Keratocystic odontogenic tumors can occur in any area of the maxilla or mandible. According to their size, location, and relations with surrounding structures, they are treated by cyst enucleation or enucleation after either marsupialization or decompression. Enucleation is performed when cysts are not large and when only minor damage to adjacent anatomical structures is expected. Although marsupialization and decompression follow the same basic bone-regeneration principle, which is to say, by reducing the pressure within the cyst, the former leaves a large defect after healing due to the large fistula necessary to induce the conversion of the cyst-lining epithelia to oral epithelia; the latter leaves only a relatively small defect, because of the continuous washing carried out by means of a tube inserted into a small hole in the cyst. In the latter case too, a decompressor appropriate for the focal position is required, owing to the importance of maintaining the device and controlling for oral hygiene. We report herein decompression treatment with a patient-customized device for an extensive cyst in the anterior region of the mandible.
Much has been written about the possibility that the lining of a dentigerous cyst might undergo neoplastic transformation to an ameloblastoma which is defined as mural ameloblastoma. Mural ameloblastomas are estimated to constitute 5% of all ameloblastomas. 85% of the mural ameloblastomas were associated with dentigerous cyst, all of which were observed before the age of 30. The frequency of recurrence after simple enucleation of the cyst containing this lesion is considerably lower than that for ameloblastomas in general that are treated in a similar manner. Therefore, further and more extensive surgery is not required for mural ameloblastoma. We diagnosed a cystic appearance of a 17 year-old female as dentigerous cyst and undergone marsupialization. For 2 years of period, no significant change was observed and therefore enucleation was undergone and diagnosed as ameloblastoma. We report this case with a review of related literatures.

Various treatment methods for huge cystic lesion of the jaw exist, such as, resection of the involved bone, enucleation and decompression. Among these methods, enucleation after decompression is a conservative technique that decreases the size of the cystic cavity and reduces the risk of intrabony defects, which could be induced by primary enucleation. In addition, it can save the adjacent anatomic structures. In these cases, the decompression combined with partial enucleation, which was performed before complete cyst enucleation was performed on huge cystic lesions of the mandible. During the process, a decrease in the size of the lesion and the growth of normal oral tissues was observed. The size of the lesion decreased until the time of complete enucleation, and surgery could be performed under local anesthesia. No damage to inferior alveolar nerve was observed. We report these cases with a review of the relevant literature.
A radicular cyst is a slowly-growing epithelial sac lining a pathologic cavity in the bone at the apex of a tooth. Large radicular cyst revealed by radiographic examination almost always appear to involved several teeth. The author observed a 31 year old woman with a labial swelling and palatal mucosal expansion of the maxillary anterior region. Radiographic examination indicated that the lesion extended from the left maxillary lateral incisor to the right lateral incisor. Cyst consisted of tan to gray yellow slightly firm to partly hard plaque like tissue measuring 3.4 x 2.2 x 0.3㎝. The author has treated the cyst by means of enucleation with apicoecomies of the involved teeth except the right central incisor. Recovery proceed satisfactorily and there were no clinical or radiologic signs of recurrence 8 months later.
The dentigerous cyst originates through alteration of the reduced enamel epithelium after amelogenesis is completed, with accumulation of fluid between the layers of the enamel epithelium or between this epithelium and the tooth crown. It is always associated with unerupted teeth. There are two methods of treament which are generally accepted. One is the enucleation of the cystic sac in its entirety, the other is the Partch operation, or the marsupialization, by which the cyst is uncovered or "deroofed" and the cystic lining made continuous with the oral avity or surrounding structures. The present case report showed that in instance of dentigerous cyst & odontogenic keratocyst surgical enucleation followed by careful excision of all cyst wall was successfully carried out and recovery and healing were rapid and uncomplicated.
The so-called eruption cyst, which occurs mostly on the third molar, is dentigerous cyst, arising at a late stage of tooth development. The authors have treated a dentigerous cyst occurred in angle region of the left mandible in 27-year-old woman who has complained of the facial asymmetry, by means of enucleation of the cyst in tis entirety and tooth removal, and resulted in a fairly good prognosis.

