Purpose: This study was carried out to describe the process and evaluation of a critical thinking class for new community health practitioners. Methods: The case study design was used to develop and evaluate a critical thinking class for 46 participants in the community health practitioners training program. The class was held two hours a week for 8 weeks. Critical thinking disposition was tested before and after the class and critical skill was graded according to the final test score. Data analysis was performed using descriptive statistics and paired t-tests using SPSS WIN 20.0. Results: Clinical critical thinking competences were identified through the literature review. The case situations with questions guiding the problem-solving process were developed and used for group discussion. Critical thinking disposition of participants was determined to have increased slightly after having taken the class. 17.4% of the participants had a competency level high enough to solve a problem and half of them stayed at the level of understanding of critical thinking. Compared with the class's satisfaction with the relevance to their jobs, the satisfaction with the learning method and instructor was high. Conclusion: The findings of this research will serve as the basis for developing critical thinking classes for community health nurses in order to improve their critical thinking competence.
Purpose: This study aimed to utilize concept analysis to obtain a better understanding of the concept of "continuity of care" in chronic diseases. Methods: The concept of continuity of care was analyzed using the Walker and Avant method. Covering literature in English from 1930 to 2018, the data sources included CINAHL Complete, Academic Search Complete, MEDLINE, PsyARTICLES, Health Source: Nursing/Academic Edition, Google Scholar, Science Direct, and the Cochrane Library. Results: A comprehensive definition of concept of continuity of care was developed based on a systematic search and synthesis. The key defining attributes were identified as (a) care over time, (b) the relationship between an individual patient and a care team, (c) information transfer, (d) coordination, and (e) meeting changing needs. The antecedents of continuity of care were having a chronic disease, inexperienced with disease management, a poorly coordinated healthcare system, and medical care limitations. The consequences of continuity of care were decreasing hospital admissions, reducing costs, reducing emergency room visits, improving the quality of life, improving patient satisfaction, and delivering good healthcare. Conclusion: The thorough concept analysis provides insight into the nature of "continuity of care" in chronic diseases and also helps ground the concept in healthcare.
The free vibration analysis of fluid conveying Timoshenko pipeline with different boundary conditions using Differential Transform Method (DTM) and Adomian Decomposition Method (ADM) has not been investigated by any of the studies in open literature so far. Natural frequencies, modes and critical fluid velocity of the pipelines on different supports are analyzed based on Timoshenko model by using DTM and ADM in this study. At first, the governing differential equations of motion of fluid conveying Timoshenko pipeline in free vibration are derived. Parameter for the nondimensionalized multiplication factor for the fluid velocity is incorporated into the equations of motion in order to investigate its effects on the natural frequencies. For solution, the terms are found directly from the analytical solution of the differential equation that describes the deformations of the cross-section according to Timoshenko beam theory. After the analytical solution, the efficient and easy mathematical techniques called DTM and ADM are used to solve the governing differential equations of the motion, respectively. The calculated natural frequencies of fluid conveying Timoshenko pipelines with various combinations of boundary conditions using DTM and ADM are tabulated in several tables and figures and are compared with the results of Analytical Method (ANM) where a very good agreement is observed. Finally, the critical fluid velocities are calculated for different boundary conditions and the first five mode shapes are presented in graphs.

The use of Supercritical Fluids(SCF) has been proposed for numerous power cycle designs as part of the Generation IV advanced reactor designs, and can provide for higher thermal efficiency. One particular area of interest involves the behavior of SCF during a blowdown or depressurization process. Currently, no data are available in the open literature at supercritical conditions to characterize this phenomenon. A preliminary computational analysis, using a homogeneous equilibrium model when a second phase appears in the process, has shown the complexity of behavior that can occur. Depending on the initial thermodynamic state of the SCF, critical flow phenomena can be characterized in three different ways; the flow can remain in single phase(high temperature), a second phase can appear through vaporization(high pressure low temperature) or condensation(high pressure, intermediate temperature). An experimental facility has been built at the University of Wisconsin to study SCF depressurization through several diameter breaks. The preliminary results obtained show that the experimental data can be predicted with good agreement by the model for all the different initial conditions.

