Cervical chondrocutaneous remnant: a case report

  • Cervical chondrocutaneous branchial remnants are very rare congenital lesions of the lateral neck; thus, our knowledge of this condition derives almost entirely from occasional case reports in the literature. They are thought to originate from the branchial arches and, therefore, can be found anywhere on the pathway along which those branchial arches migrate during embryogenesis. We report the case of a 5-year-old girl presenting with a cervical chondrocutaneous branchial remnant on the right lateral neck that had existed since birth, with no other anomalies.

Development of seven secondary neoplasms in a nevus sebaceous: a case report and literature review

  • Nevus sebaceous of Jadassohn is a congenital cutaneous hamartoma with epidermal, sebaceous, follicular, and apocrine structures that usually appears at birth or in early childhood. It has the potential to generate a variety of secondary neoplasms of different lineages, and the risk increases with patient age. Although multiple neoplasms may occasionally arise within the same lesion, the coexistence of more than five secondary tumors is extremely rare. Here we report a case of seven secondary tumors including syringocystadenoma papilliferum, desmoplastic trichilemmoma, sebaceoma, trichoblastoma, pigmented trichoblastoma, sebaceous adenoma, and tumor of follicular infundibulum arising within a nevus sebaceous. The complete diagnosis relies on the histopathological analysis of multipoint biopsies and delicate pathological sections.

Current understanding of angiosarcoma: disease biology and evolving treatment

  • Angiosarcoma is a very rare soft tissue sarcoma that originates from endothelial cells and typically has a poor prognosis. It is most commonly found in elderly white men and can occur anywhere in the body, particularly in the head, neck, and scalp. Patients who have undergone previous radiation treatment or who have chronic lymphedema also face an elevated risk of this condition. Various genetic changes are suspected to contribute to the development of angiosarcoma, and these changes have been identified as potential targets for treatment. For localized disease, wide surgical resection is often the prudent course of action. A multidisciplinary approach, which may include surgery, radiotherapy, systemic chemotherapy, or immunotherapy, is typically the most effective way to achieve favorable outcomes. In this review, we discuss the general understanding of angiosarcoma and its management, with a particular focus on the current evolving treatments for the disease.

Expression and Functional Analysis of cofilin1-like in Craniofacial Development in Zebrafish

  • Pharyngeal pouches, a series of outgrowths of the pharyngeal endoderm, are a key epithelial structure governing facial skeleton development in vertebrates. Pouch formation is achieved through collective cell migration and rearrangement of pouch-forming cells controlled by actin cytoskeleton dynamics. While essential transcription factors and signaling molecules have been identified in pouch formation, regulators of actin cytoskeleton dynamics have not been reported yet in any vertebrates. Cofilin1-like (Cfl1l) is a fish-specific member of the Actin-depolymerizing factor (ADF)/Cofilin family, a critical regulator of actin cytoskeleton dynamics in eukaryotic cells. Here, we report the expression and function of cfl1l in pouch development in zebrafish. We first showed that fish cfl1l might be an ortholog of vertebrate adf, based on phylogenetic analysis of vertebrate adf and cfl genes. During pouch formation, cfl1l was expressed sequentially in the developing pouches but not in the posterior cell mass in which future pouch-forming cells are present. However, pouches, as well as facial cartilages whose development is dependent upon pouch formation, were unaffected by loss-of-function mutations in cfl1l. Although it could not be completely ruled out a possibility of a genetic redundancy of Cfl1l with other Cfls, our results suggest that the cfl1l expression in the developing pouches might be dispensable for regulating actin cytoskeleton dynamics in pouch-forming cells.

