Frontotemporal Craniotomy for Clipping of Unruptured Aneurysm Using a Diamond-Coated Thread Wire Saw and Reconstruction Using Calcium Phosphate Cement without Metal Fixation

  • Metal fixation systems for cranial bone flaps cut by a drill are convenient devices for cranioplasty, but cause several complications. We use modified craniotomy using a fine diamond-coated threadwire saw (diamond T-saw) to reduce the bone defect, and osteoplasty calcium phosphate cement without metal fixation. We report our outcomes and tips of this method. A total of 78 consecutive patients underwent elective frontotemporal craniotomy for clipping of unruptured intracranial aneurysms between 2015 and 2019. The follow-up periods ranged from 13 to 66 months. The bone fixation state was evaluated by bone computed tomography (CT) and three-dimensional CT (3D-CT). The diamond T-saw could minimize the bone defect. Only one wound infection occurred within 1 week postoperatively, and no late infection. No pain, palpable/cosmetically noticeable displacement of the bone flap, fluid accumulations, or other complications were observed. The condition of bone fixation and the cosmetic efficacy were thoroughly satisfactory for all patients, and bone CT and 3D-CT demonstrated that good bone fusion. No complication typical of metal fixation occurred. Our method is technically easy and safety, and achieved good mid-term bone flap fixation in the mid-term course, so has potential for bone fixation without the use of metal plates.

감마 나이프 방사선 수술시 필름 선량 측정에 의한 조직 불균일성에 대한 연구 (Evaluation of Tissue Inhomogeneity for Gamma-knife Radiosurgery Using Film Dosimetry)

  • 목적 : 감마 나이프에 사용되고 있는 코발트-60의 골에 의한 흡수 선량 감소는 -3.5$\%$이다. 그러나 모든 방사선 수술 치료 계획 시스템은 치료 부위를 단위밀도 (unit density)로 가정하여 계산을 시행하므로 조직 불균일성에 대한 고려를 하지않고 있다. 중두개와 (mid cranial fossa) 부위는 여러 두께의 골로 구성되어 있기 때문에 감마 나이프를 이용한 방사선 조사시 중두개와에 위치한 병변인 뇌하수체 종양 지점이나 청신경 초종 지점의 치료시 흡수되는 방사선량은 방사선 수술 계획 시스템에 의하여 계산한 값과 차이가 날 가능성이 있다. 이 연구는 이러한 중두개와에 위치한 병변의 치료시 골에 의한 흡수 선량의 감소에 대한 검증을 위하여 시행하였다. 대상 및 방법 : Alderson Rando phantom (이하 인체 모형 판톰-human phantom-이라 명명)의 두개골 부위에 4개의 핀에 의해 Leksell stereotactoc frame을 고정시키고 뇌단층 촬영을 시행하였다. 뇌 단층 촬영 상에 뇌하수체 종양과 청신경 초종(acoustic neurimona)에 해당되는 각각의 지점을 정한 후 먼저 뇌하수체 지점에 인체 모형 판톰의 모양에 맞게 자른 코닥 X-omat V 필름을 사이에 넣고 인체 모형 판톰을 잘 고정한 후에 14 mm 조준기 (collimator)를 부착하여 방사선을 조사하였다. 한 지점당 3회 반복 시행하였다. 18 mm 조준기를 이용하여 동일한 방법으로 3회 반복 시행하였다. 두 번째 지점인 청신경 초종 지점에 필름을 넣은 뒤 첫 번째와 동일한 방법으로 14 mm 조준기와 18 mm 조준기를 사용하여 각각 3회 반복 시행하였다. 동일한 실험 조건으로 균일한 밀도로 이루어진 16 cm 지름의 구형 폴리스티렌 판톰 내에 폴리스티렌 카세트에 맞는 필름을 삽입하여 14 mm, 18 mm 조준기 각각에 대하여 방사선 조사를 시행하였으며 이 과정을 3회 반복 시행하였다. 이후 필름들을 현상하여 VXR-12 필름 광학 측정기를 이용하여 등선량 곡선을 얻는다. 여기서 얻어진 등선량 곡선 (isodose curve)을 기초로 하여 off axis ratio 곡선을 얻었다. 결과 : 중두개와에 위치한 병소에 대하여 감마 나이프를 이용한 방사선 조사를 시행할 때 골에 의한 흡수 선량 감소는 뇌하수체 종양 지점이 청신경 초종 지점보다 많이 나타났으며 (0.2-3.0 mm vs. 0.1-1.3 mm) 조준기 크기에 따라 14 mm 조준기일 경우가 18 mm 조준기보다 더 많은 흡수 선량 감소 (0.4-3.0 mm vs. 0.2-2.2 mm)를 보였다. 결론 : 흡수 전량 감소가 실제 임상에서 치료 계획을 세우는데 있어서는 큰 영향을 미치지 않는 범위 내 (1 mm)에서 일어남으로 골에 의한 흡수 선량 감소를 고려하여 치료계획을 조정해야 할 필요는 없을 것으로 생각된다.

