Background/Aims: Risk factors for progression to critical illness in hospital-acquired coronavirus disease 2019 (COVID-19) remain unknown. Here, we assessed the incidence and risk factors for progression to critical illness and determined their effects on clinical outcomes in patients with hospital-acquired COVID-19. Methods: This retrospective cohort study analyzed patients admitted to the tertiary hospital between January 2020 and June 2022 with confirmed hospital-acquired COVID-19. The primary outcome was the progression to critical illness of hospital-acquired COVID-19. Patients were stratified into high-, intermediate-, or low-risk groups by the number of risk factors for progression to critical illness. Results: In total, 204 patients were included and 37 (18.1%) progressed to critical illness. In the multivariable logistic analysis, patients with preexisting respiratory disease (OR, 3.90; 95% CI, 1.04-15.18), preexisting cardiovascular disease (OR, 3.49; 95% CI, 1.11-11.27), immunocompromised status (OR, 3.18; 95% CI, 1.11-9.16), higher sequential organ failure assessment (SOFA) score (OR, 1.56; 95% CI, 1.28-1.96), and higher clinical frailty scale (OR, 2.49; 95% CI, 1.62-4.13) showed significantly increased risk of progression to critical illness. As the risk of the groups increased, patients were significantly more likely to progress to critical illness and had higher 28-day mortality. Conclusions: Among patients with hospital-acquired COVID-19, preexisting respiratory disease, preexisting cardiovascular disease, immunocompromised status, and higher clinical frailty scale and SOFA scores at baseline were risk factors for progression to critical illness. Patients with these risk factors must be prioritized and appropriately isolated or treated in a timely manner, especially in pandemic settings.
본 연구에서는 실제적으로 베나트 대류의 발생시 유체층의 상하면은 적당한 대류 열전달 계수를 가지고 있게 되는데, 이와 같은 경계 조건을 가진 가변 점성 유 체의 안정성은 연구된 바가 없다. 이에 따라 본 연구를 수행하게 되었으며 유체의 점성이 지수 함수적으로 (.nu.=.nu.o exp(-CT)) 변화할 경우를 관찰하였다. Fig.1은 대 상이 된 유체층을 보인 것으로 하부는 고정되어 있고 상부는 고정되어 있거나 자유 표 면 상태이다. 유체층의 하부는 상부보다 더 뜨겁게 되어 있는데, 이것은 유체층의 상부는 차가운 주위로 열을 빼앗기며 하부는 뜨거운 주위로부터 가열되기 때문이다. 이때 상하부의 냉각 및 가열 경계 조건은 대류경계 조건으로 주어진다. 열팽창 계수 는 양이며 온도의 증가에 따라 점성은 감소한다.이와 같은 온도-점성 관계, 수력학 적 경계조건, 열적 조건등은 프란틀 수가 큰 유체에서 표본적으로 나타나는 것들로서 선형적 안정 이론을 적용하여 옳은 결과를 얻을 수 있다.
Critical nozzle has been frequently employed to measure the flow rate of various gases, but hydrogen gas, especially being at high-pressure condition, was not nearly dealt with the critical nozzle due to treatment danger. According to a few experimental data obtained recently, it was reported that the discharge coefficient of hydrogen gas through the critical nozzle exceeds unity in a specific range of Reynolds number. No detailed explanation on such an unreasonable value was made, but it was vaguely inferred as real gas effects. For the purpose of practical use of high-pressure hydrogen gas, systematic research is required to clarify the critical nozzle flow of high-pressure hydrogen gas. In the present study, a computational fluid dynamics(CFD) method has been applied to predict the critical nozzle flow of high-pressure hydrogen gas. Redlich-Kwong equation of state that take account for the forces and volume of molecules of hydrogen gas were incorporated into the axisymmetric, compressible Navier-Stokes equations. A fully implicit finite volume scheme was used to numerically solve the governing equations. The computational results were validated with some experimental data available. The results show that the coefficient of discharge coefficient is mainly influenced by the compressibility factor and the specific heat ratio, which appear more remarkable as the inlet total pressure of hydrogen gas increases.
Critical loads and load-carrying capacities for steel scaffolds used as shoring systems were compared using computational and experimental methods in Part I of this paper. In that paper, a simple 2-D model was established for use in evaluating the structural behavior of scaffold-shoring systems. This 2-D model was derived using an incremental finite element analysis (FEA) of a typical complete scaffold-shoring system. Although the simplified model is only two-dimensional, it predicts the critical loads and failure modes of the complete system. The objective of this paper is to present a closed-form solution to the 2-D model. To simplify the analysis, a simpler model was first established to replace the 2-D model. Then, a closed-form solution for the critical loads and failure modes based on this simplified model were derived using a bifurcation (eigenvalue) approach to the elastic-buckling problem. In this closed-form equation, the critical loads are shown to be function of the number of stories, material properties, and section properties of the scaffolds. The critical loads and failure modes obtained from the analytical (closed-form) solution were compared with the results from the 2-D model. The comparisons show that the critical loads from the analytical solution (simplified model) closely match the results from the more complex model, and that the predicted failure modes are nearly identical.
