Clinical analysis of factors affecting the failure of free flaps used in head and neck reconstruction

  • Background: Free tissue transfer is the preferred method of reconstructing head and neck defects, with a success rate of approximately 95%. Although flap failure is uncommon, it has a major impact on patient morbidity and diminishes quality of life, making it is important to investigate the causes of flap failure. Methods: This retrospective chart review analyzed patients who underwent free tissue transfer during head and neck reconstruction at a single institution between 2016 and 2021. Results: During the study period, 58 patients underwent 60 free flap procedures. Revision surgery was needed in 14 patients. Subsequent free flap surgery was performed in one patient, and three free flaps (5%) could not be salvaged. Cardiovascular disease was significantly associated with flap failure, and venous congestion (thrombosis) was the most common reason for revision surgery. Conclusion: Cardiovascular disease clearly emerged as a factor related to the failure of free flap surgery, and this issue warrants particular attention in patients for whom free tissue transfer is planned.

Alar crease as a donor site for the extension limb of modified nasolabial V-Y advancement flap

  • Background: The traditional nasolabial V-Y advancement flap is widely used for midface reconstruction, particularly for the lower third of the nose and upper lip, as its color and texture are similar to these areas. However, it provides insufficient tissue to cover large defects and cannot restore the nasal convexity, nasal ala, and adjacent tissues. The purpose of this study is to investigate the modified nasolabial V-Y advancement flap with extension limbs the along alar crease for the reconstruction of complex midface defects. Methods: A retrospective analysis of 18 patients, who underwent reconstruction with the modified nasolabial V-Y advancement flap, was performed between September 2014 and December 2022. An extension limb was added along the alar crease, adjacent to the defect area, and was hinged down as a transposition flap at the end of the advancement flap. Results: The extension limb along the alar crease successfully covered large and complicated defects, including those of the ala, the alar rim, the alar base, the nostrils, and the upper lip, with minor complications. Conclusion: The alar crease is a good donor site for the reconstruction of large and complex nasal and upper lip defects.

Influence of heritability on craniofacial soft tissue characteristics of monozygotic twins, dizygotic twins, and their siblings using Falconer's method and principal components analysis

  • Objective: The purpose of this study was to investigate the influence of heritability on the craniofacial soft tissue cephalometric characteristics of monozygotic (MZ) twins, dizygotic (DZ) twins, and their siblings (SIB). Methods: The samples comprised Korean adult twins and their siblings (mean age, 39.8 years; MZ group, n = 36 pairs; DZ group, n = 13 pairs of the same gender; and SIB group, n = 26 pairs of the same gender). Thirty cephalometric variables were measured to characterize facial profile, facial height, soft-tissue thickness, and projection of nose and lip. Falconer's method was used to calculate heritability (low heritability, $h^2$ < 0.2; high heritability, $h^2$ > 0.9). After principal components analysis (PCA) was performed to extract the models, we calculated the intraclass correlation coefficient (ICC) value and heritability of each component. Results: The MZ group exhibited higher ICC values for all cephalometric variables than DZ and SIB groups. Among cephalometric variables, the highest ${h^2}_{(MZ-DZ)}$ and ${h^2}_{(MZ-SIB)}$ values were observed for the nasolabial angle (NLA, 1.544 and 2.036), chin angle (1.342 and 1.112), soft tissue chin thickness (2.872 and 1.226), and upper lip thickness ratio (1.592 and 1.026). PCA derived eight components with 84.5% of a cumulative explanation. The components that exhibited higher values of ${h^2}_{(MZ-DZ)}$ and ${h^2}_{(MZ-SIB)}$ were PCA2, which includes facial convexity, NLA, and nose projection (1.026 and 0.972), and PCA7, which includes chin angle and soft tissue chin thickness (2.107 and 1.169). Conclusions: The nose and soft tissue chin were more influenced by genetic factors than other soft tissues.