악안면 성장양상에 따른 하악이부 헝태에 관한 연구 (A STUDY ON CORRELATIONSHIP BETWEEN CRANIOFACIAL GROWTH PATTERN AND SYMPHYSIS MORPHOLOGY)

  • 교정치료에 있어서 악안면 성장의 양상은 진단과 치료계획 수립에 있어서 매우 유용한 자료이며 교합의 달성 및 안모의 형태, 발육에 많은 영향을 미친다. 따라서 성장유형의 분류에 대한 많은 연구가 있었으며 성장예측을 위한 많은 시도가 있었다. 이에 본 연구에서는 성인 120명을 대상으로 치료전 측모두부계측 방사선사진을 이용하여 전안면고경에 대한 후안면고경의 비를 측정하여 $56\%-62\%$는 시계방향 성장군(36명), $65\%-80\%$는 반시계방향 성장군(43명)으로, 그리고 $62\%-65\%$는 정상군(41명)으로 분류하고 이에 따른 하악이부의 형태와 돌출정도 평가를 시행한 결과 다음과 같은 결과를 얻었다. 1. 남녀 하악이부 형태비교에 있어서는 남자가 여자보다 이부의 높이와 돌출정도가 크게 나타났다. 2. 시계방향 성장군이 반시계방향 성장군에 비해 하악이부의 Height, H/D ratio, Actual length는 크게 나타났고, Depth, Angle, effective length, E/A ratio는 작은 값을 나타냈다. 3. 악안면 성장양상에 있어 이부의 돌출정도가 작을수록 시계방향성장 경향을 나타내며, 돌출정도가 클수록 반시계방향성장 경향을 나타냈다.

치아 기관배양시 골형성단백의 역할에 관한 연구 (THE ROLE OF BONE MORPHOGENETIC PROTEIN IN THE TOOTH CULTURE)

  • Objectives : The proper development of the facial structures relies upon a sequence of tightly regulated signaling interactions between the ectoderm and mesoderm involving the participation of several families of signaling molecules. Among these, bone morphogenetic proteins (BMPs) have been suggested to be a key signal that regulates the development of the mandible and the initiation and morphogenesis of the teeth. The aim of this study was to examine the artificial development of the mandibular structures and to examine the role of BMPs on tooth morphogenesis and differentiation using an organ culture system. Materials and Methods : The tooth germs from Ed 11.5, 13.5 mice were dissected, and transplanted into the diastema of the mandible primordia. The mandibles containing the transplanted tooth germs were cultured in vitro. During this period, beads soaked with BMP4 were implanted around the transplanted tooth germs. In addition, a diastema block containing the transplanted tooth germ was dissected, then transferred to an adult mouse kidney. After the organ culture, the developing mandibular explant was removed from the kidney and prepared for the tissue specimens. Odontogeneis of the transplanted tooth germs was examined after Hematoxylin-eosin, Masson-trichrome staining. Results : Proliferation and differentiation of the tooth germs cultured in the diastema was observed. In the BMP4-treated tooth germs, the formation of the first and second molars was noted. The crown of the developing tooth showed the formation of a mature cusp with the deposition of enamel and dentin matrix. In conclusion, it was confirmed that BMP4 is involved in the formation of a dental crown and the differentiation of ameloblasts and odontoblasts of the molar tooth during the development of the transplanted tooth germs.

치근 및 치주조직 형성과정 동안 Sonic Hedgehog signaling의 역할 (Roles of Sonic Hedgehog Signaling During Tooth Root and Periodontium Formation)

  • 본 연구는 치근 및 치주조직의 형성과정동안 Sonic Hedgehog (SHH) signaling의 역할에 대해 알아보고자 치성간엽에서 조직특이적으로 Smoothened (Smo)가 활성화 또는 차단되는 mouse를 제작하여 분석하였다. 생후 28일경 Smo 활성화모델에서는 H-E 염색 시 얇은 두께의 치근상아질, 넓어진 치수강, 치주인대공간에 느슨하게 배열되어있는 섬유들이 관찰되었다. 면역조직화학염색 시 Smo 활성화모델은 wild type mouse와 비교하여 백악질, 치주인대 그리고/또는 백악모세포의 표식자인 Bsp, Dmp1, Periostin, Ank 같은 기질 단백질들의 발현이 현저히 감소하였다. 그러나 Smo 차단모델은 wild type mouse와 비교하여 이러한 기질 단백질들의 발현에 차이가 보이지 않았다. 본 연구를 통하여 치근 및 치주조직의 정상적인 형성과정에 낮은 수준으로 조절되는 SHH signaling이 필요하다는 것을 확인할 수 있었다.

