미세피판기법을 이용한 성대낭종제거술 (Microsurgical Extirpation of Intracordal Cyst with a Microflap Technique)

  • Background and Objectives : Microsurgical resection of intracordal cysts is technically difficult and challenging because the wall of cysts may be tightly attached to underlying vocal ligament and/or overlying epithelium, and therefore their thin wall will easily rupture during surgical dissection. We aimed to evaluate the voice outcomes of standard microflap subepithelial resection and the recurrence rate depending on the intraoperative rupturing of the cyst. Materials and Methods : Medical records of Samsung Medical Center, Seoul, Korea, were reviewed for sixty-four consecutive patients who received surgical resection of vocal cyst using microflap subepithelial dissection technique between the year 2004 and 2013. Meticulous dissection was performed to completely remove the cyst wall while preserving the mucosa and the lamina propria as much as possible. Voice outcomes and recurrence rates were compared according to the type, size and the intraoperative rupture of cyst. Results : Presence or absence of cyst rupture was clearly described in the operation records of 41 patients. Intraoperative rupture of the cyst occurred in 32 of 41 (78%) patients. The recurrence was detected in 5 of 64 (7.8%) total cases and 4 of 32 (12.5%) cases of ruptured cyst, but not in 9 cases of intact extirpation. Rupture was more common in case of mucous retention cyst compared with epidermoid cyst (p=0.036). Subjective and objective voice parameters were measured at before and 3 months after surgery, which improved regardless of the cyst rupture. Conclusion : Although complete microsurgical extirpation of intracordal cyst while keeping the cyst wall intact is technically difficult, meticulous dissection with maximal preservation of surrounding tissue may warrant the improvement of voice outcomes.

  • Thymic cyst is a relatively uncommon,usually asymptomatic benign lesion situated in the anterior mediastinum or in the neck,and sometimes in both regions. This is a report of two cases of congenital thymlc cyst in a 18 year old girl and a 43 tear old man. Those were treated with extirpation of the cystic lesion by thoracotomy successfully and had good postoperative results.

  • Cystic intrathoracic lesions of foregut origin are now well recognized and account for approximately 10% of lesions presenting as mediastinal tumors. The terminology used to describe mediastinal endodermal cysts has been confused and sometimes ambiguous. The embryological derivation of these lesions has been the cause of much speculation. It Is suggested that these lesions should be classified Into three main categories based on embryology bronchogenic cyst[resulting from a defect of lung budding], Intramural esophageal cyst[true duplication], and enteric cyst[resulting from the split notochord syndrome]. This communication describes a 26 year old man with intramural esophageal cyst who was diagnosed as posterior medlastlnai tumor preoperatively and cured with extirpation of the cyst.

골결손을 동반한 비구개관 낭종의 치험례 (A Clinical Experience of Nasopalatine Duct Cyst with Bony Defect)

  • The nasopalatine duct cyst, known as the incisive canal cyst, is the most common nonodontogenic cyst in the maxillofacial area. It is believed to arise from epithelial remnants of the embryonic nasopalatine duct. Nasopalatine duct cysts are most often detected in patients between forties and sixties. The trauma, bacterial infection, or mucous retention has been suggested as etiological factors. The cysts often present as asymptomatic swelling of the palate but can present with painful swelling or drainage. Radiologic findings include a well demarcated cystic structure in a round, ovoid or heart shape presenting with a well-defined bone defect in the anterior midline of the palate between and posterior to the central incisors. Most of them are less than 2cm in size. On MRI, the cyst is identified as a high-intensity, well-marginated lesion, which indicates that it contains proteinaceous material. We experienced a case of a 61-year-old female patient who had a $2.3{\times}2.6{\times}1.7cm$ sized nasopalatine duct cyst. The bony defect after a surgical extirpation was restored with hydroxyapatite. So we report a good results with some reviews of the literatures.

만삭 크기 거대 난소 낭종의 복강경 적출술 (Laparoscopic Extirpation of the Term Sized Huge Ovarian Cyst)

  • A 23 years old single nulligravida woman underwent laparoscopic removal of a huge cystic adnexal mass that occupied her entire abdomen, giving the appearance of a full term pregnancy. After anesthesia, a vertical infra-umbilical incision, 1 cm long, was made and a telescope was introduced through the port to determine the status of the intra-abdomen and the surface contour of the mass. A needle tipped with a laparoscopic suction apparatus was inserted into the cyst through the infra-umbilical port, directly under the mass. Subsequently, 3,200 ml of cystic fluid was aspirated without spillage. A huge cyst, reaching to the level of the xyphoid process was effectively excised through the operative laparoscopy after prelaparoscopic drainage. Operation time was 140 minutes and hospital stay was 2 days. There were no complications during hospital stay and after discharge. It seems the size of the cyst is not a criteria for the contraindication of laparoscopic surgery.

종격동 종양의 임상적 고찰 (Clinical Evaluation of the Mediastinal Tumors)

  • Generally early surgical extirpation is considered to be beneficial for the treatment of mediastinal tumors and prevention of its complication. The review of the clinical course, histopathologic characteristics and current diagnostic and treatment modalities will help us to decide when to treat and how to treat mediastinal tumors. For this purpose clinical analysis of the 42 patients who were surgically treated from July 1984 to August 1993 in the Chung Ang University Hospital is done in this report. There were 24 males and 18 females[1.3:1], and their age ranged from 1 year to 75 years. Nonspecific symptoms such as chest pain, cough and dyspnea were the most frequently encountered symptoms. Asymptomatic patients were relatively common[21.4%]. The tumors were found most frequently in the posterior mediastinum[45.2%]. Anterior mediastinum was the next and middle and upper mediastinum showed relatively rare occurrence of the tumor. Neurogenic tumor was the most common histopathologic type . Germ cell tumor, benign cyst and thymoma followed it. Curative resection was possible in 27 patients[64%], partial resection in 6 patients[14.3%]and just open biopsy in 7 patients[16.7%]. There was no early operative mortality. Postoperative complications were occurred in 5 patients[11.9%] and most of them were not serious.