Purpose: Dentigerous cyst is the most common intra-osseous lesion of the jaw. Dentigerous cysts can cause delays in eruption of the affected permanent tooth in mixed dentition. It has been suggested that the affected permanent tooth could be erupted spontaneously after the dentigerous cyst was enucleated. But in some cases, orthodontic treatment or autotransplantation technique is known to be required. This study reviews previously performed prognoses of affected permanent teeth, which will lead to a more efficient treatment plan. Patients and Methods: With 28 patients who have undergone cyst enucleation and 10 patients who have undergone autotransplantation, the prognosis of permanent teeth was observed. Results: After cyst enucleation, spontaneous eruption of a permanent tooth was observed in 56.3% patients, orthodontic treatment was performed in 25% patients. The success rate of autotransplantation was 60.0%. Discussion: The first choice for treatment of dentigerous cyst in mixed dentition is to guide spontaneous eruption of permanent teeth. For cases without enough eruption space, the orthodontic treatment should be considered. Autotransplantation should be considered when the spontaneous eruption is not expected. It should be considered that the rate of successful autotransplantation is decreased on maxilla anterior area.
Dentigerous cyst is an odontogenic cyst which occurs in unerupted tooth crown. After the crown formation, enamel epithelium remnants surrounded continuously proliferates and it forms effusionfluid cyst and expands due to increased internal osmotic pressure. Treatments of cysts are mainly enucleation, marsupialization and de-compression. When deciding the way of treatment, the age of a patient, the anatomical circumstances, the region of lesion and the size of cyst should be considered. Marsupialization is that some parts of internal cystic wall would be converted into oral mucosa if the cyst is large size and is concerned about neighboring anatomic structure. It can be accompanied by enucleation later and eruption of related tooth can be possible. If there is a limitation of spontaneous tooth eruption, eruption of tooth can be induced by orthodontic apparatus. There were 3 patients had dentigerous cyst and underwent marsupialization, their impacted teeth had preserved and had induced eruption, all showing satisfactory results.
The author has diagnosed and treated the dermold cyst patient in the 41 years old korean man. The dermold cyst in the floor of the mouth had various sign and symptom such as elevated tongue, disturbance of speech and swallowing, and etc. The cyst wall of dermoid cyst was enucleated by using of direct enucleation of cyst wall and primary suture technique. The mass of the enucleated dermoid cyst was 5×5.5×6 Cm.in size.
악골의 낭종은 크게 치성과 비치성으로 구분되며 치성낭은 치성기관이나 그 잔존 세포성분과 관련되어 주로 골내에서 발생한다. 비치성낭종은 과거에는 태아의 발육중 상피의 봉입이나 융합선을 따라 존재하는 상피잔사가 증식하여 낭성변형을 일으킨다는 설이 유력하였으나 현재에는 이러한 태아의 발육 동안상피의 봉입은 일어나지 않는 것으로 보고 있다. 함치성낭은 미맹출 치아와 관련되어 발생하고 상피의 전암 가능성이 있으므로 가능한 발리 제거하도록 하며 조대술이나 적출술이 이용될 수 있고 간혹 원인치의 발거가 추천된다. 비구개낭은 가장 일반적인 비치성낭으로 상악 중절치 사이에 경계가 명확한 방사선 투과상으로 나타난다. 적출술과 조대술로 치료할 수 있으며 전암 가능성이나 재발은 드물다. 본 증례에서는 전북대학교 병원 소아치과에 내원한 아동의 악골내 발생한 낭종을 외과적으로 제거한 후 다음과 같은 결론을 얻었다. 1. 함치성낭의 환아에서, 원인 치아를 보존하고 완전 적출술로 병소를 제거한 후 경과를 관찰한 바 영구치의 정상적인 맹출이 이루어졌다. 2. 비구개낭을 치료시, 인접치아의 생활력에 대한 정확한 진단과 그에 따른 치료법의 선택으로 불필요한 근관치료를 피할 수 있다. 3. 향후 비구개낭의 환아의 관련치에 대한 계속적인 관찰이 필요하리라 사료된다. 4. 악골내 방사선 투과성의 병소를 치료시, 정확한 감별진단과 적절한 치료법의 선택이 중요하다.