Since the propagation of a short fatigue crack is directly related to the large crack which causes the fracture of bulk specimen, the detailed study on the propagation of the short crack is essential to prevent the fatigue fracture. However, a number of recent studies have demonstrated that the short crack can grow at a low applied stress level which are predicted from the threshold condition of large crack. In present study, the threshold condition for the propagation of short fatigue crack is examined with respect to the micro-structure and cyclic loading history. Specimens employed in this study were decarburized eutectoid steels which have various decarburized ferrite volume fraction. Rotating bending fatigue test was carried out on these specimens with the special emphasis on the "critical non-propagating crack length" It is found that the reduction of the endurance limit of their particular micro-structures can be due to the increase of the length of critical non-propagating crack, and the quantitative relationship between the threshold stress ${\sigma}_{wo}$ and the critical non-propagating crack length $L_c$ can be written as ${\sigma}_{wo}{^m}{\cdot}L_c=C$ where m,C is constant. Further experiments were carried out on cyclic loading history on the length of critical non-propagating crack. It shown that the length of critical non-propagating crack is closely related to cyclic loading history.
Focusing on both small separations and baryonic acoustic oscillation scales, the cosmic evolution of the clustering properties of peak, void, wall, and filament-type critical points is measured using two-point correlation functions in ΛCDM dark matter simulations as a function of their relative rarity. A qualitative comparison to the corresponding theory for Gaussian random fields allows us to understand the following observed features: (i) the appearance of an exclusion zone at small separation, whose size depends both on rarity and signature (i.e. the number of negative eigenvalues) of the critical points involved; (ii) the amplification of the baryonic acoustic oscillation bump with rarity and its reversal for cross-correlations involving negatively biased critical points; (iii) the orientation-dependent small-separation divergence of the cross-correlations of peaks and filaments (respectively voids and walls) that reflects the relative loci of such points in the filament's (respectively wall's) eigenframe. The (cross-) correlations involving the most non-linear critical points (peaks, voids) display significant variation with redshift, while those involving less non-linear critical points seem mostly insensitive to redshift evolution, which should prove advantageous to model. The ratios of distances to the maxima of the peak-to-wall and peak-to-void over that of the peak-to-filament cross-correlation are ~2-√~2 and ~3-√~3WJ, respectively, which could be interpreted as the cosmic crystal being on average close to a cubic lattice. The insensitivity to redshift evolution suggests that the absolute and relative clustering of critical points could become a topologically robust alternative to standard clustering techniques when analysing upcoming surveys such as Euclid or Large Synoptic Survey Telescope (LSST).
A procedure for the test of independence of the observations and the null distribution are studied for a waiting-time distribution of the number of Bernoulli trials required to obtain a preassigned number of successes under Markov dependence. Selected critical values for the test statistic are tabulated.
This paper investigates the noise radiated by a cascade of flat-plate airfoils interacting with homogeneous, isotropic turbulence. At frequencies above the critical frequency, all wavenumber components of turbulence excite propagating cascade modes, and cascade effects are shown to be relatively weak. In this frequency range, acoustic power was shown to be approximately proportional to the number of blades. Based on this finding at high frequencies, an approximate expression is derived for the power spectrum that is valid above the critical frequency and which is in excellent agreement with the exact expression for the broadband power spectrum. The approximate expression shows explicitly that the acoustic Power above the critical frequency is proportional to the blade number, independent of the solidity, and varies with frequency as ${\phi}_{ww}(\omega/W$), where ${\phi}_{ww}$ is the wavenumber spectrum of the turbulence velocity and W is mean-flow speed. The formulation is used to perform a parametric study on the effects on the power spectrum of the blade number stagger angle, gap-chord ratio and Mach number. The theory is also shown to provide a close fit to the measured spectrum of rotor-stator interaction when the mean square turbulence velocity and length-scale are chosen appropriately.
본 연구에서는 곡관 채널에서의 비뉴튼 젤 추진제의 유동 특성에 대해 연구하였다. 물을 기본유체로 하는 모사젤을 Carbopol 941 젤화 작용제와 NaOH 농축액을 혼합하여 제작하였으며 입자 유무에 따른 유동 특성을 파악하기 위해 $Al_2O_3$ 나노 입자가 첨가된 젤을 제작하여 두 젤 추진제간의 유변학적 특성을 비교하였다. 두 모사젤에 대해 U-자형의 곡관부 위치별 유동특성과 Dean 와류(vortices)의 경향은 상이하였으나 나노 입자가 첨가된 모사젤 추진제의 경우 높은 컨시스턴시 지수에도 불구하고 두 모사젤 모두 비슷한 범위의 임계 Dean 수를 도출하였다. 나노 입자 첨가 유무와 무관하게 power-law 지수값이 임계 Dean 수를 결정하는데 주요 변수임을 판단할 수 있었으나 나노입자가 첨가된 젤의 경우 Dean 와류 강도의 변동폭이 상대적으로 크다는 결론을 내릴 수 있었다.