전두골에 발생한 콜레스테롤 육아종 1례 - 증례보고 - (Cholesterol Granuloma of Frontal Bone - A Case Report -)

  • Cholesterol granuloma of frontal bone is a rare disease which usually occurs at the lateral part of the supraorbital ridge. This expanding lesion grows slowly and extends into the orbit and anterior cranial fossa. The most common symptom is proptosis. This granuloma is composed of a granulomatous reaction surrounding cholesterol crystals. Surgical treatment involves aspiration of contents and stripping or curettage of the lining which is highly successful. We experienced a case of cholesterol granuloma of frontal bone with huge intracranial extension, which was cured by surgical removal. The clinical features, radiologic, and pathologic finding were discussed and the pertinent literatures were reviewed.

우측 광대뼈에 발생한 중심 거대 세포 육아종 1예 (A Case of a Central Giant Cell Granuloma in the Right Zygomatic Bone)

  • The central giant cell granuloma is a benign tumor seen generally in the mandible, but rarely in other cranial bones. Herein, we present a 51-year-old man with central giant cell granuloma in the right zygomatic bone. Physical and radiologic examinations of the central giant cell granuloma in the zygomatic bone showed that specific and preoperative diagnosis is usually difficult. Therefore, clinicians should consider the possibility that central giant cell granuloma may occur in the zygomatic bone mimicking other more frequently observed lesions.

Bone Healing Capacity of Demineralized Dentin Matrix Materials in a Mini-pig Cranium Defect

  • Purpose: In this study the bone healing ability of autogenous tooth bone graft material as a substitute material was evaluated in a mini-pig cranial defect model through histologic examinations and osteonectin reverse transcription polymerase chain reaction (RT-PCR) quantitative analysis. Materials and Methods: A defect was generated in the cranium of mini-pigs and those without a defect were used as controls. In the experimental group, teeth extracted from the mini-pig were manufactured into autogenous tooth bone graft material and grafted to the defect. The mini-pigs were sacrificed at 4, 8, and 12 weeks to histologically evaluate bone healing ability and observe the osteonectin gene expression pattern with RT-PCR. Result: At 4 weeks, the inside of the bur hole showed fibrosis and there was no sign of bone formation in the control group. On the other hand, bone formation surrounding the tooth powder granule was observed at 4 weeks in the experimental group where the bur hole was filled with tooth powder. Osteonectin gene expression; there was nearly no osteonectin expression in the control group while active osteonectin expression was observed from 4 to 12 weeks in the experimental group. Conclusion: We believe this material will show better results when applied in a clinical setting.

Assessment of stem cell viability in the initial healing period in rabbits with a cranial bone defect according to the type and form of scaffold

  • Purpose: Increased bone regeneration has been achieved through the use of stem cells in combination with graft material. However, the survival of transplanted stem cells remains a major concern. The purpose of this study was to evaluate the viability of transplanted mesenchymal stem cells (MSCs) at an early time point (24 hours) based on the type and form of the scaffold used, including type I collagen membrane and synthetic bone. Methods: The stem cells were obtained from the periosteum of the otherwise healthy dental patients. Four symmetrical circular defects measuring 6 mm in diameter were made in New Zealand white rabbits using a trephine drill. The defects were grafted with 1) synthetic bone (${\beta}$-tricalcium phosphate/hydroxyapatite [${\beta}-TCP/HA$]) and $1{\times}10^5MSCs$, 2) collagen membrane and $1{\times}10^5MSCs$, 3) ${\beta}-TCP/HA+collagen$ membrane and $1{\times}10^5MSCs$, or 4) ${\beta}-TCP/HA$, a chipped collagen membrane and $1{\times}10^5MSCs$. Cellular viability and the cell migration rate were analyzed. Results: Cells were easily separated from the collagen membrane, but not from synthetic bone. The number of stem cells attached to synthetic bone in groups 1, 3, and 4 seemed to be similar. Cellular viability in group 2 was significantly higher than in the other groups (P<0.05). The cell migration rate was highest in group 2, but this difference was not statistically significant (P>0.05). Conclusions: This study showed that stem cells can be applied when a membrane is used as a scaffold under no or minimal pressure. When space maintenance is needed, stem cells can be loaded onto synthetic bone with a chipped membrane to enhance the survival rate.

