance method. Glucose absorption band appeared at second overtone, first overtone, and combination region for all systems. By use of the multivariate technigue of partial least squares (PLS) regression, glucose concentrations can be determined with a 16, 44, and 9.1 mg/d l standard error of prediction for dispersive type, photo-diode array type, and fourier transform type system, respectively. Sensitivity of spectrometer was evaluated by absorbance for the difference of 10 mg/d l glucose. Three absorption bands, second overtone, first overtone, and combination region were suited to three types systems, dispersive type, photo-diode array type, and fourier transform type systems, respectively. This investigation showed that three types NIR spectrometer were proper method for identification and quantitative analysis of glucose and possible for noninvasive blood glucose monitoring.

포도당(葡萄糖) 및 부신피질(副腎皮質)홀몬제(劑)가 Candida albicans의 발육(發育)에 미치는 영향(影響)에 관(關)한 연구(硏究) (The Effect of Various Concentration of Glucose and Corticosteroid on the Growth of Candida albicans)

  • The growing behavior of Candida albicans in various concentration of glucose and corticosteroid media was studied with the method of modified hanging slide culture. The strains of Candida albicans used in this study were obtained from vaginal swab from outpatients and were isolated from cultured colonies on Sabouraud's glucose agar media. To detect the budding rate of blastospore, the diluted suspension of Candida albicans in normal saline were inoculated into various concentration of glucose (Gl, G2, G3, G4), corticosteroid (S1, S2, S3, S4) and corticosteroid with 10% pepton broth (D1, D2, D3, D4) respectively and cultured at room temperature $(22{\sim}25^{\circ}C)$. The number of budding of blastospore were counted under the high power field of light microscope (400X) at specific time interval, e.g, 1, 2, 3, 6, 9, 12, 18, and 24 hours after inoculation. The results are as following: 1. The most effective budding rate was seen in G4 media (1.25% glucose) in 18 hours aft inoculation (89%). 2. The budding rate in Sabouraud's glucose broth with various concentration of dexamethasone added, was not significantly different from that of simple Sabouraud's glucose broth within 18 hours after inoculation, but there was statistically. significant difference in two budding rate at 24 hours observation. 3. The budding rate in 10% pepton broth media with various concentration of dexamethasone was almost same budding rate in control group, which is normal saline and 10% pepton broth, except on 2 and 24 hours results.

마우스 T 세포 림프종 EL4 세포에 대한 metformin 단독 및 2-deoxy-D-glucose와 병용의 항암효과 (Anticancer effect of metformin alone and in combination with 2-deoxy-D-glucose on mouse T cell lymphoma EL4 cells)

  • Metformin is a treatment used widely for non-insulin-dependent diabetes mellitus with few side effects and acts by inhibiting hepatic gluconeogenesis and glucose absorption from the gastrointestinal tract. Lymphoma is one of the most common hematological malignancies in dogs. Chemotherapy is used mainly on lymphoma, but further research on developing anticancer drugs for lymphoma is needed because of its severe side effects. This study examined the anticancer effects of metformin alone and in combination with 2-deoxy-D-glucose (2-DG), a glucose analog, on EL4 cells (mouse T cell lymphoma). Metformin reduced the metabolic activity of EL4 cells and showed an additive effect when combined with 2-DG. In addition, cell death was confirmed using a trypan blue exclusion test, Hochest 33342/propidium iodide (PI) staining, and Annexin V/PI staining. An analysis of the cell cycle and mitochondria membrane potential (MMP) to investigate the mechanism of action showed that metformin stopped the G2/M phase of EL4 cells, and metformin + 2-DG decreased MMP. Metformin exhibited anticancer effects as a G2/M phase arrest mechanism in EL4 cells and showed additive effects when combined with 2-DG via MMP reduction. Unlike cytotoxic chemotherapeutic anticancer drugs, metformin and 2-DG are related to cellular glucose metabolism and have little toxicity. Therefore, metformin and 2-DG can be an alternative to reduce the toxicity caused by chemotherapeutic anticancer drugs. Nevertheless, research is needed to verify the in vivo efficacy of metformin and 2-DG before they can be used in lymphoma treatments.

  • In this study, a mechanism by which glucose level modulates glucose transport in Jurkat cells was investigated. Glucose uptake was more efficient in the cells cultivated in low glucose (2.5 mM) medium than that grown in high glucose (20 $\mu$M) medium. Vmax (0.74 n㏖/10$^6$ cells$\cdot$min) of glucose uptake measured with the cells grown in the low glucose medium was higher than the one (1.06 n㏖/10$^6$ cells$\cdot$min) in the high glucose medium while Km was almost consistent through the change of glucose levels, indicating the increase of glucose transporter number.