  • Enlow의 구조적 대응체 분석법은 개개인에서 두개안면의 구성양상이 발달되어온 해부학적, 발육학적 특징의 복합체를 설명하고자 하는 것으로 다수의 평균에 의한 정상치와 비교하는 것이 아니고 개인에게서 나타나는 값을 비교하여 분석하는 방법이다. 본 연구의 목적은 두개안면의 골격형태에 인종적, 지역적으로 골격형태의 변이가 있으므로 한국인에 Enlow의 구조적 대응체 분석법을 적용해봄으로써 한국인 정상교합자에 나타나는 두개안면골격의 특성을 규명해보고자 하였다. 본 연구에서는 안모와 교합상태가 양호하고 교정치료의 경험이 없는 정상 교합자 100명(남자 50명, 여자 50명)을 대상으로 구조적 대응체 분석법을 이용하여 계측점과 계측선을 설정하고, 통계처리하여 다음과 같은 결과를 얻었다. 1. PCF와 PMV 간의 각도는 한국인 남자 평균 $38.54{\pm}2.93^{\circ}$, 여자 $38.43{\pm}2.90^{\circ}$으로 나타났고, Wits' appraisal은 한국인 남자 평균 $-2.51{\pm}3.25$와 여자 $-2.3{\pm}2.26$으로 나타났다. 하악지 배열을 나타내는 R4는 한국인 남자 $1.89{\pm}3.15$, 여자 $2.36{\pm}2.84$를 나타냈다. 2. 후두개저와 하악지의 골격적인 수평 길이 차이를 나타내는 A3-B3에서 여자가 남자보다 유의성있게 더 큰 차이를 나타내고 있어 여자가 남자에 비해 하악지가 후두개저(PCF)보다 더 긴 것으로 나타났다. 3. 상악골과 하악체의 길이 차이를 나타내는 A4-B4에서는 남자가 여자보다 유의성있게 더 큰 차이를 나타내고 있어 남자가 여자에 비해 하악체가 상악골보다 더 긴 것으로 나타났다. 4. 상악과 하악의 수평적인 전체 길이차를 나타내는 Al-Bl에서는 남녀에서 성차를 나타내고 있지 않았다. 이상의 결과에서 한국인은 서양인에 비하여 중안면부가 함몰되고 하악골은 전돌되며 하악지가 후하방으로 회전되어 있는 양상을 보인다. 또한 한국인에서 여자가 남자에 비해 하악지가 후두개저에 비해 더 크며 이에 보상적으로 하악체는 상악골에 비해 남자가 여자에 비해 더 크게 나타나는 것을 볼 수 있었다. 이로인해 결과적으로 상악과 하악의 전체적인 길이 차이에서는 남녀간의 차이를 나타내지 않았다.

Facial injury burden of personal mobility devices: a single-center retrospective analysis

  • Background: Personal mobility devices (PMDs) have become an increasingly popular transport modality globally. With increasing social interest in and demand for PMDs, the number of individuals visiting emergency departments with PMD-related injuries has also increased annually. This study aimed to evaluate injury patterns and treatment costs for patients treated in the department of plastic surgery in a trauma center. Methods: In this retrospective study, data concerning patients with PMD-related injuries from January 2017 to December 2021 were reviewed. The data retrieved included age, sex, alcohol consumption, helmet use, the type of impact, onset of injury, place of first visit, type of injury, admission status, operation status, and treatment cost. Multiple linear regression analysis was performed to determine the effects of various factors on cost. Results: Data were collected from 93 patients. Until 2019, the annual number of PMD-related accidents was less than 10; however, this number increased sharply in 2020. The average cost of hospitalization was USD 7,698 whereas the average cost of non-hospitalization was USD 631. Only fractures had a significant association with total cost in linear regression analysis (p< 0.001). Conclusion: The prevalence of PMD use and related injuries requiring plastic surgery during the study period showed significant health and financial costs both to the patients involved and to society. This cost could be reduced through stricter regulations concerning PMD use, advocating the use of protective gear, and promoting greater awareness of safety measures and of the consequences of PMD-related accidents.

Auricle reconstruction with autologous costal cartilage versus polyethylene implants in microtia patients: a meta-analysis

  • Background: Auricle reconstruction is among the most challenging procedures in plastic and reconstructive surgery, and the choice of framework material is a critical decision for both surgeons and patients. This meta-analysis compared the outcomes of autologous auricle reconstruction using costal cartilage with those of alloplastic reconstruction using porous polyethylene implants. Methods: A literature review was conducted using the PubMed and Embase databases to retrieve articles published between January 2000 and June 2024. The outcomes analyzed included postoperative complications such as framework exposure, infection, skin necrosis, hematoma, and hypertrophic scars, as well as patient satisfaction. The proportions of reconstructive outcomes from each selected study were statistically analyzed using the "metaprop" function in R software. Results: Fourteen articles met our inclusion criteria. The group undergoing polyethylene implant reconstruction exhibited higher rates of framework exposure, infection, and skin necrosis, whereas the autologous reconstruction group experienced higher rates of hematoma and hypertrophic scars. Of all the complications, framework exposure was the only one to show a statistically significant difference between the two groups (p< 0.0001). In terms of patient satisfaction, those who underwent autologous cartilage reconstruction reported a higher rate of satisfaction, although this difference did not reach statistical significance in the meta-analysis (p= 0.076). Conclusion: There is no statistically significant difference in postoperative complications such as infection, hematoma, skin necrosis, and hypertrophic scars between auricle reconstructions using autologous costal cartilage and those using polyethylene implants. However, reconstructions with polyethylene implants show a significantly higher rate of framework exposure.

