Results of Anterior Cruciate Ligament Reconstruction with Unicondylar Arthroplasty for Medial Compartment Knee Osteoarthritis combined with Anterior Instability (전방 불안정성과 동반된 슬관절 내측 구획 진행성 관절염환자에서 전방십자인대 재건술 및 인공 관절 부분 치환술의 결과 - 3예 보고 -)

  • Three cases who had medial compartment osteoarthritis of the knee (Kellgrene-Laurence grade 3 and Outerbridge grade 4) and anterior instability of the knee due to rupture of the anterior cruciate ligament in relative young ages underwent staged anterior cruciate ligament reconstruction followed by medial unicondylar arthroplasty in 2 cases and simultaneous anterior cruciate ligament reconstruction and unicondylar arthroplasty. We evaluated clinical results some kinds of preoperative and postoperative International Knee Documentation Committee (IKDC), Lysholm score and last follow-up hospital for special surgery (HSS), knee society score (KSS). We consider that medial unicondylar arthroplasty with staged or simultaneous anterior cruciate ligament reconstruction is very good option of the treatment for the anterior instability and pain from advanced arthritis.

Conservative Treatment for Injured Anterior Cruciate Ligament - Two Cases Report - (손상된 전방십자인대의 보존적 치료 - 증례 보고 -)

  • While the cruciate ligament has a profuse vascular response following injury, spontaneous repair does not occur. This may result from the fact that synovial fluid dilution of the hematoma following injury prevents the formation of a fibrin clot and thus the initiation of the healing mechanism. Another theory suggests that the dynamic nature of the fascicles of the anterior cruciate ligament(ACL) through even small ranges of motion prohibits spontaneous union to these fibers. But we experienced two cases of spontaneous healing of partially injured ACL. Initially they showed more than grade II anterior instability. 6 mm difference by stress roentgenographs(pull view) and difference of 8 mm by KT 1000TM arthrometer between the ACL injured knee and normal side knee. Lax, nearly complete tear of ACL and synovial bleeding were noted during arthroscopic examination but the continuity of synovial membrane was seemed to be intact. These cases were treated by conservative management rather than reconstructive procedure. Postoperatively they showed excellent clinical results, no anterior instability and unlimited athletic activity. Based on our clinical experience. we think that cruciate ligament has the spontaneous healing potential in acute stage and middle aged patient. We consider the microfracture technique and initial immobilization for accelleration of healing response of the ACL.

Avulsion of the Tibial Attachment of the Posterior Cruciate Ligament in a 13-year-old child - A Case Report - (13세 소아에서 후방십자인대의 경골 부착 부위의 견열 골절 - 증례보고-)

  • The authors report one case of isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament. A 13 years old boy was admitted for right knee pain and swelling after the soccer injury on his knee. Hemarthrosis and posterior instability were present. Simple roentgenographic examination showed an isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament. Open approach through the popliteal fossa was used and bony fragment was fixed into its bed using pull-out sutures. Postoperatively, a long leg cast was applied for 4 weeks. The patient was followed for 6 months. He returned to his previous activity levels. There was no residual laxity. Isolated avulsion of the posterior cruciate ligament from the tibia before the epiphyseal fusion is very rare in children. We reported one case of our experience and its result.

  • The most recent concept in anterior cruciate ligament reconstruction is an anatomical single bundle anterior cruciate ligament reconstruction. For an anatomical anterior cruciate ligament reconstruction, the tibial tunnel is made anterior than before, and the femoral tunnel is made in a lower and oblique direction compared to the classical method using the transtibial technique. The anteromedial portal technique, outside-in technique, and modified transtibial technique have been performed to produce femoral tunnels with anatomical positions. Each method has different advantages and disadvantages and is chosen based on the operator's preferences, experience, instruments, and implants.