복수 개의 광파장에 대한 상대적 흡광 특성을 이용한 글루코스 농도 측정 (Prediction of the Glucose Concentration Based on Its Optical Absorbance at Multiple Discrete Wavelengths)

  • 본 논문에서는 복수 개의 측정 광파장 대역에서의 글루코스 수용액의 상대적인 흡광 특성을 이용한 글루코스 농도 예측 방법을 제안하고 검증하였다. 각 측정 파장에서의 상대적인 흡광도는 기준 파장에서의 흡광도를 기준하여 얻어진다. 선정된 기준 파장(1310 nm)과 네 개의 측정 파장(1064, 1550, 1685, 1798 nm) 대역에서는 글루코스에 대한 흡광도가 서로 반대의 부호를 갖도록 하였으며, 이 특성은 측정 정확도를 높이는데 도움이 된다. 최종적인 글루코스 수용액의 예측 농도는 각 측정 파장에서 얻어진 예측 값의 평균으로 결정된다. 5 mm의 광경로와 $0{\sim}1000mg/dL$ 농도 범위에서 실제로 측정된 글루코스의 흡광도를 살펴보면, 기준 파장 1310 nm에서는 $-1.42{\times}10^{-6}\;AU$/(mg/dL), 측정 파장 1685 nm에서는 $+8.12{\times}10^{-6}\;AU$/(mg/dL)로 최대였다. 그리고 제안된 방법을 이용하여 글루코스 용액의 농도를 예측할 경우 얻어진 표준예측오차(SEP: standard error of prediction)는 ${\sim}28\;mg/dL$였다. 또한, 온도와 지방층이 글루코스 농도 측정에 미치는 영향을 조사하였다. 먼저 $26{\sim}40^{\circ}C$ 온도 범위에서 측정된 흡수량 변화율은 기준 파장 1310 nm에서 $-9.1{\times}10^{-5}\;AU/^{\circ}C$였고, 측정 파장 1550 nm에서 $-2.08{\times}10^{-2}\;AU/^{\circ}C$였다. 그리고 글루코스 수용액에 존재하는 지방층 두께에 따른 흡수량 변화율은 1685 nm 파장 대역에서 +1.093 AU/mm로 측정되었다.

Coconut-derived D-xylose affects postprandial glucose and insulin responses in healthy individuals

  • Metabolic alterations including postprandial hyperglycemia have been implicated in the development of obesity-related diseases. Xylose is a sucrase inhibitor suggested to suppress the postprandial glucose surge. The objectives of this study were to assess the inhibitory effects of two different concentrations of xylose on postprandial glucose and insulin responses and to evaluate its efficacy in the presence of other macronutrients. Randomized double-blind cross-over studies were conducted to examine the effect of D-xylose on postprandial glucose and insulin response following the oral glucose tolerance test (OGTT). In study 1, the overnight-fasted study subjects (n = 49) consumed a test sucrose solution (50 g sucrose in 130 ml water) containing 0, 5, or 7.5 g D-xylose powder. In study 2, the overnight-fasted study subjects (n = 50) consumed a test meal (50 g sucrose in a 60 g muffin and 200 ml sucrose-containing solution). The control meal provided 64.5 g of carbohydrates, 4.5 g of fat, and 10 g of protein. The xylose meal was identical to the control meal except 5 g of xylose was added to the muffin mix. In study 1, the 5 g xylose-containing solutions exhibited significantly lower area under the glucose curve (AUCg) and area under the insulin curve (AUCi) values for 0-15 min (P < 0.0001, P < 0.0001), 0-30 min (P < 0.0001, P < 0.0001), 0-45 min (P < 0.0001, P < 0.0001), 0-60 min (P < 0.0001, P < 0.0001), 0-90 min (P < 0.0001, P < 0.0001) and 0-120 min (P = 0.0071, P = 0.0016). In study 2, the test meal exhibited significantly lower AUCg and AUCi values for 0-15 min (P < 0.0001, P < 0.0001), 0-30 min (P < 0.0001, P < 0.0001), 0-45 min (P < 0.0001, P = 0.0005), 0-60 min (P = 0.0002, P = 0.0025), and 0-90 min (P = 0.0396, P = 0.0246). In conclusion, xylose showed an acute suppressive effect on the postprandial glucose and insulin surges.

