Immediate implant placement and provisionalization of single implant in the esthetic region : Cases report (심미적 영역에서 즉시 임플란트 식립 및 임시 수복을 통한 단일치의 수복에 대한 증례 보고)

  • In the anterior maxillary area, dental implants for tooth replacement are challenging due to the need to satisfy high esthetic level as well as functionality. Immediate implant placement and provisionalization can dramatically reduce the edentulous period, and then fulfill patient's demand for esthetics. The aim of present case report is to demonstrate two cases that successfully restored single tooth with immediate implant placement and provisionalization in the anterior maxillary area. A 47 years old female was scheduled to replace her maxillary right central incisor due to crown-root fracture by trauma. Another 54-year-old female was planned to place dental implant following tooth extraction of maxillary right lateral incisor owing to continuous pus discharge despite repetitive treatments including apicoectomy. In these two cases, surgical and prosthetic procedures progressed in a similar way. After minimal flap elevation, atraumatic tooth extraction was performed. Implant was placed in proper 3-dimensional position and angulation with primary stability. Bone graft or guided bone regeneration for peri-implant bone defect was conducted simultaneously. Provisionalization without occlusal loading was carried out at the same day. Each definitive crown was delivered at 7 and 5 months after the surgery. Two cases have been followed uneventfully for 2 to 5 years of loading time. In conclusion, Immediate implant placement and provisionalization could lead to esthetic outcome for single tooth replacement with dental implant under proper case selection.

Corrosion Resistance Evaluation in the Co-Cr Alloys for the Full and Removable Partial Denture Metal Frameworks and the Porcelain-fused-to-metal Crown (총의치와 국소의치 금속의치상용 코발트-크롬 합금과 금속소부도재관용 코발트-크롬 합금의 부식저항성 평가)

  • Purpose: This study was conducted to evaluate the corrosion resistance of metal ions of alloys and use the results as the dental health data. These were performed by examining the corrosion levels of Co-Cr alloys for the full and removable partial denture metal frameworks and porcelain-fused-to-metal crown, among the dental casting nonprecious alloys. Methods: The alloy specimens (N = 10) were manufactured in $15mm{\times}10mm{\times}1.2mm$ and stored in two types of corrosive solutions at $37^{\circ}C$ for seven days. The metal ions were quantitatively analyzed using the Inductively Coupled Plasma-Atomic Emission Spectrometer. Results: Of the three Co-Cr alloys, the Co ion concentration of the porcelain-fused-to-metal alloy was 1.512 ${\mu}g/cm^2$, which indicated the highest metal ion dissolution. The metal corrosion was higher in the more acidic pH 2.2 solution compared with the pH 4.4 solution. In all three Co-Cr alloys, Co ion dissolution was predominant in the two corrosive solutions. Conclusion: The corrosion resistance of the three Co-Cr alloys was high, indicating a good biocompatibility.

Effect of yttrium additives on the shear bond strength of porcelain fused to Ni-Cr alloy for porcelain fused metal crown (도재용착용 Ni-Cr계 합금의 이트륨 첨가물이 도재전단결합강도에 미치는 영향)

  • Purpose: In this study, to evaluate the effect of oxide changes on the shear bond strength according to the composition of Ni-Cr alloys for porcelain fused matal crown, T-4 alloys, Zeroy alloys and Zeroy-X alloys were selected. Methods: 20 specimens were fabricated using selected Ni-Cr alloys and porcelain powders. A Ni-Cr alloy having a diameter of 5 mm and a height of 25 mm was produced and the metal surface was polished. Porcelain powder was fired on the polished metal surface to a diameter of 5 mm and a height of 3 mm. The experiment group consisted of three groups, T-4(TNA), Zeroy(ZNA) and Zeroy-X(ZXA). The fabricated specimens were mounted on a jig of a universal testing machine(UTM) and fracture strength was measured by applying a shear force at a UTM crosshead speed of 0.5 mm/min. The fracture strength was calculated as the bond strength between the porcelain and the alloy. The surface of the fractured alloy was analyzed by X-ray diffraction(XRD) and scanning electron microscopy(SEM), and the components of the oxide were measured by energy dispersive X-ray spectroscopy(EDX) line profile method. Results: In SEM, XRD and EDX analysis, yttrium tended to increase the mechanical and chemical bonding forces. The shear bond strength of ZXA group containing yttrium showed the highest value at 27.53 MPa. Conclusion: Based on the results of this study, it is considered that the yttrium-added Ni-Cr alloy is clinically acceptable in porcelain shear bond strength.