The Effects of Knee Brace on Anterior Cruciate Ligament Injuries Risk Factors During One-Legged Landing of Female Gymnasts (여자 체조선수들의 한 발 드롭 착지 시 무릎보호대가 전방십자인대 부상 위험요인에 미치는 영향)

  • The purpose of the study was to investigate the effects of knee brace on anterior cruciate ligament injuries risk factors during one-legged landing of female gymnasts. Eleven female gymnasts were recruited and performed randomly one-legged drop landing in height of her's knee with (3times) and without (3times) knee brace. Kinematics and ground reaction data were collected to estimate the anterior cruciate ligament injuries risk factors. Data were analyzed with paired samples t-test. Female gymnasts with knee brace showed more reduced the distance from ankle joint center to knee joint center in sagittal plane and knee maximum joint torque than without knee brace. In conclusion, Female gymnasts with knee brace reduced anterior cruciate ligament stress.

Comparison of the Results of One-Incision Technique Versus Two-Incision Technique of the Arthroscopic Posterior Cruciate Ligament Reconstruction using Bone-Patellar Tendon-Bone Graft (일절개법을 이용한 관절경적 후 십자 인대 복원술 - 이절개법과의 비교 -)

  • This study was done to compare the results of the one-incision technique and the conventional two incision technique for the arthroscopic treatment of the posterior cruciate ligament injury. Fifty-five patients with the posterior cruciate ligament injury underwent the arthroscopic posterior cruciate ligament reconstruction using bone-patellar tendon-bone(BTB) graft. Patients with combined ligament injuries requiring concomitant operative treatment were excluded in this study. The conventional two-incision technique was performed in ten patients(Group I) and the one-incision technique in forty-five patients(Group II). The average duration of follow-up was 45 months in Group I(range, 40 to 50 months) and 36 months in Group II(range, 24 to 68 months). Auto BTB grafts were utilized for all patients in Group I. In Group II, 34 BTB autografts and 11 BTB allografts were utilized. The functional results were evaluated according to the Lysholm Knee Scoring scale and the Hospital for Special Surgery(HSS) knee ligament rating form. The postoperative posterior laxity was measured with a KT 1000 or 2000 arthrometer. Lysholm postoperative mean values were 90.0 in Group I and 90.6 in Group II. HSS mean values were 87.7 in Group I and 92.6 in Group II. HSS postoperative mean value showed better results in Group II(p=0.037). The average side-to-side difference of the posterior translation measured by the KT 2000 arthrometer were 2.10 mm(range, 1 to 4 mm) in Group I and 2.38 mm(range, 0 to 5 mm) in Group II. But there was no statistically significant difference. In Group II, the results of the autograft and allograft showed no significant difference. The arthroscopic posterior cruciate ligament reconstruction using one-incision technique showed good results comparable to the conventional two-incision technique. This technique minimizes potential injury to the extensor mechanism, especially vastus medialis obliquus, and scar formation over the medial femoral condyle. The operation can be finished within one tourniquet time by using only one-incision.

Effect of Closed and Open Kinetic Chain Exercise after Cruciate Ligament Reconstruction (십자인대 재건술 후 닫힌사슬운동과 열린사슬운동의 효과)

  • Open kinetic chain exercise has lost favour in rehabilitation after cruciate ligament reconstruction due to concerns that this exercise is harmful to the graft and will be less effective in improving function. Therefore rehabilitation has focused over the past decade on closed kinetic chain exercise. Open kinetic chain and closed kinetic chain exercises were compared for their effects on proprioception, muscle strength and knee instability in the early period of cruciate ligament reconstruction rehabilitation. The study subjects were 14 patients in 28weeks from cruciate reconstruction surgery(11 male, 3 female; mean age = 44.36 years). Closed kinetic chain exercise group used ball, balance pad and air cushion, to perform weightbearing exercises and the open kinetic chain exercise group used elastic rope and N-K table, to perform non-weightbearing exercises. Between tests, subjects trained 5 times per week for 2 weeks. Statistical analysis was by Wilcoxon signed rank test and Mann Whitney U test. In result, this study shows that both open and closed kinetic chain exercise programs lead to an improved muscle strength and Lysholm score. But there was no improvement in proprioception at both exercises. Closed versus open kinetic chain exercise in early period of rehabilitation after cruciate ligament reconstruction surgery do not differ in their effects on knee proprioception, muscle strength and instability. But the effect of closed kinetic chain exercises was showed more improvement than open kinetic chain exercises between pre-post exercises. Therefore further study is required to assess effect of both groups in more long period.