Role of a Burr Hole and Calvarial Bone Marrow-Derived Stem Cells in the Ischemic Rat Brain : A Possible Mechanism for the Efficacy of Multiple Burr Hole Surgery in Moyamoya Disease

  • Objective : This study investigates the role of a burr hole and calvarial bone marrow-derived stem cells (BMSCs) in a transient ischemic brain injury model in the rat and postulates a possible mechanism for the efficacy of multiple cranial burr hole (MCBH) surgery in moyamoya disease (MMD). Methods : Twenty Sprague-Dawley rats (250 g, male) were divided into four groups : normal control group (n=5), burr hole group (n=5), ischemia group (n=5), and ischemia+burr hole group (n=5). Focal ischemia was induced by the transient middle cerebral artery occlusion (MCAO). At one week after the ischemic injury, a 2 mm-sized cranial burr hole with small cortical incision was made on the ipsilateral (left) parietal area. Bromodeoxyuridine (BrdU, 50 mg/kg) was injected intraperitoneally, 2 times a day for 6 days after the burr hole trephination. At one week after the burr hole trephination, brains were harvested. Immunohistochemical stainings for BrdU, CD34, VEGF, and Doublecortin and Nestin were done. Results : In the ischemia+burr hole group, BrdU (+), CD34 (+), and Doublecortin (+) cells were found in the cortical incision site below the burr hole. A number of cells with Nestin (+) or VEGF (+) were found in the cerebral parenchyma around the cortical incision site. In the other groups, BrdU (+), CD34 (+), Doublecortin (+), and Nestin (+) cells were not detected in the corresponding area. These findings suggest that BrdU (+) and CD34 (+) cells are bone marrow-derived stem cells, which may be derived from the calvarial bone marrow through the burr hole. The existence of CD34 (+) and VEGF (+) cells indicates increased angiogenesis, while the existence of Doublecortin (+), Nestin (+) cells indicates increased neurogenesis. Conclusion : Based on these findings, the BMSCs through burr holes seem to play an important role for the therapeutic effect of the MCBH surgery in MMD.

Supraorbital Keyhole Approach for Intracranial Aneurysms : Transitioning from Concerns to Confidence

  • For surgical minimalism to reduce iatrogenic traumatization, a supraorbital keyhole approach has already been successfully applied to treat many unruptured anterior circulation aneurysms. However, using this minimal approach also raises several technical concerns due to the small cranial opening and cosmetic impact of a facial incision. Yet, such technical limitations can be overcome by using favorable surgical indications, slender surgical instruments, and optimized surgical techniques, while excellent cosmetic outcomes can be achieved using a short skin incision located <1 cm from the supraorbital margin, reconstruction of any bone defects around the bone flap, and meticulous wound closure. Thus, given such reassuring surgical results, in terms of the clipping status, neurological effects, and cosmetic outcomes, any concerns can be transitioned into confidence.

Primary Osteolytic Intraosseous Atypical Meningioma with Soft Tissue and Dural Invasion : Report of a Case and Review of Literatures

  • Primary intraosseous meningioma is a rare tumor, and atypical pathologic components both osteolytic lesion and dura and soft tissue invasion is extremely rare. A 65-year-old woman presented with a 5-month history of a soft mass on the right frontal area. MR imaging revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the right frontal bone, and CT showed a destructive skull lesion. The mass was adhered tightly to the scalp and dura mater, and it extended to some part of the outer and inner dural layers without brain invasion. The extradural mass and soft tissue mass were totally removed simultaneously and we reconstructed the calvarial defect with artificial bone material. The pathological study revealed an atypical meningioma as World Health Organization grade II. Six months after the operation, brain MR imaging showed that not found recurrence in both cranial and spinal lesion. Here, we report a case of primary osteolytic intraosseous atypical meningioma with soft tissue and dural invasion.

  • Food intake and swallowing are complicated and intriguing series of movements involving voluntary and involuntary activities of cranial and spinal nerves and muscles. They have two most important functions, that is, food passage from the oral cavity to stomach and airway protection. Tongue, buccinators, and hyoid bone and its muscular attachments are anatomic structures for swallowing of special interests. The swallowing process of liquid is commonly divided into oral preparatory, oral propulsive, pharyngeal, and esophageal stages according to the location of the bolus. The movement of the food in the oral cavity and to the oropharynx differs between eating solid food and drinking liquid.