Purpose: Virtual worlds are emerging in health care as an innovative way of helping clients take responsibility for and promote their own care. Yet, the concept of health promotion education in this environment has not been elucidated. Therefore, we undertook a concept analysis to discover the underlying attributes and contextual basis to generate a conceptual understanding of virtual worlds in the context of health promotion education. Methods: Using Rodgers' evolutionary view of concept development, we analyzed literature on virtual worlds that describe their use to discover critical attributes, antecedents, and consequences of this phenomenon. Eleven studies matching inclusion criteria were reviewed. Results: Analysis of the literature highlighted three areas of important domains: user, simulated environment, and education. This concept analysis extends our understanding of a virtual world for health promotion education. Conclusion: This study may help anticipate future trends in the use of virtual worlds in the context of health promotion education, serving as a stepping-stone for further research on this aspect of the study. The next step should focus on the development of an evaluation tool that can measure the factors identified in this analysis to determine their effects on outcomes and environmental interactions.
Purpose: Cardiac arrest has multiple characteristics that need to be approached as an integrated method according to the various changes in the body system. This study was performed to develop a useful guideline for early detection of cardiac arrest by revealing the attributes of cardiac arrest through a concept analysis. Methods: This study was conducted according to the Walker and Avant's concept analysis method. Systematic literature review and in-depth interview with nurses who experienced cardiac arrest situation were conducted. Based on the literature reviews and in-depth interviews with nurses, the attributes and the empirical referents of the concept of cardiac arrest were elicited. Results: The definable attributes of cardiac arrest were 1) loss of consciousness, 2) abnormal respiratory condition, 3) abnormal cardiovascular signs. Cardiac arrest was found to occur by several antecedents such as cardiac problem, non-cardiac problem, or general problem, whereas ischemia and re-perfusion injury, which can lead to multiple organ failure and death, were derived as consequences. Conclusion: In this study, the concept analysis eliciting attributes and empirical referents is found to be useful as a guideline for understanding and managing cardiac arrest. Based on these findings, clinical providers are expected to make a precise and rapid decision on cardiac arrest and respond quickly, which may increase survival rate of the patients underwent the arrest event.
It is well recognized that case management is required to survive in the rapidly changing medical environment. One of the case management is the critical pathway(CP) which is assumed to increase the quality of care and at the same time to decrease the length of stay in hospital. The purpose of the study was to develop a CP for the management of patients with postero-lateral fusion for lumbar spinal stenosis. Through review of literature and medical records of patients with spinal stenosis, a pilot CP was designed, including 8 different care components such as medication, laboratory tests, assessment etc., from one day before surgery to 6 days of postoperative care. Every item of the pilot CP was evaluated by a panel of experts to test the content validity. The items not agreed on by more than 4 out of 6 experts were deleted or modified to be integrated in the CP. To apply the modified CP to a clinical environment, the items reflecting treatment, medication and lab work were entered into an order communication system(OCS), and doctors and nurses were taught to use the CP. Finally, the development of CP for the patients with posterolateral lumbar fusion was completed after the application and variance analysis of the CP.
In order to investigate the characteristics in tensile strength of transversely isotropic rock, a new anisotropic tensile failure function was suggested. According to the function, the tensile strength is minimum in the normal direction to a weakness plane and rises exponentially to its maximum on a plane perpendicular to the weakness plane. The anisotropic function is defined in terms of three strength parameters which can be identified trom direct tensile tests of transversely isotropic rocks. By incorporating the suggested function into the critical plane approach, a numerical procedure which enables to search the tensile strength and the direction of critical plane at failure was presented. The validity of the suggested numerical procedure was checked through the simulation of direct tensile tests reported in a literature. The numerical results from the simulation were in good agreements with those from the laboratory tests.
This paper presents a theoretical research framework that was used to analyse operational risk management (ORM) system practices in Australia. It provides a new perspective on how to use national and international operational management system standards as a basis for systematic management of operational risks. Based on the extensive literature review and the analysis of operational risk management system practices that are common in Australian organisations, this paper identifies the critical factors for effective use of an ORM system. The proposed framework could also be used as a model to research ORM system applications in other countries.