소아 수두증 환자의 치과적 관리 (Dental Management of Pediatric Hydrocephalus)

  • 수두증은 뇌척수액이 비정상적으로 뇌실 혹은 두개강 내에 축적되는 것으로, 큰 머리둘레와 성장 지연, 지능 발육 저하, 영구치의 발육 속도 차이, 악안면 비대칭 등이 나타날 수 있다. 뇌실 - 복강 단락술이 가장 많이 사용되는 치료법이고 복강에 문제가 있는 경우 다른 단락술이 시행될 수 있다. 본 증례는 소아 수두증으로 인하여 뇌실 - 복강 단락술을 받은 병력이 있는 7세 6개월 환자의 치과적 관리에 대하여 다루고 있다. 환자는 유구치의 깊은 우식과 하악각 부위의 안면 부종이 관찰되었으며, 큰 머리 둘레와 신체적 및 정신적 발달 지연을 보였다. 아산화 질소 흡입 진정과 가벼운 보호 안정 하에 치수절제술을 시행하였으며 예방적 항생제는 필요로 하지 않았다. 최근 검진 시까지 치료받은 부위는 잘 유지되고 있었으며 향후 전반적인 구강위생 및 영구치의 발육 정도, 악안면부의 비대칭 발생 여부를 주기적으로 검진 받을 예정이다. 단락술을 받은 병력이 있는 수두증 환자들은 치과 치료 시, 단락 장치의 도관에 과도한 힘이 가해져 도관의 변위가 발생하지 않도록 해야 하고, 뇌실 - 심방 단락술을 받은 환자의 경우 예방적 항생제의 필요성과 관련하여 신경외과로의 의뢰가 추천된다. 수두증 환자들은 성장하면서 악안면부의 비대칭과 영구치의 발육 수준에 차이가 발생할 수 있으므로 이에 대한 관찰이 요구된다.

Clinical Analysis of Lobular Keloid after Ear Piercing

  • Background: Lobular keloid appears to be a consequence of hypertrophic inflammation secondary to ear piercings performed under unsterile conditions. We wish to understand the pathogenesis of lobular keloids and report operative outcomes with a literature review. Methods: A retrospective review identified 40 cases of lobular keloids between January, 2005 and December, 2010. Patient records were reviewed for preclinical factors such as presence of inflammation after ear piercing prior to keloid development, surgical management, and histopathologic correlation to recurrence. Results: The operation had been performed by surgical core extirpation or simple excision, postoperative lobular compression, and scar ointments. Perivascular infiltration was noted in intra- and extra-keloid tissue in 70% of patients. The postoperative recurrence rate was 10%, and most of the patients satisfied with treatment outcomes. Conclusion: Histological perivascular inflammation is a prominent feature of lobular keloids. Proper surgical treatment, adjuvant treatments, and persistent follow-up observation were sufficient in maintaining a relatively low rates of recurrence.

다발성 낭종을 동반한 면포 모반 (Nevus Comedonicus with Multiple Cyst)

  • Purpose: Nevus comedonicus is a rare disease involving abnormal development of the pilosebaceous unit, clinically appearing confluent clusters of open comedones. It is characterized by follicular epidermic invaginations filled with keratin, with atrophic pilose or sebaceous structures which are open to the lower pole of the invagination. We report a child with inflammatory pustules and cysts correlated to the nevus comedonicus. Methods: A 15-month-old girl was referred for treatment of a nevus comedonicus that has been developed since birth. There were periodic episodes of erythema and swelling of the plaque. The patient had treated previously with retinoid cream for 2 weeks without obvious benefit at dermatology clinic. We excised the nevus comedonicus with Y-shape leaving some lesions. Results: When we incised skin, we could see multiple large whitish cysts beneath the nevus comedonicus. Histopathologic findings showed deep invagination of epidermis filled with keratin plugs, keratinous cyst in the dermis, rudimentary hair follicle. Conclusion: We treated rare nevus comedonicus with multiple large cysts as surgical excision.