1-Deoxynojirimycin Content and Blood Glucose-Lowering Effect of Silkworm (Bombyx mori) Extract Powder

  • We investigated the 1-deoxynojirimycin (1-DNJ) content of extracts from silkworm larvae at each developmental stage within three silkworm varieties. We also compared the content of the following polyhydroxylated alkaloids in the silkworm extracts: 1-DNJ, fagomine, and 1,4-dideoxy-1,4-imino-d-arabinitol (DAB). In addition, we evaluated the glucose-lowering effects of silkworm extract powder in db/db mice. The 1-DNJ content was the highest in Yeonnokjam $5^{th}$ instar $3^{rd}$ d larvae and Hansaengjam $5^{th}$ instar $3^{rd}$ d larvae, which contained 18.4 mg/100 g dry weight and 18.3 mg/100 g dry weight, respectively. The $5^{th}$ instar $3^{rd}$ d larvae exhibited a higher content of 1-DNJ than that of $5^{th}$ instar $5^{th}$ d larvae among all varieties. The glucose-lowering effects of silkworm extracts and Yeonnokjam powder were tested on db/db mice, and the blood glucose levels were found to decrease significantly in the YR70 group. Silkworm extracts (180 mg/kg, 90 mg/kg, 45 mg/kg, and 22.5 mg/kg) and acarbose (50 mg/kg) were administered orally for 4 wk. Changes in water intake were not statistically significant between control and silkworm extract-treated groups. Compared to the control group, blood glucose levels in the silkworm extract powder-treated group decreased in the 22.5 mg/kg/d group after being administered for 4 wk. This decrease was statistically significant. Furthermore, biochemical changes in the AST(Aspartate aminotransferase), ALT(Alanine aminotransferase), TCHO(Total Cholesterol), TG(Triglyceride), LDL(Low density lipoprotein), and HDL(High density lipoprotein) levels in blood were not observed. However, statistically significant decreases in blood GLU in the 22.5 mg/kg/d group compared to that of the control group occurred. In addition, the epididymal fat weight of the silkworm extract powder-treated group decreased significantly in both the 22.5 mg/kg/d group and 180 mg/kg/d group compared to that of the control group, but there were no statistically significant changes in perirenal fat weight. These results demonstrate that silkworm extracts inhibit changes in blood glucose levels in model diabetic mice.

고지방식이(HFD)/stereptozotocin(STZ) 유도 당뇨모델에서 여주가 글루카곤 분비에 미치는 영향 (Effect of Momordica charantia on Glucagon Secretion in High-fat diet(HFD)/Streptozotocin(STZ)-induced Diabetic Rat)

  • In present study, we investigated the antidiabetic effect of Momordica charantia(as well known "bitter melon"). This study was conducted to determine antidiabetic mechanism of Bitter Melon Extract (BME). We measured blood glucose, insulin, glucagon level in a Sprague-Dawley rat model of high-fat diet/streptozotocin(HFD/STZ)-induced diabetes. Five experimental groups were used: normal, HFD/STZ, BME 62.5 mg/kg HFD/STZ, BME 125 mg/kg HFD/STZ and BME 250 mg/kg HFD/STZ. BME was orally administered to the rats every other day for 9 weeks. Results showed that fasting blood glucose levels were significantly lower in the BME 125 mg/kg(150.17 ± 20.22 mg/dL) and 250 mg/kg(124.17 ± 22.17 mg/dL) groups than in the vehicle group(188.83 ± 26.63 mg/dL)(p<0.05). In addition, glucagon levels were lower in the three BME treatment groups than in the vehicle group(p<0.05). Oral glucose tolerance tests revealed that the BME 250 mg/kg group had significantly(p<0.05) reduced 120-minute blood glucose levels and areas under the curve. Our results suggest that BME induces antidiabetic effects via the reduction of glucagon and blood glucose levels.

구아바 잎 열수추출물의 당류분해효소 저해 효과 및 OGTT에 미치는 효과 (Effect of Aqueous Extract of Guava (Psidium guajava L.) Leaves on the Oral Glucose Tolerance Test and Inhibition of Glycoside Hydrolase)

  • Guava ($Psidium$ $guajava$) contain a great deal of polyphenol compound and work on the treatment of $Diabetes$ $mellitus$ effectively. In this study, the bioactivities of aqueous extract (GLEx) of guava leaf were investigated. Total phenolic contents of GLEx was 266.9 mg tan/g. The effects of GLEx on digestive enzymes, ${\alpha}$-amylase, maltase and sucrase were investigated. $IC_{50}$ values of GLEx against ${\alpha}$-amylase, maltase and sucrase were 0.65 mg/$m{\ell}$, 2.0 mg/$m{\ell}$ and 3.5 mg/$m{\ell}$ respectively. The effect of GLEx on oral glucose tolerance test (OGTT) was performed in normal ICR mouse, control (dstilled water) and GLEx (aqueous extract of Guava leaf) were co-administered orally with glucose, showed reducing effect on the blood glucose level. The guava is likely to useful for prevention or improvement of hyperglycemia by lowering the blood glucose level and inhibiting glycoside hydrolase activity.