Three-dimensional comparison of 2 digital models obtained from cone-beam computed tomographic scans of polyvinyl siloxane impressions and plaster models

  • Purpose: This study was performed to evaluate the dimensional accuracy of digital dental models constructed from cone-beam computed tomographic (CBCT) scans of polyvinyl siloxane (PVS) impressions and cast scan models. Materials and Methods: A pair of PVS impressions was obtained from 20 subjects and scanned using CBCT (resolution, 0.1 mm). A cast scan model was constructed by scanning the gypsum model using a model scanner. After reconstruction of the digital models, the mesio-distal width of each tooth, inter-canine width, and inter-molar width were measured, and the Bolton ratios were calculated and compared. The 2 models were superimposed and the difference between the models was measured using 3-dimensional analysis. Results: The range of mean error between the cast scan model and the CBCT scan model was -0.15 mm to 0.13 mm in the mesio-distal width of the teeth and 0.03 mm to 0.42 mm in the width analysis. The differences in the Bolton ratios between the cast scan models and CBCT scan models were 0.87 (anterior ratio) and 0.72 (overall ratio), with no significant difference (P>0.05). The mean maxillary and mandibular difference when the cast scan model and the CBCT scan model were superimposed was 53 ㎛. Conclusion: There was no statistically significant difference in most of the measurements. The maximum tooth size difference was 0.15mm, and the average difference in model overlap was 53 ㎛. Digital models produced by scanning impressions at a high resolution using CBCT can be used in clinical practice.

Pyogenic Granuloma: A Retrospective Analysis of Cases Treated Over a 10-Year

  • Background: Pyogenic granuloma (PG) is a benign vascular lesion of the mucosa and skin. Recent studies of the epidemiology of PG are rare. We aimed to retrospectively analyze characteristics of PG cases in South Korea. Methods: We reviewed the medical records of 155 patients treated for PG between March 2005 and May 2014. The male-to-female ratio was 1:1.2 (70 males, 85 females). The mean age of patients was 35.3 years. Results: A high occurrence was observed in the first and third decades in males, and the fourth to fifth decades in females. There was a statistically significant difference between genders according to age group (p<0.05). The average lesion diameter was $0.84{\pm}0.46cm$ (long axis). The most frequently involved site was the face (n=47). Bleeding was the primary complication (n=41). PG was mostly treated with excisional biopsy (n=74). The recurrence rate was 7.7% (n=12). Conclusion: We concluded that most common site of PG was the face, the age of female with PG is higher than previous studies, and finger is associated with trauma more than other sites. The most recent epidemiological information on PG of this study will support the treatment and diagnosis of PG and future research objectives.

Metastatic Squamous Cell Carcinoma of the Lower Lip: Analysis of the 5-Year Survival Rate

  • Background: The author analyse the impact of extracapsular lymph node spread and bone engagement in the ipsilateral neck of patients suffering squamous cell carcinoma (SCC) of the lower lip. Methods: The data of 56 neck dissections performed in patients suffering SCC of the lower lip between January 2000 and December 2008 were retrospectively analysed. Statistical analysis was performed with the Kaplan-Meier life table method, and the survival rate was investigated with the log rank statistic and significance test. The values were considered statistically significant at p<0.05. Results: Nine patients took advantage from simultaneous treatment of tumor and prophylactic neck dissection (level I-III), reaching 100% survival rate. Patients suffering metastasized disease, who received radical neck dissection at the time of tumor treatment, presented 83.3% survival rate. Patients who underwent previous surgery and radiotherapy presented worse prognosis although radical neck dissection in case of extra-capsular spread only (24.7%) and osseous engagement (22.2%). Conclusion: Prophylactic neck dissection (level I-III) is recommended in T3-T4 N0 SCC. Simultaneous treatment of tumor and cervical lymph nodes provides a better prognosis as respect to delayed nodal management. Extra-capsular spread with or without bone engagement represents independent risk factor responsible for high mortality rate of SCC of the lower lip.

A retrospective computed tomography analysis of maxillary fractures and the clinical outcomes of their unreduced parts

  • Background: Some parts of a maxillary fracture-for example, the medial and posterior walls-may remain unreduced because they are unapproachable or hard to deal with. This study aimed to investigate the self-healing process of unreduced maxillary membranous parts of fractures through a longitudinal computed tomography (CT) analysis of cases of unilateral facial bone injuries involving the maxillary sinus walls. Methods: Thirty-two patients who had undergone unilateral facial bone reduction surgery involving the maxillary sinus walls without reduction of the medial and posterior walls were analyzed in this retrospective chart review. Preoperative, immediate postoperative, and 3-month postoperative CT scans were analyzed. The maxillary sinus volume was calculated and improvements in bone continuity and alignment were evaluated. Results: The volume of the traumatized maxillary sinuses increased after surgery, and expanded significantly by 3 months postoperatively (p< 0.05). The significant preoperative volume difference between the normal and traumatized sides (p= 0.024) resolved after surgery (p> 0.05), and this resolution was maintained at 3 months postoperatively (p > 0.05). The unreduced parts of the maxillary bone showed improved alignment and continuity (in 75.0% and 90.6% of cases, respectively), and improvements in bone alignment and bone continuity were found to be correlated using the Pearson chi-square test (p= 0.002). Conclusion: Maxillary wall remodeling through self-healing occurred concomitantly with an increase in sinus volume and simultaneous improvements in bone alignment and continuity. Midfacial surgeons should be aware of the natural course of unreduced fractured medial and posterior maxillary walls in complex maxillary fractures.