Cone-beam computed tomography-guided three-dimensional evaluation of treatment effectiveness of the Frog appliance

  • Objective: To evaluate the effectiveness of the Frog appliance in three dimensions by using cone-beam computed tomography (CBCT) images. Methods: Forty patients (21 boys and 19 girls), averaged 11.7 years old, with an Angle Class II division 1 malocclusion were included in our study. They had either late mixed dentition or early permanent dentition, and the maxillary second molars had not yet erupted. All patients underwent CBCT before and after the treatment for measuring changes in the maxillary first molars, second premolars, central incisors, and profile. Paired-samples t-test was used to compare the mean difference in each variable before treatment and after the first phase of treatment. Results: The maxillary first molars were effectively distalized by 4.25 mm (p < 0.001) and 3.53 mm (p < 0.05) in the dental crown and root apex, respectively. The tipping increased by $2.25^{\circ}$, but the difference was not significant. Moreover the teeth moved buccally by 0.84 mm (p < 0.05) and 2.87 mm (p < 0.01) in the mesiobuccal and distobuccal cusps, respectively, whereas no significant changes occurred in the root apex. Regarding the anchorage parts, the angle of the maxillary central incisor's long axis to the sella-nasion plane increased by $2.76^{\circ}$ (p < 0.05) and the distance from the upper lip to the esthetic plane decreased by 0.52 mm (p = 0.01). Conclusions: The Frog appliance effectively distalized the maxillary molars with an acceptable degree of tipping, distobuccal rotation, and buccal crown torque, with only slight anchorage loss. Furthermore, CBCT image demonstrated that it is a simple and reliable method for three-dimensional analysis.

FRACTURE STRENGTH OF THE IPS EMPRESS CROWN: THE EFFECT OF OCCLUSAL DEPTH AND AXIAL INCLINATION ON UPPER FIRST MOLAR (IPS Empress 도재관의 파절강도 : 상악 제1대구치에서 교합면 두께와 축면경사도에 따른 영향)

  • The purpose of this study was compare the fracture strength of the IPS Empress ceramic crown according to the occlusal depth (1.5mm, 2.0mm, 2.5mm) and axial inclination ($4^{\circ},\;8^{\circ},\;12^{\circ}$) of the upper first molar. After 10 metal dies were made for each group, the IPS Empress ceramic crowns were fabricated and were cemented with resin cement. The cemented crowns mounted on the testing jig were inclined 30 degrees and a universal testing machine was used to measure the fracture strength. The results of this study were as follows: 1. The fracture strength of the ceramic crown with 2.5mm depth and $8^{\circ}$ inclination was the highest (1393N). Crowns of 1.5mm depth and $4^{\circ}$ inclination had the lowest strength (1015N) 2. There were no significant differences of the fracture strength according to occlusal depth and axial inclination. 3. Most fracture lines began at the loading area and extended through proximal surface perpendicular to the margin, irrespective of occlusal depth. 4. There was positive correlation between the fracture strength and the fracture surface area of crowns.

  • The purpose of the study was to observe the time serial changes in proximal contact tightness after single crown restoration. Initial proximal contact tightness before crown preparation and after prosthesis restoration were measured repeatedly in 12 subjects. In consequence proximal contact tightness of temporary setting on prosthesis was bigger than contact tightness before initial preparation, contact tightness of the prostheses of 1-2 weeks after the setting was similar to that before the preparation, tightness of 2-4 weeks after the setting showed little change and maintained constant contact tightness.

  • Purpose: This study was to analyze the castability, surface oxide characteristic of Co-Cr alloy for porcelain fused to metal crown and the bonding strength of porcelain fused to metal crown. Co-Cr and Ni-Cr alloy for porcelain fused to metal crown was used for tests of the castability and surface oxide state and shear bonding strength by various porcelain. The aim of this study was to suggest the differences of result according to Co-Cr and Ni-Cr alloy. Methods: The kinds of alloy as test specimen was Co-Cr and Ni-Cr alloy. The castability index on the alloy specimens. The surfaces of two alloys were analyzed by SEM and EDX in order to observe oxide characteristic. And the shear test was performed by MTS. Results: The castability index of Co-Cr alloy was 96.8% and Ni-Cr alloy was 94.4%. The strongest bonding strength of Co-Cr alloy was shown 67.37 MPa. Conclusion: The shear bonding strength between Co-Cr alloy and EX3 porcelain was the strongest comparing with others. And all of each alloy was indicated as same level about the castability.

  • A rare case of the third molar on the site of right maxilla has observed from a 25 years old Korean female before and after extraction. The characteristics were as following ; 1. The morphology of third molar crown had resemblance to maxillary second molar, and sizes were smaller than G.V. Black's mearsurments. 2. In occlusal view, lingual cusp was largest and distolingual cusp was not seen, and decayed to the extent of degreei. 3. Numbers of root were 5 and they were not fused.
